A 3 year prospective study of survival rate of narrow diameter implants (좁은 직경 임플란트 3년간의 생존율에 관한 후향적 연구)

  • Purpose: The purpose of present study was to retrospectively analyze the survival rate of narrow diameter implant less than 3.6 mm by initial stability and radiographic measurements. Materials and Methods: In total, 24 patients who received 38 narrow diameter implants (${\leq}3.6mm$ in diameter, ${\geq}7mm$ in length) were enrolled in this retrospective study. The cumulative survival rate was calculated and various factors were investigated according to the implant platform diameter, body diameter, length, position, concomitant use of guided bone regeneration in implant placement and final prosthesis type. Initial stability was investigated with implant stability quotient (ISQ) value. The mesial and distal marginal bone level (MBL) change was calculated with radiography. Results: The overall survival rate was 92.11%. Mean ISQ value and MBL change of survival implants was 66.26 and $0.14{\pm}0.31mm$, respectively. None of the implants with platform diameters larger than the body diameter failed. Conclusion: In conclusion, the findings of present study suggest that narrow diameter implant could be predictable treatment in narrow alveolar ridge.

THE EFFECT OF FRUCTOSE ON THE METABOLISM OF SUCROSE BY STREPTOCOCCUS MUTANS (Streptococcus mutans의 자당 대사에 미치는 과당의 영향)

  • Statement of problem: Streptococcus produces energy and forms extracellular polysaccharides by metabolizing sucrose. Insoluble glucan, a kind of extracellular polysaccharide, is the important material of dental plaque. Fructose affects the metabolism of sucrose. Purpose: The purpose of this study was to evaluate the effect of fructose on the metabolism of sucrose in Streptococcus mutans. Materials and methods: To determine the effect of fructose on the formation of artificial plaque by Streptococcus mutans Ingbritt, S. mutans and fructose were placed in beakers containing M17 broth and sucrose. The wires were hung on frameworks inserted into cork stoppers, and then immersed in each of the beakers. After the incubation with gentle shaking, each wire was weighed. To analyze the effect of fructose on the sucrose metabolism by S. mutans or glucosyltransferase, S. mutans and fructose were placed in M17 broth containing sucrose. After the incubation. the remaining sucrose and polymers were analysed by thin layer chromatography. Results: The following results were obtained; 1. When Streptococcus mutans was cultured in the media containing 3% sucrose for 8 hours, the mean weight of formed artificial plaque on the wires was $124.3{\pm}3.0mg$, whereas being reduced to $20.7{\pm}10.2mg$ in the media added with 3% sucrose and 4% fructose(p<0.05) 2. When the control containing glucose was added with sucrose, the optical density of Streptococcus mutans solution cultured for 24 hours was not increased compared with the control, while being increased by adding with fructose. 3. When Streptococcus mutans was incubated in the media added with sucrose and fructose for 8 hours, the number of viable cells was increased compared with the media added with sucrose. 4. The amount of remained sucrose was increased in Streptococcus mutans culture supernatant of media added with sucrose and fructose than with sucrose only. but the amount of produced insoluble glucan was decreased. 5. The amounts of remained sucrose and produced soluble glucan were increased in the culture of glucosyltransferase-contained media added with sucrose and fructose than with sucrose only, but the amount of produced insoluble glucan was decreased . Conclusion: These results indicated that the sucrose metabolism and the production of insoluble glucan were inhibited in Streptococcus mutans by adding fructose in the media containing sucrose.

A study of incidence of palatal side abfractions in maxillary posterior teeth (상악 구치에서의 구개측 굴곡파절 발현 비율 연구)

  • Purpose: Most of studies dealing with abfractions are limited to the buccal surfaces of the teeth. In this study, we analyzed the cause for abfraction by investigating the incidence of palatal side abfractions in maxillary posterior teeth. Materials and Methods: We investigated a total of 3193 maxillary posterior teeth by an intraoral examination, model observation, and observation of virtual model fabricated using model scanning. We recorded the results and classified them depending on the type of teeth, age, gender, and side of arches. We also performed Chi-square test to evaluate the statistical significance among the groups (α = 0.05). Results: The incidence of palatal side abfraction of the maxillary molars (10.8%) was higher than the premolars (6.8%), and among them, the incidence of the 1st molars (39.1%) were the highest. The incidence of palatal side abfraction increased with age and was statistically significant (P < 0.05). There was no statistical significance in the difference by gender (P > 0.05); in the case of arches, left arch showed higher incidence and it was statistically significant (P < 0.05). Conclusion: Palatal side abfraction in maxillary posterior teeth was frequently observed in the maxillary 1st molars, and the incidence increased with age. This result suggests that the main reason for abfraction is due to occlusal force.

  • Purpose: This study was to determine the possible effects of trichloroacetic acid (TCA) and epidermal growth factor (EGF) through cell survival genes of the PI3K-AKT signaling pathway when applying an hydrophobically modified glycol chitosan (HGC)-based nanocontrolled release system to human gingival fibroblasts in oral soft tissue regeneration. Materials and Methods: An HGC-based nano-controlled release system was produced, followed by the loading of TCA and EGF. The group was divided into control (CON), TCA-loaded nano-controlled release system (EXP1), and the TCA- and EGF- individually loaded nano-controlled release system (EXP2). A total for 29 genes related to the PI3K-AKT signaling pathway were analyzed after 48h of culture in human gingival fibroblasts. Real-time PCR, 1- way ANOVA and multiple regression analysis were performed. Results: Cell survival genes were significantly upregulated in EXP1 and EXP2. From multiple regression analysis, ITGB1 was determined to be the most influential factor for AKT1 expression. Conclusion: The application of TCA and EGF through the HGC-based nano-controlled release system can up-regulate the cell survival pathway.

REAL-TIME MEASUREMENT OF DENTINAL TUBULAR FLUID FLOW DURING AND AFTER AMALGAM AND COMPOSITE RESTORATIONS (아말감과 복합레진의 수복 과정과 수복 후 발생하는 상아세관액 흐름의 실시간 측정)

  • The aim of this study was to measure the dentinal tubular fluid flow (DFF) during and after amalgam and composite restorations. A newly designed fluid flow measurement instrument was made. A third molar cut at 3 mm apical from the CEJ was connected to the flow measuring device under a hydrostatic pressure of 15 $cmH_2O$. Class I cavity was prepared and restored with either amalgam (Copalite varnish and Bestaloy) or composite (Z-250 with ScotchBond MultiPurpose: MP, Single Bond 2: SB, Clearfil SE Bond: CE and Easy Bond: EB as bonding systems). The DFF was measured from the intact tooth state through restoration procedures to 30 minutes after restoration, and re-measured at 3 and 7days after restoration. Inward fluid flow (IF) during cavity preparation was followed by outward flow (OF) after preparation, In amalgam restoration, the OF changed to IF during amalgam filling and slight OF followed after finishing. In composite restoration, application CE and EB showed a continuous OF and air-dry increased rapidly the OF until light-curing, whereas in MP and SB, rinse and dry caused IF and OF, respectively. Application of hydrophobic bonding resin in MP and CE caused a decrease in flow rate or even slight IF. Light-curing of adhesive and composite showed an abrupt IF. There was no statistically significant difference in the reduction of DFF among the materials at 30 min. 3 and 7 days after restoration (p > 0.05).

Cyclic fatigue resistance, torsional resistance, and metallurgical characteristics of M3 Rotary and M3 Pro Gold NiTi files

  • Objectives: To evaluate the mechanical properties and metallurgical characteristics of the M3 Rotary and M3 Pro Gold files (United Dental). Materials and Methods: One hundred and sixty new M3 Rotary and M3 Pro Gold files (sizes 20/0.04 and 25/0.04) were used. Torque and angle of rotation at failure (n = 20) were measured according to ISO 3630-1. Cyclic fatigue resistance was tested by measuring the number of cycles to failure in an artificial stainless steel canal ($60^{\circ}$ angle of curvature and a 5-mm radius). The metallurgical characteristics were investigated by differential scanning calorimetry. Data were analyzed using analysis of variance and the Student-Newman-Keuls test. Results: Comparing the same size of the 2 different instruments, cyclic fatigue resistance was significantly higher in the M3 Pro Gold files than in the M3 Rotary files (p < 0.001). No significant difference was observed between the files in the maximum torque load, while a significantly higher angular rotation to fracture was observed for M3 Pro Gold (p < 0.05). In the DSC analysis, the M3 Pro Gold files showed one prominent peak on the heating curve and 2 prominent peaks on the cooling curve. In contrast, the M3 Rotary files showed 1 small peak on the heating curve and 1 small peak on the cooling curve. Conclusions: The M3 Pro Gold files showed greater flexibility and angular rotation than the M3 Rotary files, without decrement of their torque resistance. The superior flexibility of M3 Pro Gold files can be attributed to their martensite phase.

Real-time measurement of dentinal fluid flow during desensitizing agent application (상아질 지각과민 처치제의 적용 중에 일어나는 상아세관액 유동의 실시간 측정)

  • Objectives: The aim of this study was to examine changes in the dentinal fluid flow (DFF) during desensitizing agent application and to compare permeability after application among the agents. Materials and Methods: A Class 5 cavity was prepared to exposure cervical dentin on an extracted human premolar which was connected to a sub-nanoliter fluid flow measuring device (NFMD) under 20 cm water pressure. DFF was measured from before application of desensitizing agent (Seal&Protect, SP; SuperSeal, SS; BisBlock, BB; Gluma desensitizer, GL; Bi-Fluoride 12, BF) through application procedure to 5 min after application. Results: DFF rate after each desensitizing agent application was significantly reduced when compared to initial DFF rate before application (p < 0.05). SP showed a greater reduction in DFF rate than GL and BF did (p < 0.05). SS and BB showed a greater reduction in DFF rate than BF did (p < 0.05). Conclusions: Characteristic DFF aspect of each desensitizing agent was shown in NFMD during the application procedure.

Influence of root canal curvature on the screw-in effect of nickel-titanium rotary files in simulated resin root canal (모형 레진근관에서 근관의 만곡도가 니켈-티타늄 전동 파일의 screw-in effect에 미치는 영향)

  • Objectives: Nickel-titanium (Ni-Ti) rotary instruments have some unexpected disadvantages including the tendency to screw-in to the canal. The purpose of this study was to evaluate the influence of root canal curvatures on the screw-in effect of Ni-Ti rotary files. Materials and Methods: A total of 80 simulated root canals in clear resin blocks were used in the study. Canals with curvature of 0, 10, 20 and 30 degrees were instrumented with ProTaper instruments SX, S1, S2 and a ProFile of #25/0.06 to 1.0-2.0 mm beyond the initial point of root curvature. The screw-in force was measured with a specially designed device while canal was instrumented with a ProFile of #30/0.06 at a constant speed of 300 rpm. The data were subjected to one-way ANOVA and Scheffe multiple range test for post-hoc test. Results: Larger degree of canal curvature generated significantly lesser screw-in forces in all groups (p < 0.001). Conclusions: More attention needs to be paid when using rotary instruments in canals with less curvature than canals with more curvatures to prevent or reduce any accidental overinstrumentation.

The evaluation of surface roughness and polishing time between polishing systems (연마시스템에 따른 복합레진의 표면거칠기와 연마시간에 대한 평가)

  • Objectives: The purpose of this experiment was to evaluate four different polishing systems of their polishability and polishing time. Materials and Methods: 4 mm diameter and 2 mm thickness Teflon mold was made. Z-250 (3M ESPE) hybrid composite resin was slightly overfilled and pressed with slide glass and cured with Optilux 501 for 40 sec each side. Then the surface roughness (glass pressed: control group) was measured with profilometer. One surface of the specimen was roughened by #320 grit sand paper and polished with one of the following polishing systems; Sof-Lex (3M ESPE), Jiffy (Ultradent), Enhance (Dentsply/Caulk), or Pogo (Dentsply/Caulk). The surface roughness and the total polishing time were measured. The results were analyzed with one-way ANOVA and Duncan's multiple range test. Results: The surface roughness was lowest in Pogo, and highest in Sof-Lex. Polishing times were shortest with Pogo, and followed by the Sof-Lex, Enhance and Jiffy. Conclusions: One-step polishing system (Pogo) is very effective to get the smooth surface in a short time, therefore it can be recommended for final polishing system of the restoration.

Study of endodontic working length of Korean posterior teeth (한국인의 구치부 근관작업장에 관한 연구)

  • Objectives: The aim of this study was to investigate average working lengths of Korean posterior teeth and evaluate validity of endodontic file length. Materials and Methods: The endodontic working length of the posterior teeth of 670 Korean patients were measured than each mean value and standard deviation were investigated than the frequency deviation and standard deviation per each length were calculated. Results: Among the canals of premolar, 66.5% of canal length was marked under 20 mm by endodontic working length and 95.4% could be measured under 22 mm and Among the canals of molars, 95.5% of canal length was marked under 20 mm endodontic working length. Conclusions: With the result of measurement of endodontic working length of premolars of Korean, it suggested that 23 mm endodontic file is more proper than the 21 mm and 25 mm file on the market.
