A study of Clinical DW for utilizing analysis of medical treatment information (진료정보 분석 활용을 위한 Clinical DW에 관한 연구)

  • So far, DW(data warehouse) of hospital has been used as tool for analyzing patient-focused data. However, EMR(Electronic Medical Record) is established these days, so informal data which is record and video record could be useful to get some information for patient remedy, not as DW data. This study claims that need of establishing treatment-focused DW, not for hospital administration-focused DW which has been used lots of hospital DW. Also we discussed how CDW can be applied for real medication situation. At last, we deduct a relation past record of sick and wounded patient as Thesaurus searching method by real hospital data for establishing base of early-treatment system.

Comparison of Virulence Factors of Enterococci from Intestinal Drugs, Infant Feces and Clinical Isolates (정장제, 신생아 분변 및 병원에서 분리한 장구균의 병독성인자 비교)

  • Three isolates, E. faecium P1, P2 and P3, from intestinal drugs of three phamaceutical companies, four clinical vancomycin resistant isolates, E. faecium V1, V2, V3 and E. faecalis V4, and three isolates, E. faecalis DW01, DW07 and DW14, from infant feces were tested for the presence of virulence genes, ace, agg, esp, efaA, gelE, sprE, vanA and vanB as well as fsrABC, regulatory genes of gelE and sprE, cylMBA, cytolysin activation genes and cpd, cob and ccf, pheromone genes by PCR and for their phenotype activities such as protease, biofilm formation, cell clumping and hemolysis. The genes encoding cell surface adherence proteins, ace, agg, esp and efaA, were predominantly amplified from the vancomycin resistant strain V4 and the fecal isolates DW01, DW07 and DW14. Both protease and biofilm formation activity were detected only from E. faecalis V4 from which the PCR products of gelE and spreE as well as fsrABC were amplified. The pheromone genes were amplified from the V4, DW01, DW07 and DW14 strains and these strains showed clumping activity. Biofilm formation was observed from the strains DW01, DW07 and DW14, all of which produced PCR products of pheromone, and V4 as well. Whole cytolysin regulator genes were amplified from DW01, DW07 and DW14 and ${\beta}$-hemolysis activity was detected from these strains. Any virulence genes or activities except the pheomone gene ccf were not detected from the pharmaceutical isolates, E. faecium P1, P2 and P3.

Bactericidal Activity of DW286, a New Fluoroquinolone Antibiotic (새로운 퀴놀론 항균제 Dw286의 살균작용)

  • The bactericidal activities of DW286, a new fluoroquinolone were investigated by comparing the minimal bactericidal concentrations (MBCs) and the time-kill curve of it against some Gram-positive and Gram-negative bacterial strains. The MBCs of DW286 for the strains tested were either equal to or two-fold higher than the MICs, as were observed for the other fluoroquinolones. And DW286 exhibited rapid killing curves against the strains. Accordingly, it could be said that DW286 has bactericidal activity comparable to other fluoroquinolones.

  • Antigenic potential of DW-116, a newly synthesized fluoroquinolone, was examined by conduc-ting active systemic anaphylaxis (ASA), passive cutaneous anaphylaxis (PCA) and passive hemagglutination (PHA) tests. In ASA test, mild to moderate signs of anaphylactic responses were observed in the groups sensitized with low (2 mg/body) and high (10 mg/body) doses of DW-116 alone and the group sensitized with DW-116 plus adjuvant. Some moderate to severe anaphylactic reactions were observed in the group sensitized with a DW-116-bovine serum albumin (BSA) conjugate plus adjuvant when challenged with a DW-116-guinea pig senHn albumin (GSA) conjugate. However these reactions were considered to be a cross-reaction between BSA and GSA since similar reactions were induced when challenged by GSA alone. In heterologous PCA test using mice and rats, positive responses were not detected in any of the experimental groups. In PHA test, positive responses were observed in the groups sensitized with low and high doses of DW-116 alone and the group sensitized with DW-116 plus adjuvant. However, these responses were not considered to be drug-specific because some positive responses were also seen in the negative control group. From these results, it was concluded that DW-116 is not likely to have specific antigenic potential in clinical use.

Implementing Data warehouse Methodology Architecture: From Metadata Perspective

  • Recently, many enterprises have attempted to construct data warehousing systems for decision-support. Data warehouse is an intelligent store of data that can aggregate vast amounts of information. Building DW requires two important development issues:(i) DW for the decision making of business users and (ii) metadata within it. Most DW development methodologies have not considered metadata development; it is necessary to adopt a DW development methodology which develops a DW and its metadata simultaneously. Metadata is a key to success of data warehousing system and is critical for implementing DW. That is, metadata is crucial documentation for a data warehousing system where users should be empowered to meet their own information needs; users need to know what data exists, what it represents, where it is located, and how to access it. Furthermore, metadata is used for extracting data and managing DW. However, metadata has failed because its management has been segregated from the DW development process. Metadata must be integrated with data warehousing systems. Without metadata, the decision support of DW is under the control of technical users. Therefore, integrating data warehouse with its metadata offers a new opportunity to create a more adaptive information system. Therefore, this paper proposes a DW development methodology from a metadata perspective. The proposed methodology consists of five phases: preparatory, requirement analysis, data warehouse (informational database) development, metastore development, and maintenance. To demonstrate the practical usefulness of the methodology, one case is illustrated

Diffusion-Weighted MR Neurography with Unidirectional Motion-Probing Gradient to Evaluate Lumbar Nerve Roots at 1.5T MR (요추 신경근 평가를 위한 1.5T MR의 단일 방향 경사자장을 사용한 확산강조 자기공명신경조영)

  • Purpose Recent studies have demonstrated the usefulness of diffusion-weighted MR neurography (DW MRN) for assessing nerve roots. This study aimed to evaluate the utility of DW MRN with a unidirectional motion-probing gradient (MPG) for the lumbar nerve roots at 1.5T MR. Materials and Methods Sixty-four lumbar spine MRI scans with DW MRN using anteroposterior unidirectional MPG were retrospectively analyzed. Any changes in the 512 lumbar spinal nerve roots from L3 to S1 were evaluated using T2-weighted imaging (T2WI), contrast-enhanced T1-weighted imaging (CE T1WI), and DW MRN, with agreement and correlation analysis. Results T2WI revealed compression of 78 nerve roots, and CE T1WI revealed 52 instances of nerve root enhancement. Sixty-seven nerve roots showed swelling and hyperintensity on DW MRN. A total of 42 nerve roots showed changes in the CE T1WI and DW MRN sequences. Moderate to substantial agreement and moderate positive correlation were observed between DW MRN and CE T1WI, as well as DW MRN and T2WI (κ = 0.59-0.65, ρ = 0.600-0.653). Conclusion DW MRN with unidirectional anteroposterior MPG can help evaluate neuritisrelated changes in spinal nerve roots and could serve as a sequence capable of complementing or substituting gadolinium CE imaging.

The Extracts from Natural Product Complex (DW) Suppress Lipid reroxidation and Inhibit Liver Fibrosis (Cirrhosis) in Rats (실험동물에서 복합천연물(DW)의 지질 과산화 억제와 간섬유화 저해 효과 검색)

  • The chronic cholestasis induce to biliary liver fibrosis (cirrhosis) and the increased products of ROS(reactive oxygen species) cause to the liver damage. In this study ; the antioxidant and antifibrotic effect of dried extracts of oriental medicine (DW) was investigated under the liver fibrotic (cirrhotic) condition. The female Sprague-Dawley rats were divided in 5 groups (Normal, Op-2, Op-4, OpDW-2, OpDW-4). Except for normal group, the rats were induced to biliary liver fibrosis (cirrhosis) by the operation of bile duct ligation/scission (BDU/S) and were observed in 2 weeks or 4 weeks. And the prepared DW was treated p.o.2 ml/day/rats in 2 weeks or 4 weeks for OpDW groups. At the time of sacrifice, the liver, kidney, spleen were weighed and the ratio of organ weight/body weight was calculated. The MDA, the hyp and biochemical parameters (GOT GTP, ALP, t-bili) were measured in sera and liver tissue of rats. The biochemical change was observed on liver tissue. In the result, the hepatomegaly and spleenomegaly appeared in all BDL/S operated rats, and significantly lower liver weight was observed in OpDW-4 group compared with in Op-4 group (p<0.05). The level of clinical parameters in sera of all liver fibrosis (cirrhosis) developed rats was higher than in normal group. Especial1y, the value of GOT in OpDW-2 group and ALP in OpDW-4 group showed significantly lower than in Op-2 group and Op-4 group (p<0.01, p<0.005). The content of hyp in all operation groups was significantly higher than in normal group (p<0.05∼<0.005), and showed significantly lower value in the OpDW-4 group than in Op-4 group (p<0.05). The product of lipid peroxidationUDA) increased significantly under the fibrotic(cirrhotic) condition (p<0.05∼ <0.005), and the MDA value in OpDW-4 group decreased significantly in Op-4 group (p<0.005). The histological change (bile duct proliferation, fibrosis, collagen bundle) was similarly observed in Op-2 group and in OpDW-2 group, but the weak fibrosis and bile duct proliferation were observed in OpDW-4 group compared with in Op-4 group. In conclusion, lipid peroxidation and severe liver damage were activated by bile duct obstruction, and the measurement of MDA and hap can be useful monitor for the screening of antioxidant and antifibrotic effect in experimental liver fibrosis (cirrhosis). The 4 weeks treatment with DW extracts suppressed lipid peroxidation and inhibited fibrotic (cirrhotic) process in BDL/S operated rats.

Environmental Adaptation of Tall Fescue Varieties in Mountainous Pastures IV. Evaluation of winter hardiness by DW / RTAR (Tall Fescue 품종의 환경적응성 IV. 상대경수 출현율당 개체중에 의한 월동성의 평가)

  • This experiments was carried out to evaluate the potential winter hardiness of tall fescue varieties based on the data of dry weight of plant per relative tiller appearance rates(DW/RTAR) in mountainous pastures of Taekwalyon area. The results obtained were as follows; 1. The varieties were classified into 2 groups by dry weight of plant per relative tiller appearance rates (DW/RTAR). Thus. Forager and Fuego belong to the group with more than 2.0 in DW/RTAR but Barcel, Barvetia, Demeter, Enforcer. Johnstone, Safe and Stef belong to the group with less than 2.0 in DW/RTAR respectively. 2. The dry weight of plant per relative tiller appearance rates(DM/RTAR) and dry weight of plant(DW) of 1st cutting were significantly positive correlated with number of heading tillers and dry weight of heading tillers. 3. The dry weight of plant per relative tiller appearance rates(DW/RTAR) was significantly positive correlated with dry weight of plant(DW) of final cutting and of 1st cutting. 4. The varieties with high DW/RTAR showed a high dry weight of plant(DW) of 1st cutting. Accordingly, it was concluded that DW/RTAR is a valuable character for evaluating potential winter hardiness of tall fescue on mountainous pastures of Taekwalyon area.

  • DW2282, (S)- (+)-4-phenyl -1-[N-(4-am mob enzoyl) -indolin-5- sulfonyl]-4,5- dihydro-2-imidazolone hydrochloride, is a novel anticancer agent thought to have an unique mechanism of action on the inhibition of tumor growth. In this study, we estimated in vivo antitumor activities and pharmacokinetics of Dw2282 depending on various vehicles. The inhibition rate of tumor growth was increased by 50, 100 and 200 mg/kg of Dw2282 in a dose-dependent manner. When Dw2282 dissolved in 4 sorts of vehicles was orally single dosed to rats at 50 mg/kg, Cmax of Dw2282 in 0.5% CMC.Na was a half as high as those in PG, PG+CP and PG+CP+DW. When Dw2282 was orally administered to mice for 5 days, antitumor activity of 130 mg/kg suspended in 0.5% CMC.Na was as effective as that of 65 mg/kg dissolved in the rest of vehicles. Taken together, it is thought that antitumor activities of Dw2282 are resulted from the absorption extent of it and related to the vehicle used.

  • Direct observation of the DW dynamics in a bifurcated wire reveals that the propagation is via the splitting of DW at the junction, resulting in individual DW in each branch. The DMI induced DW tilting leads to quasi-selective propagation through the network structure, with favored branch determined by the tilting angle of the DW surface. This results in the DW in the individual branches having different depinning fields. Our work shows that by tuning the DMI constant in a material, selective DW motion through a network can be achieved.
