ACTIVATION OF H-RAS ONCOGENE IN RAT SALIVARY GLAND TUMORS INDUCED BY DMBA AND IRRADIATION (DMBA 매식과 방사선 조사로 유도된 백서 타액선 종양에서 H-ras 암유전자의 활성화)

  • Cellular transforming genes have been identified in a number of different tumor cell lines and tumor types. A significant number of these oncogenes belong to the ras gene family. The ras gene family consists of three closely related genes:H-ras, K-ras and N-ras which code for a related 21 kDa protein. Mutations in codon 12, 13 and 61 of one of the three ras genes convert these genes into acute oncogenes. The presence of H-ras gene mutations has important prognostic implications in various tumors. Each genomic DNA was isolated from tumors induced by implantation with DMBA, or by treatment with DMBA -implantation/irradiation. When genome DNA was transfected into NIH 3T3 cells and investigated by two-step PCR-RFLP, the fOllowing results were concluded: 1. Transformation foci developed in two groups when the genome DNA of two experimental groups were transfected into NIH 3T3 cells. 2. Transformation efficiency was 0.01-0.02 foci/㎍DNA in the experimental group with the DMBA-implantation, 0.01-0.03 foci/㎍lgDNA in the experimental group with the DMBA-implantation/irradiation according to results of transfection assay. 3. When the point mutation of H-ras gene was investigated by a two-step PCR-RFLP, there was 13.9% (5/36) in the experimental group with the DMBA implantation, 15.4 % (6/39) in the experimental group with the DMBA -implantation/irradiation. 4. The point mutation in codon 12 and 61 of H-ras was 5.6%(2/36) and 8.3%(3/36) in the experimental group with the DMBA implantation. 5. The point mutation in codon 12 and 61 of H-ras gene was 7.7%(3/39) in the experimental group with the DMBA -implantation/irradiation.

  • As popularity of dental implantation is increasing, the number of cases associated with complications also increase. Evaluation on diabetes mellitus is often neglected due to the disease's irrelevance to implantability. However, patients with diabetes mellitus are susceptible to infection due to impaired bactericidal ability of neutrophils, cellular immunity and activity of complements. Due to this established connection between diabetes mellitus and infection, a couple of cases were selected to present patients with diabetes mellitus with glycemic incontrollability, suffering from post-implantation dentigerous inter-fascial space abscess.

  • In the recent studies, many authors have reported that the success rate of immediate implantation has no difference compared to conventional staged implantation. Although the immediate implantation has many advantages over conventional approach, many clinicians don't seem to practice because they think that most of the extraction sockets will have some bony gab with defects around implant and that this situation makes the result of immediate implantation unpredictable. We clinically analyzed 23 implanted sites of 18 patients treated with immediate implantation in our hospital from September 2003 to January 2004. The $ITI^{(R)}$. dental implant system was used and GBR procedure with $Bio-Oss^{(R)}$. and $Bio-Gide^{(R)}$. was done simultaneously. The pre & post-op. measurements were recorded such as alveolar crest-adjacent tooth CEJ distance, gingival crest-adjacent tooth CEJ distance, existence of periapical lesion, vertical defect around the extraction socket, horizontal defect around the extraction socket, probing depth, radiologic change of alveolar crest height. We report a positive outcome about immediate implantation with review of literatures.

Scintigraphic Evaluation of Alveolar Bone Following Dental Implantation in Dogs (개에서 치아 임플란트 식립후 치조골의 신티그라피 평가)

  • This study aimed to determine of osseointegration following dental implantation in the dog mandible using bone scintigraphy. Five mongrel dogs, weighing approximately 8.5 kg and averaging 1.8 years of age, without active periodontal disease were used. During the entire study period, all dogs were fed a soft commercial diet and water ad libitum to minimize functional loading of the implant. Titanium alloy implant systems 10 mm in length and 4 mm in diameter were chosen for insertion. Twelve weeks prior to implantation, the second and third left mandibular premolars in each dog were extracted for the dental implant insertion. Before the dental implantation procedures and 0, 4, 8, and 12 weeks after the insertions, clinical observation, radiography and bone scintigraphy were conducted. The scintigraphy was obtained using a large field of view gamma camera equipped with a paralled-hole, low-energy collimator about 3 hours after intravenous injection of Tc-99m-MDP (8 mCi/dog) to the dogs. There were not inflammation sign after insertion of dental implants on the mandible in dogs. Implants were slightly movable at the first and fourth weeks, and there was no mobility after 8 weeks. Twelve weeks after dental implantation, the bone uptake scintigraphy of peri-implant bone was similar to that of normal alveolar bone, indicating that peri-implant bone was completely regenerated by new bone. In conclusion, we recommend stable implant fixation with alveolar bone for the accurate and safe repair of teeth loot due to decacy, trauma or peridontal disease. Titanium alloy implants were optimal due to their biocompatibility.

  • The restorative treatment using titanium implants has become a standard procedures in everyday clinical practice. However, there are several concerns about the use of titanium as a materials. Above all, the grayish metal color provokes esthetic problems when the soft tissue around the implants is not optimal. Recently, zirconia has been emerging as an alternative to titanium due to its white color and superior physicochemical properties. In this paper, the current findings about zirconia implants was reviewed based on the results from animal and clinical experiments. In addition, the immediate implantation protocols using the natural tooth form zirconia implants was introduced as a novel procedures and future perspectives were discussed.

CLINICAL EVALUATION OF DELAYED VERSUS SIMULTANEOUS IMPLANTATION WITH SINUS LIFTING (상악동거상술시 임프란트의 지연식립과 즉시식립에 대한 임상적 연구)

  • Purpose: Simultaneous implantation has been traditionally limited to patients with at least 5 mm of residual bone to ensure that the implant is completely stabilized. This is considered to be one of the most critical factors for primary implant stability and parallelism and, subsequently osseointegration. Recently, improved initial implant stability is provided by advancement of surgical techniques, implant designs and surface treatments. This has led surgeons to extending simultaneous treatment option to patients with below 4 mm of residual bone height, despite the lack of much data. The purpose of this study was to retrospectively evaluate and compare the status of implants which are installed by delayed and simultaneous methods. Material and methods: The subjects were patients(121 patients, 278 implants) who had been operated with sinus lifting from 2003 to 2007 in Sun Dental Hospital. Lateral window approach with autograft and xenograft(1:1 ratio) were carried out for all sinus lifting. 4 types of implant were used. The mean follow up period was 26.3 months(19 - 58 months) in delayed group, and 22.8 months(18 - 43 months) in simultaneous group. Results: The survival rate of implant restoration of this study was 98.2% in delayed group and 91.7% in simultaneous group. In simultaneous group, wide-platform type implants showed 100% survival rate. The total average of marginal bone loss in radiographs was $0.96\;{\pm}\;0.29\;mm$ in delayed group and $1.02\;{\pm}\;0.31\;mm$ in simultaneous group. Conclusion: Simultaneous implantation with sinus lifting(below 4mm of residual bone height) could be predictable treatment.

  • Tooth autotransplantation using socket lifting like this case had been seldom reported. So this report demonstrates a case using this technique which is frequently used for maxillary posterior implantation. Clinical considerations and pitfalls will be discussed.

  • Purpose. This multicenter retrospective study was performed to evaluate the survival and success rates of immediate placement of USII and SSII Osstem implant (Osstem implant Co, Korea) on the maxillary and mandibular anterior and premolar areas. Materials and methods. Dental records were obtained in 37 patients who were treated with immediate implantation on the maxillary and mandibular anterior and premolar areas in 6 different clinics. The 98 implants were evaluated both clinically and radiographically using predefined success criteria. Results. There was no failed implant in all patients. The mean follow up period was 24.7 months (ranged from 12 to 58 months), and 25.1 months (ranged from 16 to 35 months) in USII and SSII implants, respectively. The crestal bone loss was 3 mm in 3 USII implants during 41 months, and in 1 SSII implant during 22 months. The overall success rate was 94.2% and 97.7% in USII and SSII implants, respectively. The age, gender, diameter, or length of implants, and type of surgery were not influenced to the success rate of immediate implantation. Conclusion. These results suggest that USII and SSII Osstem implant can be used successfully in immediate implantation on the maxillary and mandibular anterior and premolar areas.

COMPARATIVE STUDY ON THE MARGINAL BONE LOSS OF IMMEDIATE NONSUBMERGED AND SUBMERGED ENDOSSEOUS DENTAL IMPLANTS PLACED INTO EXTRACTION SOCKETS OF DOGS (발치후 즉시 매식한 Nonsubmerged와 Submerged 임플랜트의 변연골 흡수양상에 관한 비교연구)

  • The conventional osseointegration protocol calls for waiting up to 12 months for ossification of an extraction socket to heal before placing an endosseous implant. In this study, the possibility of placing a pure titanium implant directly into an extraction socket immediately after extraction was investigated. And the marginal bone loss of immediate nonsubmerged and submerged endosseous dental implants placed into extraction sockets was also compared. Pure titanium Nobelpharma Branemark implants and solid screw type ITI implants were placed into premolar extraction sockets of two adult dogs and allowed to heal for a period of 3 months, followed by functional loading of the implant. Radiographic examination was performed before implantation, immediately after implantation and 3, 6, 9, 12 months after implantation. The results obtained were as follows : 1. Immediately placed nonsubmerged ITI implants and submerged Branemark implants showed favorable radiographic osseointegration status and there were minimum marginal bone loss. 2. There were no significant differences in radiographic finding of osseointegration between conventional and immediate implantation. 3. Gingival tissue around implants showed more inflammatory signs than that of adjacent natural teeth. This study suggest that pure titanium Branemark implants and submerged ITI implants have the potential to integrate when placed immediatly after extraction of the teeth and warrants further investigation.

  • Immediate implantation is widely applied as it is recognized as a useful implant treatment in dental clinic due to its various advantages as well as being able to shorten the treatment period. Immediate implant treatment was performed on 5 patients who needed extraction of the mandibular first molar for various reasons, shortening the overall treatment period, and obtaining satisfactory results. Immediate implantation after extraction is a high-level treatment that requires proper initial fixation at the exact location of the extraction socket and the surrounding soft tissues, but is considered to be a clinically very useful prosthetic restoration method
