Spatial modeling of mortality from acute lower respiratory infections in children under 5 years of age in 2000-2017: a global study

  • Background: Over the past few decades, various goals have been defined to reduce the mortality of children caused by acute lower respiratory infections (ALRIs) worldwide. However, few spatial studies to date have reported on ALRI deaths. Purpose: We aimed to assess the spatial modeling of mortality from ALRI in children under 5 years of age during 2000-2017 using a global data. Methods: The data on the mortality of children under 5 years old caused by ALRI were initially obtained from the official website of the World Health Organization. The income status of their home countries was also gathered from the Country Income Groups (World Bank Classification) website and divided into 5 categories. After that, in the ArcGIS 10.6 environment, a database was created and the statistical tests and related maps were extracted. The Global Moran's I statistic, Getis-Ord Gi statistic, and geographically weighted regression were used for the analyses. In this study, higher z scores indicated the hot spots, while lower z scores indicated the cold spots. Results: In 2000-2017, child mortality showed a downward trend from 17.6 per 100,000 children to 8.1 and had a clustered pattern. Hot spots were concentrated in Asia in 2000 but shifted toward African countries by 2017. A cold spot that formed in Europe in 2007 showed an ascending trend by 2017. Based on the results of geographically weighted regression test, the regions identified as the hot spots of mortality from ALRI in children under 5 years old were among the middle-income countries (R2=0.01, adjusted R2=8.77). Conclusion: While the total number of child deaths in 2000-2017 has decreased, the number of hot spots has increased among countries. This study also concluded that, during the study period, Central and Western Africa countries became the main new hot spots of deaths from ALRI.

Public Exposure to Natural Radiation and the Associated Increased Risk of Lung Cancer in the Betare-Oya Gold Mining Areas, Eastern Cameroon

  • Background: This study aims to reevaluate natural radiation exposure, following up on our previous study conducted in 2019, and to assess the associated risk of lung cancer to the public residing in the gold mining areas of Betare-Oya, east Cameroon, and its vicinity. Materials and Methods: Gamma-ray spectra collected using a 7.62 cm×7.62 cm in NaI(Tl) scintillation spectrometer during a car-borne survey, in situ measurements and laboratory measurements performed in previous studies were used to determine the outdoor absorbed dose rate in air to evaluate the annual external dose inhaled by the public. For determining internal exposure, radon gas concentrations were measured and used to estimate the inhalation dose while considering the inhalation of radon and its decay products. Results and Discussion: The mean value of the laboratory-measured outdoor gamma dose rate was 47 nGy/hr, which agrees with our previous results (44 nGy/hr) recorded through direct measurements (in situ and car-borne survey). The resulting annual external dose (0.29±0.09 mSv/yr) obtained is similar to that of the previous study (0.33±0.03 mSv/yr). The total inhalation dose resulting from radon isotopes and their decay products ranged between 1.96 and 9.63 mSv/yr with an arithmetic mean of 3.95±1.65 mSv/yr. The resulting excess lung cancer risk was estimated; it ranged from 62 to 216 excess deaths per million persons per year (MPY), 81 to 243 excess deaths per MPY, or 135 excess deaths per MPY, based on whether risk factors reported by the U.S. Environmental Protection Agency, United Nations Scientific Committee on the effects of Atomic Radiation, or International Commission on Radiological Protection were used, respectively. These values are more than double the world average values reported by the same agencies. Conclusion: There is an elevated level of risk of lung cancer from indoor radon in locations close to the Betare-Oya gold mining region in east Cameroon. Therefore, educating the public on the harmful effects of radon exposure and considering some remedial actions for protection against radon and its progenies is necessary.

  • A magnitude of 7.3 in Richter scale earthquake the strongest in-land earthquake in hundred years struck central Taiwan on September 21, 1999. It caused over 2,400 deaths and 30 some trillion won losses. To give an overview of this devastating earthquake this presentation will cover the following topics: 1) Introduction to Taiwan historical and 921 earthquake. 2) Damages to people landslide building dam bridge tank power facility etc. 3) Strong motion data and its characteristics. 4) Some changes to the building code triggered by the experience of the earthquake. Finally a concluding remark will be made.

  • It's declining the number of deaths in total traffic accidents, but the death of elder drivers has increasing than younger drivers. So this paper wish to prevent the traffic accident of the elder drivers using driving simulator. It can help to make better policies and planning for elder drivers.

  • The current use of leading causes of death is an outmoded, misleading counterproductive measure of importance of disease. The 'new' concept of productive years of life discounted to the future and combined with morbidity, disability, and direct cost measures will give a much more meaningful, useful measure of the magnitude of disease problems in Korea. I am looking forward to seeing refinements and improvements on this technique carried out by some readers of this paper.

  • Bronchial Adenomas are rare entity among the primary bronchopulmonary neoplasms. But they have received attention because of debate on their malignant nature, prognosis, and mode of therapy. We have four carcinoids and two cylindromas from Jan. 1972 to Jun. 1983. Operations include two pneumonectomies two lobectomies and one bilobectomy. In one cylindroma, resection was impossible due to its invasion into mediastinum and pericardium. There were no hospital deaths.

  • Improvements in the operative management of traumatic aortic transection have resulted in safe and expeditious repair. Nonetheless, multisystem injuries continue to inflict significant numbers of deaths. We have experienced a case of acute traumatic aortic transection in 41 years old male patient by a traffic accident. The transection was just distal to the origin of the left subclavian artery. We have done a synthetic graft interposition under left atrium to left femoral artery bypass with centrifugal pump. His postoperative course was smooth, and discharged without any complications.

  • During the Dec.1990 to April.1994, 14 patients were diagnosed in the Department of Thoracic and Cardiovascular surgery, Medical college of Chonnam National University,as having congenital Bochdalek hernia. All of them diagnosed and operated before the age of 20 days, neonatal period. 3 of 14 were died after operation, so mortality rate was 21%, the deaths occurred in 1,1,13 days neonate. In this retrospective study we describe our experience and results with review of the literature.

  • Still a lethal injury, traumatic rupture of thoracic aorta occurs more frequently than we expect and comprises significant part of causes of deaths by blunt trauma. We recently experienced a thoracic aortic rupture accompanied by multiple injuries including pericardial and interatrial septal rupture and myocardial contusion in a patient who had been injured in a fall accident. Literatures are reviewed with the concern of early diagnosis, surgical technique and the result of operation.
