Impact Factors and Validity of Blood Variables on Death in COVID-19 patient: Using Data of Korea Disease Control and Prevention Agency

  • In this paper, we propose impact factors and validity of blood variables on death of COVID-19 patients. The clinical-epidemiological data of 5628 COVID-19 patients, provided from Korea Disease Control and Prevention Agency as day of 30th April 2020, were used. As results, impact factors of death were dementia, older age, high lymphocyte, cancer, dyspnea, COPD, change of consciousness, heart disease, high platelets, abnormal diastolic pressure and fever. The validities of blood variables for death were high in the order of lymphocyte, hemoglobin, hematocrit, platelet and WBC. Therefore, risk factors such as initial clinical characteristics, underlying disease and blood test results, could be regarded for efficient management of COVID-19 patients.

  • Construction industry has a relatively higher accident rate compared to other industries, especially the serious accident rate including death. Ministry of Employment and Labor has not only implemented safety consciousness but also initiated the basic occupational safety and health training for the construction industry. Despite the effort, only a few sites follow the obligatory task and there are reports stating the effect of the education is minimal. As a result, this research aims to determine and analyze the current state of the basic occupational safety and health training including its lesson plan and method. Furthermore, by analyzing the preliminary study on the effective instructional method, this research suggests an effective method to implement safety consciousness to the workers. An improvement plan is also suggested by supplying appropriate educational material to different workers by differentiating the characteristics and the cause of death at various work types in construction sites.

Review on the Justifiable Grounds for Withdrawal of Meaningless Life-sustaining Treatment -Based on a case of Supreme Court's Sentence No. 2009DA17417 (May 21, 2009)- (무의미한 연명치료 중단 등의 기준에 관한 재고 - 대법원 2009.5.21 선고 2009다17417사건 판결을 중심으로 -)

  • According to a case of Supreme Court's Sentence No. 2009DA17417 (May 21, 2009), the Supreme Court judges that 'the right to life is the ultimate one of basic human rights stipulated in the Constitution, so it is required to very limitedly and conservatively determine whether to discontinue any medical practice on which patient's life depends directly.' In addition, the Supreme Court admits that 'only if a patient who comes to a fatal phase before death due to attack of any irreversible disease may execute his or her right of self-determination based on human respect and values and human right to pursue happiness, it is permissible to discontinue life-sustaining treatment for him or her, unless there is any special circumstance.' Furthermore, the Supreme Court finds that 'if a patient who is attacked by any irreversible disease informs medical personnel of his or her intention to agree on the refusal or discontinuance of life-sustaining treatment in advance of his or her potential irreversible loss of consciousness, it is justifiable that he or she already executes the right of self-determination according to prior medical instructions, unless there is any special circumstance where it is reasonably concluded that his or her physician is changed after prior medical instructions for him or her.' The Supreme Court also finds that 'if a patient remains at irreversible loss of consciousness without any prior medical instruction, he or she cannot express his or her intentions at all, so it is rational and complying with social norms to admit possibility of estimating his or her own intentions on withdrawal of life-sustaining treatment, provided that such a withdrawal of life-sustaining treatment meets his or her interests in view of his or her usual sense of values or beliefs and it is reasonably concluded that he or she could likely choose to discontinue life-sustaining treatment, even if he or she were given any chance to execute his or her right of self-determination.' This judgment is very significant in a sense that it suggests the reasonable orientation of solutions for issues posed concerning withdrawal of meaningless life-sustaining medical efforts. The issues concerning removal of medical instruments for meaningless life-sustaining treatment and discontinuance of such treatment in regard to medical treatment for terminal cases don't seem to be so much big deal when a patient has clear consciousness enough to express his or her intentions, but it counts that there is any issue regarding a patient who comes to irreversible loss of consciousness and cannot express his or her intentions. Therefore, it is required to develop an institutional instrument that allows relevant authority to estimate the scope of physician's medical duties for terminal patients as well as a patient's intentions to withdraw any meaningless treatment during his or her terminal phase involving loss of consciousness. However, Korean judicial authority has yet to clarify detailed cases where it is permissible to discontinue any life-sustaining treatment for a patient in accordance with his or her right of self-determination. In this context, it is inevitable and challenging to make better legislation to improve relevant systems concerning withdrawal of life-sustaining treatment. The State must assure the human basic rights for its citizens and needs to prepare a system to assure such basic rights through legislative efforts. In this sense, simply entrusting physician, patient or his or her family with any critical issue like the withdrawal of meaningless life-sustaining treatment, even without any reasonable standard established for such entrustment, means the neglect of official duties by the State. Nevertheless, this issue is not a matter that can be resolved simply by legislative efforts. In order for our society to accept judicial system for withdrawal of life-sustaining treatment, it is important to form a social consensus about this issue and also make proactive discussions on it from a variety of standpoints.

Actual Experience of the Oracle of the I Ching-Death, God and Love: In Front of My Father's Spirit (주역 점(占)의 실제 체험-죽음, 신 그리고 사랑: 아버지의 영전(靈前)에서)

  • The oracle of the I Ching, divination can be understood as 'synchronicity phenomenon' in analytic psychology. In order to experience divination actually, it requires a religious attitude that asks questions with a serious mind when a person is in trouble that consciousness reaches its limit. It is not just a passive attitude, but a modest, active attitude to ask what I can do now. The experience of the oracle of the I Ching connected to supra-consciousness is similar to 'active imagination'-talking with the archetype of collective unconsciousness-and is 'the process of finding the rhythm of Self-archetype, the absolute wisdom of unconsciousness.' One month before my father's death, I took care of him who couldn't communicate verbally and I divination with a question 'What can I do for my father and me now?' The I Ching's answer was hexagram 19 Lin 臨, nine at the beginning. It's message was '咸臨貞吉 joint approach. perseverance brings good fortune.' 志行正也 we must adhere perseveringly to what is right.' Through this phrase, I learned the attitude of waiting for life after death as if 'joyful obedient' to the providence of nature that spring comes after winter. And I found that keeping the touching emotion of meeting infinity (in analytical psychological terms, 'Self') with perseveration is to do the true meaning of life beyond popular money-mindedness. And six months before my father's death, I had a dream about the afterlife. In the process of interpreting that dream, I learned not only from the shock of the direct message that 'it is a truth that there is something after death,' but also the regeneration of the mind through introversion from the similarity between the closed ward and '黃泉'-chinese underworld through amplification. And I learned the importance of an open attitude to accept new things through the 'window to eternity' symbolized by the white iron gate. In my father's catholic funeral ritual, I had hope that the catholic doctrine 'Communio Sanctorum'-A spiral cycle in which the living and the dead help each other may be real as well as a symbol of the individuation process in which consciousness and unconsciousness interact in our minds. Through the consolation received through the funeral visit of many people I met in my life, I found the answer that the path to contact with infinity begins with loving the beings in front of me. I tried to understand this continuous experience by the perspective of analytical psychology.

Convergence Factors Influencing Attitude Toward Advance Directive in Healthcare Workers (의료종사자의 사전의료의향서에 대한 태도에 미치는 융합적 영향요인)

  • This study was a descriptive study to investigate the effects of healthcare workers' consciousness of biomedical ethics, recognition of good death, and self-esteem on attitude toward advance directive. Participants in this study were 292 healthcare workers in the three general hospitals located Y city. The data were collected from April 2 to April 23, 2019 and analyzed descriptive statistics, t-test, one-way ANOVA, Pearson correlation coefficient, and multiple regression using the SPSS Win. 22.0 program. The result of the multiple regression indicates the intention to awareness regarding advance directives, intention to execute advance directives, time to complete advance directives and recognition of good death predict 23.5% (F=12.39, P=.000) of attitudes toward advance directives. Therefore, it is necessary to develop a program to establish attitudes toward advance directives by providing education on recognition of good death of healthcare workers.

  • This study analyzes the animal symbolism shown in the novella by Hyoseok Lee and seeks to shed light on the consciousness of the author expressed through such symbolism. Lee used animal symbolism since 1933 which was a literary turning point for him. Animal symbolism in his novels are more than an analogical theme but is used as a core meaning and image. This has a clear intention of the author underneath. Novella which is seen as the starting point of pure literature for Lee, uses the animal pig as a key symbol. The pig serves as a symbol of the capital for poor farmers, while also serving as an amplifier for the desires of the protagonist. The death of the pig that appears at the closing of the novella stands for the tragic reality of the farmers of the time who could not seek growth in their capital or realization of their desires. As such, has characteristics of pure literature but also represents the consciousness of the author who brings attention to the tragic reality of farmers. That is, is a work that realized the unique writing style of Lee and his animal symbolism to bring to life a middle way appraoch to realism and romanticism.

An Interpretation of "Golden Bird" from the Perspective of Analytical Psychology (그림형제 동화 '황금 새'의 분석심리학적 해석)

  • This thesis is an interpretation of "Golden bird" of Grimm's fairy tale and similar other tales from the perspective of analytical psychology. Golden bird story start loss of golden apple in king's garden. King and three sons are one-sided structure live without female members in family. They need feminine aspect. Adventure and journey of stupid youngest son can be seen as a individuation process in analytic psychology. Ego meets several painful experience during journey. New Kingdom has a new specific collective consciousness. When ego fixed dominated specific collective consciousness he can only be liberated after completing the difficult task of the next phase in crisis of death. This process is initiation, which is a supplement to feminity after recovery of increased consciousness, marriage with the princess, which is possible to unify the sexes (coniunctio oppositorum). The ego who got the apple, bird, horse, and princess which is shown to be a supplement to feminity, does not attain a complete success. Although the youngest comes out after brothers' failure, yet separation and abandonment of persona are difficult challenges and after all he achives a higher consciousness and then he primarily marries the princess, his Anima, after he executed his brothers for his repeated hardships, when the intellectual capacity is sacrificed as offering. Also, the fox restores himself from magic after the youngest son kills him and cut his head and feet, as the fox's wish. To be transformed into a human being. It experiences another consciousness. Finally, unifying the coniunctio oppositerum is fulfilled and a divine bond achieves the wholeness and it is accomplished individuation. In clinical practice, fox is compared to a therapist as a guide of individuation process who lead clients.

Symptom Prevalence During Terminal Cancer Patients' Last 48 Hours of Life (말기 암 환자에서 임종 전 48시간 동안 나타나는 신체적 증상 빈도)

  • Purpose : The prevalence of symptoms in patients with terminal cancer varies considerably and these symptoms are very difficult to control. However, patients can spend their last days or hours of life without suffering pain with appropriate care. One of the major concerns during last days of life is to predict the time of death. We would like to investigate symptom prevalence during terminal cancer patients' last 48 hours in Korea, and therefore contribute to predict the time of death and to help to determine appropriate treatments. Methods : The data for this study was recorded from 92 of 132 patients who died with terminal cancer at the hospital between February 1 and October 31, 2000. We investigated the symptom prevalence during the last 48 hours through medical obligation record and analyzed the changes of symptom prevalence at the admission, $48{\sim}24$ hours and $24{\sim}0$ hours before death. Results : The predominant symptom prevalence was pain (57.6%), followed by confusion (55.4%), dyspnea (48.9%), voiding difficulty (42.4%) in the last 48 hours before death. From the statistical analysis for the changes of symptom prevalence in time, pain, nausea and vomiting were decreased but noisy and moist breathing, sweating, groan, restlessness and agitation, and loss of consciousness were increased (P<0.05). Conclusion : The results from this study show the tendency to increase of prevalence of noisy and moist breathing, sweating, groan, restlessness and agitation as well as loss of consciousness in 48 hours before death. Therefore the symptoms above can be used for the important indicators to predict the imminent death.

  • The number of motor vehicle registrations in Korea is increasing steadily each year, driven by industry development and economic growth. The number of traffic accidents is also rapidly increasing. Korea has a relatively high number of traffic accidents among OECD member countries, and it ranks among the highest in traffic accident death rates. This death rate is higher compared to death rates as a proportion of the number of traffic accidents in each country. It is very common for drivers to lose consciousness in traffic collisions, which leads to a failure to carry out early emergency measures. In order to prevent such situations as well as hit-and-runs and people left uncared for after traffic accidents, there is a need for motor vehicle black boxes and accident report systems. This study addressed the need for an emergency evacuation system for people injured in traffic accidents and a secondary traffic accident prevention system by developing a motor vehicle emergency situation detection and report system combined with a black box, and materializing it as an actual system.
