Core muscle Strengthening Effect During Spine Stabilization Exercise

  • Core spinal muscles are related to trunk stability and assume the main role of stabilizing the spine during daily activities; strengthening of core muscles around the spine can therefore reduce the chance of back pain. The objective of the study was to investigate the effect of core muscle strengthening in the spine during spine stabilization exercise using a whole body tilt device. To achieve this, a validated musculoskeletal (MS) model of the whole body was used to replicate the input motion from the whole body tilting exercise. An inverse dynamics analysis was executed to estimate spine loads and muscle forces depending on the tilting angles of the exercise device. The activation of long and superficial back muscles such as the erector spinae (iliocostalis and longissimus) were mainly affected by the forward direction (-40°) of the tilt, while the front muscles (psoas major, quadratus lumborum, and external and internal obliques) were mainly affected by the backward tilting direction (40°). Deep muscles such as the multifidi and short muscles were activated in most directions of the rotation and tilt. The backward directions of the tilt using this device could be carefully applied for the elderly and for rehabilitation patients who are expected to have less muscle strength. In this study, it was shown that the spine stabilization exercise device can provide considerable muscle exercise effect.

긴장성 근육통 증후군으로 인한 만성 경항통 환자에 대한 이완 요법 적용 1례(例) (The Report on Relaxation Therapy Application for Chronic Neck Pain with Tension Myositis Syndrome Trend)

  • 2005년 5월 자생한방병원 한방신경정신과에 입원한 환자에 대하여 긴장성 근육통 증후군의 관점에서의 접근을 통해 이완 요법치료를 실시하여 환자 동통 치료에 대하여 양호한 결과를 보였고 환자의 통증 회복 의지에 대한 심리 상태에도 변화가 있었다. 그 결과 다음과 같은 소견을 보였다. 1. 기질적인 원인이 있는 만성 경항부 통증 환자에서 환자의 심리적 상태를 고려한 이완요법을 병행하여 통증 지각 감소와 심리적 자신감 회복에 양호한 결과를 보였다. 2. 긴장성 근육통 증후군에서의 환자의 내재되어 있는 심리상태가 통증을 일으킨다고 보는 관점은 한의학적 관점에서는 기(氣)의 순환 장애에서 오는 기통증(氣痛症), 기울증(氣鬱症)으로 볼 수 있으며, 이완 요법은 환자 치료에 정서, 심리적인 측면을 고려한 점에서 이정변기요법(移精變氣療法)등과 같은 한방정신요법과 유사성을 찾아 볼 수 있다. 3. 이완 요법은 통증 치료에 있어서 심리적 이완, 안정을 도모하고, 통증에 대한 과도한 관심을 억제하여 통증 지각 완화에 도움이 된다.

The Ability of Muscle Functional MRI to Detect the Slight Effect of Exercise on Trunk Muscle Activity

  • Purpose: In this study, we provide a way to assess even a slight effect of exercise on trunk-muscle activity. Materials and Methods: Seven healthy male participants (mean age, 24.7 ± 3.2 years; height, 171.2 ± 9.8 cm; and weight, 63.8 ± 11.9 kg) performed 15 sets of an exercise with 20 repetitions of 90° hip and right-knee flexion while lying supine. The exercise intensity was measured using the 10-point Rating of Perceived Exertion Scale after the first and 15th sets of exercises. Although cross-sectional areas and functional T2 mapping using ultrafast imaging (fast-acquired muscle functional magnetic resonance imaging, fast-mfMRI) have been proposed for imaging to evaluate exercise-induced muscle activity in real time, no previous studies have reported on the evaluation of trunk-muscle activity using functional T2 mapping. As a method for assessing trunk-muscle activity, we compared functional T2 mapping using ultrafast imaging (fast-mfMRI) with cross-sectional areas. Results: Although the muscle cross-sectional areas were increased by the exercise, there was no significant difference at rest. On the other hand, for all sets, the changes in T2 were significant compared with those at rest (P < 0.01). These results demonstrate that T2, calculated from fast-mfMRI images can be used to detect even a small amount of muscle activity induced by acute exercise, which was impossible to do with cross-sectional areas. Conclusion: Fast-mfMRI, which can also display functional information with detailed forms, enabled non-invasive real-time imaging for identifying and evaluating the degree of deep trunk-muscle activity induced by exercise.

Impact of Applied Progressive Deep Muscle Relaxation Training on the Level of Depression, Anxiety and Stress among Prostate Cancer Patients: A Quasi-Experimental Study

  • Background: The aim of this study was to determine the impact of applied progressive muscle relaxation training on the levels of depression, anxiety and stress among prostate cancer patients. Materials and Methods: A quasi-experimental study was conducted at the University Malaya Medical Centre (UMMC) and Universiti Kebangsaan Malaysia Medical Centre (UKMMC) over six months. Prostate cancer patients from UMMC received the intervention and patients from UKMMC were taken as controls. The level of depression, anxiety and stress were measured using Depression, Anxiety Stress Scales - 21 (DASS-21). Results: A total of 77 patients from the UMMC and 78 patients from the UKMMC participated. At the end of the study, 90.9% and 87.2% of patients from the UMMC and UKMMC groups completed the study respectively. There were significant improvements in anxiety (p<0.001, partial ${\eta}^2$=0.198) and stress (p<0.001, partial ${\eta}^2$=0.103) at the end of the study in those receiving muscle training. However, there was no improvement in depression (p=0.956). Conclusions: The improvement in anxiety and stress showed the potential of APMRT in the management of prostate cancer patients. Future studies should be carried out over a longer duration to provide stronger evidence for the introduction of relaxation therapy among prostate cancer patients as a coping strategy to improve their anxiety and stress.

흡연자와 비흡연자의 앉은 자세에서 일어서는 동작 시 배근육 활성도 차이 (Differences in the Activation of Abdominal Muscles During Sit-to-Stand between Smokers and Non-smokers)

  • Purpose: This study aimed to determine whether the abdominal muscles, which are the major lumbar stabilizers along with the respiratory muscles, are affected by smoking. We compared abdominal muscle activity between smokers and non-smokers during the sit-to-stand movement. Methods: A total of 28 healthy adult males (14 smokers and 14 non-smokers) in their 20s-30s voluntarily participated in the study. The subjects performed the sit-to-stand movement, and then their abdominal muscle activity was measured. The surface electromyography system was employed to measure the maximal voluntary isometric contraction (MVIC) values of the subjects' rectus abdominis, external oblique abdominal, internal oblique abdominal, and transversus abdominis muscles. Then, the values were quantified into %MVIC. Results: The activity of the rectus abdominis muscle was higher in the smokers than in the non-smokers. Conversely, the activity of the internal oblique abdominal and transversus abdominis muscles, which are deep abdominal muscles, was higher in non-smokers than in smokers, but the difference was not statistically significant. Conclusion: Smoking can inhibit the activity of smokers' deep abdominal muscles and increase the activity of their superficial muscles, thus making it detrimental to their musculoskeletal system.

고주파심부투열 치료가 앞쪽머리자세를 가진 성인의 자세정렬, 통증 그리고 기능에 미치는 즉각적인 효과 (Immediate Effects of High-Frequency Diathermy on Neck, Shoulder Alignment, Pain and Function in Adults with Forward Head Posture)

  • Purpose : Forward head posture (FHP), characterized by the anterior positioning of the head relative to the spine, is a common postural deviation that can lead to neck pain, reduced mobility, and muscle imbalances. Recently, high-frequency deep heat therapy (HFDT) has been gaining attention for the intervention of FHP. This research aims to investigate the efficacy of HFDT in comparison to instrument assisted soft-tissue mobilization (IASTM) for treating FHP among 30 young adults. Methods : Participants were randomly assigned to either the HFDT or IASTM group. The study focused on examining changes in neck joint mobility, pain thresholds, rounded shoulder distance, lower trapezius muscle strength, and neck dysfunction. Measurements were taken before and after the interventions. Paired t-tests were used for within-group analyses, and independent t-tests were employed for between-group comparisons. The statistical significance level α was set to .05. Results : Statistically significant improvements were observed across all measured parameters in both groups (p<.05). The HFDT group showed significantly greater enhancements in neck joint mobility, pain thresholds, rounded shoulder distance, lower trapezius muscle strength, and neck dysfunction parameters. Specifically, HFDT was more effective than IASTM in improving neck joint mobility, right upper trapezius pain threshold, left rounded shoulder distance, and right lower trapezius strength. The only exceptions were neck flexion range of motion, left upper trapezius pain threshold, right rounded shoulder distance, and left lower trapezius strength, where no significant differences were found between the groups. Conclusion : The findings suggest that HFDT, by combining the benefits of high-frequency therapy and manual therapy, effectively alleviates upper trapezius muscle pain and tension, enhances neck mobility, and strengthens lower trapezius muscles. Thus, HFDT could be considered a valuable intervention for clinicians aiming to address FHP and associated musculoskeletal problems.

깊은목굽힘근 강화운동이 만성 목통증 환자의 통증과 목-어깨각도, 장애지수 및 통증에 미치는 영향 (Effects of Deep Cervical Flexors Strengthening Exercise on Cervical-Shoulder Angle, Disability index and pain and in Patients with Chronic Neck Pain)

  • Background: The purpose of this study on the effects of deep cervical flexors strengthening exercise to forward head posture and pain of neck in patients with chronic neck pain. Method: We selected 30 subjects among neck pain patients and carried out measurements. The subjects' forward head posture was measured including head tilt angle (HTA), neck flexion angle (NFA) and forward shoulder angle (FSA), neck disability index (NDI), numeric rating scale (NRS). The subjects underwent deep cervical flexors muscle strengthening with pressure bio-feedback device for 4 weeks. Then, after intervention, the subjects' forward head posture was measured again. It was performed Wilcoxon signed-ranks test for confirming the effect of deep cervical flexor muscles strengthening exercise. Results: As a result of comparison of measurements before and after intervention, we found that there were statistically significant improvement in the subjects' NDI, NRS and forward head posture including head tilt angle, neck flexion angle and forward shoulder angle. Conclusion: We concluded that deep cervical flexors strengthening exercise has the effect of alleviation of neck pain patients and is effective for forward head posture.

안정적 반전, 내려치기 그리고 들어올리기를 통한 하부체간 심층근육 강화운동이 초기 편마비 환자의 균형에 미치는 영향 - 증례 보고 - (The effect of a balance on deep abdominal muscles in an acute hemiplegic patient through stabilizing reversal, chopping and lifting)

  • Purpose : The purpose of this study was to evaluate the effect of core stability training at deep abdominal muscle for balance control of hemiplegic patient. Method : The subject of this study was a 47-year-old man with right hemiplegia. He was treated five times a week for three weeks with core stability training at deep abdominal muscles. Evaluation tool was used Functional reach test(FRT), timed up and go test(TUG) and one leg standing for stroke patients. Result : The FRT distance increase, TUG time decrease, one leg standing time increase core stability training at deep abdominal muscles for right hemiplegia improved was the ability for maintain balance. Posture and control of trunk stability are changing posture, and so which showed significant improve of total balance control. Conclusion : The result of this study showed that core stability training at deep abdominal muscles is an effective treatment for balance control. Therefore, it could be considered as a treatment method in the rehabilitation of stroke patient with poor postural control and imbalance, although further studies are needed.

신장 질환 맞춤형 AI 운동요법 제공 시스템 구현 (Implementation of AI Exercise Therapy System customized for Kidney Disease)

  • 본 논문에서는 신장 질환자를 대상으로 맞춤형 운동요법을 제공하기 위해 딥러닝과 같은 AI 방식을 적용한다. 딥러닝 적용을 위해 신장 질환을 판별할 수 있는 데이터셋을 학습시켜 신장 질환 여부를 판단하고 사용자의 신체정보와 질환여부에 따른 근력량인 1RM 또한 딥러닝을 통해 산출할 수 있도록 구성하였다. 산출된 근력량 1RM은 신체 부위별 산출 식을 통해 부위별 저항성 운동으로 변환하여 사용자의 신체정보에 맞춘 유산소 운동량과 함께 제공되도록 구성하였다. 본 논문에서 제안한 방식으로 지속적인 연구가 진행될 경우 다양한 질환에 대해 맞춤형 운동요법을 제공할 수 있을 것이다.

긴장성 두통환자의 두경부 압력통각 역치에 관한 연구 (Pressure-Pain Thresholds(PPT) of Head and Neck Muscles in Tension-type Headache Patients)

  • The pressure pain thresholds of head and neck muscles of patients suffering from tensiontype headache220, all female, ages ranged from 13 to 50 years (28.4$\pm$9.6) and 39 healthy controls, all female, ages ranged from 14 to 46 years (24.4$\pm$9.2) were recorded by the electronic algometer (Electyronic Algometer Type I, Somedic, Stockholm, Sweden). And the obtained results were as follows : 1. The pressure pain thresholds of patient group were lower than those of controls in superior sternocleidomastoid muscle, middle sternocleidomastoid muscle, and trapezius insertion muscle (P<0.001) 2. The pressure pain thresholds of patient group were not different from those of controls in anterior temporal, middle temporal, posterior temporal, deep masseter, anterior masseter, inferior masseter, medial pterygoid, posterior digastric, splenius capitus and upper trapezius muscle (P>0.05). 3. Seventy-one percent of tension-type headache patients had more than one muscle, of whicb pressure pain threshold was lowered significantly (less than mean of control - 1.5SD). 4. The pressure pain thresholds of head and neck muscles should be considered as a criterion for the diagnosis of tension-type headache.
