Differences in dietary intakes, body compositions, and biochemical indices between metabolically healthy and metabolically abnormal obese Korean women

  • BACKGROUND/OBJECTIVES: There are various factors that affect metabolic abnormalities related to obesity. The purpose of this study is to analyze the differences in dietary intakes and body compositions of obese women according to metabolic risks and to classify them as metabolically healthy obese (MHO) or metabolically abnormal obese (MAO). SUBJECTS/METHODS: This study was conducted on 59 obese Korean women aged 19 to 60 years. NCEP-ATPIII criteria were applied and the women classified as MHO (n = 45) or MAO (n = 14). Body composition of each subject was measured by using dual-energy x-ray absorptiometry (DEXA). Three-day food records were used to analyze dietary intake. Eating habits and health-related behaviors were determined through questionnaires. Indirect calorimetry was used to measure resting metabolic rate and respiratory rate. RESULTS: The average age of the subjects was 43.7 years. The analysis of body composition according to phenotype revealed significantly higher body fat mass (P < 0.05), arm fat mass (P < 0.05), and android fat mass (P < 0.05), as measured by DEXA, in the MAO group than in the MHO group. There was no significant difference in the dietary intake of the two groups. However, eating behaviors differed. Compared to the MHO group, the MAO women had a shorter meal time (less than 10 minutes), a preference of oily foods, and a tendency to eat until full. Therefore, the eating habits of MHO women were more positive than those of MAO women. CONCLUSIONS: The results suggest that fat distribution in each body region affects various metabolic abnormalities. A high level of arm fat mass in obese Korean women may increase metabolic risk. In addition, eating habits of obese Korean women are considered to be environmental factors affecting the metabolic phenotype of obese Korean women.

Reduction of the Useless Radiation Exposure to Patients and Improvement of the Skill to Manage the Test according to Minimizing Changes of Posture in Bone Mineral Density (골밀도 검사에서 환자 자세 변경 최소화에 따른 수검자의 불필요한 피폭선량 감소 및 생산성 향상)

  • As we recognize the health care, test of BMD is increased. There are various methods in BMD, Dual Energy X-ray absorptiometry (DEXA) which has high precision and accuracy, and low medical exposure dose has been widely used. To measure the changes of BMD, the test must be done as same posture at first and follow up study performed a year. we analyze the difference in the hour taken before and after the examination by radiologic technologist, frequency of scout scan, and the amount of the radiation exposure. The hour of the examination and frequency of the scout scan were shorten and the dose of the radiation exposure is reduced. As the numerical value of total subjects is converted into that of one person, the duration for the test was 52 seconds, the frequency for the scout scan 0.79 time, and the dose for the radiation exposure $13{\mu}Sv$. When the health care provider perform the precise and easy methods to minimize changes of posture and the skillful ability to manage the test, useless radiation is decreased.

Correlation between lumbar spinal stenosis and bone mineral density : a clinical survey of 9 cases (요추관 협착증과 골밀도와의 상관성에 관한 임상례 보고)

  • Objective : The aim of this study was to evaluate association between lumbar spinal stenosis, one of degenerative diseases, and bone mineral density. Methods : We monitored 9 lumbar spinal stenosis patients in women above 50 years, visiting outpatient clinic of the Jaseng Oriental Medicine Hospital between January 5 2006 and March 31, 2006. They were diagnosed by radiologist ist after taking Magnetic Resonance Imaging(MRI). The cases were Investigated the bone mineral density using Dual Energy X-ray Absorptiomerty(DEXA). and then Picture Archiving Communication System(PACS) were used to assess correlation between lumbar spinal stenosis and bone mineral density. Results : 1. In comparison of the spinal canal area and lumbar spine 2 level bone mineral density, the data showed a significant result 2. The data, between spinal canal area and lumbar spine 1-2 level bone mineral density, indicated a significant result. 3. Also, the result of comparison between spinal canal area and the lowest value of bone mineral density showed significance. Conclusions : It showed that there is a statistically significant correlation between lumbar spinal stenosis and bone mineral density.

The Study of Diagnostic Results Discordance Analysis on BMD Using DEXA (이중에너지 X선 흡수 계측법을 이용한 골밀도 검사 시 진단불일치에 대한 분석)

  • Purpose : This study was conducted to understand for the diagnostic discordance of bone mineral density(BMD) in DEXA(Dual Energy X-ray Absorptiometry). And the diagnostic difference between lumbar spine and femur neck, forearm was evaluated using T-score. Materials and Methods : We studied 220 females measured BMD on lumbar spine, and femur neck, forearm including ward's triangle and ultra digital(UD). We were distinguished T-score into normal, osteopenia, osteoporosis(WHO classification) and evaluated discordance rate according to age and degree of bone loss. Correlation analysis and chi-square test between L-spine, L-4, femur neck, Ward, Forearm, UD were carried out. Results : In the lumbar spine, the number of normal were in 57(25.9%), osteopenia in 86(39.1%), osteoporosis in 77(35.0%). In the L-4 and ward's triangle, the number of osteoporosis were in 78(35.5%), in 126(57.3%). There was significant correlation between lumbar, femur neck and forearm BMD in all cases. The discordance of BMD between lumbar and femur were 57%, lumbar and forearm 43%, forearm and femur 51%. The discordance rates of normal, osteopenic, osteoporotic groups were 39%, 64%, 43%, respectively, showing the highest discordance rate in osteopenia patients. In normal group of lumbar spine, the discordance rate was 25%, 23%, 11%, 65%, 86% in 30', 40', 50', 60', 70', respectively. In osteopenia, osteoporosis group of lumbar spine, the discordance rate was 62%, 55%, 36%, 20%, 9% in 30', 40', 50', 60', 70', respectively. Conclusion : It was different of the results of BMD with lumbar, femur and forearm site. The discordance rate was decreased with age in osteopenia, osteoporosis lumbar spine. In osteopenia group, the discordance rate was the highest. So, it is necessary that the BMD of lumbar, femur neck and forearm should be checked.

The Study of Technical Error Analysis on BMD Using DEXA (이중 에너지 X선 흡수 계측법을 이용한 BMD 검사 시 발생할 수 있는 기술적인 오류 분석)

  • Purpose: This study was conducted to search for the type of technical error in DEXA(dual-energy X-ray absorptiometry) and the effect of error to measurement of BMD. Materials and Methods: The changes of BMD($g/cm^2$, T-score) by patients information(Age, Weight, Height, Manopause age) input error and Confirming ROI error were investigated. Using spine phantom, we canned 10 times by age(5, 10), weight(10, 20 kg), height(5, 10 cm), manopause age(5, 10) increase & decrease respectively. Scanning region(L-spine, femur, Forearm) of 10 patients was calculated by changing ROI respectively. Analysis of difference for mean(precision 1%) were carried out. Results: The error of patient information(Age, Weight, Height, Manopause age) was not changed differently. In confirming ROI, the BMD and T-score of L-spine involving T-12 was decreased to $0.063\;g/cm^2$, 0.3 and involving L-5 increased to $0.077\;g/cm^2$, 0.5. In narrowing 1 cm of vertical line of ROI, the BMD and T-score decreased to $0.006\;g/cm^2$, 0.1 and in 2 cm, $0.021\;g/cm^2$, 0.15, each. In hip ROI, Upper and left shift(0.5 cm) of line was not influenced BMD and T-score. In 0.5 cm lower shift(lesser trochanter below), the BMD and T-score increased $0.031\;g/cm^2$, 0.3 and in 1 cm $0.094\;g/cm^2$, 0.65, each. In forearm ROI, the BMD and T-score decreased $0.042\;g/cm^2$, 0.9 involving 1 cm lower wrist. And expanding 1 cm of vertical line, the BMD and T-score decreased $0.008\;g/cm^2$, 0.1 and in 2 cm, $0.021\;g/cm^2$, 0.3, each. The L-spine, hip, forearm ROI error was changed differently. Conclusion: There are so many kinds of technical error in BMD processing. Errors according to age, weight, height, manopause age did not influent to $BMD(g/cm^2)$ and T-score. There are mean differences BMD and T-score in confirming ROI. For the precision exam, in L-spine processing, L1-4 have to confirmed without shift of ROI vertical line. In hip processing, the ROI have to included greater trochanter, femur head and lesser trochanter. In forearm processing, the ROI have to included wrist, radius and ulnar.

Germinated Rhynchosia nulubilis Hydrolysate Ameliorates Dexamethasone-induced Muscle Atrophy by Downregulating MAFbx Expression in C2C12 Cells and C57BL/6 Mice (발아 서목태 가수분해물의 근위축 억제 효과)

  • Sarcopenia is the age-related loss of muscle mass and function. It is a natural part of aging and can lead to decreased mobility and increased frailty. The ubiquitin-proteasome pathway, which is involved in muscle protein degradation, is closely linked to sarcopenia. Germinated Rhynchosia nulubilis hydrolysate (GRH) has been reported to have anti-inflammatory and antioxidant properties, but there have been no reports on its inhibitory effect on muscle reduction. However, no study has yet explored the relationship between GRH and muscle loss inhibition. In this study, we evaluated the effects of GRH on muscle atrophy inhibitory activity in dexamethasone (Dexa)-induced muscle atrophy C2C12 myotubes and mouse models. Moreover, we identified a molecular pathway underlying the effects of GRH on skeletal muscle. May Grunwald-Giemsa staining showed that the length and area of myotubes increased in the groups treated with GRH. In addition, the GRH-treated group significantly reduced the expression of muscle ring finger protein 1 and muscular atrophy F-box (MAFbx) in the Dexa-induced muscular atrophy C2C12 model. GRH also improved muscle strength in C57BL/6 mice with Dexa-induced muscle atrophy, resulting in prolonged running exhaustive time and increased grip strength. We found that muscle strengthening by GRH was correlated with a decreased expression of the MAFbx gene in mouse muscle tissue. In conclusion, GRH can attenuate Dexa-induced muscle atrophy by inhibiting the ubiquitin-proteasome pathway via downregulation of the MAFbx gene expression.

A Study of Bone Mineral Density of Lumbar Spine by Dual Energy X-ray Absorptiometry (DEXA) in Children (Dual Energy X-ray Absorptiometry를 이용한 소아들의 골밀도에 대한 연구)

  • The bone mineral density(BMD) of the lumbar spine (L2-L4) was measured by using dual energy x-ray absorptiometry(Norland XR26 DEXA) in 80 children aged between 2months and 15years (group 1 : 2month-1years, group 2 : 1year-5years, group 3 : 6years-10years, group 4 : 11years-15years). The correlation coefficient of BMD with age, body weight, height and Tanner stage were 0.696, 0.693, 0.717 and 0.636 respectively. There were significant difference in BMD($g/cm^2$) between group 1(BMD : $0.335{\pm}0.175$) and group 2(BMD : $0.627{\pm}0.200$). and group 3(BMD : $0.714{\pm}0.189$) and group 4(BMD : $0.873{\pm}0.163$)(P<0.05). There was no significant difference of BMD between boys and girls(P<0.05). BMD also increased significantly with development of Tanner stages(Tanner stage 1 : $0.547{\pm}0.234$, Tanner stage 2 : $0.783{\pm}0.136$, Tanner stage 3 : $0.998{\pm}0.080$ )(P<0.05). These data indicate that the BMD was correlated with age, body weight, height and Tanner stage significantly and BMD increased significantly during growth spurt occured in 1 to 4years of age and puberty.

  • This study was conducted to investigate the relationship of calcium intake , serum calcium level to calcium supplementation and the effect of nutritional education in middle-aged women. The age distribution of the subjects was 49-55 years. Daily nutrient intakes and dietary sources of calcium were analyzed by convenient method and food frequency questionnaire. At the same time, fasting blood samples were collected and serum calcium and magnesium concentrations were measured before and after nutritional education. The BMD of subjects were measured by Dual Energy X-ray Absorptiometry(DEXA). Serum calcium and magnesium concentrations were significantly(p<0.05) different between pre and post nutritional education. After nutritional education, nutrient intakes were significantly increased(p<0.05), especially calcium. This results suggested that dietary calcium intake as well as nutritional education seem to be important in preventing in middle-aged women.

  • There were few reports about the fluoride concentration in bone and osteoporotic women. This study was designed to evaluate the relationship between the urinary fluoride concentration and periodontal condition in osteoporotic old women. Twentyeight postmenopausal women(Test group) and twenty-one premenopausal women(Control group) were examined. Bone mineral density(BMD) of lumbar spine(L2-L4) was measured by dual energy X-ray absorptiometry(DEXA). The urine samples were collected at early morning and determined with the help of a fluoride-specific electrode and Tisabbufferd samples. The results were as follows. 1. The mean urinary fluoride concentration in test and control group showed statistically no difference. 2. The bone mineral density(BMD) of the spine in test group was significantly lower than control group(p<0.05). 3. The significant negative correlation was found between BMD level and age after menopause $(p<0.001,\;{\gamma}=-0.526$. 4. The urinary fluoride concentration was not correlated with age, age after menopause and bone mineral density. 5. The urinary fluoride concentration was not correlated with periodontal condition.

  • This study was designed to examine the morphological measurement and characteristics of trabecullae based on femoral radiographic image for prediction of osteoporosis. Study subjects were 34 females (average age of 62.1 years) and 6 males (average age of 60.1 years), they were categorized into normal group and osteoporosis group in accordance with the T-score value. Measurement of the bone density of femoral bone was measured with DEXA(Dual Energy X-ray absorptiometry). ROI(Region of interests) was selected on femoral neck and trochanter. Characteristics of trabecullae was analyzed by using the skeletonization analysis of trabecular image. Morphological measurement was analyzed through femoral radiographic image in order to examine the correlation with osteoporosis. The result demonstrated statistically significant correlation between neck cortical thickness, shaft width, shaft cortical thickness, periphery, mean gray level and trabeculae area with BMD average (T-score) of femoral part. The results show that morphological measurement and characteristics of trabecullae based on femoral radiographic images for osteoporosis prediction could be effective.
