임플란트와 상부구조물 사이의 micromotion에 관한 연구 (A study on the micromotion between the dental implant and superstructure)

  • Treatment with implants of single tooth missing cases is both functional and esthetic. Although the success rate of single-tooth implant treatments is increasing, sometimes it makes some problems. Problems with single-tooth implant treatments include soft tissue complications, abutment screw fracture, and most commonly, abutment screw loosening, and these involve the instability of the dental implant-superstructure interface. This study investigated and compared dental implant screw joint micromotion of various implant system with external connection or internal connection when tested under simulated clinical loading, Six groups (N=5) were assessed: (1) Branemark AurAdapt (Nobel Biocare, Goteborg, Sweden), (2) Branemark EsthetiCone (Nobel Biocare, Goteborg, Sweden), (3) Neoplant Conical (Neobiotec, Korea), (4) Neoplant UCLA (Neobiotec, Korea), (5) Neoplant 5.5mm Solid (Neobiotec, Korea), and (6) ITI SynOcta (Institute Straumann, Waldenburg, Switzerland). Six identical frameworks were fabricated. Abutment screws were tightened to 32-35 Ncm and occlusal screw were tightened to 15-20 Ncm with an electronic torque controller. A mechanical testing machine applied a compressive cyclic load of 20kg at 10Hz to a contact point on each implant crown. Strain gauge recorded the micromotion of the screw joint interface once a second. Data were selected at 1, 500, 5,000, 10,000, 20,000, 30,000, 40,000 and 50,000 cycle and 2-way ANOVA test was performed to assess the statistical significance. The results of this study were as follows; The micromotion of the implant-superstructure in the interface increased gradually through 50,000 cycles for all implant systems. In the case of the micromotion according to cycle increase, Neoplant Conical and Neoplant UCLA system exhibited significantly increasing micromotion at the implant-superstructure interface (p<0.05), but others not significant. In the case of the micromotion of the implant-superstructure interface at 50,000 cycle, the largest micromotion were recorded in the Branemark EsthetiCone, sequently followed by Neoplant Conical, Neoplant UCLA, Branemark AurAdapt, ITI SynOcta and Neplant Solid. Internal connection system showed smaller micromotion than external connection system. Specially, Neoplant Solid with internal connection system exhibited significantly smaller micromotion than other implant systems except ITI SynOcta with same internal connection system (p<0.05). In the case of external connection, Branemark EsthetiCone and Neoplant Conical system with abutment showed significantly larger micromotion than Branemark AurAdapt without abutment (p<0.05).

수종의 내측연결 임플랜트 시스템에서 고정체-지대주-나사계면의 적합에 관한 연구 (Fit of Fixture/Abutment/Screw Interface of Internal Connection Implant Systems)

  • 이 연구의 목적은 내측연결 임플랜트 시스템의 고정체-지대주-나사간의 계면 적합도를 알아보기 위한 것이다. 실험에 이용된 임플랜트 시스템은 Certain, Xive, Replace, Ankylos, SSII 이다. 이 시스템의 고정체와 지대주를 제조사의 권장 토크로 조인후 액상의 불포화 폴리에스터 용액에 고정한 후 절단하였다. 모든 표본은 절단, 연마후 주사전자 현미경으로 관찰하였다.내측연결 시스템의 적합도를 관찰한 결과, 각 제조사들 사이에 차이는 있으나 대부분의 내측연결 시스템의 경우엔 고정체와 지대주의 접촉은 긴밀히 접촉하는 경향을 가지고 있음을 알 수 있었다. 그러나 지대주와 나사, 고정체와 나사 간에는 다양한 형태의 틈이 존재하였다. 이러한 틈들이 나사의 불안정, 미생물의 군집, 악취 등에 어떠한 영향을 미칠지는 앞으로 계속적인 연구가 필요할 것으로 사료된다.

기공실에서의 임플란트 토크값에 따른 적합도 평가 (Evaluation of fitness in implant screw as tightening torque in dental laboratory)

  • 목적: 본 연구는 토크 게이지를 이용하여, 치과 기공사들이 임플란트 기공을 진행할 때 가하는 토크의 양을 측정해 분석하고, 이러한 결과가 보철물에 어떠한 결과를 미치는 알아보기 위함이다. 연구 재료 및 방법: 치과 기공사들이 임플란트 기공시 임플란트 나사에 가하는 토크를 디지털 토크 게이지를 이용하여 측정한다. 이후 기성지대주와 analog를 이용하여, 제조사의 권장 토크로 조였을 때의 길이를 통계학적으로 비교 분석하였다. 결과: 치과 기공사들의 임플란트 기공시 평균 토크는 $1.563{\pm}0.332Ncm$로 나타났으며, 제조사의 권장토크 값으로 조였을 때와 비교하면, 외부연결형의 침하량 차이는 없으나, 내부연결형의 침하량의 차이는 통계적으로 유의한 차이를 보였다(P < 0.05). 결론: 치과기공실에서도 제조사의 권장토크를 이용하여 보철물 기공작업을 시행하고, 정확한 토크 게이지의 사용법을 익히면, 기공오차에 따른 임플란트의 부적합을 어느 정도 해소할 수 있을 것이라 사료된다.

맞춤형 지대주 각도에 따른 지지골의 유한요소 분석 (Finite Element Analysis of Supporting Bone according to Custom Abutment Angles)

  • Purpose: The purpose of this study is a finite element analysis of supporting bone according to custom abutment angle. Methods: Implant fixture was selected with a diameter of 4 mm and the length of 13 mm. The fixture and abutment was designed by a combination of the abutment screw clamping force to produce a custom abutment model of $0^{\circ}$, $15^{\circ}$, $25^{\circ}$ and $35^{\circ}$. The loading condition of 176 N was applied to the lingual surface of the crown, near to the incisor edge, and horizontal load. An oblique load of $90^{\circ}$ was applied long axis of the implant fixture analyze the stress of supporting bone. Results: The result of mechanical analysis was observed that the supporting bone stress analysis of the horizontal load, the von Mises stress values (MPa) are given in the order of TH00 (432.6) > TH25 (418.0) > TH15 (417.4) > TH35 (415.8), the oblique load, the von Mises stress values are given in the order of TO00 (459.3) > TO15 (399.6) > TO25 (374.8) > TO35 (343.4) Conclusion: The $35^{\circ}$ abutment over the current clinical tolerance limits will be available for clinical application.

골드링을 이용한 완전 밀폐형 어버트먼트 개발 및 성능평가 (Development and Performance Evaluation of a Sealing-Type Abutment Inserted with a Gold Ring)

  • When chewing forces are repetitively applied to a dental implant, gaps are often generated at the interface surfaces between the abutment and the fixture, which results in some deterioration, such as loosening of the fastening screw, dental retraction, and fixture fractures. To cope with such problems, a sealing-type abutment having a number of grooves along the conical-surface circumference was previously developed, and it showed better sealing performance than conventional ones. To enhance the sealing performance, a new model in which a gold ring is inserted into the top groove of the sealing-type abutment is developed in this study. In addition, a manufacturing process is developed, and performance evaluations, such as fatigue and sealing tests, are carried out. The evaluation results show that the gold-ring-inserted sealing abutment yields better performance than conventional ones.

골유착성 치과 임플랜트 고정체 직경에 따른 지지골의 응력분포에 관한 삼차원 유한요소 분석적 연구 (A 3-dimensional Finite Element Analysis of Stress Distribution in the Supporting Bone by Diameters of Dental Implant Fixture)

  • The objective of this finite element method study was to analyze the stress distribution induced on a supporting bone by 3.75mm, 4.0mm, 5.0mm diameter of dental implant fixture(13mm length). 3-dimensional finite element models of simplified gold alloy crown(7mm height) and dental implant structures(gold cylinder screw, gold cylinder, abutment screw, abutment, fixture and supporting bone(cortical bone, cancellous bone) designs were subjected to a simulated biting force of 100 N which was forced over occlusal plane of gold alloy crown vertically. Maximum von Mises stresses(MPa) under vertical loading were 9.693(3.75mm diameter of fixture), 8.885(4.0mm diameter of fixture), 6.301(5.0mm diameter of fixture) and the highest von Mises stresses of all models were concentrated in the surrounding crestal cortical bone. The wide diameter implant was the good choice for minimizing cortical bone-fixture interface stress.

체결력을 고려한 내부 및 외부연결형 임플란트의 반복 하중에 대한 풀림 연구 (Loosening behavior of Internal and External Connection Dental Implants under Cyclic Loads Considering Pre-fastening Force)

  • This paper presents the loosening behavior that occurs after the application of an external load to internal and external connection types of dental implants using the finite element method. We use fastening force between an abutment and a fixture to clamp the dental implant system; however, loosening and breakage may occur owing to cyclic external loads. In this study, we considered the initial fastening condition in the pre-load analysis and then investigated the change in stress and contact surface status when applying external loads. After the application of the initial fastening load, we verified that the internal connection-type model exhibited a relatively lower stress distribution than that of the external connection-type one. Moreover, we found that the former model showed a lower stress concentration after the application of the external load. In addition, after the application of this load, we found that the higher the shear load acting on the implant system, the higher the possibility of loosening. The study results showed the change in stress distribution and contact surface according to the connection type of the dental implants and the phenomenon of loosening by cyclic loads. We expect that the results of this study will be useful for the study of reliability and design of dental implant systems.

THE EFFECT OF INTERNAL IMPLANT-ABUTMENT CONNECTION AND DIAMETER ON SCREW LOOSENING

  • Statement of problem. One of the common problems of dental implant prosthesis is the loosening of the screw that connects each component, and this problem is more common in single implant-supported prostheses with external connection, and in molars. Purpose. The purposes of this study were: (1) to compare the initial abutment screw detorque values of the six different implant-abutment interface designs, (2) to compare the detorque values of the six different implant-abutment interface designs after cyclic loading, (3) to compare the detorque values of regular and wide diameter implants and (4) to compare the initial detorque values with the detorque values after cyclic loading. Material and methods. Six different implant-abutment connection systems were used. The cement retained abutment and titanium screw of each system were assembled and tightened to 32Ncm with digital torque gauge. After 10 minutes, initial detorque values were measured. The custom titanium crown were cemented temporarily and a cyclic sine curve load(20 to 320N, 14Hz) was applied. The detorque values were measured after cyclic loading of one million times by loading machine. One-way ANOVA test, scheffe’s test and Mann-Whitney U test were used. Results. The results were as follows : 1. The initial detorque values of six different implant-abutment connections were not significantly different(p>0.05). 2. The detorque values after one million dynamic cyclic loading were significantly different (p<0.05). 3. The SS-II regular and wide implant both recorded the higher detorque values than other groups after cyclic loading(p<0.05). 4. Of the wide implants, the initial detorque values of Avana Self Tapping Implant, MIS and Tapered Screw Vent, and the detorque values of MIS implant after cyclic loading were higher than their regular counterparts(p<0.05). 5. After cyclic loading, SS-II regular and wide implants showed higher detorque values than before(p<0.05).

지대주 혹은 지대주 나사 파절이 발생한 임플란트 보철물에서 효과적인 수리 방법 (Repairment of abutment and abutment screw fracture in implant prosthesis: A case report)

  • 치과 임플란트는 상실된 치아의 수복에 효과적이고 예측 가능한 치료 방법이다. 하지만 임플란트 사용이 증가함에 따라 합병증도 증가하고 있다. 이 중 기계적 합병증은 발생 빈도가 높을 뿐 아니라 임플란트의 수명이 길어질수록 더 많이 발생하므로 이에 대한 대책이 필요하다. 이에 임플란트의 가장 흔한 기계적 합병증인 지대주 파절과 지대주 나사 파절의 증례에서 파손된 부위만 재제작하여 파손되지 않은 구성요소를 최대한 보존할 수 있는 방법을 소개하고자 한다.

The effect of the digital manufacturing technique of cantilevered implant-supported frameworks on abutment screw preload

  • PURPOSE. The purpose of this study was to investigate the misfit and screw preload at the implant abutment connection of implant supported fixed dental prosthesis with cantilever (ICFDP) manufactured using different digital manufacturing techniques and to compare the screw preload before and after cyclic loading. MATERIALS AND METHODS. Mandibular jaw model with four intra-foraminal implants was scanned using digital scanner. Stereolithography file was used to design a framework with nonengaging (NE) abutments and 10 mm cantilever distal to one terminal implant. Five frameworks were constructed using combined digital-conventional techniques (CAD-cast), and five frameworks were constructed using three-dimensional printing (3DP). Additional CAD-cast framework was constructed in a way that ensures passive fit (PF) to use as control. Scanning electron microscope (SEM) measured the implant abutment connection misfit. Sixty screws were used on the corresponding frameworks. Screws were torqued and pre-cyclic loading reverse torque value (RTV) was recorded. Frameworks were subjected to 200,000 loading cycles with a loading point 9 mm from the center of terminal implants adjacent to the cantilever and post-cyclic loading RTVs were recorded. RESULTS. Microscopic readings showed significant differences between frameworks. PF demonstrated the lowest measurements of 16.04 (2.6) ㎛ while CAD-cast demonstrated the highest measurements of 29.2 (3.1) ㎛. In all groups, RTVs were significantly lower than the applied torque. Post-cyclic loading RTV was significantly lower than pre-cyclic loading RTV in PF and 3DP frameworks. Differences in RTVs between the three manufacturing techniques were insignificant. CONCLUSION. Although CAD-cast and three-dimensionally printed (3DP) both produce frameworks with clinically acceptable misfit, 3DP might not be the technique of choice for maintaining screw's preload stability under an aggressive loading situation.
