한국치매노인 가족의 주거환경 개선요구 (Housing Needs of Korean Family with Demented Elderly)

  • Most elderly victims of senile dementia in Korea are cared for at home. This creates hardship for their families. Recent research suggests that the physical environment can affect the treatment of dementia. In light of these findings, a prudent approach to the design of the housing environment of family with dementia sufferers is advisable. This study aims to determine the design factors that need to be considered for the housing environment of dementia, particularly for those suffering from Alzheimer's or Cerebrovascular dementia. For this study, and in-depth qualitative investigation method was employed. Principal caregivers were interviewed using various investigative tools, including objective measures, structured and non-structured questionnaires. The investigation results suggest that the need for housing environment improvement varies with the severity and type of dementia. The housing needs in this study, is expected to promote a focus on improved design management for the environment and housing of dementia and their families. The results also suggest the establishment of a welfare policy for senile dementia victims and their families.

치매환자 부양가족의 치매환자 구강관리 실태 및 우울과 사회적지지 (Oral care practices for dementia patients and social support and depression in family caregivers of dementia patients)

  • Objectives: This study aims to contribute to devising systems for family caregivers of dementia patients by examining the state of oral care of dementia patients, and depression and social support among family caregivers of dementia patients. Methods: Family caregivers of dementia patients in the metropolitan area were selected in this study. The inclusion criteria were individuals who have provided care for a dementia patient at home for at least six months and those who come in contact (including phone calls) with the patient at least twice a week. Results: Oral health knowledge of the elderly, caregiving burden, depression, and social support were examined. The mean scores for oral health knowledge of the elderly and caregiving burden were $57.11{\pm}16.94$ out of 100 and $17.33{\pm}8.61$ out of 48, respectively. Further, the mean caregiving behavior score, depression score, and social support score were $8.49{\pm}13.71$ out of 100, $5.11{\pm}3.05$ out of 10, and $72.75{\pm}17.03$ out of 100, respectively. Factors affecting oral health knowledge of the elderly were examined. The results showed that the level of oral health knowledge of the elderly increased with an increasing perception of a need for oral health education (p<0.05), caregiving burden (p<0.01), and social support (p<0.01). Conclusions: These findings suggest that developing and popularizing oral care intervention programs for family caregivers of dementia patients are necessary to ensure systematic oral care for dementia patients.

건강신념모델을 적용한 치매 선별검사 수행에 영향을 미치는 요인 (Factors Affecting the Performance of the Dementia Screening Test Using the Health Belief Model)

  • Purpose: This study was conducted to investigate factors affecting the dementia screening test based on the health belief model. Methods: The survey was conducted with structured questionnaires assessing the knowledge of dementia, health beliefs, self-efficacy and cue to action for dementia screening. Study participants were 156 older adults and data from 121 respondents were analyzed. Descriptive statistics, frequency percentage, chi-squared (${\chi}^2$) test, t-test and logistic regression analysis were conducted using SPSS version 23.0. Results: Factors influencing the dementia screening test were found to be gender, self-efficacy on dementia screening, and the presence of family or friends who underwent dementia screening. Older adults who were female (OR=4.92, p=.003), showed an increasing average score of self-efficacy for dementia screening (OR=2.85, p=.002), and had family members or friends who underwent dementia screening (OR=4.60, p=.003) were more likely to receive dementia screening. Conclusion: This study showed that education and programs enhancing self-efficacy associated with the dementia screening test, as well as utilizing the network of family or friends are necessary to increase the rate of dementia screening.

치매부모에 대한 성인자녀의 자원이전에 관한 연구 (A study on the Family Resource Transfers from Adult Children to their Parents with Dementia)

  • The purpose of this study is to analyze the influence of parent-to-child financial transfers, providing household chores, and inheritance on financial transfers and time transfers from adult children to their elderly parents with dementia. Analyzing data from the sample of 343 adult children of parents with dementia, this study finds a strong positive effect of prior parent-to-child financial transfers on child-to-parent financial transfers under controlling parent characteristics, respondent characteristics and sibling's transfers to their parents. The effects of providing household chores and inheritance on time transfers are also positively significant. The results of this study point out the importance of reciprocity in resource transfers between adult children and their parents with dementia.

치매노인 주부양자의 노인복지시설에 대한 인식 (Family Care-givers' Perceptions of Welfare Facility for Elderly People with Dementia)

  • The purpose of this study was to investigate the family care-givers' perceptions of welfare facility for the elderly people with dementia, to analyze the related variables, and to provide the suggestions from the perspectives of the well-being of family care-givers. The survey was done using questionnaires in 1999, and the subjects were 198 primary care-givers for senile dementia patients. Frequency distribution, mean, t-test, and one way ANOVA were used to analyze the data. The major findings of this study were as follows: The family care-givers' perceptions of the welfare facility for elderly people with dementia was relatively low. The variables associated were sex, education level, occupation and relationship with the demented elderly. The conclusion suggested from this study was that the social welfare facilities and services were necessary for both the demented elderly people themselves and their family care-givers.

  • Even though enormous governmental expenses and scientists' efforts to find out definite causes and treatment methods of senile dementia have been investigated, little has been known in this area. Along with knowledge development of the etiology and treatment of the dementia, researchers have started to focus on improving the quality of life of the older adults with dementia through psychosocial intervention. This study was designed to propose a theoretical framework for establishing therapeutic environment for the older adults with dementia and for developing principles and strategies of caring. The results of this study were expected to help family members of the older adults with dementia to understand behavioral problems of the demented persons. The results can be utilized for health professionals to provide nursing interventions to reduce family caregivers' burden and to improve the quality of life of the older adults with dementia and their family. Caring principles developed from this study were as follows: 1. To minimize the stressors that can stimulate older adults with dementia. 2. To assess demented person's needs for safety and provide intervention based on the assessment. 3. To provide therapeutic environment for older adults with dementia to reduce confusion and to improve orientation. 4. To organize simple regular daily activities that older adults can anticipate. 5. To enhance demented person's self-esteem and self-confidence by providing supportive care. 6. To promote social interaction of the older adults with dementia by utilizing adequate activity programs.

치매노인 부양주부의 가정자원이 부양스트레스에 미치는 영향 (The Influences of Family Resources on Caregiving Stress for Dementia Elderly in Homemakers)

  • The purpose of this study was to explore the influences of family resources, i.e. objective resources, perceived adequacy of resources, locus of control, social support, family cohesion, and family adaptability on caregiving stress for dementia elderly in homemakers. The subjects of this study were 141 married women who had cared for dementia elderly. The survey method was a questionnaire. And the data was analyzed by means of Cronbach's, factor analysis, and multiple regression. The main findings were as follows: First, Homemakers' caregiving stress was positively correlated to their academic background and monthly income, but negatively correlated to perceived adequacy of human resources, perceived adequacy of material resource, psychical support resource from relatives, and internal locus of control. Second, caregiving stress of homemakers was affected by family cohesion, monthly income, psychical support resource from relatives, perceived adequacy of human resources, and external locus of control.

부산지역 치매 재활의 현황 분석 (Analysis of the Current Status of Dementia Rehabilitation in Busan, South Korea)

  • Purpose: The purpose of this study is to understand and analyze the current status of dementia management and rehabilitation services in Busan, South Korea, in response to the rapid increase of people with dementia due to the aging of the population. Methods: To investigate the current status of dementia rehabilitation in Busan, a survey was disseminated to dementia safety centers and day- and night-care centers in 16 districts/counties in Busan. Of the 209 day- and night-care centers, 23 institutes were registered in the National Health Insurance Service and received the highest grade (Grade A) in the institute evaluation that was implemented in each district. A telephone interview was conducted, and survey questions were related to the existence of an ongoing dementia rehabilitation program, program presenter, number of participants, progress method, program time, program duration, program contents, and participation path. Results: Dementia safety centers were implementing dementia prevention program, cognitive enhancing program, dementia program, self-help meeting and education program for family of dementia patient. The majority of the presenters of all four dementia-related programs were occupational therapists. The highest number of participants in the dementia prevention program was 15, and the highest number of participants in the cognitive enhancing program, dementia program, self-help meeting and education program for family of dementia patient was 10. All institutes' programs delivered group therapy. As for the time and frequency of the program, most dementia program included three-hour sessions five times a week. Most dementia prevention program, cognitive enhancing program, self-help meeting and education program for family of dementia patient included 60-minute sessions once a week. The most frequently observed program duration for the dementia prevention program and cognitive enhancing program was six months, and the most frequently observed duration for the dementia program was three months. Lastly, study participants most often reported that self-help meeting and education program for family of dementia patient lasted for two months. Among day- and night-care centers in Busan, programs related to cognition were implemented in 18 institutes, and the majority of the program presenters were social workers. Conclusion: In response to the rapidly growing number of dementia patients due to the aging of the population, this study examined the current status of dementia rehabilitation in Busan. The study results underscore the need to develop systems that consider the circumstances in Busan and continuously and systematically support dementia programs.

지역사회 경증치매노인의 행동심리증상과 가족의 부양스트레스 및 자기효능감 (Behavioral and Psychological Symptoms of the Elderly with Mild Dementia in Local Communities and Care-giving Stress and Self-efficacy of Family Members)

  • 본 연구는 경증치매노인의 행동심리증상과 가족의 부양스트레스 및 자기효능감의 관계를 파악함으로써 치매노인 가족의 부양스트레스를 감소시키는데 기초자료를 제공하기 위함이다. 이에 따른 관계를 규명하기 위한 분석은 기술통계, t-test, ANOVA를 이용하였다. 경증치매노인의 행동심리증상에 대한 가족의 부양스트레스와 자기효능감에 대해 알아본 결과, 치매노인의 행동심리증상과 가족의 부양스트레스는 양의 상관관계 (r=.655, p<.001), 치매노인 가족의 부양스트레스와 자기효능감은 음의 상관관계(r=-.334, p<.001)가 있었다. 본 연구결과를 토대로 경증치매노인의 행동심리증상에 대한 적절한 대처법 개발을 통해 지역사회 경증치매노인 가족의 부양스트레스를 감소시키고 자기효능감을 향상시키기 위한 방안이 필요할 것으로 사료된다.

치매노인환자 간병지원을 위한 스마트 돌봄 플랫폼 설계 (The Design of Smart Care-platform for Supporting Care of Elderly Patients with Dementia)

  • In modern society, dementia patients are increasing due to busy daily life. Due to the nature of dementia, family members are having a hard time with their caring activities. Dementia diseases reduce the quality of life not only of the patient, but also of the family, and nursing activities lead to social problems such as unemployment or frequent turnover. This study aims to reduce the difficulty and stress of the nursing process by supporting daily life of dementia patients and nursing activities through smart care platform. Ultimately, we will reduce the cost of treatment for people with dementia and delay the worsening of symptoms through a systematic care program for people with dementia. The smart dementia care platform features smart technology to help the dementia patients' daily lives and support their caregivers.
