Cost-Utility of "Doxorubicin and Cyclophosphamide" versus "Gemcitabine and Paclitaxel" for Treatment of Patients with Breast Cancer in Iran

  • Purpose: A cost-utility analysis was performed to assess the cost-utility of neoadjuvant chemotherapy regimens containing doxorubicin and cyclophosphamide (AC) versus paclitaxel and gemcitabine (PG) for locally advanced breast cancer patients in Iran. Materials and Methods: This cross-sectional study in Namazi hospital in Shiraz, in the south of Iran covered 64 breast cancer patients. According to the random numbers, the patients were divided into two groups, 32 receiving AC and 32 PG. Costs were identified and measured from a community perspective. These items included medical and non-medical direct and indirect costs. In this study, a data collection form was used. To assess the utility of the two regimens, the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Core30 (EORTC QLQ-C30) was applied. Using a decision tree, we calculated the expected costs and quality adjusted life years (QALYs) for both methods; also, the incremental cost-effectiveness ratio was assessed. Results: The results of the decision tree showed that in the AC arm, the expected cost was 39,170 US$ and the expected QALY was 3.39 and in the PG arm, the expected cost was 43,336 dollars and the expected QALY was 2.64. Sensitivity analysis showed the cost effectiveness of the AC and ICER=-5535 US$. Conclusions: Overall, the results showed that AC to be superior to PG in treatment of patients with breast cancer, being less costly and more effective.

정규화 기법 기반의 설계VE 기능 및 비용 점수 산출 모델 (Calculation Model for Function & Cost Score based on Normalization Method in Design VE)

  • VE는 공공건설사업의 예산절감과 기능향상, 구조적 안전, 품질 확보를 목적으로 하고 있다. 그러나 관련 내용이 설계의 경제성 검토 위주로 구성되어 있어 구조적 안전과 품질확보를 위한 검토가 미흡해 질 가능성이 문제점으로 지적되고 있고, VE를 단순한 원가절감방법으로 생각하는 인식으로 인해 VE 수행 시 비용절감에 치중된 대안이 제시되어 왔다. 이에 대한 개선방안으로 정부에서는 생애주기비용절감 가치향상 제안서를 도입하고 원안과 대안의 비용 점수와 기능 점수, 가치 변화를 명시하도록 하였다. 그러나 구체적인 방법과 기준이 제시되어 있지 않아 이론적 근거보다는 실무적 편의 위주의 방법이 사용되고 있다. 현행 방법은 원안과 대안의 비(比)를 이용하기 때문에 간단하고 사용이 편리하다. 하지만 현행 방법을 이용하여 평가 값을 점수화 하면 설계안 속성에 따라 다양한 값이 산출되어 광범위한 점수 분포를 나타낸다. 따라서 많은 양의 VE 안을 평가하여 수정 설계를 위한 최적 안을 선택하는 의사결정과정에서 평가자에게 혼란을 줄 수 있다. 이는 합리적인 의사결정을 저해하는 요소로서, VE대안의 비용과 성능 속성 정보를 이용하여 산출한 가치 점수의 신뢰성 및 객관성 검증의 어려움으로 이어진다. 본 연구는 VE 안 평가 시 의사결정의 효율성 제고를 위하여 기능 점수와 비용 점수 산출 절차와 방법을 고찰하여 문제점을 제시하고, 개선 방안으로서, 정규화 기반의 기능 및 비용 점수 산출 모델을 제시하였다.

불확실성을 고려한 철도 교량의 LCC분석 시스템 개발 (Development of Uncertainty-Based Life-Cycle Cost System for Railroad Bridges)

  • Recently, the demand on the practical application of life-cycle cost effectiveness for design and rehabilitation of civil infrastructure is rapidly growing unprecedentedly in civil engineering practice. Accordingly, it is expected that the life-cycle cost in the 21st century will become a new paradigm for all engineering decision problems in practice. However, in spite of impressive progress in the researches on the LCC, so far, most researches in Koreahave only focused on roadway bridges, which are not applicable to railway bridges. Thus, this paper presents the formulation models and methods for uncertainty-based LCCA for railroad bridges consideringboth objective statistical data available in the agency database of railroad bridges management and subjective data obtained form interviews with experts of the railway agency, which are used to anew uncertainty-based expected maintenance/repair costs including lifetime indirect costs. For reliable assessment of the life-cycle maintenance/repair costs, statistical analysis considering maintenance history data and survey data including the subjective judgments of railway experts on maintenance/management of railroad bridges, are performed to categorize critical maintenance items and associated expected costs and uncertainty-based deterioration models are developed. Finally, the formulation for simulation-based LCC analysis of railway bridges with uncertainty-based deterioration models are applied to the design-decision problem, which is to select an optimal bridge type having minimum Life-Cycle cost among various railway bridges types such as steel plate girder bridge, and prestressed concrete girder bridge in the basic design phase.

Economical selection of optimum pressurized hollow fiber membrane modules in water purification system using RbLCC

  • A water treatment utility in South Korea operates a large system of pressurized hollow fiber membrane (PHFM) modules. The optimal selection of membrane module for the full scale plant was critical issue and carried out using Risk-based Life Cycle Cost (RbLCC) analysis based on the historical data of operation and maintenance. The RbLCC analysis was used in the process of decision-making for replacing aged modules. The initial purchasing cost and the value at risk during operation were considered together. The failure of modules occurs stochastically depending on the physical deterioration with usage over time. The life span of module was used as a factor for the failure of Poisson's probability model, which was used to obtain the probability of failure during the operation. The RbLCC was calculated by combining the initial cost and the value at risk without its warranty term. Additionally, the properties of membrane were considered to select the optimum product. Results showed that the module's life span in the system was ten years (120 month) with safety factor. The optimum product was selected from six candidates membrane for a full scale water treatment facility. This method could be used to make the optimum and rational decision for the operation of membrane water purification facility.

다기준의사결정기법과 수정 A-STAR 알고리즘을 이용한 목적지 최적경로 탐색 기법 개발 (Development of Destination Optimal Path Search Method Using Multi-Criteria Decision Making Method and Modified A-STAR Algorithm)

  • In this paper, we propose a destination optimal route algorithm for providing route finding service for the transportation handicapped by using the multi-criteria decision-making technique and the modified A-STAR optimal route search algorithm. This is a method to set the route to the destination centering on safety by replacing the distance cost of the existing A-STAR optimal route search algorithm with the safety cost calculated through AHP/TOPSIS analysis. To this end, 10 factors such as road damage, curb, and road hole were first classified as poor road factors that hinder road driving, and then pairwise comparison of AHP was analyzed and then defined as the weight of TOPSIS. Afterwards, the degree of driving safety was quantified for a certain road section in Busan through TOPSIS analysis, and the development of an optimal route search algorithm for the transportation handicapped that replaces the distance cost with safety in the finally modified A-STAR optimal route algorithm was completed.

  • The purpose of this research calculates the map manufacture cost and there are we even though we determine proper map price. We distinguished the budget item of NGI in 2003 with the direct cost to the overhead cost. A map manufacture cost classified a direct and overhead cost as a nap manufacture cost commonness cost and no relation cost and was calculated. The result to analyze the item of the charge, It is impossible to withdraw the map manufacture cost selling the map. This is because a map sale price inexpensive than the map manufacture price. We presented a two alternative in research to see consequently about a map price raising.

  • TCA(Total Cost Assessment) can be defined by a tool to evaluate actual profit about investment. Also, It can be used by method to make more reasonable decision-making in business. Enforcement of 'direct payment system' for environment friendship farmhouse of government is spreading recent times. But, it is true that it is no definite standard about appropriation of disbursement amount of money. TCA method can be utilized usefully for solution of these problem. Also, It will help to decide price grasping invisible cost and environment cost. Therefore, We tried that may can apply TCA theory in farmhouse class, and attempted economic performance estimation which use this. This study introduces TCA theory and developed applicable expense list in agriculture field. Also, We applied TCA theory to farmhouse. This theories referenced domestic and th6 foreign countries, connection literature. But, It handled part about invisible cost and social cost etc..

  • A making a decision of construction cost has important meaning and function for both contractor and owner in construction projects. Especially, it should be premised that estimating the construction cost in efficient and rational way in public construction, which is invested by government funds, for efficient execution of the budget and investment as a side of government. The systematic methodology for estimating construction cost approximately of a river facility construction project has not yet been established because of its unique characteristics including its relatively small project size in terms of cost. On this study, It collect and analyze a river facility construction historical cost data for develop an approximate cost estimating model for river applied by typical embankment section method and rate application of the others activity type. And it verify suitability of model through a that result of application of real river facility construction statement at developed model. By this study, it is expected to reasonable and systematic estimating construction cost through application of developed model.

  • 태양열 주택 및 급탕의 보급 정책은 민수용 에너지의 절감 또는 대체의 의미에서 그 필요성 및 당위성을 인정받고 있으나, 다른 대체에너지와 마찬가지로 태양열 이용시스템의 경제성이 가상 문제가 되고 있는 실정이다. 본 연구에서는 태양열 주택 및 급탕의 경제성을 상세히 분석함으로써 실질적인 보급정책 자료를 제공하며, 실수요자를 위한 계몽근거를 마련하고자 한다. 본 연구에서는 태양열 이용시스템의 경제성을 정확히 수행하기 위한 의사결정지원 시스템(decision support system)을 확립시켰으며, 이에 따른 태양열 이용시스템의 열적성능 해석과 동시에 경제성 분석은 시행오차(trial & error)하에 수행할 수 있도록 각종전산 프로그램을 개발하였다. 태양열 주택 및 급탕의 열적성능 해석은 모의실험을 통하여 기상 조건과의 상관관계를 도출하고 태양열 이용시스템의 추가비용 및 연료가 이용을 현재가 (present worth) 개념으로 적응하여 산출하고, 순익분기 해석법 (break-even point analysis method) 및 수명가 산정법(life-cycle cost analysis method)으로 경제성 분식을 중점적으로 수행하였다.

거래비용 관점에서 본 온라인 구매와 오프라인 구매의 비교 (The Comparison of Online and Offline Purchases: A Transaction Cost Perspective)

  • 본 연구에서는 제품구매과정의 각 단계에서 발생되는 거래비용이 온라인과 오프라인 구매에서 어떻게 다른지를 살펴보기 위하여 제품유형, 구매의사결정단계, 그리고 거래비용을 연계하는 모형을 통합적으로 제시하였다. 구체적으로, 구매의사결정의 각 단계에서 발생하는 거래비용에 기초하여, 소비자가 인지하게 되는 오프라인과 온라인에서의 거래비용의 차이를 살펴보고, 그 차이가 제품유형에 따라 다르다는 것을 비교하였다. 제품구매단계별 거래비용으로는 탐색비용, 비교비용, 제품성능에 대한 위험비용, 그리고 구매 후 서비스 위험비용을 고려하였으며 온오프라인구매의 거래비용 차이를 제품유형별로 분석하기 위하여 FCB 틀에서 제시하는 4가지 제품유형을 도입하였다. 연구결과, 총 24개의 가설 중 19개가 채택되어 온라인과 오프라인 구매의 거래비용의 차이와 제품유형에 따는 차이에 대한 가설이 대부분 지지되는 것으로 나타났다. 반면, 기각된 가설 중 4개는 비유의적이었으며 1개만이 가설과 반대되는 결과를 보였다.
