성인의 치매 지식, 치매 두려움 및 치매 예방행위 의도가 치매 예방교육 요구에 미치는 영향요인 (Factors Influencing Adult Dementia Knowledge, Fear of Dementia, and Intention of Dementia Prevention Behavior on the Demand for Dementia Prevention Education)

  • 본 연구는 성인의 치매 지식, 치매 두려움 및 치매 예방행위 의도가 치매 예방교육 요구에 미치는 영향요인을 분석하고자 수행되었다. 연구대상자는 충남, 충북에 거주하고 있는 19~64세 성인으로 연구의 목적을 이해하고 연구에 참여할 것을 동의한 265명을 대상으로 설문조사하였다. 수집된 연구 자료는 빈도, 백분율, 평균, 표준편차, t-test, ANOVA, 다중 회귀분석을 이용하여 분석하였다. 연구 결과 성인의 치매 지식과(p=.055) 치매 두려움은(p=.302) 치매예방교육 요구에 유의한 영향을 미치지 않는 것으로 나타났고, 치매 예방행위 의도가(β=.329, p<.001) 치매 예방교육요구에 유의한 영향을 미치는 것으로 나타났다. 이 같은 연구결과를 토대로 치매의 이해와 치매관리사업, 치매조기검진과 치매예방 건강수칙, 치매예방을 위한 만성질환 관리, 치매예방을 위한 운동, 치매예방을 위한 건강한 식생활, 치매예방을 위한 인지증진프로그램 등 국가의 치매 예방 홍보 및 국가 차원의 대응 노력과 더불어 지역사회를 중심으로 치매예방 프로그램에 적극 참여하여 올바른 치매 예방행동을 실천하고 예방할 수 있는 기회를 충분히 제공해야 한다고 사료된다.

경도인지장애와 경도 치매의 감별을 위한 대면 이름대기와 범주 이름대기의 예측 성능 비교 (Prediction Performance of Naming Tests for Differentiating Mild Cognitive Impairment and Mild Dementia)

  • 본 연구는 정상 노인과 초기 단계의 노년기 인지 장애(경도인지장애(MCI), 경도 치매)의 선별검사인 대면 이름대기와 범주 이름대기의 예측력을 파악하였다. 노년기 인지장애로 진단을 받은 340명(정상 노인 203명, MCI 106명, 경도 치매 31명)을 분석하였다. 대면 이름대기는 단축형 한국판 보스턴 이름대기 검사로 측정하였고, 범주 이름대기는 통제연상단어검사의 의미검사와 음소검사를 이용하여 측정하였다. 이름대기 검사의 예측 성능을 비교하고자 다항 로지스틱 회귀분석을 수행한 결과, 대면 이름대기와 범주 이름대기 검사 모두 일반노인에서 MCI와 경도 치매를 감별하는데 유의미한 효과가 확인되었다(p<0.05). 반면, MCI에서 경도 치매를 감별할 때, 범주 이름대기의 음소검사는 교차비가 유의미하지 않았다. 본 연구의 결과는 MCI에서 경도 치매를 감별할 때, 범주 이름대기의 총점만을 측정기준으로 분석하는 것은 유의하지 않을 가능성이 높음을 시사한다.

노인성치매 발생요인과 돌보는 가족원의 스트레스에 관한 조사 연구 (A Study on risk factors for senile dementia)

  • This study was conducted to investigate risk factors for senile dementia as well as care givers' stresses and thier needs for nursing care. It was done using a retrospective survey. A convenience sample or In senile dementia patients and l20 nor-mal elders in a rural area was used. The tools used in the study were the MMSE-K(Mini-Mental State Examination-Korea) for dementia screening test and a questionaire developed by the research team. Data were collected through home visits by Com-munity Health Practitioners. Data were analyzed using descriptive statistics, T-test, and Chi-square test. The findings are as follows : 1. There were significant differences in age, marital status, and religions between the two groups. 2. There was a significant difference in smoling behavior between the two groups. 3. There was a significant difference in past his-tory of cancer between groups. 4. There was a significant difference in past and present elderftmily relationship between the two groups. 5. There were significant differences in intellectual activities, assuming major role in family and seeking other's help in daily life troubles between the two groups. 6. There were significant differences in stress factors such as child problem, family conflict, health problem and illegal behavior between the two groups. 7. The major problems out by families in caring for dementia patient were catastrophic reactions, dirtiness, mood change, devouring and tremor. The most serious problems faced by families was dirtiness. with catastrophic reactions, sleep distrubance, changeableness, and a suspcio-usness following. The care givers expressed chronic fatigue, anxiety, tension, depression, disorder in daily life, shamefulness, blame from neighbours and guiltiness. 8. There is need for geriatric hospitals, nursing homes, burden sharing, and counselling or education for family care givers. A replicate study in the urban area is recommended to validate the findings of this study. To explore the impact of stress in life and ‘han’ on senile dementia, a qualitative study is recommended.

Application of AIG Implemented within CLASS Software for Generating Cognitive Test Item Models

  • Scale scores for cognitive domains have been used as an important indicator for both academic achievement and clinical diagnosis. For example, in education, Cognitive Abilities Test (CogAT) has been used to measure student's capability in academic learning. In a clinical setting, Cognitive Impairment Screening Test utilizes items measuring cognitive ability as a dementia screening test. We demonstrated a procedure of generating cognitive ability test items similar as in CogAT but the theory associated with the generation is totally different. When creating cognitive test items, we applied automatic item generation (AIG) that reduces errors in predictions of cognitive ability but attains higher reliability. We selected two cognitive ability test items, categorized as a time estimation item for measuring quantitative reasoning and a paper-folding item for measuring visualization. As CogAT has widely used as a cognitive measurement test, developing an AIG-based cognitive test items will greatly contribute to education field. Since CLASS is the only LMS including AIG technology, we used it for the AIG software to construct item models. The purpose of this study is to demonstrate the item generation process using AIG implemented within CLASS, along with proving quantitative and qualitative strengths of AIG. In result, we confirmed that more than 10,000 items could be made by a single item model in the quantitative aspect and the validity of items could be assured by the procedure based on ECD and AE in the qualitative aspect. This reliable item generation process based on item models would be the key of developing accurate cognitive measurement tests.

Comparison of Feasibility of Touch-Based Cognitive Training Games in Community Elderly and Elderly with Minor Dementia

  • Objective: The purpose of this study is to select a cognitive training game that can evaluate five cognitive domains and to study their validity with existing cognitive evaluation tools. Design: Methods: Delphi survey was conducted through the 2nd questionnaire for 30 experts to select games suitable for training 5 cognitive domains. Five cognitive training games and Mini Mental State Examination - Korea(MMSE-K), and cognitive impairment screening test(CIST) were conducted for 82 elderly in the community. Pearson correlation analysis was performed to find out the correlation of the three tests. The ROC curve was used to calculate the selection criteria for the game results for the screening evaluation of the presence or absence of mild cognitive impairment. Results: The coefficient of variation to evaluate the stability of the Delphi survey was less than 0.50 in most game items. The 'correct answers' and 'level' of the five final selected game items showed a statistically significant positive correlation with the CIST and MMSE-K scores. CIST score and 'time' of all game items except 'number making_time' showed a statistically significant negative correlation. Conclusions: The validity of the cognitive training program using smart devices was evaluated, and the criteria for classifying the cognitive domain and distinguishing the presence or absence of cognitive impairment were confirmed.

감각활동기반 인지재활 그룹 프로그램이 시설거주 치매노인의 인지기능과 우울 및 삶의 질에 미치는 효과 (Effect of Sensory Activity-Based Cognitive Rehabilitation Group Program on Cognitive Function, Depression and Quality of Life of Dementia Patients Living in a Facility)

  • 목적 : 본 연구는 감각활동기반 인지재활 그룹 프로그램이 시설거주 치매노인의 인지기능과 우울감, 삶의 질에 미치는 영향에 대해 알아보고자 실시하였다. 연구방법 : 강원도 H군에 위치한 노인 요양시설에 거주하는 65세 이상 치매노인 8명을 대상으로 감각기반 인지재활 그룹 프로그램 총 20회기(사전/사후평가 포함)를 실시하였다. 대상자들의 인지기능, 우울수준과 삶의 질 수준을 프로그램 전과 후에 측정하였다. 결과 : 인지기능 평가에 사용한 한국어판 간이 정신상태 검사(Mini-Mental State Examination-Dementia Screening)의 평균점수는 증가하였지만 통계적으로 유의미한 차이는 없었다. 주관적 기억감퇴 설문지(Subjective Memory Complaints Questionnaire) 결과 통계적으로 유의한 향상이 있었다(p < .05). 우울정도 변화를 확인하기 위해 측정한 한국형 단축형 노인우울 척도(Short Geriatric Depression Scale, SGDS-K)의 사후평가 평균점수는 사전평가보다 향상을 보였으나 통계적으로 유의미한 차이는 없었다. 마지막으로 삶의 질의 변화를 확인하기 위해 측정한 세계보건기구 삶의 질 간편형 척도(World Health Organization Quality of Life assessment instrument-Brief) 결과에서 통계적으로 유의미한 향상이 있었다(p < .001). 결론 : 본 연구 결과를 통해서 시설에 거주하는 치매노인을 위한 감각활동기반 인지재활 그룹 프로그램 개발의 필요성과 유용성을 제시하는 데 기초적 근거를 마련할 수 있을 것이다.

뇌졸중 환자를 대상으로 한 인지선별검사(CIST), 한국판 간이정신상태검사 2판(K-MMSE~2) 및 임상치매척도(CDR)의 상관성 (Correlation Between Cognitive Impairment Screening Test (CIST), Korean-Mini Mental State Examination, 2nd Edition (K-MMSE~2) and Clinical Dementia Rating (CDR) of Patients with Stroke)

  • 목적 : 본 연구는 뇌졸중 환자를 대상으로 한 인지기능 평가 도구 간의 상관성을 알아봄으로써 임상에서 평가 도구를 선정하거나 사용하는데 도움이 될 수 있는 기초자료를 제공하고자 하였다. 연구방법 : 본 연구는 병원 등 의료기관에 입원하여 작업치료를 받는 뇌졸중 환자 51명을 대상으로 실시하였다. 평가 도구는 인지선별검사(Cognitive Impairment Screening Test: CIST), 한국판 간이정신상태검사 2판(Korean-Mini Mental State Examination, 2nd Edition: K-MMSE~2)과 임상치매척도(Clinical Dementia Rating: CDR)를 사용하였다. 평가 도구 점수 간 상관관계는 Spearman's rank correlation coefficient를, 전체 CDR 점수에 따른 CIST와 K-MMSE~2의 총점 비교는 Wilcoxon signed rank test를 이용하여 분석하였다. 결과 : CIST와 K-MMSE~2의 총점 및 전체 CDR 점수 간 상관관계는 CIST와 K-MMSE~2(ρ=.956), CIST와 CDR(ρ=-.957), K-MMSE~2와 CDR(ρ=-.971)에서 통계학적으로 유의미한 상관관계를 보였다(p<.01). CIST와 K-MMSE~2의 하위영역별 점수 간 상관관계는 모든 하위영역에서 통계학적으로 유의미한 상관관계를 보였다(p<.01). 전체 CDR 점수에 따른 CIST와 K-MMSE~2의 총점 비교는 모든 전체 CDR 점수에서 통계학적으로 유의미한 차이를 보이지 않았다(p>.05). 결론 : 본 연구를 통해 뇌졸중 환자를 대상으로 한 CIST, K-MMSE~2 및 CDR 간에 상관성이 있다는 점을 알 수 있었다. 향후에는 다른 대상자를 통한 연구가 진행되기를 바라며, 본 연구의 결과가 병원 등의 임상에서 인지기능 평가 도구를 선정 또는 사용함에 있어 참고자료로 활용되기를 기대한다.

Association of head circumference with cognitive decline and symptoms of depression in elderly: a 3-year prospective study

  • Background: Brain volume is associated with dementia and depression in the elderly. An easy way to predict relative brain volume is to measure head circumference. In this study, we investigated the relationship between head circumference and cognition as well as depression in a non-demented elderly community. Methods: Baseline and follow-up surveys were conducted in 2007 and 2010. At baseline, community residents aged 65 years or over (n=382) within a rural area of South Korea were screened for dementia and symptoms of depression and were followed using the same screening battery after 3 years (n=279). Data from anthropometric measurements (head circumference, height, and body weight), demographics, and blood tests were gathered. Neuropsychological tests, including the Korean version of mini-mental state examination (K-MMSE), clinical dementia rating (CDR) including the CDR-sum of boxes, the Korean version of instrumental activities of daily living, and geriatric depression scale (GDS), were performed. None of the 279 subjects followed were demented. Results: Baseline performance on the K-MMSE and GDS was poorer for participants with smaller head circumferences. Follow-up performance on the MMSE was also poorer for participants with smaller head circumferences. Interestingly, participants with smaller head circumference showed worse GDS scores at baseline but on follow-up examination, participants with larger head circumference showed rapid worsening than those with smaller head circumference with marginal significance by ANOVA test. In regression coefficient analysis, GDS decline showed significant difference. Conclusion: Head circumference was not associated with cognitive change but was associated with symptoms of depression in non-demented community residents.

Effect of Education on Discriminability of Montreal Cognitive Assessment Compared to Mini-Mental State Examination

  • Background and Purpose: The Montreal Cognitive Assessment (MoCA) has been known as a screening test for detecting mild cognitive impairment (MCI) better than Mini-Mental State Examination (MMSE). However, in previous domestic studies, no significant difference was found in the discriminability between MoCA and MMSE. Researchers have suggested that this might be because older Koreans are less educated than older Westerners. This study was conducted to examine the effect of education on the discriminability of MoCA compared to the MMSE. Methods: Participants were 123 cognitively normal elderly, 118 with vascular MCI, 108 with amnestic MCI, 121 with vascular dementia, and 113 with dementia of the Alzheimer's type. The Korean-MoCA (K-MoCA) and Korean-MMSE (K-MMSE) were administered. Multiple regression analyses and receiver operating characteristic (ROC) curve analyses were performed. Results: In all participants, education significantly affected both K-MoCA and K-MMSE scores along with age. The effect of education was re-examined by subgroup analysis after dividing subjects according to the level of education. Effect of education on K-MoCA and K-MMSE was only shown in the group with <9 years of education. ROC curve analyses revealed that the discriminability of K-MoCA to differentiate between vascular MCI and normal elderly was significantly higher than that of K-MMSE. When re-examining subgroups divided by education level, however, this higher discriminability of K-MoCA disappeared in the group with <9 years of education. Conclusions: These results indicate no difference in discriminating cognitive deficits between K-MoCA and K-MMSE in Korean elderly with <9 years of education.
