Clinical Practice Guideline for Soyangin Disease of Sasang Constitutional Medicine: Yin-Deficit Diurnal-Heat (Eumheo-oyeol) symptomatology (소양인체질병증 임상진료지침: 음허오열병)

  • Objectives This research was carried out to establish the clinical practice guideline(CPG) for Yin-Deficit Diurnal-Heat (Eumheo-oyeol) symptomatology of Soyangin disease. Methods Dongeuisusebowon(sinchuk edition) and several kinds of literatures including journal articles concerning this symptomatology of Soyangin disease were collected and classified. Sasang constitutional specialists' conference was held to make an agreement on the conflicting issues as well. Consensus was drawn as a result of the conference. Results & Conclusions 3 papers were selected as an inclusion and exclusion criteria for the relevant articles to Yin-Deficit Diurnal-Heat (Eumheo-oyeol) symptomatology of Soyangin disease. Yin-Deficit Diurnal-Heat (Eumheo-oyeol) symptomatology consists of two aspects : Yin-Deficit Diurnal-Heat (Eumheo-oyeol) severe pattern and Yin-Deficit Diurnal-Heat (Eumheo-oyeol) critical pattern. In Yin-Deficit Diurnal-Heat (Eumheo-oyeol) severe pattern contains 1 disease, namely, Clear Yang Depletion of Large Intestine (Daejang-cheongyang Moson) pattern (Lower wasting-thirst (Haso) pattern). In Yin-Deficit Diurnal-Heat (Eumheo-oyeol) critical pattern contains 2 diseases, Yin-Deficit Diurnal-Heat (Eumheo-oyeol) pattern and Yin-Deficit Diurnal-Heat (Eumheo-oyeol) advanced pattern. Yin-Deficit Diurnal-Heat (Eumheo-oyeol) symptomatology has several kinds of symptoms like dry mouth, disliking to drink much water, diurnal body fever, coldness on the back and nausea as well as body fever, chest discomfort, constipation or dry stool as a common symptoms of Interior Heat disease. Clear Yang Depletion of Large Intestine (Daejang-cheongyang Moson) pattern (Lower wasting-thirst (Haso) pattern) has above mentioned symptoms and much urine/turbid urine, thin thigh and knee joints and twinge of joint pain over the body. Yin-Deficit Diurnal-Heat (Eumheo-oyeol) pattern has symptoms like indigestion and epigastric discomfort, abdominal pain and vomiting in addition. Yin-Deficit Diurnal-Heat (Eumheo-oyeol) advanced pattern has symptoms like hematemesis as well.

Characteristics of Semi-diurnal and Diurnal Currents at a KOGA Station over the East China Sea Shelf

  • The long-term mooring performed at a KOGA station, located at about $30^{\circ}20^{\prime}N$, $126^{\circ}12^{\prime}E$ in the East China Sea shelf, shows some different behaviors between "semi-diurnal" and "diurnal currents" defined as the currents with periods around, respectively, a half day and a day. They appear to be predominantly tidal having significant coherences with sea level changes around the semi-diurnal and diurnal frequencies. The "semi-diurnal current" is strongly barotropic all year round. However, contrastingly, it is largely baroclinic in summer in the area about 70 km nearer to the continental slope, referred to as the "slope-area", as was found in previous current observations. The "diurnal current" of tidal origin is strongly barotropic in winter. In spring and summer, however, it becomes more baroclinic although it still remains largely barotropic, also showing more of its barotropic nature than in the "slope-area". The inertial oscillation contributing to the "diurnal current" appears to be more prominent when the current is baroclinic, indicating the important role played by stratification in generation of inertial oscillations. Downward energy propagation of inertial oscillation is not observed, suggesting that it is not created at the surface by wind. Considering that the study area is both near a critical latitude and proximity to the continental slope, it is suggested that parametric subharmonic instability (PSI) plays a significant role in creating the baroclinic inertial oscillation.

Characteristics of Diurnal Variation of Wind over the South Korean (우리 나라의 바람 일변동 특성)

  • The purpose of this study is to find out the temporal and spatial characteristics of the diurnal wind variation between coastal and inland areas using the hourly wind data of 58 AWS-stations in the South Korea which are collected during the 10 years from 1980. Diurnal variation is investigated by using the Fast Fourier Transform(FFT), and the wind direction in investigated by comparing C(sub)r with C(sub)v represented the constancy of wind. For the scalar wind speed, the maximum wind speed occurs in the daytime 14h to 16h. The maximum diurnal amplitude at coastal areas occurs from 12h to 16h, and is about 2 hours faster than that at inland areas. Vector mean wind speed is strong at coastal areas and Chupungnyong, Kumi, Taegu of inland areas. The diurnal variation ellipses make a right angle with coastline show that the land and sea breezes are prevailing. The constancy of wind is strong at all observations in January. In the relationship between $C_r and C_v, C_v is higher than C_r$.

Study on Blood Cortisol in Patients treated with Oral Corticosteroid Drugs (경구 Corticosteroid계 약물 치료한 환자에 있어서 혈중 cortisol에 관한 연구)

  • This is an analysis of 32 patients who received long continuous corticosteroid drug due to some diseases. Patients were collected from June 1976 to March 1980. Blood cortisol level, variation of diurnal rhythm and side effects were studied. The Result as follows: 1. Side effects were observed in 24 patients(75%) and most common complaint was obesity. 2. Diurnal rhythm analysed by Doe's method shows abnormal diurnal rhythm is 21 out of 32(66%). 3. Mean durations or therapy of abnormal diurnal rhythm and normal diurnal rhythm were $55.7{\pm}4.4$ months and $43.9{\pm}7.0$ months respectively which shows statistically significant difference. 4. Mean cortisol value of steroid treated patients were lower than normal. 5. Reverse diurnal rhythm was 4 out of 21 patients. 6. 8 A.M. cortisol value is lower than 2 times of 8 P.M. in all patients who showed abnormal diurnal rhythm except one.

Analysis of Diurnal and Semidiurnal Cycles of Precipitation over South Korea (한반도 강수의 일주기 및 반일주기 성분 분석)

  • The hourly precipitation data from 1973 to 2007 observed at 60 weather stations over Korea are used to characterize the diurnal and semidiurnal cycles of total precipitation amount, intensity and frequency and examine their spatial patterns and interannual variations. The results show that the diurnal cycle peaks in the morning (03-09LST) and the semidiurnal cycle peaks in the late afternoon (16-20LST). It is found that the spatial variations of the peak phase of diurnal or semidiurnal cycle relative to their corresponding seasonal mean cycle are considerably small (large) for total precipitation amount and intensity (frequency, respectively) in both winter and summer seasons. Also, the diurnal phase variations for individual years relative to the seasonal mean precipitation show the significant interannual variability with dominant periods of 2-5 years for all three elements of precipitation and the slightly decreasing trend in total precipitation amount and intensity. To compare the relative contributions of frequency and intensity to the diurnal and semidiurnal cycles (and their sum) of total precipitation amount, the percentage variance of each cycle of precipitation amount explained by frequency is estimated. The fractional variance accounted for by precipitation intensity is greater than that of frequency for these three cycles. All above analyses suggest that intensity plays a more important role than frequency in the diurnal variations of total precipitation amount.

Relationship between the Diurnal Temperature Range and Wound Healing of Diabetic Foot: Animal Study (일교차가 당뇨병성 창상의 치유에 미치는 영향: 동물 실험 연구)

  • Purpose: Diabetic foot ulcers are closely related to body surface heat, which can be affected easily by temperature differences. This study examined the correlation between the healing process of diabetic wounds and abnormal diurnal temperature through an animal study. Materials and Methods: Rats in the abnormal diurnal temperature group and control group were given a 10 mm sized full-thickness skin ulcer. Wound size progression was observed in both groups. H&E and Masson's trichrome staining was performed at 14 days after wound formation, and the number of vessels per unit area and histology analysis were performed. The changes in the ulcer were measured through three dimensional cross-section area using INSIGHT® devices. Results: The wound recovery period (granulation ingrowing) was 24 days in the abnormal diurnal temperature model and 20 days in the control group. The thickness of scar tissue was 402±23.19 ㎛ in the control group and 424.5±36.94 ㎛ in the diurnal temperature model. Neovascular formation was counted as 5.1±0.97 for the control group and 4.16±0.94 for the diurnal temperature model group. Conclusion: Delayed and inferior diabetic wound healing was observed in the abnormal diurnal temperature group, which was characterized by greater diurnal variations than the typical growth environment.

Diurnal Variation and Sleep Pattern in Depressive Patients (우울증 환자의 주간기분변동과 수면 양상)

  • Object: Diurnal variation is included in the diagnostic criteria of the major depressive disorder, melancholic specifier. But there has been controversy over whether diurnal variation is an unique depressive symptoms or a symptom related to a change of sleep patterns, or that of another mechanism, when the previous studies are reviewed. We investigated the existence of diurnal variation according to the subtype of depression and whether diurnal variation is charateristic of melancholic depression or not. We also compared sleep variables according to the existence of diurnal variation. Method: We examined diurnal variation, sleep patterns, severity of depression using the Visual Analogue Mood Scale, Pittsburgh Sleep Quality Index, and Hamilton Depression Rating Scale. Patients recorded their mood state on the Visual Analogue Mood Scale twice a day, morning and evening, for diurnal variation. We divided depressive patients into two groups,-diurnal variation group and nondiurnal variation group,-and compared the mood and sleep variables using SPSS. Results: The frequency of diurnal variation is not significantly different among the subtypes of depression. Significant differences between the diurnal variation group and the nondiurnal variation group existed in middle insomnia and sleep time (p<0.05). In melancholic type, al significant difference between the diurnal variation group and the nondiurnal variation group was noticed in PSQI total, sleep latency, sleep disturbances, daytime dysfunction as well as middle insomnia and sleep time (p<0.05). Conclusions: Diurnal variation existed in other types of depression as well as melancholic type. The results showed that diurnal variation was not a specific symptom of melancholic type, and existence of diurnal variation might be related to sleep patterns.

Exploring Subjective Stress, Sleep and Diurnal Variation of Salivary Cortisol in Korean Female Adults (여대생의 스트레스, 수면, 타액 코티솔 일중변동 연구)

  • Purpose: The purpose of this exploratory study was to find a specific time of day with a stable cortisol level and to investigate the relationship between salivary cortisol and sleep. Methods: A total of 36 Korean female college students participated in the study. Salivary specimens were collected 6 times a day for 2 days in different stressful situations. Sleep characteristics were measured using an actigraph while salivary specimens were collected. Perceived stress was evaluated using the Global Assessment of Recent Stress. Results: Depending on whether there were morning peak and/or afternoon elevations in the cortisol levels, the type of diurnal cortisol pattern was classified into 4 types. None of the cortisol levels in different times of the day showed significant relationships to perceived stress levels. Cortisol levels in the morning, levels of peak cortisol and diurnal differences of cortisol were significantly correlated with sleep duration. The time with most stable cortisol level was 9-10 pm. Conclusion: It is recommended that measurements of salivary cortisol are taken from 9-10 pm since it showed a stable value regardless of diurnal cortisol rhythm and sleep. Sleep duration should be considered as an important confounding factor in measuring cortisol levels in the morning and the diurnal differences of cortisol.

An Estimation of Tidal Currents from Satellite-tracked Drifters and its Application to the Yellow Sea

  • A simple but effective method has been developed for estimating diurnal and semi-diurnal tidal currents from trajectories of satellite-tracked drifters. The estimation method consists of separation of tidal current signals contained in the drifter trajectories, computation of undulations by diurnal and semi-diurnal currents, and correction of dominant diurnal and semi-diurnal tidal constituents. M$_2$ tidal currents estimated from drifter trajectories in the Yellow Sea are well consistent with those observed by moored current meters and this supports the validity of this method. We have constructed M$_2$ tidal current chart in the Yellow Sea by applying this method to available drifter trajectories collected during 1994-1998. According to this chart, M$_2$ current in the Yellow Sea rotates in the clockwise direction south of 35$^{\circ}$ 30'N but in the counterclockwise one to the north. Also it is found that the M$_2$ current is strong in the bank area northeast of the Changjiang River mouth and in the Korean coastal area, while it is weak in the deep central trough.

Preschooler's Internal Representation Profile Types and Diurnal Cortisol Regulation Pattern at Home (유아 내적 표상 유형과 가정에서의 코티솔 패턴 변화)

  • The purposes of this study were to explore the relations of preschoolers' narrative representation profile types to diurnal cortisol regulation pattern at home. Fourteen story stems from the MacArthur Story Stem Battery (MSSB, Bretherton, et al., 1990) were administered to 40 preschoolers(22 boys, 18 girls, aged 5) recruited from 8 kindergartens in the Seoul and Gyeonggi areas. And also, their saliva cortisol was collected. The children's responses were aggregated into 5 dimensions, based on content themes and performance scores, which included emotions expressed and narrative coherence using the MacArthur Narrative Coding System(Robinson, et al., 2004). Data were analyzed by means of cluster analysis. Five response profiles emerged over the course of this research: Prosocial, Anxiety, Dyregulated aggression, Restricted, and Avoidance profiles. Cortisol at home showed decrease diurnal cortisol regulation patterns, however it showed a difference according to narrative response profiles. Restricted, Prosocial, and Avoidance profiles showed decrease diurnal cortisol regulation patterns. Anxiety profiles showed flat pattern, and Dyregulated aggression profile showed rising diurnal cortisol regulation pattern. These results show the preschoolers' internal representation may affect the diurnal cortisol reaction in daytime.
