Sa traduction française montre bien « form is content, content is form ».

  • Long QT syndrome is a cardiac repolarization disorder and is associated with an increased risk of torsades de pointes. The acquired form is most often attributable to administration of specific medications and/or electrolyte imbalance. This review provides insights into the risk for QT prolongation associated with drugs frequently used in the treatment of chronic pain. In the field of pain medicine all the major drug classes (i.e. NSAIDs, opioids, anticonvulsive and antidepressant drugs, cannabinoids, muscle relaxants) contain agents that increase the risk of QT prolongation. Other substances, not used in the treatment of pain, such as proton pump inhibitors, antiemetics, and diuretics are also associated with long QT syndrome. When the possible benefits of therapy outweigh the associated risks, slow dose titration and electrocardiography monitoring are recommended.

  • Although amiodarone is generally regarded as safe with a low incidence of associated arrhythmias, torsade de pointes (TdP) has been observed usually in the presence of predisposing factors. We report a case of amiodarone-induced TdP after long-term administration of alow dose of oral amiodarone in the absence of predisposing factors.

  • Cornelia de Lange syndrome (CdLS) is a rare multisystemic disorder that is characterized by mental retardation, prenatal and postnatal growth retardation, limb anomalies, and distinctive facial features, which include arched eyebrows that often meet in the middle (synophrys), long eyelashes, low-set ears, small and widely spaced teeth, and a small and upturned nose. Ophthalmic manifestations include long eyelashes, nasolacrimal duct obstruction, myopia, ptosis, and strabismus. There has been no report of surgical treatment for esotropia and unilateral ptosis in patients with CdLS in Korea. I report a patient with CdLS who underwent surgical treatment for esotropia and unilateral ptosis with a good surgical outcome.

  • The origin of magnetic fields in the universe remains an outstanding problem in cosmology. We propose that these fields are produced by shocks during the large-scale structure formation. We discuss the mechanism of the field generation via the counter-streaming (Weibel) instability. We also show that these Weibel-generated fields are long-lived and weakly coupled to dissipation. Subsequent field amplification by the intra-cluster turbulence may also take place, thus maintaining the magnetic energy density close to equipartition.

  • The numerical solution of the Navier-Stokes equations in a constricted stepped channel problem has been obtained using a fourth order numerical method. Transformations are made to have a fine grid near the sharp corner and a long channel downstream. The derivatives in the Navier-Stokes equations are replaced by fourth order central differences which result a 29-point computational stencil. A procedure is used to avoid extra numerical boundary conditions near the solid walls. Results have been obtained for Reynolds numbers up to 1000.

  • The covid 19 pandemic led to an economic collapse and multiple impacts upon Argentina's labour dynamics. As well as in other parts of the region, falls in employment rates (both due to an increase in unemployment and significant withdrawals from the labour force) were combined with wage reductions for those who were able to keep their jobs. Thus, two important processes for the labour market complimented each other: a structural shock associated with a tasks automation as a reorganisation and substitution of factors, with a cyclical recession caused by the pandemic. The international experience shows the amplifying impact the latter has on the former, generating long-term consequences mainly in routine-intensive jobs. However, the knowledge-intensive services sector appears to be the most capable of cushioning the recessionary shock (both in terms of wages and labour absorption), even with nuances within the sector depending on the extent of the shutdown measures and its capability to switch to remote work. Finally, the task approach is decisive in capturing the ability to adapt both the cyclical and structural processes, absorbing a large part of the explanatory potential that sectoral classifications tend to bring about.

  • The Monte Carlo criticality simulation of decoupled systems, as for instance in large reactor cores, has been a challenging issue for a long time. In particular, due to limited computer time resources, the number of neutrons simulated per generation is still many order of magnitudes below realistic statistics, even during the start-up phases of reactors. This limited number of neutrons triggers a strong clustering effect of the neutron population that affects Monte Carlo tallies. Below a certain threshold, not only is the variance affected but also the estimation of the eigenvectors. In this paper we will build a time-dependent diffusion equation that takes into account both spatial correlations and population control (fixed number of neutrons along generations). We will show that its solution obeys a traveling wave dynamic, and we will discuss the mechanism that explains this biasing of local tallies whenever leakage boundary conditions are applied to the system.

  • Torsades de pointes refers to polymorphic ventricular tachycardia (PMVT), which is caused by the suppression of potassium channels owing to genetic and electrolytic abnormalities, resulting in the extension of the QT interval. Symptoms range from spontaneous circulation recovery to fainting and sudden death. Defibrillation, magnesium correction, and the use of lidocaine as an antiarrhythmic agent are recommended as treatments for persistent torsades de pointes. Currently, only amiodarone is available in the ambulance; however, torsades de pointes does not respond efficiently to amiodarone because it suppresses potassium channels and increases the refractory period of the myocardium. Lidocaine, in contrast, reduces the relative refractory period of the myocardium caused by suppressing sodium channels; thus, it inhibits the occurrence of and treats arrhythmia. In cases where PMVT did not respond to defibrillation, the administration of lidocaine showed no difference in survival and discharge rates compared to amiodarone. Thus, ambulances must be equipped with provisions to administer lidocaine.
