Oblique transnasal wiring canthopexy via Y-V epicanthoplasty for telecanthus correction in a patient with Waardenburg syndrome

  • Telecanthus is a common symptom accompanied by Waardenburg syndrome, a rare genetic disorder. The optimal surgery for telecanthus correction is still debated. A 28-year-old patient with Waardenburg syndrome underwent transnasal wiring canthopexy using a Y-V epicanthoplasty for telecanthus correction. A Mini-Monoka stent was used to prevent damage to the lacrimal apparatus. The intercanthal distance decreased from 50 mm to 43.2 mm. The easily designed Y-V epicanthoplasty incision provides sufficient operative field for oblique transnasal wiring, which is effective in properly positioning the medial canthal tendon. It has minimal scarring resulting in satisfactory cosmetic outcomes.

  • Through articles published in the 『Seungjeongwon Ilgi』, the following conclusions were obtained by analyzing how the Joseon royal family used Asi-acupoint therapy. Various Asi-acupoints were used in the Joseon royal family to treat diseases; King Sukjong was the king who was treated the most with Asi-acupoint therapy, followed by King Yeongjo, King Hyeonjong, King Hyojong and King Injo. The body parts with the most Asi-acupoint therapy are lower extremities, followed by interbody, upper extremities, other parts, and head. Asi-acupoint therapy was treated evenly throughout the body. The most common disease using Asi-acupoint therapy is pain, followed by abscesses, other symptom, internal damage, and external infections. When Asi-accupoint was executed, the ratio of acupunture and moxiubustion was 65:35, indicating that acupuncture was somewhat frequently used.

  • Objective : Recently, Seguele of Traffic Accident patients are increasing, especially cervical injury bring only severe sequela but also mortal damage to the patient. In treatment of the disease, the east medical treatment is another effective method. Methods : The patient resisted the west theraphy improved greatly in movement and sensation by acupuncture, moxibustion, herb medication and Physical treatment. In MRI, The treatment result is proved. Acupuncture on Stomach Meridian(足陽明胃經) referring to Naijing(內經) is thought effective. Results : Tetraplegia by Traffic Accident is considered as Wei Symptom in term of the east medicine, By means of radial and physical study of the patient treated by the east medicine, we are conviced that the east medicine is the excellent method Conclusions : Referring to the past documentary data, The only Treatment on this disease is Surgical operation, but the operation isn't the successful one because seg. of spinal cord injury is mortal. The east treatment, acupuncture, moxibustion herb medication and physical treatment isn't only effective but also satisfactory. In the future, The study on this disease by the east medicine should be made thoroughly.

Physiological Responses of Soybean Cultivars to Fusarium solani f. sp. glycines Causing Sudden Death Syndrome

  • Six soybean cultivars having different SDS susceptibility were planted with sorghum seedinoculum infested with F. solani isolate 171 in the greenhouse. First leaf symptoms appeared on unifoliar leaves at 9 days after inoculation and all cultivars showed the typical leaf symptoms at 13 days after inoculation, when trifoliar leaves emerged. Leaf symptoms development in susceptible cultivars was faster than in resistant cultivars. Leaf symptom severities during the period of 25 to 29 days after inoculation showed a significant difference between cultivars which had SDS resistance and sus ceptibility. In this period, area under the diseaseprogress curve (AUDPC) of Hartz 6686 was the highest and that of PI 520733 was the lowest. SDS caused serious damage to the growth of soybean in all cultivars. Average reductions of growth rate of root fresh weight and dry weight were greater than those of plant tops. Duyu-kong showed less severe leaf symptoms than that of SDS suscetible cultivars; however, average growth rate of plants top and roots of this cultivar was less but not significantly different than those of SDS susceptible cultivars. In all cultivars, as severity of leaf symptoms increased, plant top weight decreased. Root rot symptoms were observed in all cultivars before leaf symptoms appeared. Average proportions of tap root reddish-brown discoloration of all cultivars was up to 75 % at 15 days after inoculati on; however there was no significant differenc between cultivars at each rating date. Appearances of leaf symptoms on leaves varied in each cultivar. SDS resistant cultivars had a significantly higher level of crinkling than susceptible cultivars and SDS susceptible cultivars had a significantly higher level of necrosis than resistant cultivars. Further study will be needed to identify the relationships between the physiological growth rate and SDS severities in soybeans.

  • The current market size of insect industry in Korea is estimated at 300 million dollars and more than 500 local farms are related to many insect industry. One of the strong candidates for insect industry is Korean horn beetle, Allomyrina dichotoma. Early this year, we reported a viral disease extremely fatal to A. dichotoma larvae. While we were proceeding a nationwide investigation of this disease, it was informed that similar disease symptom has been occurred occasionally during past over 10 years. The symptom can be easily confused with early stage of bacterial infection or physiological damage such as low temperature and high humidity. A peroral infection with the purified virus to healthy larvae produced a result that only 21% of larvae survived and became pupae. Although some of the survived adult beetle was deformational, many of them had no abnormal appearance and even succeeded in mating. Later, these beetles were examined if they were carrying the virus, and all except one were confirmed as live virus carrier. This implies that these beetles may fly out and spread the disease to the nature. We found the evidence for this possibility by collecting a few wild A. dichotoma larvae which were virus infected, near two local farms rearing A. dichotoma larvae. So far, transovarial transmission of this virus to the eggs, or horizontal transmission to other commercially reared insects is not known yet.

  • The Diagnosis and Treatment of Jaundice According to the Morbid Pulse in the Chapter 15 of Synopsis of Golden Chamber sets forth the classification, symptom, method of treatment, formula, prognosis of jaundice. The contents of this chapter can be induced by(from) yellowing and jaundice These two disease patterns assume various aspects of disease cause, pathomechanism, symptom, prognosis, and treatment. Jaundice was at first classified into liquor jaundice,dietary irregularly jaundice, black jaundicem abd sextual taxation jaundice, later classified into liquor jaundice, dietary irregularly jaundice, black jaundice, sextual tatation jaundice, and yellow sweat. the disease cause and pathomechanism of jaundice are said to be said to be due to the damp-heat, which is attacked with the evil of wind and dampness to affect the spleen-stomach and the liver. and to the inveterate drinking, damage by food, sextual taxation jaundice, sextual taxation, and vacuity taxation. This chapter mainly refers to jaundice as damp-heat, so that it lays down fundamental principles of clear heat and disinhibit dampness. In the prognosis of jaundice, ingibited urination shows symptoms of jaundice, and neither urination nor damp-heat develops symptoms of jaundice. and jaundice with thirst is difficult to cure, and that the symptoms of the abdominal fullness is also difficult to cure, while jaundice without thirst is ease to cure. Of the 18 days of suffering from jaundice, the symptoms of jaundice get better before and after 10 days. I think, however, that getting worse of its symptoms makes more meaningful clinically.

  • Objective: Facial nerve palsy is caused by damage to the 7th cranial nerve. It is the main symptom of facial muscle paralysis on the affected side. Usually, recovery from this disease begins 2-3 weeks after onset and most patients recover in 4-8 weeks. If the patients cannot receive proper treatment, severe permanent impairments, both physical and mental, may remain, so this disease should be treated appropriately. In this study, a patient with facial nerve palsy was admitted to the Korean medicine hospital for treatment. We report on the patient's progress and the effects of treatment. Methods: We cured the patient with herbal medicines, acupuncture, herbal acupuncture therapy, and physical therapy. We used a numerical rating scale, the House Brackmann grading system, and a weighted regional grading system to assess symptom changes. Result: The patient with facial nerve palsy was hospitalized for 23 days and recovered from symptoms without significant problems on the face or in motor function.

Visual Analysis for Detection and Quantification of Pseudomonas cichorii Disease Severity in Tomato Plants

  • Pathogen infection in plants induces complex responses ranging from gene expression to metabolic processes in infected plants. In spite of many studies on biotic stress-related changes in host plants, little is known about the metabolic and phenotypic responses of the host plants to Pseudomonas cichorii infection based on image-based analysis. To investigate alterations in tomato plants according to disease severity, we inoculated plants with different cell densities of P. cichorii using dipping and syringe infiltration methods. High-dose inocula (${\geq}10^6cfu/ml$) induced evident necrotic lesions within one day that corresponded to bacterial growth in the infected tissues. Among the chlorophyll fluorescence parameters analyzed, changes in quantum yield of PSII (${\Phi}PSII$) and non-photochemical quenching (NPQ) preceded the appearance of visible symptoms, but maximum quantum efficiency of PSII ($F_v/F_m$) was altered well after symptom development. Visible/near infrared and chlorophyll fluorescence hyperspectral images detected changes before symptom appearance at low-density inoculation. The results of this study indicate that the P. cichorii infection severity can be detected by chlorophyll fluorescence assay and hyperspectral images prior to the onset of visible symptoms, indicating the feasibility of early detection of diseases. However, to detect disease development by hyperspectral imaging, more detailed protocols and analyses are necessary. Taken together, change in chlorophyll fluorescence is a good parameter for early detection of P. cichorii infection in tomato plants. In addition, image-based visualization of infection severity before visual damage appearance will contribute to effective management of plant diseases.

Up-regulation of NHE8 by somatostatin ameliorates the diarrhea symptom in infectious colitis mice model

  • $Na^+/H^+$ exchangers (NHEs) have been shown to be involved in regulating cell volume and maintaining fluid and electrolyte homeostasis. Pooled evidences have suggested that loss of $Na^+/H^+$ exchanger isoform 8 (NHE8) impairs intestinal mucosa. Whether NHE8 participates in the pathology of infectious colitis is still unknown. Our previous study demonstrated that somatostatin (SST) could stimulate the expression of intestinal NHE8 so as to facilitate $Na^+$ absorption under normal condition. This study further explored whether NHE8 participates in the pathological processes of infectious colitis and the effects of SST on intestinal NHE8 expression in the setting of infectious colitis. Our data showed that NHE8 expression was reduced in Citrobacter rodentium (CR) infected mice. Up-regulation of NHE8 improved diarrhea symptom and mucosal damage induced by CR. In vitro, a similar observation was also seen in Enteropathogenic E. coli (EPEC) infected Caco-2 cells. Seglitide, a SST receptor (SSTR) 2 agonist, partly reversed the inhibiting action of EPEC on NHE8 expression, but SSTR5 agonist (L-817,818) had no effect on the expression of NHE8. Moreover, SST blocked the phosphorylation of p38 in EPEC-infected Caco-2 cells. Taken together, these results suggest that enhancement of intestinal NHE8 expression by SST could ameliorate the symptoms of mice with infectious colitis.

Compression of the Superficial Radial Nerve by Schwannoma: A Case Report (신경초종에 의한 표재요골신경의 압박)

  • Purpose: Schwannoma, a benign peripheral nerve tumor, is slow-growing, encapsulated neoplasm that originates from the Schwann cell of the nerve sheath. Schwannoma most frequently involves the major nerve. Schwannoma occurring in the superficial radial nerve rare. This is a report of our experience with schwannoma arising from the superficial radial nerve with neurologic symptom. Methods: A 55-year-old woman presented with eight-month history of progressive numbness and paresthesia in dorsum of the thumb and index finger. Physical examination revealed a localized mass on the midforearm. Sonographic examination showed an ovoid, heterogenous, hypoechoic lesion, located eccentrically in related to the superficial radial nerve. The lesion was mobile in the transverse but not in the longitudinal axis of the nerve, which was thought to favour schwannoma rather than neurofibroma. At operation, a $20{\times}15mm$ ovoid, yellowish grey mass was seen arising from the superficial radial nerve. The tumor present as eccentric masses over which the nerve fibers are splayed. Using operating microscope, the tumor was removed, preserving the surrounding nerve. Results: Histology confirmed that the mass was a benign schwannoma. There were no postoperative complications. After two months the patient had no clinically demonstrable sensory deficit. Conclusion: An unsusual case of a schwannoma of the superficial radial nerve is presented. In case with neurologic symptom, prompt surgical decompression must be made to prevent further nerve damage and to restore nerve function early.
