Marginal and internal fit according to the shape of the abutment of a zirconia core manufactured by computer-aided design/computer-aided manufacturing (CAD/CAM으로 제작된 지르코니아 코어의 지대치 형태에 따른 변연 및 내면 적합도에 관한 연구)

  • Background: In this study, zirconia copings were fabricated by setting clinically acceptable inner values for prostheses using computer-aided design/computer-aided manufacturing (CAD/CAM). The processed copings were evaluated for the marginal and internal fit of each abutment shape with a CAD program using the silicone replica technique. Methods A total of 20 copings was produced by selecting models commonly used in clinical practice. After injecting the sample, the minimum thickness, internal adhesion interval, and distance to the margin line were set to 0.5, 0.05, and 1.00 mm using a dental CAD program, respectively. It was measured using a 2D section function in a three-way program of the silicon replication technology. Although the positions and number of measurements of the anterior and posterior regions differed, nine parts of each pre-tube were designated and measured by referring to a previous study to compare the two samples. Results As a result, the average margin of the mesial, distal, and buccal (labial) surfaces was 59.90 ㎛ in the anterior region and 60.40 ㎛ in the posterior region. The mean axial wall margin was 67.25 ㎛ in the anterior region and 69.25 ㎛ in the posterior region. In occlusion, the anterior teeth (77.70 ㎛), posterior teeth (77.60 ㎛), and both anterior and posterior regions were within the clinically acceptable range. Conclusion The edge and inner fit of zirconia coping manufactured using the CAD/CAM system showed clinically applicable results. To reduce errors and increase accuracy, materials and machine errors that affect the manufacture of prosthetics should be investigated. Based on our results, the completeness of prosthetics could increase if the inner value and characteristics of the material are adjusted when applied in clinical practice.

  • An accurate implant placement with ideal location is significant for long-term success of the implant. An exact evaluation of nearby anatomic structures such as quality of residual bone, an inferior alveolar bone and a maxillary sinus is required. For a prosthetic-driven treatment, planned surgery, precise prosthesis and communication with the patient are significant requisites especially for full-mouth rehabilitation. In this case, the patient with severe alveolar bone resorption had a CT guided surgery supported by CT data and the data from scanning diagnostic wax-up. Afterward, edentulous area was restored by full mouth implant-supported prosthesis by using monolithic zirconia and CAD/CAM technique. This paper reports the outcome of the procedure which was remarkable both esthetically and functionally.

Evaluation of Marginal Gap of Fixed Dental Prostheses Fabricated by Soft Metal Material Using Dental CAD/CAM (치과용 캐드캠을 이용하여 연질금속 재료로 제작한 고정성 보철물의 변연 간격 평가)

  • The purpose of this study was to evaluate marginal gap of fixed dental prostheses (FDPs) fabricated by soft metal material with using dental computer aided design/computer aided manufacturing (CAD/CAM) system and to compare gap of its by a conventional method. Ten same cases of study models were manufactured and scanned for digital models fabricating. Ten FDPs were fabricated by soft metal material using dental CAD/CAM (SMB group). Then, ten FDPs were fabricated by cast metal using lost wax technique and casting method (LWC group). Marginal gap was measured by silicone replica technique. Gap was measured by digital microscope (${\times}160$). Mann-Whitney test for statistical analysis was executed (${\alpha}=0.05$). The mean (standard deviations) of marginal gap was $76.5{\mu}m$ (35.2) for the SMB group, and $82.9{\mu}m$ (22.1) for the LWC group. Statistically significant differences were not found between SMB and LWC. As results, FDPs fabricated by soft metal material were clinically acceptable range.

Computer-guided implant surgery and immediate provisionalization by chair-side CAD-CAM: A case report (진료실 CAD-CAM에 의한 컴퓨터 가이드 임플란트 수술과 즉시 임시보철치료: 증례보고)

  • This report demonstrates a method of generating a chair-side and computer-aided template for implant surgery based on the Top-Down and restoration-driven concept. Compared to the traditional CAD-CAM process which requires multiple steps to be taken between dental clinic and laboratory, this alternative procedure, VARO guide system (VARO Guide, CAD, Pre-Guide, VARO-mill, NeoBiotech, Seoul, South Korea) enables accurate and patient-friendly implant surgery as well as immediate provisional restoration in a single visit. First, bite-registration at centric jaw relation and CBCT were taken using the Pre-Guide. The CBCT data was then reorganized directly through the chair-side CAD, and we could determine the most appropriate 3-dimensional position of implant. The STL file was extracted and put into the chair-side CAM (VARO-mill) to fabricate a VARO. This surgical guide allowed the implants to be accurately positioned into the planned sites within an hour.

  • Recently, the digital solution of fabricating removable prosthesis by applying haptic input device, electronic surveying, and rapid prototyping was introduced. This review article covers the concept of electronic surveying, computer-aided denture framework designing procedure, discussions after several digital denture cases, directions of future development, such as digital tooth arrangement and RP flasking.

  • 치과보철학 분야에서 컴퓨터를 이용한 기술의 발달로 치료 계획, 수술, 보철물 수복에 이르기까지 CAD-CAM의 광범위한 적용이 가능해지고 있으며, 비용과 시간의 절약으로 효율적인 진료가 가능하다는 점을 인지한 많은 회사들의 투자로 다양한 프로그램과 기계들이 소개되고 있다. 그러나, 이렇게 활발한 제품 개발에도 불구하고 아직 실제 임상으로의 적용은 보편화되었다고 보기는 어려운데, 가장 큰 이유는 정확도에 대한 의심과 복잡하다고 느껴지는 초기의 숙련 과정에 있다고 하겠다. 제작회사들의 보고에 따르면 컴퓨터를 이용하여 디자인하고 절삭하는 것이 기존의 주조 기술보다 더 정교하다고 하였으나, 실제 임상에서는 미세한 오차를 간과한 데서 오는 최종 결과물과의 차이를 얘기하는 임상가들이 있는 것이 현실이다. 또, 그 사용 재료에 있어서도 초기 티타늄 금속 재료에 이어 심미적 요구를 위한 알루미나, 보다 향상된 강도를 가진 지르코니아에 이르기까지 다양한 재료들이 쏟아져 나오고 있다. 이에 본 논문에서는 다양한 CAD-CAM 시스템 중 대표적인 것 중의 하나인 Nobel Biocare사의 $Procera^{(R)}$ 시스템을 중심으로 지르코니아 수복물에 관한 최신 정보를 제공하며, 그 다양한 치과적 적용을 누구나 쉽고 정확하게 사용할 수 있는 임상 증례와 함께 소개함으로써 실제 임상 적용에 도움이 되고자 한다.

Comparative Study of Color Correspondence According to Size of Vita CAD/CAM Ceramic Block Using ShadeEye-Ncc$^{(R)}$ System (ShadeEye$^{(R)}$ NCC system을 이용한 Vita CAD/CAM Ceramic Block의 크기에 따른 색조 일치성 비교연구)

  • Purpose: The purpose of this study was to compare color correspondence of different sizes of Vita Mark II$^{(R)}$ and TriLuxe$^{(R)}$ Feldspar blocks. Methods: The three commercially available shades(1M2, 2M2, 3M2) of Mark II & TriLuxe blocks for the CEREC$^{(R)}$ CAD/CAM system were examined. For each of three colors, three different sizes were tested, 5 blocks each. The measurements were made using a spectrophotometer equipped with an integrating sphere using the CIE $L^*$, $a^*$, $b^*$ colorimetric system. Results: The $L^*$, $a^*$, $b^*$ value of Vita Mark II$^{(R)}$ ceramic block showed significantly higher than TriLuxe$^{(R)}$ ceramic block(p<0.05). In comparing the Vita Mark II$^{(R)}$ specimen of the three different shade, color differences between materials(${\Delta}E^*$) showed the lowest value of 2.09, and the highest was 2.24. ${\Delta}E^*$ values of the materials of Vita Mark II$^{(R)}$ were higher than 2. As the size of ceramic block differed, the color correspondence of Vita Mark II$^{(R)}$ showed statistically significant difference but, this result is clinically acceptable. Conclusion: All the different sizes of the different shades of Vita TriLuxe$^{(R)}$ blocks for the CEREC$^{(R)}$ system showed the high degree of color correspondence necessary in industrially prefabricated CAD/CAM blocks. The results of the present study suggested that it would be necessary to establish the reproducible and constant color specification system for an esthetic restoration.

Three-dimensional evaluation of the internal adaptation of single and three-unit fixed dental restoration by CAD/CAM milling system (CAD/CAM 밀링 시스템을 활용한 단일 치관과 3본 교의치의 3D 적합도 평가)

  • Purpose: The purpose of this paper was to evaluate the occurrence of errors regarding adaptation by conducting a three-dimensional assessment comparing the bridge type dental restoration after the cutting process, which has multiple abutments, with a single type dental restoration. Methods: By using ten identical files obtained by scanning the master model, thirty designs were created consisting of ten maxillary right first premolars and ten maxillary right first molars with single crown abutments, along with ten bridge designs with the identical abutment. A 5-axis milling machine was used to produce the design file. The produced denture prostheses were scanned using a silicone replica for a STL file. An evaluation was conducted using 3D analysis software on the master model and each of the thirty data files. Results: The RMS value of the pre-molar (14) was $38.4{\pm}4{\mu}m$ for single and $54.7{\pm}6{\mu}m$ for bridge abutment; therefore, a statistically significant difference was observed for single and bridge designs although both shared the same abutment form (P<.05). Also, the RMS value of the molar (16) was $47.6{\pm}2{\mu}m$ and $56.6{\pm}5{\mu}m$ for the single and bridge designs, respectively, thereby presenting a statistically significant difference (P<.05). Conclusion: As a result, dental prosthesis fabricated using the single method presented better internal adaptation outcomes.

Reconstruction of anterior guidance using duplication technique of CAD/CAM: a case report (CAD/CAM의 복제 기법을 이용한 전방 유도의 재현 증례)

  • Comprehensive prosthetic treatment requires considerations from various points of view. The anterior guidance is important factor in prosthodontic treatment of anterior teeth. Lingual surface contour of anterior restoration is so critical that a small mistake of laboratory or clinical process can cause discomfort of patient and disharmony of entire dentition. There are no guidelines for lingual surface contour that fit all patients. Therefore the lingual surface of provisional restoration is most accurately described as a customized one. The dentist transfers the exact information of anterior guidance that has made through long term provisional restoration to the technician. This case introduce that the duplication technique of CAD/CAM system to reproduce the anterior guidance of provisional restoration. This method can improve satisfaction of both patient and dentist.

Perceptions of dental hygienists toward digital dentistry (치과 디지털 활용에 관한 치과위생사의 인식)

  • Objectives : The purpose of this study was to investigate the perceptions of dental hygienists toward digital dentistry. Methods : The subjects were 393 dental hygienists in Daegu Gyeongbuk. A total of 380 data were analyzed except the incomplete questionnaires. Data were analyzed by descriptive statistics, chi-square test, t-test, one-way ANOVA using SPSS version 18.0. Results : 1. Difference of perception by general characteristics was affected by age, academic background, working career, and married. 2. Difference of perception by utilization of digital facilities was affected by digital camera, CAD/CAM system, oral scanner, and computer system. 3. Difference of perception by digital education experience and intention of participation was affected by stronger intention to participate in digital education and practical exercise. Digital X-ray system and CAD/CAM system were the must-be equipment in education. Conclusions : Utilization of digital dentistry is the motive for education experience in dental hygienists.
