Objective: In spite of increasing dental treatment for the disabled, there was a few collected data on dental treatment status and dentists' perception in Korea. The purpose of this' study is to research the current dental treatment status for the disabled in Korea and to suggest public health policies for the disabled. Material and methods: Total of 68 dentists who have treated the disabled regularly were asked to fill questionnaires regarding dental treatments for the disabled. Results: 1. It is necessary to set up public dental center for the disabled in rural area. 2. In case of dental treatments relating to cerebral palsy, ADHD, autism, the compensation for treatment time and extra investment is needed. 3. Only 39.7% of dentists charged treatment fees to the disabled as same as normal patients. Moreover 38.2% of dentists answered that they didn't charge treatment fees at all when their patient are disabled. 4.23.5% of dentists provided all of dental treatments including dental implant and orthodontic appliances in Korea. However, 54.3% of dentists reported the improvement of access to dental implant, orthodontic and esthetic treatment is needed. 5. It is imperative for the government to improve the support system for the disabled. Conclusions: Through this research only 26.5% of dentists answered that they had received the fund from the government and 11.1% of them were satisfied with the support for the disabled in Korea. This indicated the improvement of government financial support system for the disabled is needed.

Purpose: This study compared the effects of coating implants with hydroxyapatite (HA) using an ion beam-assisted deposition (IBAD) method prepared with machined, anodized, sandblasted and large-grit acid etched (SLA) surfaces in minipigs, and verified the excellency of coating method with HA using IBAD. Material and Methods: 4 male Minipigs(Prestige World Genetics, Korea), 18 to 24 months old and weighing approximately 35 to 40 kg, were chosen. All premolars and first molars of the maxilla were carefully extracted on each side. The implants were placed on the right side after an 8 week healing period. The implant stability was assessed by resonance frequency analysis (RFA) at the time of placement. 40 implants were divided into 5 groups; machined, anodized, anodized plus IBAD, SLA, and SLA plus IBAD surface implants. 4 weeks after implantation on the right side, the same surface implants were placed on the left side. After 4 weeks of healing, the minipigs were sacrificed and the implants were analyzed by RFA, histology and histometric. Results: RFA showed a mean implant stability quotient (ISQ) of $75.625{\pm}5.021$, $76.125{\pm}3.739$ ISQ and $77.941{\pm}2.947$ at placement, after 4 weeks healing and after 8 weeks, respectively. Histological analysis of the implants demonstrated newly formed, compact, mature cortical bone with a nearby marrow spaces. HA coating was not separated from the HA coated implant surfaces using IBAD. In particular, the SLA implants coated with HA using IBAD showed better contact osteogenesis. Statistical and histometric analysis showed no significant differences in the bone to implant contact and bone density among 5 tested surfaces. Conclusion: We can conclude that rough surface implants coated with HA by IBAD are more biocompatible, and clinical, histological, and histometric analysis showed no differences when compared with the other established implant surfaces in normal bone.
The purpose of this study was to evaluate the effect of demineralized freeze dried bone and demineralized bone gel with guided tissue regeneration treatment around titanium implants with dehisced bony defects and also evaluate space maintaining capacity of demineralized bone gel type and DFDB powder type under e-PTFE membrane. In 3 Beagle dogs, mandibular premolar was extracted and four peri-implant osteotomies were formed for dehiscence. After insertion of implants, the four peri-implant defects were treated as follows. 1) In control group. no graft material and barrier membrane were applied. 2) In experimental group.1, the site was covered only with the e-PTFE membrane. 3) In experimental group 2,received DFDB powder and covered by the e-PTFE membrane. 4) In experimental group 3, demineralized bone gel and e-PTFE membrane were used. By random selection, animals were sacrificed at 4, 8, 12 weeks. The block sectioned specimens were prepared for decalcified histologic evaluation(hematoxylin and eosin staining) and undecalcified histologic evahiation(Von Kossa's and toluidine blue staining) with light microscopy. The results of this study were as follows. 1) In control group, there was a little new bone formation and connective tissue was completely filled in the defect area. 2) Experimental group 1 showed lesser quantity of bone formation as compared to the bone grafted group. Thin vertical growth of new bone formation around implant fixture was shown. 3) Experimental group 2 showed thick bucco-lingual growth of new bone formation and grafted bone particles were almost resorbed in 12 week group. 4) In experimental group 3, most grafted bone particles were not resorbed in 12 week group and thick bucco-lingual bone formation was shown in dehisced defect base area. 5) There was no remarkable differences in space making capacity and new bone formation procedure between demineralized freeze-dried bone powder type and demineralized bone gel type.

Purpose: Resonance frequency analysis (RFA) , a non-invasive technique for the clinical measurement of implant stability, was investigated. Peri-implant bony defect may contribute to implant failure. This in vitro study evaluated the resonance frequencies according to various bony defects and determined whether the directional bone defect can affect the value of frequency analysis. Material and Method: Fifteen 3.75 mm in diameter and 10 mm in length, machined self-tapping implant future were used. Twelve types of bone defects that have different horizontal and vertical dimensions were simulated. Embedded implants were attached to the dental surveyor. Then, the transducer was connected with the implant fixture and the ISQ value was measured at four different directions. Two-way analysis of variance and post hoc $Sch\`{e}ffe'$ test were performed at the 95% significance level. Results: The control group showed the highest ISQ value and 5 thread-$360^{\circ}$ group had the lowest one. As the vertical exposure of implants in each angle was increased, the ISQ value was decreased. Although the horizontal exposure in each thread was increased, the ISQ value was not significantly decreased. Conclusion : Although the simulated defect type was different from each other, the ISQ value was similar among groups.
In the present study, a simple method was successfully used for hydroxyapatite coatings on Ti-6Al-4V substrates deposited by using a sol-gel derived precursor. Prior to hydroxyapatite coating the samples were micropolished (0.1 micron) and divided into three sets. The first set, were the micropolished samples kept as such. The second set were coated with titania sol and the third set was treated with 5M NaOH. After three repetitions of hydroxyapatite coating procedures on each set and heat treatment at 600 $^{\circ}\Delta C$, the formation of hydroxyapatite has been confirmed by XRD analyses and the substrate material was found to be oxidized with negligible amount of CaO in the coating. The SEM studies revealed surface morphology. Hydroxyapatite, calcined at 600$^{\circ}\Delta C$, displaying a porous structure arisen from heating of the bulk. But, it is very meaningful in trying to approach morale management plans with an object of dental technicians. It is necessary that dental technicians should make efforts to control themselves.
Statement of problem: Br${\aa}$nemark or ITI are two currently most widely used implant systems but with contrasting design, surgical and restoration methods. Purpose: The purpose of this study was to compare changes and its statistical significance in bone height and shape which may rise due to the differences between two implant systems. Also to analyse the effect of inter-implant distance on annual bone height changes. Material & Method: Those patients who were treated with two or more of either Br${\aa}$nemark or ITI implants at posterior mandibular area at Yonsei University Dental Hospital, Implant Clinic were selected. At annual examination appointments, standardised radiographs using parallel technique were taken. Marginal bone and inter-implant crestal bone changes were measured and following results were obtained. Results: 1) When ITI and Br${\aa}$nemark system were compared, both annual marginal and inter-implant crestal bone height changes in ITI system in the first two years were smaller than Br${\aa}$nemark and they were statistically significant. On the third year, however, there was no statistical difference between two implant systems on their annual bone level changes (p>0.05). 2) The Marginal and inter-implant crestal bone changes were compared when inter-implant distance was less than 4mm. Statistically significant bone level changes were noted on the first year only for ITI implants but in the first and second year for Br${\aa}$nemark implants (p>0.05). 3) When comparing angulation changes between marginal bone and implant fixture, ITI system had smaller angulation changes but the annual changes were not statistically significant (p>0.05). Conclusion: Within the limitation of this study, it could be concluded that Br${\aa}$nemark implant systems had more changes in marginal and inter-implant crestal bone level in the first and second year after loading with statistical significance. Further studies are recommended to see the effects of these bone loss during the first and second year after loading on the long term prognosis of Br${\aa}$nemark Implants.
Statement of problem: Crestal bone loss, a common problem associated with dental implant, has been attributed to excessive bone stresses. Design of implant's transgingival (TG) part may affect the crestal bone stresses. Purpose: To investigate if concavely designed geometry at a dental implant's TG part reduces peri-implant bone stresses. Material and methods: A total of five differently configured TG parts were compared. Base model was the ITI one piece implant (Straumann, Waldenburg, Switzerland) characterized by straight TG part. Other 4 experimental models, i.e. Model-1 to Model-4, were designed to have concave TG part. Finite element analyses were carried out using an axisymmetric assumption. A vertical load of 50 N or an oblique load of 50 N acting at $30^{\circ}$ with the implant's long axis was applied. For a systematic stress comparison, a total of 19 reference points were defined on nodal points around the implant. The peak crestal bone stress acting at the intersection of implant and crestal bone was estimated using regression analysis from the stress results obtained at 5 reference points defined along the mid plane of the crestal bone. Results: Base Model with straight configuration at the transgingival part created highest stresses on the crestal bone. Stress level was reduced when concavity was imposed. The greater the concavity and the closer the concavity to the crestal bone level, the less the crestal stresses. Conclusion: The transgingival part of dental implant affect the crestal bone stress. And that concavely designed one may be used to reduce bone stress.
Statement of problem. Higher stresses at the cervical bone around dental implants have been seen as a primary cause of the bone resorption at the site. Purpose : To determine the possibility of stress reduction by assembly of different abutment and implant in diameters. Material and methods. Abutments of several different diameters assembled on the top of XiVE$^{(R)}$ implants were axisymmetrically modeled for a series of finite element analyses. Abutments of 3.4, 3.8, 4.5, and 5.5 mm diameters were assumed to be sit on implants of the same or bigger diameters. All the abutments with an exception of 3.4mm dia, are technically possible to be assembled on bigger implants. Main consideration was given to the stresses at the cervical cortical bone induced by loads of parallel to the implant axis. Results and conclusions. 1. Higher stresses were observed at the cervical area of all the models of the same diameters of abutment and future. The peak stresses, which were shown to be a function of the fixture diameter, were from 1-1.85MPa. 2. Difference in the diameters of the abutments and the implants actually reduced the cervical bone stresses. 3. Downsizing of the abutment by one step resulted in 0.1MPa (5%) reduction of the stresses. In light of the relatively lower bone stress, however, this amount of stress reduction was decided to be biomechanically insignificant.
The purpose of this study was to compare the changes of retentive force in different stud attachment systems for implant retained overdenture. Two commercially available attachments with different retentive forces were investigated: Kerator (pink, blue, red) and O-ring (orange, red). Two implant fixtures were vertically embedded in base mountings. Five pairs of each attachment were evaluated. A universal testing machine was used to evaluate the retentive force of two attachments during wear simulation. Surface characteristics of each attachment system were evaluated with scanning electron microscopy. Five pairs of each attachment were evaluated. Kerator pink showed the highest initial retention. After 2,500 cycles of wear-simulation, Kerator pink noted the largest decrease in retention. According to results of surface analysis, Worn surfaces were obtained in matrices and patrices. Heavy wears were observed in matrices. After 2 year-wear simulation, most attachments exhibited retention loss. Attachments using different kind of material exhibited dissimilar surface alterations.

Fracture causes serious problems in many instance of prosthetic failures. But it is hard to find the definite causes when fractures occur. Fractography encompasses the examination of fracture surfaces that contain features resulting from the interaction of the advancing crack with the microstructure of the material and the stress fields. All fractured specimens(implant retaining screw) retrieved from Gangneung-Wonju national university dental hospital for 3 years(from 2007 to 2009). After pretreatment of samples, the scanning electon microscope were used for surface examination and fracture analysis. In case of most of the fractured specimens, fracture took place by fatigue fracture and fractured surface represents fatigue striation. Fatigue striation indicate the progression of the crack front under cyclic loading, are characteristic of stage 2 crack growth. The site of crack initiation and stage 1 crack growth were not easily identified in any of the failure, presumably because of the complex microstructural features of the polycrystalline sample. In case of fractured by overload, dimpled or cleavage surface were observed. Using the interpretation of characteristic markings(ratchet mark, fatigue striation, dimple, cleavage et al) in fracture surfaces, failure events containing the crack origin, crack propagation, material deficiency could be understand. Using the interpretation of characteristic markings in fracture surfaces, cause and mechanism of fractures could be analyzed.
