Solitary Ancient Schwannoma in Lower leg -Case Report- (하퇴부에 발생한 고립성 퇴행성 신경초종 -증례 보고-)

  • When the old schwannoma has the degenerative changes, it is named as the ancient schwannoma which was rarely reported. The degenerative changes are perivascular hyalinization, calcification, cystic necrosis, marked decrease of Antoni type A area, and degenerative nuclei and the ancient schwannoma may be misinterpreted as sarcomatous pleomorphisms. We experienced the ancient schwannoma which has massive hemorrhage and hematoma, cystic change, calcification, and marked decrease of Antoni A area in leg on 75 year old male patient, and report it.

Effects of Spinal Stabilization Exercises on the Cross-sectional Areas of the Lumbar Multifidus and Psoas Major Muscles of Patients with Degenerative Disc Disease

  • Purpose: To evaluate, in patients with degenerative disc disease (DDD), the efficacy of using spinal stabilizing exercises for the reversal? of atrophy of the multifidus and psoas major, reductions in pain and disability, and for increases in paraspinal muscle strength. Methods: Nineteen patients diagnosed with DDD participated for 10 weeks in a spinal stabilization exercise program. Pain and disability were measured before and after exercise using, respectively, a visual analogue scale (VAS) and the Oswestry Disability Index (ODI). Paraspinal muscular strength in four directions was evaluated using CENTAUR. Both before and after exercise we used computed tomography (CT) too measure cross-sectional areas (CSAs) of both the left and right multifidus and the psoas major at the upper & lower endplate of L4. Results: After 10 weeks of a spinal stabilization exercise program, pain was significantly decreased from $5.7{\pm}0.9$ to $2.5{\pm}0.9$ (p<0.01); the ODI score decreased from $16.7{\pm}4.9$ to $7.3{\pm}3.1$. Paraspinal muscle strength was significantly increased (p<0.01) and the CSAs of the left and right multifidus and psoas major muscles were significantly increased (p<0.01). Conclusion: Spinal stabilization exercise is effective in reversing atrophy in DDD patients, in reducing pain and disability, and in increasing paraspinal muscle strength. It is an effective treatment foro aiding rehabilitation in these cases.

Retrolisthesis as a Compensatory Mechanism in Degenerative Lumbar Spine

  • Objective : Posterior vertebral translation as a type of spondylolisthesis, retrolisthesis is observed commonly in patients with degenerative spinal problems. Nevertheless, there is insufficient literature on retrolisthesis compared to anterolisthesis. The purpose of this study is to clarify the clinical features of retrolisthesis, and its developmental mechanism associated with a compensatory role in sagittal imbalance of the lumbar spine. Methods : From 2003 to 2012, 230 Korean patients who underwent spinal surgery in our department under the impression of degenerative lumbar spinal disease were enrolled. All participants were divided into four groups : 35 patients with retrolisthesis (group R), 32 patients with simultaneous retrolisthesis and anterolisthesis (group R+A), 76 patients with anterolisthesis (group A), and 87 patients with non-translation (group N). The clinical features and the sagittal parameters related to retrolisthesis were retrospectively analyzed based on the patients' medical records. Results : There were different clinical features and developmental mechanisms between retrolisthesis and anterolisthesis. The location of retrolisthesis was affected by the presence of simultaneous anterolisthesis, even though it predominantly manifest in L3. The relative lower pelvic incidence, pelvic tilt, and lumbar lordosis compared to anterolisthesis were related to the generation of retrolisthesis, with the opposite observations of patients with anterolisthesis. Conclusion : Retrolisthesis acts as a compensatory mechanism for moving the gravity axis posteriorly for sagittal imbalance in the lumbar spine under low pelvic incidence and insufficient intra-spinal compensation.

A Study on Osteoarthritis of The Knee And Associated Factors (퇴행성 슬관절염에 관련요인연구)

  • Purpose: We defined osteoarthritis of the knee as knee pain and crepitus in over 40 years old. The usual clinical manifestation include pain, stiffness, crepitus and loss of function. Methods: We studied 40 cases of the degenerative arthritis of knee clinically. The body mass index(BMI, weight(kg)/height($m^2$) was used as a measure of obesity. Pain self assessment scale by Million Index in according to age, occupation, BMI. Results: Gender by pain self assessment scale was 6.5 in male and 6.6 in female(P<0.05). The prevalence rates obese was 40%. Occupation by pain self assessment scale was 7.0 in Sitting and 5.7 in Standing(P<0.05). Prevalence was increased with age by pain self assessment scale in aged 40-69 years. Conclusion: It has been known that the obesity is one of the predispsing factors of the primary degenerative arthritis of knee. A flexion weight bearing view of the knee obtained at $30^{\circ}$ to $40^{\circ}$ of the joint flexion may be more sensitive in assessing damages to hyaline cartilage because the knee flexion is an important component of the stance phase.

Anti-inflammatory Effect of Herbal Complex HP-04 on Degenerative Osteoarthritis (해동피를 포함한 생약복합제제 HP-04의 퇴행성 골관절염에 대한 항염효과)

  • To investigate the anti-inflammatory effect on Degenerative Osteoarthritis, Male sprague-Dawley rats were randomized and classified into three different concentration groups. We measured weekly weight change, dietary intake, drinking water intake, blood analysis like TNF-α (tumor necrosis factor-α), IL-6 (interleukin 6), TIMP-1 (tissue inhibitor of metalloprotease-1), MMP-2 (matrix metalloprotease-2), MMP-9 (matrix metalloprotease-9) and micro CT analysis. The results suggest that the treatment with herbal complex HP-04 improved the Monosodium iodoacetate (MIA) induced degenerative osteoarthritis and it could be applicable for the improvement of arthritic symptoms as a new therapeutic agent.

An Antioxidative and Antiinflammatory Agent for Potential Treatment of Osteoarthritis from Ecklonia cava

  • Osteoarthritis is thought to be induced by the ageing-related loss of homeostatic balance between degeneration and repair mechanism around cartilage tissue in which inflammatory mediators such as reactive oxygen species, cytokines and prostaglandins are prone to overproduction under undesirable physiological conditions. Phlorotannins are unique polyphenolic compounds bearing dibenzo-1,4-dioxin skeleton which are not found in terrestrial plants but found only in some brown algal species such as Ecklonia and Eisenia families. Phlorotanninrich extracts of Ecklonia cava including LAD103 showed significant antioxidant activities such as DPPH radical scavenging, ferric ion reduction, peroxynitrite scavenging, and inhibition of LDL oxidation, indicating their possible antioxidative interference both in onset and downstream consequences of osteoarthritis. LAD103 also showed significant down regulation of $PGE_2$ generation in LPS-treated RAW 246.7 cells, and significant inhibition of human recombinant interleukin-$1{\alpha}$-induced proteoglycan degradation, indicating its beneficial involvement in pathophysiological consequences of osteoarthritis, the mechanism of which needs further investigation. Since LAD103 showed strong therapeutic potentials in arthritic treatment through several in vitro experiments, it is highly encouraged to perform further mechanistic and efficacy studies.

A study on the relationship of lumbar extensor muscle power and static spinal loaded test for old female patients with lumbar degenerative kyphosis (요추후만증을 가진 여자 노인 환자의 정적 척추부하 검사와 허리신전근력과의 관계)

  • The purpose of this study was to investigate relationship of lumbar extensor muscle power & spinal column curve for old female patients with LDK(Lumbar degenerative kyphosis). Subjects were composed of 37 old female with LDK. The subjects were tested in their spinal segment movement and spinal column curve with Spinal-$Mouse^{\leq}$ in $1^{st}$ loaded test and $2^{nd}$ loaded test and then tested lumbar extensor muscle power with $Medx^{\leq}$ lumbar extension machine. The results of this study, were as follow; There were statistically significant difference $1^{st}$ loaded test and $2^{nd}$ loaded test in upright position increase spinal column forwardly(p<0.01) and decrease lumbar lordosis angle(p<0.01), but no statistically significant difference $1^{st}$ loaded test and $2^{nd}$ loaded test thoracic and hip & sacrum curve angle. Their lumbar extensor muscle poser is very weakness, 61.4% of normal people.

Ultrastructural Changes in the Ganglion and Granule-Containing Cells in the Heart of Vacor-Induced Diabetic Mongolian Gerbil (Vacor 유발 당뇨 모래쥐의 심장신경절과 과립함유세포의 미세구조)

  • The ultrastructural changes of the cardiac ganglion and granule-containing cells in the heart of vacor-induced diabetic Mongolian gerbils were studied by electron microscopy. After one month of vacor-induced diabetes the ganglion cells showed increase in numbers of dense bodies and mitochondria compared with the normal cardiac ganglion. Most of the satellite cells were filled with numerous phagosomes containing digested debris. Both electron-dense and lucent types of degenerating axon terminals were observed. The former was characterized by clusters of agranular vesicles and numerous mitochondria. The electron lucent type of degenerating axon terminal contained a few agranular vesicles and swollen mitochondria. Degenerating unmyelinated and myelinated axons contained large numbers of dense bodies, lamellar bodies, and mitochondria. Numerous macrophages containing phagosomes were reveled in the interstitial spaces. Some of the granule-containing cells in the heart showed a variety of degenerative changes and a decreased number of dense-cored vesicles. After three months of vacor-induced diabetes the unmyelinated and myelinated axons showed degenerative changes, whereas no structure changes could be demonstrated in intraatrial ganglion and granule containing cells. The satellite cells containing engulfed debris were observed in the cardiac ganglion cells. These results suggest that the degenerative changes occur in the cardiac ganglion cells of vacor-induced diabetic Mongolian gerbils as well as atrial granule-containing cells.

Thread Embedding Acupuncture and Complex Korean Medicine Treatment for Lumbar Spinal Stenosis with Degenerative Scoliosis: A Clinical Case Report

  • Lumbar spinal stenosis (LSS) is a common degenerative spinal condition that can have unpredictable improvement and worsening of symptoms which include low back pain, radiating pain, claudication, and degenerative lumbar scoliosis affecting quality of life. In this study, thread embedding acupuncture (TEA) was used as a conservative treatment for LSS in combination with complex Korean medicine treatments (acupuncture, herbal medicines, and physical therapy). The treatment was evaluated using the numerical rating scale, walking distance and duration, and inclination of radiological lumbar scoliosis according to antalgic posture. TEA was performed 27 times between June 8, 2020, and March 16, 2021. The patient showed improvement in numerical rating scale score from 7 to 2, pain-free walking distance from 10 m to 900 m, and scoliosis inclination following treatment. The findings of this study suggest that TEA may be helpful in the treatment of LSS.

Effects of the Tai Chi Exercise Program on Physical Functional and Physiological Variables in Patients with Degenerative Arthritis (타이치 운동이 퇴행성 관절염 환자의 신체적 기능과 생리적 지수에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of a Tai Chi exercise program on physical function and physiological variables in patients with degenerative arthritis. Methods: The study utilized a nonequivalent control group with pretest-posttest design. Data collection was done with the elders from two welfare institutions in C-city between July I and September 22, 2007. The participants were assigned either to an experimental group (n=24) or to a control group (n=22). The experimental group participated in Tai Chi exercise for 60 minutes per session, twice a week for 12 weeks and the control group received the education about arthritis for 3 weeks. Results: Except for $VO_2max$, weight, and body fat rate, the elders in the experimental group showed significant improvement in physical function (grip strength, flexibility, balance), and physiological variables (BP) compared to the control group. Conclusion: The results suggest that Tai Chi exercise would partially improve physical function, and physiological variables. Further studies are needed to determine the effects on physical fitness and physiological variables after Tai Chi exercise in this population.
