근육내 자극요법 후 목빗근내 농양의 치험례 (A Case of Intramuscular Abscess in the Sternocleidomastoid Muscle after Intramuscular Stimulation (IMS))

  • Purpose: Intramuscular stimulation (IMS) shows good results in the treatment of chronic pain patients who did not respond to other treatments such as oral analgesics, trigger point injection, nerve block and epidural steroid injection. But, IMS procedure especially, patients with diabetes mellitus (DM) has sometimes serious problem. So, we present a very rare case of intramuscular abscess in the sternocleiomastoid muscle after IMS with literature review. Methods: A 66 year old male visited our department 7 days after IMS in the neck. His premorbid conditions and risk factors of deep neck infection was DM and old age. Computed tomographic scans of the head and neck region were performed in this patient: signs of deep neck infection, were seen enhanced abscess in the sternocleidomastoid muscle, cellulitis overlying tissue of the neck, and air bubbles involved muscle. Necrotic wound was excised serially and we treated this with the Vacuum-assisted closure (VAC, Kinetics Concepts International, San Antonio, Texas) system device. After appropriately shaping the sponge in the pockets, continuous negative pressure of 125 mmHg was applied. The VAC therapy was utilized for a period of 12 days. Results: We obtained satisfactory results from wide excision, drainage of the abscess with the VAC system, and then primary closure. The postoperative course was uneventful. Results: We suggest that many of the infectious complications may be preventable by strict adherence to aseptic techniques and that some of the other complications may be minimized by refining the techniques with a clear understanding of the medical disorders of patients. And, the refined technique using the VAC system can provide a means of simple and effective management for the cervical intramuscular abscess, with better cosmetic and functional results.

머리-목굽힘 운동 시 앞쪽머리자세와 목굽힘근 근두께 간의 상관성 연구 (The Correlation Between Forward Head Position and Neck Flexor Thickness During Cranio-Cervical Flexion Exercise)

  • Background: As the increase of forward head position, we studied the change of thickness of deep cervical flexor (DCF) compared of with sternocleidomastoid (SCM). we measured and analysed the change of thickness of the neck flexor for forward head position and cranio-cervical flexion exercise (CCFE). Methods: Using a cross-sectional design, we conducted the study selecting 35 healthy adults (12 males, 23 females). We measured the craniovertebral angle (CVA), instructed them to perform the CCFE, and measured the DCF and SCM using ultrasonography during the contraction and relaxation period. Results: Intra-reliability of SCM is .96, longus capitis is .92 and longus coli is .97. we compared according to the change of forward head position, Correlation of DCF is .841, and DCF/SCM is .754 by significant positive correlation. At the comparison of CCFE and Resting muscle thickness, SCM and DCF is .00, DCF/SCM is .68. Conclusion: There is a strong positive correlation between the change amount of DCF and DCF/SCM as the increase of CVA.

Comparison of the Thickness of the Neck Flexor Muscles of Subjects With and Without a Forward Head Posture on the Two Initial Head Positions During Cranio-Cervical Flexion Exercise

  • This study compared the effects of the initial head position (i.e., a HHP versus a relaxed head position) of subjects with and without a FHP on the thickness of the deep and superficial neck flexor muscles during CCF. The study recruited 6 subjects with a FHP and 10 subjects without a FHP. The subjects performed CCF in two different head positions: a HHP, with the head aligned so that the forehead and chin formed a horizontal line, and a relaxed head position (RHP), with the head aligned in a self-selected comfortable position. During the CCF exercise, the thickness of the longus colli (LCo) and the thickness of the sternocleidomastoid (SCM) were recorded using ultrasonography. The thickness of each muscle was measured by Image J software. The statistical analysis was performed with a two-way mixed-model analysis of variance. The thickness of the SCM differed significantly (p<.05) between the subjects with and without FHP. According to a post $h^{\circ}C$ independent t-test, the change in thickness of the SCM increased significantly during CCF in the subjects with FHP while adopting a HHP compared to that in the subjects without FHP. The change in thickness of the SCM was not significantly different between the two positions in subjects without FHP, and there was no significant change in thickness of the LCo muscle during the CCF exercise according to the initial position in both subjects with and without FHP. The results suggest that CCF should be performed in RHP to minimize contraction of the SCM in subjects with a FHP.

Neck Pain and Functioning in Daily Activities Associated with Smartphone Usage

  • Purpose: The aim of the study was to investigate neck posture, range of motion, muscle endurance and self-report of pain and disability in smartphone users. Methods: Seventy-eight university student volunteers, aged between 18 and 30 years (mean age 23.2), were assessed for: a head-neck posture by measuring cranial vertical angle, neck range of motions using cervical range of motion device, and a deep neck flexor endurance using a stabilizer. Finally, subjects were asked about their neck pain and completed disability questionnaires, ie, Short Form McGill Pain Questionnaire, Neck Disability Index, and World Health Organization Disability Assessment Schedule 2.0. Results: Thirty-eight subjects experienced recurrent neck pain with/without upper limb pain (neck pain group) and 40 reported no current neck pain with/without upper limb pain (no neck pain group). Differences were found between groups on pain and disability questionnaires. Subjects with neck pain had significantly higher disability scores than those of no neck pain group. However, there were no differences observed between groups in a head-neck posture, neck range of motions, and deep neck muscle endurance time. The smartphone usage time was negatively correlated with neck pain intensity and disability score whereas it had positive relationship with flexibility and posture. Conclusion: Group differences were observed as lower capacity not only for neck specific daily activities but for general functioning in daily routine when the neck pain and no neck pain groups were compared. Therefore, functioning in daily activities should be investigated as prevention for further developing neck pain in smartphone users.

거북목을 가진 사람에게 메켄지 폄운동과 어깨안정화운동이 앞쪽머리자세에 미치는 영향 (Effect of Shoulder stabilization exercise and McKenzie Exercise on Forward Head Posture)

  • 본 연구의 목적은 거북목을 가진 사람에게 메켄지 폄운동과 어깨안정화운동이 앞쪽머리자세에 미치는 영향에 대해 알아보는 것이다. 연구대상자는 30명의 거북목을 가진 평범하고 건강한 사람들로 이루어져 있다. 대상자는 어깨안정화운동과 메켄지 폄운동으로 나누어(인원=30, 남자 15명, 여자 15명) 무작위로 나누어 6주 동안 실시되었다. 본 연구는 마름근, 아래등세모근의 운동프로그램과 깊은목굽힘근, 위등세모근, 어깨올림근의 신장프로그램을 통한 목뼈의 머리척추각(Cervical Vertebra Angle) 값과 각 근육별 근전도의 차이를 알아보았다. 앞쪽머리자세와 경우에는 목과 등근육의 활성도가 감소하고 피로도는 증가함을 보였다. 이러한 결과는 목과 머리의 불안정한 상태를 안정시키기 위해서는 위등세모근, 깊은목굽힘근, 어깨올림근의 신장과 마름근, 아래등세모근의 운동을 통해 거북목의 개선에 긍정적인 변화를 줄 것으로 보인다. 이러한 근육들의 불균형한 수축과 과도한 긴장으로 근육의 경직과 피로가 누적되며 이러한 상태가 계속 유지되게 되면 만성화상태가 되어 근육의 불균형으로 인해 통증을 유발할 수 있으며, 피로의 누적으로 인하여 자극에 대한 반응과 근력을 약화시켜서 목 디스크가 발생할 확률이 높아질 것으로 사료된다.

복부와 경부 관통상 환자에 대한 임상적 고찰 (Clinical Analysis of Patients with Abdomen or Neck-penetrating Trauma)

  • Purpose: Recently, the change to a more complex social structure has led to an increased frequency of traumas due to violence, accident and so on. In addition, the severity of the traumas and the frequency of penetrating injuries have also increased. Traumas to cervical and abdominal areas, what are commonly seen by general surgeons, can have mild to fatal consequences because in these areas, various organs that are vital to sustaining life are located. The exact location and characteristics of the injury are vital to treating patients with the trauma to these areas. Thus, with this background in mind, we studied, compared, and analyzed clinical manifestations of patients who were admitted to Wonju Christian hospital for penetrating injuries inflicted by themselves or others. Methods: We selected and performed a retrospective study of 64 patients who had been admitted to Wonju Christian Hospital from January 2005 to December 2009 and who had cervical or abdominal penetrating injuries clearly inflicted by themselves or others. Results: There were 51 male (79.7%) and 13 female (20.3%) patients, and the number of male patients was more dominant in this study, having a sex ratio of 3.9 to 1. The range of ages was between 20 and 86 years, and mean age was 43.2 years. There were 5 self-inflicted cervical injuries, and 19 self-inflicted abdominal injuries, making the total number of self-inflicted injury 24. Cervical and abdominal injuries caused by others were found in 11 and 29 patients, respectively. The most common area involved in self-inflicted injuries to the abdomen was the epigastric area, nine cases, and the right-side zone II was the most commonly involved area. On the other hand, in injuries inflicted by others, the left upper quadrant of the abdomen was the most common site of the injury, 14 cases. In the neck, the left-side zone II was the most injured site. In cases of self-inflicted neck injury, jugular vein damage and cervical muscle damage without deep organ injury were observed in two cases each, making them the most common. In cases with abdominal injuries, seven cases had limited abdominal wall injury, making it the most common injury. The most common deep organ injury was small bowel wounds, five cases. In patients with injuries caused by others, six had cervical muscle damage, making it the most common injury found in that area. In the abdomen, small bowel injury was found to be the most common injury, being evidenced in 13 cases. In self-inflicted injuries, a statistical analysis discovered that the total duration of admission and the number of patients admitted to the intensive care unit were significantly shorter and smaller, retrospectively, than in the patient group that had injuries caused by others. No statistically significant difference was found when the injury sequels were compared between the self-inflicted-injury and the injury-inflicted-by-others groups. Conclusion: This study revealed that, in self-inflicted abdominal injuries, injuries limited to the abdominal wall were found to be the most common, and in injuries to the cervical area inflicted by others, injuries restricted to the cervical muscle were found to be the most common. As a whole, the total duration of admission and the ICU admission time were significantly shorter in cases of self-inflicted injury. Especially, in cases of self inflicted injuries, abdominal injuries generally had a limited degree of injury. Thus, in our consideration, accurate injury assessment and an ideal treatment plan are necessary to treat these patients, and minimally invasive equipment, such as laparoscope, should be used. Also, further studies that persistently utilize aggressive surgical observations, such as abdominal ultrasound and computed tomography, for patients with penetrating injuries are needed.

Deep neck flexor endurance in university students: normative data and reliability

  • Objective: Deep neck flexor muscle endurance (DNFET) is important to cervical pain patients. However, there is no normative data of the DNFET hold time of Korean university students. The purpose of this study was to provide normative data and the reliability of the DNFET times of Korean university students and to compare the DNFET hold times between male and female subjects. Design: Cross-sectional study. Methods: The participants included 39 male and 39 female students with no cervical pain. Each DNFET was measured while the subjects kept their chin tucked in while in a supine (hook-lying) position and with the head lifted 2.5 cm off the bed. The DNFET was conducted on each subject twice and the mean values were obtained. After each measurement, the participants rested for 5 minutes. Inter-rater reliability was measured by intraclass correlation coefficient (ICC) by three separate evaluators. Results: The inter-rater reliability was good, showing an ICC (2,3) value = 0.785 (95% confidence interval, 0.370-0.942). The DNFET time scores for men and women were $25.14{\pm}9.96$ seconds and $15.23{\pm}6.10$ seconds, respectively, in which the time scores of the men were significantly longer compared to women (p<0.05). Conclusions: Asymptomatic men displayed higher DNFET scores than women. This study can help clinicians test cervical function of patients and set an interventional goal. These findings serve as a basis that insists Korean women to increase their amount of physical activity.

Effects of Stabilization Exercise Applying Sling Neurac Method on Pain and Cervical Alignment in Young Adults with Forward Head Posture

  • 이 연구의 목적은 전방머리자세를 가진 젊은 성인을 대상으로 슬링뉴렉안정화운동이 중재방법이 통증 및 목뼈 앞굽음각, 중력중심선에 어떠한 영향을 미치는지 알아보고자 실시되었다. 연구의 대상자는 슬링뉴렉안정화운동그룹(SNSEG) 10명, 통제그룹(CC) 10명으로 선정하였으며, 8주간 주3회 70분간 실시하였다. 이 연구 결과 슬링뉴렉안정화운동은 통증은 감소(p<.001), 목뼈 앞굽음각은 증가(p<.001)와 중력중심선은 감소에(p<.01) 유의한 효과가 나타났다. 이상의 결과 종합해 보면 슬링뉴렉안정화운동은 속근육(LM)군의 고유수용성감각 개선 및 근활성화에 효과적이며, 대근육(GM)군과 공동활성화하여 앞굽은각과 중력중심선에 개선에 효과적인 것으로 나타났다. 따라서 깊은목굽힘근과 뒤통수밑근의 재활성화는 통증 조절과 자세 정렬에 중요한 요소로 전방머리자세 개선시킬 수 있는 효과적인 중재방법으로 제안한다.

A Novel Approach to Submandibular Gland Ptosis: Creation of a Platysma Muscle and Hyoid Bone Cradle

  • Submandibular gland ptosis is a common impediment to obtaining superior surgical aesthetic results in neck lift surgery. Techniques for suspending the submandibular gland have been proposed, but these procedures have the drawbacks of disturbing the floor of the mouth mucosa and periosteum. We present an approach of submandibular gland suspension for the treatment of gland ptosis by employing a platysma and hyoid bone fascia cradle. Our technique was performed on cadaveric models. The platysma muscle and hyoid bone cradle for submandibular gland ptosis was created on the left side of the neck in two cadavers. A submental incision with sharp dissection was performed to raise a supraplatysmal flap. A subplatysmal plane was developed until the submandibular gland was identified. Sutures were used to pexy the platysma to the hyoid bone periosteum and deep cervical fascia, tightening the overlying muscle and in turn elevating the submandibular gland. Submandibular gland ptosis must be corrected in order to achieve exemplary aesthetic results. Our approach of creating a cradle with the platysma and hyoid bone avoids the potential complications of previously described sling procedures, while still maintaining the integrity of the gland and surrounding tissues.

경부 통증 유무에 따른 심부 경부 굴곡근의 근력과 지구력 비교 (Strength and Endurance of the Deep Neck Flexors of Industrial Workers With and Without Neck Pain)

  • The purpose of this study was to investigate the strength and endurance of the deep neck flexor muscles in individuals with work-related neck pain. Subjects consisted of two groups: twenty industrial workers with neck pain and twenty age-matched healthy subjects. To evaluate the strength and endurance of deep cervical flexors, maximum voluntary contractile strength (MVCS) and a sustained time at sub-maximal voluntary contractile strength (SMVCS) (80% and 50% of MVCS) were measured using a pressure biofeedback unit and a stop watch in supine. The MVCS of deep neck flexor muscles was 29.67${\pm}$4.56 in neck pain group and 54.27${\pm}$6.78㎜Hg in normal group. The sustained time at 80% SMVCS was 12.42${\pm}$2.64 seconds and 55.12${\pm}$12.76 seconds in the groups with and without neck pain. The sustained time at 50% SMVCS was 25.40±5.88 seconds and 109.70${\pm}$31.50 seconds in the groups with and without neck pain. The difference of the lower jaw position was 16.75${\pm}$3.57㎜ and 23.03${\pm}$2.51㎜. The MVCS, endurance at the two sub-maximal levels and the difference of the lower jaw position were significantly greater in the group without neck pain than with neck pain (p$<$0.05). The findings indicate that the maximal strength and endurance of the deep neck flexors were decreased in the workers with neck pain compared to those without neck pain. Therefore, it is necessary to include strengthening and endurance exercises of the deep neck flexor muscles in therapeutic program of work-related musculoskeletal disorders involving neck pain.
