Epigenetic Field for Cancerization

  • Epigenetic alterations, represented by aberrant DNA methylation, are deeply involved in human cancers. In gastric cancers, tumor-suppressor genes are inactivated more frequently by promoter methylation than by mutations. We recently showed that H. pylori infection, a potent gastric carcinogenic factor, induces methylation of specific genes in the gastric mucosae. When the methylation levels were analyzed in the gastric mucosae of healthy volunteers, cases with a single gastric cancer, and cases with multiple gastric cancers, who have increasing levels of risks for gastric cancers, there was a significant increasing trend in the methylation levels among the individuals without current H. pylori infection. This finding unequivocally showed the presence of an epigenetic field for cancerization. The degree of the field defect was measured more conveniently using methylation levels of marker genes than using those of tumor-suppressor genes. The presence of an epigenetic field for cancerization has been indicated for liver, colon, Barrett's esophageal, lung, breast, and renal cancers. Since decreased transcription is involved in the specificity of methylated genes, it is likely that specific genes are methylated according to carcinogenic factors. These findings emphasize the usefulness of DNA methylation as a marker for past exposure to carcinogens and future risk of cancer development.

Treatment of Aplasia Cutis Congenita on Scalp using Hyalomatix$^{(R)}$: A Case Report (하이알로매트릭스를 이용한, 두개골결손을 동반한 선천성피부무형성증의 치료: 1례 보고)

  • Purpose: Aplasia Cutis Congenita (ACC) is a rare disease characterized by the focal defect of the skin at birth, frequently involving scalp, but it may affect any region of the body. There are no etiology known but some conditions such as intrauterine vascular ischemia, amniotic adherences and viral infections are associated. The ideal treatment for the ACC is not known. Superficial and relatively small sized defects (< $3{\times}5\;cm$) may heal spontaneously and large defects related with risks of infection and bleeding may require aggressive surgical treatment. Hyalomatrix$^{(R)}$ is a bilayer of an esterified hyaluronan scaffold beneath a silicone membrane. It has been used as a temporary dermal substitute to cover deep thickness skin defect and has physiological functions derive from the structural role in extracellular matrix and interaction with cell surface receptor. This material has been used for the wound bed pre-treatment for skin graft to follow and especially in uncooperative patient, like a newborn, this could be a efficient and aseptic way of promoting granulation without daily irritative wound care. For this reason, using Hyalomatrix$^{(R)}$ for the treatment of ACC was preferred in this paper. Methods: We report a case of a newborn with ACC of the vertex scalp and non-ossified partial skull defect. The large sized skin and skull defect ($6{\times}6\;cm$) was found with intact dura mater. No other complications such as bleeding or abnormal neurologic sign were accompanied. Escharectomy was performed and Hyalomatrix$^{(R)}$ was applied for the protection and the induction of acute wound healing for 3 months before the split-thickness skin graft. During the 3 months period, the dressing was renewed in aseptic technique for every 3 weeks. The skin graft was achieved on the healthy granulation bed. Results: The operative procedure was uneventful without necessity of blood transfusion. Postoperative physical examination revealed no additional abnormalities. Regular wound management was performed in out-patient clinic and the grafted skin was taken completely. No other problems developed during follow-up. Conclusion: Hyalomatrix$^{(R)}$ provides protective and favorable environment for wound healing. The combination of the use of Hyalomatrix$^{(R)}$ and the skin graft will be a good alternative for the ACC patients with relatively large defect on vertex.

The Use of Pedicled Buccal Fat Pad Graft in Dental Surgery: Case Reports (치과 수술에서 유경 협지방대의 이용: 증례보고)

  • Euccal fat pad is special fat tissue which is different from subdermal fat. Anatomically, buccal fat pad is easy to harvest in the course of dental surgery procedure. In 1802, it was introduced by Bichat, Since Egyedi used buccal fat pad flap for the closure of oro-antral fistula and oro-nasal fistula, it has been widely used as an alternative method for the reconstruction of small to medium-sized intraoral defects in oral and mzxillofacial surgery. Kim et al. reported successful results in the all cases they applied buccal fat pad for the reconstruction of intraoral defect from their 31 months follow-up data. Because intraonal wounds are difficult to complete the layered suture and there are high risks of infection related with wound dental implant surgery, double layer closure using some kind of local flaps or other procedure is recommended. So we are to introduce the useful applications of the pedicled buccal fat pad in the dental surgery procedure from the various case presentations.

  • In this paper, we proposed a prioritization method of test cases using a value estimation model of test sets, that are key elements for highly effective software testings as well as involve a large cost factor in software developments and maintenances. Based on previous studies, our idea includes introducing some practical factors of the test case prioritization which critically influence the value of a test case: Relative values of test sets before and after the test running, Average value of these two relative values, Severity of the defect, Risks that are covered, Frequency of use, Change related values, Systematic elicitations, etc. Finally we discussed the usefulness and the expected effects of the proposed scheme.

Proposing a Method for Robustness Index Evaluation of the Structures Based on the Risk Analysis of Main Shock and Aftershock

  • Investigating remained damages from terrible earthquakes, it could be concluded that some events including explosion because of defect and failure in the building mechanical facilities or caused by gas leak, firing, aftershocks, etc., which are occurred during or a few time after the earthquake, will increase the effects of damages. In this paper, by introducing a complete risk analysis which included direct and indirect risks for earthquake (the main shock) and aftershock, the corresponding robustness index was created that called as "robustness index sequential critical events risk-based". One of the main properties of the intended robustness index is using progressive collapse percentage in its evaluation. Then, in a numerical example for a 4-storey moment resisting steel frame structure, a method is presented for obtaining all effective parameters in robustness index evaluation based on the intended risk and at last its results were reported.

  • The development of fetal surgery has led to promising options for many congenital malformations, such as congenital diaphragmatic hernia (CDH), obstructive uropathy, twin-to-twin transfusion syndrome (TTTS), and sacrococcygeal teratoma. However, preterm labor (PTL) and premature rupture of membranes continue to be uniquitous risks for both mother and fetus. To reduce maternal morbidity and the risk of prematurity, minimal access techniques were developed and are increasingly employed recently. Lift-threatening diseases as well as severely disabling but not life-threatening conditions are potentially amenable to treatment. Recently, improvement of video-endoscopic technology has boosted the development of operative techniques for feto-endoscopic surgery, which has been demonstrated to be less invasive than the open approach. Fetal surgery for repair of cleft lip and palate, a congenital anomaly which is not life threatening, is inappropriate until such time that the benefits are shown to outweigh the risks of both the procedure itself and preterm delivery. Further animal studies will be needed before intrauterine surgery for humans should be considered. For the better understanding of recent techniques and complications associated with fetal intervention of congenital facial defect patients, we reviewed recent related articles about the current knowledge and new perspectives of experimental fetal fetal surgery in the cleft lip and palate defects.

  • Cracks on the asphalt road surface can affect the speed of the car, the consumption of fuel, the ride quality of the road, and the durability of the road surface. Such cracks in roads can lead to very dangerous consequences for long periods of time. To prevent such risks, it is necessary to identify cracks and take appropriate action. It takes too much time and money to do it. Also, it is difficult to use expensive laser equipment vehicles for initial cost and equipment operation. In this paper, we propose an effective detection method of road surface defect using ROI (Region of Interest) setting and cany edge detection method using UAV image. The results of this study can be presented as efficient method for road surface flaw detection and maintenance using UAV. In addition, it can be used to detect cracks such as various buildings and civil engineering structures such as buildings, outer walls, large-scale storage tanks other than roads, and cost reduction effect can be expected.

  • Recently the necessity of study on optical measuring method using laser to detect the pipeline's defect in nuclear facilities, chemical industries and power plants has been increased. Because laser light can be delivered to a remote area without any difficulties, the application of laser in many industries can solve several difficulties from the limitation of access in danger area and reduce the risks of workers. Therefore, we applied a new experimental technique to the measurement of internal defects in pressure vessels with the combination of shearography and image processing technique and detected the internal cracks of pressure vessels in the former paper. In this paper, we used the same optical system as in the former study and found the optimum shearing magnitude by comparing the real length of specimen with experimental results. A variety of conditions were applied to certify the validity of this method. Actually, several specimens which have different lengths and depths were used in this experiment under the three diverse pressure. Consequently, we have carried out this experiment to determine the limit of measurement ability with analyzing errors.

Developing a Quality Risk Assessment Model for Product Liability Law (제조물 책임(PL)법 대응을 위한 품질 리스크 진단 모델 개발)

  • As the global uncertainty of manufacturing has increased and the quality problem has become global, the recall has become a fatal risk that determines the durability of the company. In addition, as the convergence of PSS (product-service system) product becomes common due to the development of IT convergence technology, if the function of any part of hardware or software does not operate normally, there will be a problem in the entire function of PSS product. In order to manage the quality of such PSS products in a stable manner, a new approaches is needed to analyze and manage the hardware and software parts at the same time. However, the Fishbone diagram, FTA, and FMEA, which are widely used to interpret the current quality problem, are not suitable for analyzing the quality problem by considering the hardware and software at the same time. In this paper, a quality risk assessment model combining FTA and FMEA based on defect rate to be assessed daily on site to manage quality and fishbone diagram used in group activity to solve defective problem. The proposed FTA-FMEA based risk assessment model considers the system structure characteristics of the defect factors in terms of the relationship between hardware and software, and further recognizes and manages them as risk. In order to evaluate the proposed model, we applied the functions of ITS (intelligent transportation system). It is expected that the proposed model will be more effective in assessing quality risks of PSS products because it evaluates the structural characteristics of products and causes of defects considering hardware and software together.

  • As developers and testers today we all suffer from increasing project complexity, the risks of late defect discovery, repetitive manual processes, and the risk of release delays. In this paper, we introduce an effective framework for automated testing to help solve such problems. Those that are new to testing do not need to delve into complex automation tools or test scripts. This framework helps automate the distribution, execution and results analysis of test cases. It also aids communication among the various stakeholders, using tables for representing tests and for reporting the results of automatically checking those tests. This paper describes the practices and benefits of using the proposed framework.
