Evaluation of the reproducibility of various abutments using a blue light model scanner

  • PURPOSE. To evaluate the reproducibility of scan-based abutments using a blue light model scanner. MATERIALS AND METHODS. A wax cast abutment die was fabricated, and a silicone impression was prepared using a silicone material. Nine study dies were constructed using the prepared duplicable silicone, and the first was used as a reference. These dies were classified into three groups and scanned using a blue light model scanner. The first three-dimensional (3D) data set was obtained by scanning eight dies separately in the first group. The second 3D data set was acquired when four dies were placed together in the scanner and scanned twice in the second group. Finally, the third 3D data set was obtained when eight dies were placed together in the scanner and scanned once. These data were then used to define the data value using third-dimension software. All the data were then analyzed using the non-parametric Kruskal-Wallis H test (${\alpha}=.05$) and the post-hoc Mann-Whitney U-test with Bonferroni's correction (${\alpha}=.017$). RESULTS. The means and standard deviations of the eight dies together were larger than those of the four dies together and of the individual die. Moreover, significant differences were observed among the three groups (P<.05). CONCLUSION. With larger numbers of abutments scanned together, the scan becomes more inaccurate and loses reproducibility. Therefore, scans of smaller numbers of abutments are recommended to ensure better results.

Ceramic molar crown reproducibility by digital workflow manufacturing: An in vitro study

  • PURPOSE. This in vitro study aimed to analyze and compare the reproducibility of zirconia and lithium disilicate crowns manufactured by digital workflow. MATERIALS AND METHODS. A typodont model with a prepped upper first molar was set in a phantom head, and a digital impression was obtained with a video intraoral scanner (CEREC Omnicam; Sirona GmbH), from which a single crown was designed and manufactured with CAD/CAM into a zirconia crown and lithium disilicate crown (n=12). Reproducibility of each crown was quantitatively retrieved by superimposing the digitized data of the crown in 3D inspection software, and differences were graphically mapped in color. Areas with large differences were analyzed with digital microscopy. Mean quadratic deviations (RMS) quantitatively obtained from each ceramic group were statistically analyzed with Student's t-test (${\alpha}=.05$). RESULTS. The RMS value of lithium disilicate crown was $29.2\;(4.1){\mu}m$ and $17.6\;(5.5){\mu}m$ on the outer and inner surfaces, respectively, whereas these values were $18.6\;(2.0){\mu}m$ and $20.6\;(5.1){\mu}m$ for the zirconia crown. Reproducibility of zirconia and lithium disilicate crowns had a statistically significant difference only on the outer surface (P<.001). The outer surface of lithium disilicate crown showed over-contouring on the buccal surface and under-contouring on the inner occlusal surface. The outer surface of zirconia crown showed both over- and under-contouring on the buccal surface, and the inner surface showed under-contouring in the marginal areas. CONCLUSION. Restoration manufacturing by digital workflow will enhance the reproducibility of zirconia single crowns more than that of lithium disilicate single crowns.

세 가지 방식의 스캐너 종류에 따른 모형 정확도 평가 (Evaluation of the Model Accuracy according to Three Types of Dental Scanner)

  • 본 연구는 세 가지 종류 스캐너에 따른 정확도를 비교 평가하기 위하여 시행되었다. 경석고 모형과 스캐너로 채득한 디지털 모형에 각 4지점씩 계측지점을 선정하였고, 이들 4지점사이를 DVC와 디지털 모형 측정 프로그램을 이용하여 길이를 측정하고, 다음과 같은 결론을 도출하였다. 세 가지 종류의 스캐너 가운데 레이저 스캐너가 백색광 스캐너나 청색광 스캐너보다 높은 정확도를 나타내었으나 이 세 가지 종류의 스캐너 모두가 임상적으로 사용하는 것이 가능하다.

  • Purpose : 1. Retrospectively evaluate the accuracy of tentative diagnosis or impression from the clinico-radio-graphic materials of jaw lesions which showed mixed lesions associated with teeth. 2. To observe the diagnostic importance of the calcified part of the lesions which appear as radiopaque areas. Materials and Methods: 14 cases of jaw lesions which showed mixed lesions associated with teeth were reviewed. These lesions were mostly diagnosed as adenomatoid odontogenic tumors (6 cases) or calcifying odontogenic cysts with (4 cases) or without odontomas (4 cases). The calcified elements of the lesions which demonstrated various sizes and patterns of radiopaque shadows resembled odontoid tissues in some cases but could not be defined in some other cases radiographically. Results : The final histopathologic diagnosis confirmed adenomatoid odontogenic tumors in 4 of the 6 cases. The remaining 2 cases turned out to be odontoma and ameloblastic fibroodontoma. The 4 cases of calcifying odontogenic cysts with odontomas were correct in 3 cases but remaining 1 case was just odontoma. The 4 cases of calcifying odontogenic cysts were proved to be odontogenic keratocyst, calcified peripheral fibroma, unicystic ameloblastoma and squamous cell carcinoma. Conclusion : The diagnostic accuracy of the adenomatoid odontogenic tumors and calcifying odontogenic cysts were high when the lesions show typical appearance. The calcifications which show radiopaque areas could be odontomas or dystrophic calficifations or remnants of bone fragments from resorption.

  • All patients who will Undergo irraidiation of the oral cavity cancer will need dental before and during Radiotherapy. The extent of the region and the presence of numerous critical normal tissues(mucosa, gingiva, teeth and the alveolar ridge, alveolar bony structure, etc) in the oral cavity area, injury to which could result in serious functional impairment. 치과용 스캐너 평가를 위한 국제표준모델의 재료 및 표면 상태에 따른 스캔 영상 결과물 비교 연구 (Comparative study on quality of scanned images from varying materials and surface conditions of standardized model for dental scanner evaluation)

  • 목적: 본 연구는 현재 시판되고 있는 구강스캐너를 사용하여 구강스캐너 정확도 평가를 위해 국제표준이 제안하고 있는 모형의 스캔이미지 획득이 가능한 지 분석하고, 이를 통해 표준모델이 가지고 있는 문제점을 파악하는 데 있다. 연구 재료 및 방법: ISO12836과 ANSI/ADA no.132에서 규정하는 국제표준을 참고하여 3D 프린터기를 이용하여 모델을 제작하였으며, 모델스캐너와 두 가지 구강스캐너를 이용하여 스캔을 하였다. 스캔이미지 획득 정도를 3등급으로 분류하여 스캐너의 성능을 비교하였으며, 모델 표면의 상태에 따른 이미지 획득 능력도 비교하였다. 결과: 모델 스캐너가 모든 모델에서 가장 우수한 이미지를 얻을 수 있었으며 TRIOS3는 둥근 형태의 구조물, CS3500은 각진 형태의 구조물에 대한 이미지 재현이 좋은 결과를 보였다. 표준 모델의 표면상태에 따른 스캔이미지 재현에서는 초경석고 모델이 스캐너 종류와 관계없이 가장 우수하였다. 3D 프린팅 모델의 경우, 표면에 파우더 처리를 한 모델에서 가장 우수한 스캔이미지를 얻을 수 있었다. 결론: ISO12836과 ANSI/ADA 132의 표준모델의 경우, 구강스캐너의 field of view (FOV)를 벗어나는 구조물을 스캔할 때 서로 다른 면인 것을 구분하는 기준점이 존재하지 않게 되면 연속적인 스캔 및 정합과정에서 정확한 이미지를 나타내지 못한다는 것을 알 수 있었다. 그러므로 단순한 패턴의 반복과 대칭구조를 가지지 않는 새로운 표준모델이 필요하다고 여겨진다.

Evaluation of internal fit of interim crown fabricated with CAD/CAM milling and 3D printing system

  • PURPOSE. This study is to evaluate the internal fit of the crown manufactured by CAD/CAM milling method and 3D printing method. MATERIALS AND METHODS. The master model was fabricated with stainless steel by using CNC machine and the work model was created from the vinyl-polysiloxane impression. After scanning the working model, the design software is used to design the crown. The saved STL file is used on the CAD/CAM milling method and two types of 3D printing method to produce 10 interim crowns per group. Internal discrepancy measurement uses the silicon replica method and the measured data are analyzed with One-way ANOVA to verify the statistic significance. RESULTS. The discrepancy means (standard deviation) of the 3 groups are $171.6\;(97.4){\mu}m$ for the crown manufactured by the milling system and 149.1 (65.9) and $91.1\;(36.4){\mu}m$, respectively, for the crowns manufactured with the two types of 3D printing system. There was a statistically significant difference and the 3D printing system group showed more outstanding value than the milling system group. CONCLUSION. The marginal and internal fit of the interim restoration has more outstanding 3D printing method than the CAD/CAM milling method. Therefore, the 3D printing method is considered as applicable for not only the interim restoration production, but also in the dental prosthesis production with a higher level of completion.

A new method to measure the accuracy of intraoral scanners along the complete dental arch: A pilot study

  • PURPOSE. The purpose of this study is to assess the accuracy of three intraoral scanners along the complete dental arch and evaluate the feasibility of the assessment methodology for further in vivo analysis. MATERIALS AND METHODS. A specific measurement pattern was fabricated and measured using a coordinate measuring machine for the assessment of control distances and angles. Afterwards, the pattern was placed and fixed in replica of an upper jaw for their subsequent scans (10 times) using 3 intraoral scanners, namely iTero Element1, Trios 3, and True Definition. 4 reference distances and 5 angles were measured and compared with the controls. Trueness and precision were assessed for each IOS: trueness, as the deviation of the measures from the control ones, while precision, as the dispersion of measurements in each reference parameter. These measurements were carried out using software for analyzing 3-dimensional data. Data analysis software was used for statistical and measurements analysis (α=.05). RESULTS. Significant differences (P<.05) were found depending on the intraoral scanner used. Best trueness values were achieved with iTero Element1 (mean from 10 ± 7 ㎛ to 91 ± 63 ㎛) while the worst values were obtained with Trios3 (mean from 42 ± 23 ㎛ to 174 ± 77 ㎛). Trueness analysis in angle measurements, as well as precision analysis, did not show conclusive results. CONCLUSION. iTero Element1 was more accurate than the current versions of Trios3 and True Definition. Importantly, the proposed methodology is considered reliable for analyzing accuracy in any dental arch length and valid for assessing both trueness and precision in an in vivo study.

In-vitro evaluation of marginal and internal fit of 3-unit monolithic zirconia restorations fabricated using digital scanning technologies

  • PURPOSE. This study aimed to compare the marginal and internal fit of 3-unit monolithic zirconia restorations that were designed by using the data obtained with the aid of intraoral and laboratory scanners. MATERIALS AND METHODS. For the fabrication of 3-unit monolithic zirconia restorations using impressions taken from the maxillary master cast, plaster cast was created and scanned in laboratory scanners (InEos X5 and D900L). The main cast was also scanned with different intraoral scanners (Omnicam [OMNI], Primescan [PS], Trios 3 [T3], Trios 4 [T4]) (n = 12 per group). Zirconia fixed partial dentures were virtually designed, produced from presintered block, and subsequently sintered. Marginal and internal discrepancy values (in ㎛) were measured by using silicone replica method under stereomicroscope. Data were statistically analyzed by using 1-way ANOVA and Kruskal Wallis tests (P<.05). RESULTS. In terms of marginal adaptation, the measurements on the canine tooth indicated better performance with intraoral scanners than those in laboratory scanners, but there was no difference among intraoral scanners (P<.05). In the premolar tooth, PS had the lowest marginal (86.9 ± 19.2 ㎛) and axial (92.4 ± 14.8 ㎛), and T4 had the lowest axio-occlusal (89.4 ± 15.6 ㎛) and occlusal (89.1 ± 13.9 ㎛) discrepancy value. In both canine and premolar teeth, the D900L was found to be the most marginally and internally inconsistent scanner. CONCLUSION. Within the limits of the study, marginal and internal discrepancy values were generally lower in intraoral scanners than in laboratory scanners. Marginal discrepancy values of scanners were clinically acceptable (< 120 ㎛), except D900L.

Evaluation of marginal and internal gaps of Ni-Cr and Co-Cr alloy copings manufactured by microstereolithography

  • PURPOSE. The purpose of this study was to evaluate the marginal and internal gaps of Ni-Cr and Co-Cr copings, fabricated using the dental ${\mu}-SLA$ system. MATERIALS AND METHODS. Ten study dies were made using a two-step silicone impression with a dental stone (type IV) from the master die of a tooth. Ni-Cr (NC group) and Co-Cr (CC group) alloy copings were designed using a dental scanner, CAD software, resin coping, and casting process. In addition, 10 Ni-Cr alloy copings were manufactured using the lost-wax technique (LW group). The marginal and internal gaps in the 3 groups were measured using a digital microscope ($160{\times}$) with the silicone replica technique, and the obtained data were analyzed using the non-parametric Kruskal-Wallis H test. Post-hoc comparisons were performed using Bonferroni-corrected Mann-Whitney U tests (${\alpha}=.05$). RESULTS. The mean (${\pm}$ standard deviation) values of the marginal, chamfer, axial wall, and occlusal gaps in the 3 groups were as follows: $81.5{\pm}73.8$, $98.1{\pm}76.1$, $87.1{\pm}44.8$, and $146.8{\pm}78.7{\mu}m$ in the LW group; $76.8{\pm}48.0$, $141.7{\pm}57.1$, $80.7{\pm}47.5$, and $194.69{\pm}63.8{\mu}m$ in the NC group; and $124.2{\pm}52.0$, $199.5{\pm}71.0$, $67.1{\pm}37.6$, and $244.5{\pm}58.9{\mu}m$ in the CC group. CONCLUSION. The marginal gap in the LW and NC groups were clinically acceptable. Further improvement is needed for CC group to be used clinical practice.
