급성 복증 환자에서 현장검사 크레아티닌을 이용한 의사결정 시간의 단축 (The influence of the decision making time by using point-of-care creatinine in patients with acute abdomen)

  • Objective: Radio-contrast abdomino-pelvic computed tomography (APCT) is considered the gold standard diagnostic tool for an acute abdomen in the emergency department. On the other hand, APCT has a risk of contrast-induced nephropathy. Emergency physicians evaluate the creatinine (Cr) level prior to taking a APCT for the above reason but it takes time to evaluation the serum Cr level. This study hypothesized that Cr measured by a point-of-care test (POCT) can shorten the time to making clinically important decisions for patients with an acute abdomen. Methods: This prospective randomized study was conducted between March 2017 and October 2017. The subjects were divided into two groups (Cr measured by laboratory vs. Cr measured by POCT). To analyze the clinical acceptability for creatinine, agreement was demonstrated graphically by Bland-Altman plots. This study compared the time to make a clinically important decision by physicians and the length of stay at the emergency department in both groups. Results: A total of 76 patients were eligible for the study, 38 patients were assigned to each group. There was no statistically significant difference in the time to the first medical examination (P=0.222) and emergency department stay time (P=0.802). On the other hand, the time to recognition of the Cr level (P<0.001), time to performing APCT (P<0.001), time to decision making (P<0.001), and time to initiation of treatment (P<0.001) were shortened significantly in the point-of-care creatinine group. Conclusion: In this study, the POCT for creatinine can allow rapid decision making by shortening the time to performing the radio-contrast APCT than the laboratory for patients with an acute abdomen.

임상간호사의 시간관리 요인이 조직몰입 및 직무만족에 미치는 영향 (The Effects of Time Management on the Clinical Nurse's Organizational Commitment and Job Satisfaction)

  • Purpose: The aim of this study was to analyze the effects of time management on the clinical nurse's organizational commitment and job satisfaction. Methods: Subjects were recruited in two general hospitals in Seoul and Incheon. Data collection was done using a self-report questionnaire. Time management was measured using the questionnaire developed by Han (1992). Organizational commitment and job satisfaction were measured using the questionnaire developed by Yoon (2000), based on Mowday et al. (1979) and Stamps et al. (1978). The data were analyzed using the SAS statistical package program, version 10.0. Specifically, descriptive statistics and stepwise multiple regression were performed. Results: The predictive time management factors for organizational commitment included deadline decision, simplification, and goal-setting. The predictive time management factors for job satisfaction included planning/making the priority order, deadline decision, simplification, asking for help, and responsibility reduction. Conclusion: Time management factors are highly correlated with organizational commitment and job satisfaction in clinical nurses. Deadline decision and simplification are common predictive factors for organizational commitment and job satisfaction. These results can be used to develop more effective time management strategies for increasing organizational effectiveness in clinical nurses.

  • 기업의 전략적 의사결정자는 불확실성과 복잡성에 직면해왔다. 그리고 그들은 이러한 환경 하에서 의사결정을 내려야 함에도 불구하고 그들의 의사결정에 필요한 충분한 시간, 인력, 예산, 그리고 지식이 충분히 주어지지 않는다. 그래서 그들은 그 분야에 대한 암묵적 지식을 지닌 전문가들의 지원을 받는 수 밖에 없다. 그러나 의사결정자들이 어떤 문제에 직면할 때 마다 의사결정에 필요한 새로운 지식을 창조하고 변형하고 결합하고 응용하는 다른 절차나 방법을 발견하는 것은 매우 어렵다. 본 논문은 노나카에 의해 제안된 지식변환과정을 이용함으로써 전략적 의사결정을 지원할 새로운 방법을 제안한다. 본 연구는 컨설팅 산업분야에서 전략적 의사결정의 응용사례를 예시한다. 이 논문은 제안된 방법에 기초한 의사결정지원기법으로서 인지지도를 사용한다.

EEZ내 외국 침몰선박 잔존유 제거 의사결정 효율화 방안 고찰 (A Consideration of the Decision-Making Efficiency Concerning the Removal of Oil Spills by Foreign Ships in the EEZ)

  • 우리나라 영해 밖 EEZ내 침몰한 외국선박의 잔존유 제거작업이 국내 최초로 2019년 5월 이행되었다. 우리 관할수역이지만 기름 제거작업은 홍콩 선주 측 P&I 보험사와 계약을 맺은 일본업체가 수행하고 국내 업무 대행은 S법무법인과 J해운이 맡았다. 해당 선박은 총톤수 4,433톤의 일반화물선으로 2015년 4월 제주도 남동방 약 48마일 해상에서 침몰하였다. 침몰 후 기름 제거 작업 개시까지 만 4년이 넘는 기간이 소요되었지만 잔존유 제거작업은 단 22일 만에 끝났다. 대형선박이고 잔존유량이 많았던 Erika호나 Prestige호 등 외국사례 등과 비교하면 기름 제거 결정까지 긴 시간이 걸렸다. 그 원인을 파악하고자 해당선박 침몰 시부터 잔존유 제거까지 생산된 모든 문서 93건을 확보하여 분석하였다. 그 결과, 가장 우선적으로 이루어져야 할 행정적 절차 즉, 잔존유 제거에 대한 관할 행정기관의 의사결정이 지연되었음이 확인되었다. 관할청에서 긴 시간 동안 많은 검토를 하였지만 최종 결과는 초기방안과 별로 달라지지 않았다. 4년의 시간이 흐르는 동안 선내 잔존유 상당량이 유출되고 화물 전체가 유실되었다. 의사결정 지연에 영향을 미친 원인으로 관할청의 사고 관련 기본 사실이나 자료 확인 소홀부터 법률적, 기술적, 환경적 그리고 인적 측면 등 여러 문제점이 식별되었다. 연구 마지막에 식별된 문제점에 대한 개선안, 즉 의사결정 효율화 방안을 제시한다. 이 연구는 국내 최초 EEZ내 외국 침몰선박 잔존유 제거 사례를 고찰하여 향후 유사한 작업계획이나 정책수립 등에 도움이 될 것으로 기대한다.

지식집약형 컨설팅프로세스 지원을 위한 경영의사결정지원 기술모델 개발연구 (Development of managerial decision-making support technology model for supporting knowledge intensive consulting process)

  • 21세기 들어와 기업은 갈수록 복잡해지고 변동성이 커지는 경영환경에 직면하게 되었다. 또한 다양한 고객의 요구와 치열해지는 경쟁에 대응하기 위하여 경영 전 분야에 걸쳐 긴밀하고 체계적인 의사결정 대안을 선택하고 관리해야 하는 부담이 가중되고 있다. 본 연구에서는 경영의사결정을 지원할 수 있는 기술모델을 컨설팅 문제해결 절차와 기법, 경영프로세스 지식체계와 연계하여 구성하고 문제해결도구로써 지식체계에 기반한 시뮬레이션 도구 활용에 대하여 논의하였다. 나아가 제안하는 경영의사결정지원 기술모델의 시스템구현을 통한 체계적인 지식체계의 확충과 발전 필요성을 논의하였다. 경영의사결정지원 기술모델은 크게 세 가지 요소로써 구성되는데 첫 번째는 문제해결기법으로 참조자료로써 활용이 되며, 두 번째는 표준비즈니스 프로세스와 참조프로세스 모델 정보를 포함하는 프로세스관련 지식체계이다. 세 번째 요소는 문제해결기법과 프로세스 관련 지식체계를 정보로 활용하여 대안을 생성하고 분석하는 도구인 시뮬레이터로 정의하였다. 위의 세 가지 주요요소들은 컨설팅 과정전반에서 표준화된 문세해결 절차에 따라 체계적 분석을 수행하도록 하는 가이드라인을 제세하고 각 분석단계별로 분석기법에 대한 정보를 제공하여 의사결정의 정확성와 객관성 확보를 지원한다. 경영의사결정 지원기술 모델은 궁극적으로 지식집약형 컨설팅 프로세스를 지원하여 다양한 컨설팅 지식을 축적하고 컨설팅기법의 발전과 활용을 촉진하여 컨설팅 산업 발전의 기반기술 개발에 기본 프레임워크를 제공하는데 의의를 지닌다.

계층적 분석기법을 활용한 그룹의사결정 지원 (Group Decision Support with Analytic Hierarchy Process)

  • The Analytic Hierarchy Process (AHP) is well suited to group decision making and offers numerous benefits as a synthesizing mechanism in group decisions. To date, the majority of AHP applications have been in group settings. One reason for this may be that groups often have an advantage over individual when there exists a significant difference between the importance of quality in the decision and the importance of time in which to obtain the decision. Another reason may be the best alternative is selected by comparing alternative solutions, testing against selected criteria, a task ideally suited for AHP. In general, aggregation methods employed in group AHP can be largely classified into two methods: geometric mean method and (weighted) arithmetic mean method. In a situation where there do not exist clear guidelines for selection between them, two methods do not always guarantee the same group decision result. We propose a simulation approach for building group consensus without efforts to make point estimates from individual diverse preference judgments, displaying possible disagreements as is natural in group members'different viewpoints.

  • The linear search problem is concerned with finding a hiden target on the real line R. The position of the target governed by some probability distribution. It is desired to find the target in the least expected search time. This problem has been formulated as an optimization problem by a number of authors without making use of Markov Decision Process (MDP) theory. It is the aim of the paper to give a (MDP) formulation to the search problem which we feel is both natural and easy to follow.

  • The objective of this paper is to apply Extended AND-OR Graph (EAOG)-related techniques to extract knowledge from a specific problem-domain and perform analysis in complicated decision making area. Expert systems use expertise about a specific domain as their primary source of solving problems belonging to that domain. However, such expertise is complicated as well as uncertain, because most knowledge is expressed in causal relationships between concepts or variables. Therefore, if expert systems can be used effectively to provide more intelligent support for decision making in complicated specific problems, it should be equipped with real-time inference mechanism. We develop two kinds of EAOG-driven inference mechanisms(1) EAOG-based forward chaining and (2) EAOG-based backward chaining. and The EAOG method processes the following three characteristics. 1. Real-time inference : The EAOG inference mechanism is suitable for the real-time inference because its computational mechanism is based on matrix computation. 2. Matrix operation : All the subjective knowledge is delineated in a matrix form, so that inference process can proceed based on the matrix operation which is computationally efficient. 3. Bi-directional inference : Traditional inference method of expert systems is based on either forward chaining or backward chaining which is mutually exclusive in terms of logical process and computational efficiency. However, the proposed EAOG inference mechanism is generically bi-directional without loss of both speed and efficiency.

RSS를 이용한 실시간 상품정보 수집시스템의 설계 (System Design for Collecting Real-Time Product Information Using RSS)

  • It is well known that internet shoppers are very sensitive to sale prices. They visit the various shopping malls and collect the product information including purchase conditions for goods purchase decision-making. Recently the necessity of information support is increasing because of increase of information amount which is necessary and complexity of goods purchase decision-making process. The comparison shopping agent systems have provided price comparison information which is collected from various shopping malls to satisfy internet shoppers information craving. But the frequent price change caused by keen price competition is becoming the primary reason of information quality decline among price comparison sites. RSS which is a family of web feed formats used to publish frequently updated is applied even in on-line shopping malls. This paper develops a RSS product information collection system to get real-time product information. The proposed product information system consists of (1) web crawler module for searching RSS feed shopping malls automatically, (2) RSS reader module for parsing product information from RSS feed file, (3) product DB and (4) product searching module. Performance of the proposed system is higher than the comparison shopping agent systems when it is defined with the volume of collecting product information per unit time.

  • As the ticket issuing methods have been diversified for the convenience of the passengers such as ticketless service(SMS ticket, e-ticket, home ticket), automatic ticket issuing machine and consignment ticket sale, maintaining the current number of ticket booth has been becoming a issue. Too many booth can cause the inefficiency of the cost of labor. According to the Charter of Customer Service of Korail, on the other hand, 95% of passengers have to purchase a train ticket within 5 minutes. This study was designed to present a decision making model for the optimum number of ticket booth which can affect an efficient operation of train station and improvement of customer convenience. And, this paper shows the proper manpower of ticket booth and the change of customer waiting time by analyzing the arrival and ticket issuing time of passengers based on 'Queueing Theory'. However, it is insufficient to be generalized due to some limitations of analysis. This study will contribute to improve customer satisfaction by reducing the waiting time at the ticket booth. In addition, presenting the optimum number of booth is expected to have an effect on the increase of productivity and cost savings.
