Closure of Myelomeningocele Defects Using a Limberg Flap or Direct Repair

  • Background The global prevalence of myelomeningocele has been reported to be 0.8-1 per 1,000 live births. Early closure of the defect is considered to be the standard of care. Various surgical methods have been reported, such as primary skin closure, local skin flaps, musculocutaneous flaps, and skin grafts. The aim of this study was to describe the clinical characteristics of myelomeningocele defects and present the surgical outcomes of recent cases of myelomeningocele at our institution. Methods Patients who underwent surgical closure of myelomeningocele at our institution from January 2004 to December 2013 were included in this study. A retrospective chart review of their medical records was performed, and comorbidities, defect size, location, surgical procedures, complications, and the final results were analyzed. Results A total of 14 patients underwent surgical closure for myelomeningocele defects. Twelve cases were closed with direct skin repair, while two cases required local skin flaps to cover the skin defects. Three cases of infection occurred, requiring incision and either drainage or removal of allogenic materials. One case of partial flap necrosis occurred, requiring secondary revision using a rotational flap and a full-thickness skin graft. Despite these complications, all wounds eventually healed completely. Conclusions Most myelomeningocele defects can be managed by direct skin repair alone. In cases of large defects, in which direct repair is not possible, local flaps may be used to cover the defect. Complications such as wound dehiscence and partial flap necrosis occurred in this study; however, all such complications were successfully managed with simple ancillary procedures.

수열 합성된 나노구조를 갖는 ZnO 에 대한 표면 및 계면 결함의 상대적인 영향 (Relative Influence of Surface and Interfacial Defects in Hydrothermally Grown Nanostructured ZnO)

  • 온도를 달리하여 수열합성 시킨 두 ZnO nanostructure 의 자외선 검출 소자에 대해 표면 결함과 기판과의 계면 결함의 상대적인 영향을 분석했다. 실험은 laser 가 인가된 상태에서 bias voltage sweep rate을 조절하여, 그에 따른 전류-전압 곡선을 통해 이루어졌다. 수열 성장이 적게 된 ZnO nanostructure의 경우 405, 355 nm laser 인가시, bias voltage sweep rate 을 느리게 할 수록, 전류-전압 기울기가 낮아졌으며, 대조적으로 성장이 크게 된 시료의 경우 기울기가 높아졌다. 이에 대한 이유는 계면과 표면 결함 영향의 차이로 발생됨이 고려됐다. 이와 같이 laser 가 인가된 상태에서 bias voltage sweep rate 에 따른 전류-전압 곡선 분석 실험은 M-S-M (Metal-Semiconductor-Metal) 구조를 갖는 수열 성장된 ZnO 의 표면 및 계면 결함을 관찰하는데 도움을 줄 것으로 생각된다.

Which Factors Related to the Renal Cortical Defects in Infants Under 3 Months of Age with Urinary Tract Infections?

  • Purpose: We used technetium-99m dimercaptosuccinic acid (DMSA) scintigraphy to identify factors predictive of renal cortical defects in infants <3 months of age with urinary tract infections (UTIs). Methods: We retrospectively reviewed data on infants <3 months of age with culture-proven UTIs treated at a single center from March 2010 to February 2016. Blood samples were obtained for laboratory evaluation prior to commencement of antibiotic therapy. The therapeutic delay time (TDT) and therapeutic response time (TRT) were recorded. All patients were divided into two groups depending on features of their DMSA scans. We compared the demographic, clinical, and laboratory characteristics of the two groups. Results: A total of 119 infants (94 males and 25 females; mean age, $56.9{\pm}21.3days$) were included. Cortical defects were evident in the DMSA scans of 47 cases (39.5%). In infants with such defects, the peak temperatures ($38.9{\pm}0.57^{\circ}C$ vs. $38.4{\pm}0.81^{\circ}C$, P=0.001), the absolute neutrophil counts ($8,920{\pm}4,460/mm$ vs. $7,290{\pm}4,090/mm$, P=0.043), and the C-reactive protein (CRP) levels ($6.49{\pm}4.33mg/dL$ vs. $3.21{\pm}2.81mg/dL$, P=0.001) were significantly higher than those in infants without cortical defects. The TDT was also longer in those with cortical defects (P=0.037). Conclusion: We found that a TDT ${\geq}8.5hr$ (odds ratio [OR] 5.81), a peak temperature ${\geq}38.3^{\circ}C$ (OR 6.19), and a CRP level ${\geq}4.96mg/dL$ (OR 7.26) predicted abnormal DMSA scan results in infants <3 months of age with UTIs.

  • 비파괴검사중의 하나인 초음파 탐상검사는 교량, 발전설비 및 석유화학플랜트 등의 각종 다양한 구조물들의 안전성 확보를 위한 내부결함 및 손상평가를 위하여 일반적으로 폭넓게 사용되고 있다. 초음파를 이용한 데 파괴 평가 기술은 각종 구조물에 존재하는 내부결함에 의한 산란신호를 통해 건전성을 평가하는 기법이므로 결함의 신뢰성 녹은 정량적 평가를 위해서는 결함으로부터의 초음파 산란산호특성에 대한 기본적 이해가 필수적이며 따라서 이를 위한 모델링 수치해석 연구가 요구된다. 본 연구에서는 동탄성 경계요소법을 이용하여 무한체내에 존재하는 다중기공 결함에 의한 초음파 수평횡파의 근거리 산란특성에 대하여 결함의 형상과 결함사이의 상호작용을 고려하여 해석하였다. 본 연구에서 얻어진 결과들은 결함검출 민감도의 개선 및 역변환 해석에 의한 정량적 비파괴 평가에 큰 도움을 줄 수 있다.

두개안면골 결손부 재건에 있어서 티타늄 그물판의 임상적 유용성 (Clinical Usefulness of Titanium Mesh in Reconstruction of the Craniofacial Bone Defects)

  • Purpose: Reconstruction of the craniofacial defects can be carried out with autogenous tissues, allogenic implants, or alloplastic materials. Titanium mesh systems have been used for bony reconstruction in non load-bearing areas. They offer several advantages: immediate availibility without any donor site morbidity, easy handling, stable 3-D reconstruction, and low susceptibility to infection. The aim of this study is to evaluate the usefulness and complications of titanium mesh system in the reconstruction of the craniofacial defects. Methods: From Jan. 2000, to Dec. 2004, we performed reconstruction of craniofacial bone defects in 21 patients who had benign or malignant tumor and fracture events in the cranium, orbit, nasal bone, maxilla, zygoma and the mandible. The size of the defects ranged from $1.0{\times}1.5cm$ to $12{\times}10cm$. Two different mesh systems, micro-titanium augmentation mesh and dynamic mesh was used for bony reconstruction in non load-bearing areas. The patients were evaluated from 1 to 4 yrs clinically and radiographically with a mean follow up period of 1.5 yrs. Results: There were no serious complications, including wound infection, foreign body reaction, exposures or loos of the mesh, central infection and pathologic findings of bone around mesh exception of one patient, who had expired of skull base tumor recurrence. Long-term stability of the reconstructions and the overall functional and aesthetic outcome was excellent. Conclusion: Our experiences demonstrate that the Titanium mesh system is a relatively safe and efficient method in the craniofacial reconstruction and have broadens our choices of therapeutic procedures in the craniomaxillofacial surgery.

고온 열처리에 의한 결정결함의 재용해 (The annihilation of the flow pattern defects in CZ-silicon crystal by high temperature heat treatment)

  • 규소 결정의 용융 온도 근처인 $1350^{\circ}C$에서 Ar과 $O_{2}$gas를 이용하여 규소 wafer의 열처리시 vacancy ty[e 결함의 거동에 대해 알아보았다. 이 열처리에서는 wafer의 표면보다 wafer내부에서 결함의 용해속도가 매우 높음을 확인하였다. 이는 $1350^{\circ}C$에서는 규소내의 평형산소농도가 대부분의 CZ silicon에서의 산소농도보다 높아 산소의 understaturation현상과 silicon interstitial농도의 영향에 기인된 것으로 예상된다. 열처리 분위기의 영향을 알아보기 위하여 Ar과 $O_{2}$ 분위기에서 열처리한 결과 vacancy type 결함의 용해속도는 wafer의 내부에서는 차이가 없었고, wafer의 표면에서는 Ar이 $O_{2}$의 경우보다 결함의 용해속도가 높았다. $O_{2}$의 경우에는 표면산화막 성장시 유입된 silicon interstitial의 농도가 높아 결함의 용해속도가 떨어지는 것으로 판단된다. 이는 기존 연구에서 예상된 silicon interstitial이 vacancy cluster로 알려진 결정결함의 제거에 기여한다는 예상과는 상반된다. 본 연구의 결과 silicon interstitial의 존재는 void외부 산화막의 용해속도를 늦추어 결함 용해속도를 떨어뜨리는 것으로 예상된다.

고령자 주거지원을 위한 주택시설설비결함과 주거만족에 관한 연구 - 전북지역의 노인가구를 중심으로 - (Housing Defects and Housing Satisfaction for Supporting to the Aged - Emphasis on the Jeonbuk Aged Households -)

  • This paper investigates housing defects and housing satisfaction of the aged according to the personal characteristics and their housing environment. Personal interview was made on the 280 senior persons living in Jeonbuk area. Research results are following. Low levels were found in education, self privately-owned property, monthly income. Most of them live in a detached house that is outdated and obsolete. The facility defects of housing were the most serious level in case of the female senior citizen who lives alone. Their house aging was old but had the high deviation among households. Main influencing factors on facility defects of housing were perceived economic status and ownership of house, the next was age of the aged. Main influencing factors on the housing satisfaction were perceived economic status. Housing satisfaction was higher, the better economic level of the aged was, male than female, the less the age was, in case of hot-water usage in bathroom, owner of houses, no difference in floor level, larger storage space of kitchen was. According to the empirical results, we propose the supporting policy of hosing safety for the aged in order to reduce the risk and social cost. Especially the rate of population aging, the aged household, and the detached houses are higher in Jeonbuk rural areas than the other area. Housing facility defects of the elder who lives alone and house aging of theirs are serious level. Local government should have supporting system that considers local difference and characteristics of the aged housing.

현장타설말뚝의 네킹 결함 평가를 위한 전자기파의 적용성 연구 (Application of Electromagnetic Wave for Evaluating Necking Defects in Bored Piles)

  • 본 연구의 목적은 현장타설말뚝에 발생한 네킹 결함을 평가하기 위해 전자기파의 적용성을 검증하는데 있다. 본 연구에서는 네킹 결함이 있는 직경 150mm, 길이 270mm의 소형 모형 말뚝에 대해 실험을 수행하였다. 말뚝에 모사된 결함은 총 4개이며, 이 중 두 개의 결함은 모형 말뚝의 서로 다른 면의 상부와 하부에 각각 모사되었으며, 나머지 두 개의 결함은 같은 면의 상부와 하부에 모사되었다. 전선을 철망을 이루고 있는 스테인리스 재질의 철사를 따라 설치하여 두개의 도체로 구성된 전송선로를 구성하였으며, time-domain reflectometer를 이용하여 전자기파를 송 수신하였다. 실험 결과, 모형 말뚝의 결함부와 선단부에서 전자기파의 반사 신호가 나타났다. 측정된 반사 신호로부터 결함의 위치를 산정한 결과, 실제의 결함 위치와 거의 같게 산정되었다. 본 연구의 결과는 전자기파가 현장타설말뚝의 네킹 결함을 평가하는데 있어 유용한 기법이 될 수 있음을 보여준다.

Four-week histologic evaluation of grafted calvarial defects with adjunctive hyperbaric oxygen therapy in rats

  • Purpose: The aim of this study was to characterize the healing in the grafted calvarial defects of rats after adjunctive hyperbaric oxygen therapy. Methods: Twenty-eight male Sprague-Dawley rats (body weight, 250-300 g) were randomly divided into two treatment groups: with hyperbaric oxygen therapy (HBO; n=14) and without HBO (NHBO; n=14). Each group was further subdivided according to the bone substitute applied: biphasic calcium phosphate (BCP; n=7) and surface-modified BCP (mBCP; n=7). The mBCP comprised BCP coated with Escherichia-coli-derived recombinant human bone morphogenetic protein-2 (ErhBMP-2) and epigallocatechin-3-gallate (EGCG). Two symmetrical circular defects (6-mm diameter) were created in the right and left parietal bones of each animal. One defect was assigned as a control defect and received no bone substitute, while the other defect was filled with either BCP or mBCP. The animals were allowed to heal for 4 weeks, during which those in the HBO group underwent 5 sessions of HBO. At 4 weeks, the animals were sacrificed, and the defects were harvested for histologic and histomorphometric analysis. Results: Well-maintained space was found in the grafted groups. Woven bone connected to and away from the defect margin was formed. More angiogenesis was found with HBO and EGCG/BMP-2 (P<0.05). None of the defects achieved complete defect closure. Increased new bone formation with HBO or EGCG/BMP-2 was evident in histologic evaluation, but it did not reach statistical significance in histometric analysis. A synergic effect between HBO and EGCG/BMP-2 was not found. Conclusions: Within the limitations of this study, the present findings indicate that adjunctive HBO and EGCG/BMP-2 could be beneficial for new bone formation in rat calvarial defects.

Oil Pipeline Weld Defect Identification System Based on Convolutional Neural Network

  • The automatic identification and classification of image-based weld defects is a difficult task due to the complex texture of the X-ray images of the weld defect. Several depth learning methods for automatically identifying welds were proposed and tested. In this work, four different depth convolutional neural networks were evaluated and compared on the 1631 image set. The concavity, undercut, bar defects, circular defects, unfused defects and incomplete penetration in the weld image 6 different types of defects are classified. Another contribution of this paper is to train a CNN model "RayNet" for the dataset from scratch. In the experiment part, the parameters of convolution operation are compared and analyzed, in which the experimental part performs a comparative analysis of various parameters in the convolution operation, compares the size of the input image, gives the classification results for each defect, and finally shows the partial feature map during feature extraction with the classification accuracy reaching 96.5%, which is 6.6% higher than the classification accuracy of other existing fine-tuned models, and even improves the classification accuracy compared with the traditional image processing methods, and also proves that the model trained from scratch also has a good performance on small-scale data sets. Our proposed method can assist the evaluators in classifying pipeline welding defects.
