Effect of Resistant Starch on Body Weight and Defecation in Loperamide-induced Constipated Rats (저항전분 투여가 loperamide 유도 변비 쥐의 체중변화와 배변상태에 미치는 영향)

  • The present study was conducted to evaluate the effects of resistant starches (RS2, RS3) on weight change and defecation states in rats with constipation induced by loperamide. Sixty male Sprague-Dawley rats were divided into the following six groups: normal control-5% cellulose (NC), constipated-5% cellulose (CC), constipated-5% pectin (CP), constipated-5% RS2 (CR2), constipated-8% RS2 (CR2-H) and constipated-5% RS3 (CR3). The animals were fed AIN76-based experimental diets for 5 weeks. Supplementation of RS2 and RS3 resulted in significantly lower feed efficiency ratios and body weight gain as compared to cellulose supplemented group. Large intestine length was significantly longer in the resistant starch and cellulose groups than in the pectin group. Relative fecal weight to feed intake was significantly higher in the RS3 resistant starch group than in the pectin group, and the appearance of feces was similar to that of the cellulose group. Supplementation of RS3 or cellulose significantly shortened gastrointestinal transit time in the constipated rats as compared to pectin. The results of the present study demonstrated that resistant starch supplementation, especially RS3, may help in depress body weight gain and alleviate constipation through an increase of fecal weight and shortening gastrointestinal transit time in constipated animals.

Korean medicine treatment, including Nangan-jeon, of chronic constipation and abdominal pain in Lambert-Eaton myasthenic syndrome : A case report (Lambert-Eaton myasthenic syndrome 환자의 만성 변비 및 복통에 대한 난간전을 포함한 한방 치료 증례보고)

  • This case study reports the effects of complex Korean medicine treatment, including Nangan-jeon, on Lambert-Eaton myasthenic syndrome (LEMS) accompanied by chronic constipation and abdominal pain as the main symptoms. A 39-year-old woman diagnosed with LEMS with major symptoms, including chronic constipation and acute abdominal pain, received Western treatment. The treatment efficacy was weak and symptoms recurred, so the patient received outpatient treatment and 13 days of hospitalization for active Korean medicine treatment, including Nangan-jeon. During outpatient treatment and hospitalization, defecation frequency and the numeric rating scale (NRS) for abdominal pain and abdominal cold feeling were measured. The NRS for abdominal pain and cold decreased from 8 at admission to 3 and 0, respectively, at discharge. Defecation frequency increased significantly from once or twice a month to once every 2-3 days during hospitalization. This study results that complex Korean medicine treatment, including Nangan-jeon may be useful for treating patients who mainly complain of autonomic neurological symptoms, among patients diagnosed with LEMS. In addition, it is believed that it could be basic data applicable to more LEMS patient treatment cases.

Functional Independence Measure of Disabled Persons Living at Home in Rural Community (농촌지역 재가장애인의 일상생활 수행능력 실태조사)

  • Although the availability of home care rehabilitation services have been greatly increased since community-based rehabilitation was introduced to Korea, there is still a dearth of studies investigating the performance level of ADL (activities of daily living) for the physically disabled in the rural areas. The purposes of this study were to investigate the ADL performance level of disabled persons living at home in the rural areas of Wonju city, Korea, using FIM (functional independence measure) and to identify the specific areas of the ADL to be trained or evaluated by physical therapists or occupational therapists. 298 disabled people were interviewed by 10 physical therapy students. Analysis of the interviews indicated: 1) Forty seven point seven percent of the respondents were elderly-disabled persons whose ages were 61 years old or above, and 69.5% of respondents has the history of chronic disability period of 5 years or more. 2) FIM score of bathing, and stairs climbing showed severe dependent trend 3) FIM scores of self-feeding, urination, defecation, and comprehension ability were mildly dependent. These results revealed that functional evaluation/treatment for discharging from hospitals to the rural areas should be emphasized on the specific ADL performance areas such as bathing, and stairs climbing.

The Decision Tree to Analyze the Cases' Ordinary Symptoms Prescribed Yeoldahanso-tang and Taeeumjowi-tang·Choweseuncheng-tang (열다한소탕과 태음조위탕·조위승청탕의 소증 분석을 위한 의사결정나무 구성)

  • Objectives The purpose of this study is to analyze the decision making process of prescribing Yeoldahanso-tang and Taeeumjowi-tang Choweseuncheng-tang using decision tree. Methods We used collected the prospective clinical data of TE type from September 2012 to July 2015. In this study, we used gender, BMI, blood pressure, pulse and clinical symptoms (digestion, sweat, defecation, urination, sleep, physical status, emotion, heat-coldness, water consumption, facial color) as variables. Decision trees were analyzed using open source R version 3.3.2. Results & Conclusions We found that the decision trees differed among institutions. However, in all institutions, it was found that stool type (ordinary symptom), urine frequency (ordinary and present symptom) and anxiety (ordinary symptom) were important in the decision of prescription. Besides, clinical informations such as sex, Body Mass Index and blood pressure affected the prescription decision.

A Study on Child-Care in Rural Korea : Feeding, Weaning and Toilet-Training (농어촌지역의 양육방법에 관한 연구 - 수유·이유·배변훈련을 중심으로 -)

  • The purpose of this study was the description of general trends in feeding, weaning and toilet training in agricultural and fishing communities in Korea, where the traditions are better preserved; thereby to present useful material for improving child rearing practices. The results of this study showed that: 1) Breast-feeding is the dominant form of nursing. The mother nurses freely whenever the baby wants to be fed. The mother caresses the baby while she nurses him. The nursing period is relatively long, up to 1 and a half years. 2) Generally, the time of weaning starts late - between 9 months and 18 months. Many mothers apply a bitter tasting solution to their nipples to aid weaning. The many things give a baby foods, the recipe for which the acquire knowledge from his family and acquaintances. 3) Toilet training generally starts after 1 full year. Potties are commonly used. The infant is reminded of its error whenever it makes a mistake and cleanliness is stressed. Urination and defecation are taught to be dirty things. In summary, discipline is not strict enforced but given naturally as the infants grow: This was interpreted in terms of the traditional way of life and geographical characteristics.

ICF Core Sets for Measuring Functional Status of Acute Stroke Patients (급성기 뇌졸중 환자의 기능상태 측정을 위한 ICF 핵심지표)

  • Purpose: This study was conducted to extract international classification of functioning, disability, and health (ICF) core sets for measuring functional status in acute stroke patients, and to evaluate clinical applicability of the core sets. Methods: A set of 22 ICF items on functional status in acute stroke patients were extracted from the Korean general ICF core sets and ICF core sets for stroke patients. The extracted ICF functional items were assessed at the time of admission and discharge among 100 stroke patients who were admitted in a university hospital. Results: Comparing to functional status at admission of acute stroke patients, the overall functional status at discharge was improved. However, functions on defecation, skin protection, and relationship with immediate family at the time of discharge were not significantly changed. Conclusion: The set of ICF functional items identified in this study may be reliable and valid to assess acute stoke patients' body functions, activities and participation and environmental factors in the holistic and comprehensive nursing context. Nursing interventions on bowel elimination and skin protection for acute stroke patients need to be developed.

  • Purpose: The purpose of this study was to explore and describe the distress of the patients with ostomy. Methods: A descriptive research design was adopted for data collection and analysis. Six patients with ostomy participated in the study. The consent from the participants was obtained for ethical protection. Data were collected from July 31, 2009 to January 10, 2010 using in-depth interview technique. Krippendorff's content analysis method was utilized for data analysis. Results: Eight categories and 26 themes were extracted from the data which illustrated the lived distress of the patients with ostomy. The categories were "do not have any freedom to eat whatever I wish to eat", "uncontrolled defecation"tomy", "can not live with comfort", "easily take a pessimistic view", and "see how the wind blows in daily family life". Conclusion: Ostomates were affected by the distress of having an ostomy in their physical, psychological, social and spiritual life. Individualized and continued nursing education program has to be developed in hospital and community settings in order to improve the quality of life of the ostomates.

Failure of Urination and Defecation after Intrathecal Phenol-glycerin Block for Management of Chronic Perineal Pain -A case report- (만성회음부 통증 치료를 위한 지주막하 페놀글리세린 차단 후 발생한 배변 및 배뇨장애 -증례 보고-)

  • Intrathecal phenol-glycerin block is a potent tool in stepwise approach to intractable perineal pain. At outpatient clinic, intrathecal phenol-glycerin block have been frequently used to control the cancer pain, as this procedure does not require special equipment. However, undesirable effects may occur with any given intrathecal injection. Although most side effects do not last long, they deserve well to prevent. Recently, we have been controlled the perineal and perianal cancer pain with intrathecal phenol-glycerin block without any specific complications. However, one patient we managed experienced that one patient, with this procedure for chronic non-malignant perineal pain control experience gait disturbance and one-week of voiding and defication difficulties. This research reports these practices, including the review of literature.

  • In July, 2000, A 71-year-old man complained of these symptoms: frequent defecation, urgent need to defecate, fecal leakage, anal pain, flatus, and diarrhea after surgery for colorectal cancer. The symptoms are caused specifically by injury to the bowel and sphincter function. The patient suffered physically and psychologically; but, there was no special treatment in western medicine. Therefore, we treated him with electrical acupuncture for the management of sphincter tone and the alleviation of anal pain. We also treated him with herbal medicine for the recovery of strength and gastro-colic function. As a consequence of these treatments, the bowel disorders improved immediately and successfully, even though there were some limitations. So, We report this case with a brief review of related literatures.

Pyogenic Spondylitis with Diffuse Spinal Epidural Abscess - A Case Report - (미만성 척수 경막외 농양을 동반한 화농성 척추염 - 증 례 보 고 -)

  • We report a case of pyogenic spondylitis on L2 and L3 with diffuse epidural abscess up to T4 to L3 and large psoas abscess. A forty-nine-year old male was presented with progressive back pain, left flank pain and ab-dominal distention, weakness of the both legs and voiding and defecation difficulty during last 2 months. Initially multiple coronal hemilaminectomies from T4 to T12 were done for the treatment of diffuse thoracic epidural ab-scess. Then second operation via left retroperitoneal approach was performed for lumbar spondylitis and psoas abscess on third day after initial operation. After removal and curettage of pyogenic psoas and epidural abscess and spondylitis (L2-L3), iliac bone grafting with Keneda instrumentation from L1 to L4 was done simultaneously. Postoperative course has been unevenful without recurrent infection. The literature on diffuse epidural and large psoas abscess with pyogenic spondylitis are reviewed and instrumentation for stabilization of pyogenic spondylitis is also discussed.
