The association of oral diseases and chronic diseases in Korean adult population (우리나라 성인의 구강질환과 만성질환의 관련성)

  • Objectives : The purpose of this research is to use data from the third year of the 4th National Health and Nutrition Examination Survey to study relationship between oral disease and chronic disease that generally persist from 6 months to over a year, or more specifically, circulatory disease, diabetes, and osteoporosis. Methods : Of the data from the third year of the 4th National Health and Nutrition Examination Survey, 7,893 adults over 19 years old who completed medical examination, health survey, and nutrition survey were selected as the final research subjects. Relationship between chronic disease and oral disease was analyzed by cross tabulation (2-test) and logistic regression analysis using SPSSWIN ver 18.0. Results : 1. Differences in the rate of prevalence of periodontal disease and the rate of prevalence of missing teeth were statistically significant with respect to age, gender, marital status, education level, residential area, income level, and occupation. Rate of prevalence of dental caries was statistically significant with respect to age, education level, and income level. 2. After examining the relationship between existence of chronic disease as diagnosed by doctor with oral disease, rate of prevalence of periodontal disease and missing teeth, hypertension, hyperlipidemia, stroke, myocardial infarction, angina, diabetes, and osteoporosis showed statiscally significant difference. 3. Examination of rate of prevalence of chronic disease with respect to oral disease, periodontal disease and missing teeth exerted statistically significant influence on hypertension, hyperlipidemia, stroke, myocardial infarction, angina, diabetes, and osteoporosis (p<0.05), while dental caries did not have statistically significant effect. 4. Analysis of coupling effect of periodontal disease and missing teeth on chronic disease showed that they were related in all chronic diseases examined in this study (hypertension, hyperlipidemia, stroke, myocardial infarction, angina, diabetes, and osteoporosis) (p<0.05). Conclusions : Periodontal disease and missing teeth were found to increase the rate of prevalence of chronic disease.

  • 1. Purpose 'Gu-Bon' of ${\ulcorner}$Soose Bowon${\lrcorner}$ found lately was written in 1940, through comparing with 'Gu-Bon' and 'In-Bon'.   • Objectives : The purpose of this study is to find the principal of the assignment of Sidong disease and Sosaeng disease(是動病 所生病) into 12 meridians and suggest the author's opinion. Methods : 1. The authors investigated the conception of Sidong disease and Sosaeng disease through several literatures. 2. The authors investigated the line course of 12 meridians(經脈流注) and their Sidong disease and Sosaeng disease. 3. The authors classified Sidong disease and Sosaeng disease following the study by Kim et al. 4. The authors suggested the opinions about the diseases that are difficult to be understood direct relation with the course of meridian. Results : 1. The result of classification of Sidong disease and Sosaeng disease into 5 shows that the percentages were 32.96% for meridian's own disease(本經病), 13.97% for organic own disease(本臟腑病), 12.85% for other organic own disease(他臟腑病), 20.67% for related organic disease(有關器官病), 19.55% for etc.(其他病). 2. Therefore, 19.55% of the whole Sidong disease and Sosaeng disease is that which occurred on the site that is not related directly with the meridian. Conclusions : 1. The exterior and interior relation(表裏關係) and mutual communication between organ and bowel(臟腑相通) are associated with the basic principal of the assignment of Sidong disease and Sosaeng disease that is not related with the course of meridian. 2. The cause of assignment of Sidong disease and Sosaeng disease can be explained according to the profound medical theories.

Longterm Benefit and Unmet Needs in Enzyme Replacement Therapy of Gaucher (고셔병 환자에서 효소대체치료의 장기적 효과와 한계)

  • Gaucher disease is a multisystemic disorder arising from a deficient activity of the lysosomal enzyme glucocerebrosidase, which leads to accumulation of glycosylceraide and other glycolipids in the regiculoendothelial system. The characteristics of Gaucher disease are anemia, thrombocytopenia, hepatosplenomegaly, and skeletal disease. Enzyme replacement therapy (ERT) has been proven to prevent progressive manifestations of Gaucher disease and effective in improving anemia, thrombocytopenia, bone markers and biomarkers. However, some patient needs still remain unmet because of the inaccessibility of certain sites including brain, bone and various organs. ERT could not Improve the irreversible lesion such as liver fibrosis, hepatopulmonary syndrome, and necrosis or infarction of bone and other organs. Adult patients with Gaucher disease should be screened for longterm complication such as bone disease, pulmonary hypertension, gallstone, and cancer, especially in patients with splenectomy. Parkinsonism and polyneuropathy was also reported among patients with type 1 Gaucher disease, but ERT does not improve neurological function. We need to review the benefits and unmet needs of ERT in Gaucher disease.

Factors Affecting the Preventive Behavior of Cardiocerebrovascular Disease in Blue Color Workers (생산직 근로자의 심뇌혈관질환 예방실천행위에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to investigate blue color workers' knowledge and attitudes about cardiocerebrovascular disease, and these influence on their preventive health behavior. Methods: Data were collected from 235 blue color workers using questionnaire in 'H' city. Data were analyzed using hierarchical multiple regression analysis with SPSS/WIN 17.0 version. Results: Health behaviors for preventing cardiocerebrovascular disease were correlated positively with knowledge about cardiocerebrovascular disease. In addition, preventive behaviors for cardiocerebrovascular disease was influenced by marital status, monthly income, past history of cardiocerebrovascular disease, and knowledge about cardiovascular disease. Conclusion: Knowledge about cardiocerebrovascular disease has an effect on preventive behaviors regarding cardiocerebrovascular disease. For preventing cardiocerebrovascular disease, national campaign and health education program in each industry need to be built.

  • From documentary comparative study on the chief virtues of the Back-Su(兪) points and the Front-Mo(募) points, the following results are obtained : 1. Contrast the Back-Su(兪) points with the Front-Mo(募) points, the needle-steadying depth of the Front-Mo(募) points is deeper than that of the Back-Su(兪) points. and Moxibustion dosage of the Front-Mo(募) points is more than that of the Back-Su(兪) points. 2. In the case of Kimun(LR14) and Kansu(BL18), Kergwol(CV12) and Shimsu(BL15), Chungwan(CV12) and Wisu(BL21), Kyungmun(GB25) and Shinsu(BL23), the Back-Su(兪) points can be chiefly used for the treatment of acute disease, external disease, exess disease and Jang(臟)-disease. contrary the Front-Mo(募) points can be chiefly used for the treatment of chronic disease, internal disease, deficiency disease and Bu(腑)-disease. 3. In the case of Kwangwon(CV4) and Sojangsu(BL27), Serkmoon(CV5) and Samchosu(BL22), Changmun(LR13) and Bisu(BL20), Chungbu(LV1) and Pesu(BL20), Chungguk(CV3) and Banggwangsu(BL28), the Front-Mo(募) points can be used for the treatment of unbalanced visceral funtion. contrary the Back-Su(兪) points can be used for not only the treatment of unbalanced visceral funtion but also the treatment of attached organs. 4. The Back-Su(兪) points can be used for the treatment of acute disease, external disease, exess disease, Jang(臟)-disease and attached organs-disease. contrary the Front-Mo(募) points can be chiefly used for the treatment of chronic disease, internal disease, deficiency disease, and Bu(腑)-disease.

Relationship of Follow-up Management, Trend of Possible Occupational Disease and Probable Occupational Disease: Focusing on Lead or Cadmium Workers (직업병 유소견자 및 요관찰자 추이 및 사후관리와의 관련성: 납 및 카드뮴 취급근로자를 중심으로)

  • Objectives: This study identifies the trend of possible occupational disease and probable occupational disease in lead or cadmium workers. It is also required to check the usefulness of follow-up management in lead or cadmium workers by reviewing the relevance between the results of follow-up management and the trend of possible occupational disease and probable occupational disease. Methods: From 2009 to 2018, the results of the Ministry of Employment and Labor's special health-screening program for hazard agents for workers were used. The correlation between the ratio of possible occupational disease and probable occupational disease of lead or cadmium workers and the ratio of those who observed were and underwent observed follow-up management was analyzed to confirm the usefulness of follow-up management. Results: Over the past decade, the average annual proportion of possible occupational disease and probable occupational disease among lead workers has been on the decline. Among cadmium workers, it has generally shown a trend of increasing and decreasing. After the implementation of follow-up management, possible occupational disease in lead workers showed significant relevance to work prohibitions and restrictions, and probable occupational disease in lead workers showed significant relevance to the work prohibitions and restrictions, on-duty treatment, and boundary lines. However, there was no significant correlation between persons involved in cadmium workers. Conclusion: In this study, more active managements such as work ban and restrictions, on-duty treatment among follow-up management of possible occupational disease and probable occupational disease of lead worker and observers are related to a decrease in the ratio of those who have been diagnosed with possible occupational disease and probable occupational disease.

Anxiety, Depression, and Quality of Life in Parents of Adolescents with Inflammatory Bowel Disease: A Longitudinal Study

  • Purpose: The parents of adolescents with inflammatory bowel disease may experience impaired mental health and quality of life. This longitudinal study aimed to verify whether the mental health and quality of life of the parents of adolescents with inflammatory bowel disease declined when their children had active disease. Methods: Sociodemographic data, parental anxiety, depression, and quality of life were analyzed using validated questionnaires for each variable. After the baseline survey, the second and follow-up surveys were conducted at 3 and 12 months, respectively. The active disease group comprised eight parents whose children had active disease during the baseline and second surveys. The remission group comprised 14 parents whose children remained in remission during both surveys. The improved group comprised nine parents whose children experienced active disease at baseline and remission during the second survey. Parental mental health and quality of life were compared among the groups. Results: Significantly higher levels of anxiety were observed in the active disease group in all surveys (p<0.050). Although depression levels and quality of life did not differ significantly among the three groups, pairing the active disease group with other groups showed some large effect sizes. Conclusion: Parents tended to experience decreased mental health and quality of life when their adolescents experienced active inflammatory bowel disease. Consequently, our hypothesis was partially verified. Therefore, parents need support when their children have active disease; this finding highlights the need for parental support systems.

Treatment and management of patients with inherited metabolic diseases (유전성 대사질환의 치료 및 관리)

  • Inherited metabolic disease is rare disorders that show symptoms mainly in pediatric age and early treatment is important for preventing complications of the disease. Recent development in molecular and biochemical techniques help clinicians with proper diagnosis of patients, however, many of the disease still remain lack of effective therapeutic strategies. Better understanding on biochemical and molecular basis of pathogenesis of the disease combined with advanced medical care would provide new sight on the disease that can also improve the quality of life and long-term prognosis of patients. Traditionally, there are several modalities in the treatment of metabolic diseases depend on the biochemical basis of the disease such as diet restriction, removing or blocking the production of toxic metabolites, and stimulating residual enzyme activity. The inherited metabolic disease is not familiar for many clinicians because the diagnosis is troublesome, treatment is complicated and prognosis may not as good as expected in other diseases. Recently, new therapeutic regimens have been introduced that can significantly improve the medical care of patients with metabolic disease. Enzyme replacement therapy has showed promising efficacy for lysosomal storage disease, bone marrow transplantation is effective in some disease and gene therapy has been trying for different diseases. The new trials for treatment of the disease will give us promising insight on the disease and most clinicians should have more interest in medical progress of the metabolic disease.
