  • Purpose : To evaluate our clinical experience with the combination of teletherapy and intraluminal brachytherapy in patients with unresectable or inoperable esophageal cancers. Materials and Methods : From Nov 1989 to Mar 1993, twenty patients with esophageal cancer were treated with radical radiotherapy and intraluminal brachytherapy at Yonsei Cancer Center. All patients had squamous histolgy and stage distribution was as follows: stage II, 4($20{\%}$)patients; III, 15 ($75{\%}$)patients; IV, 1($5{\%}$)patients. A dose of S-12Gy/1-3weeks with intraluminal brachytherapy (3-5Gy/fraction) to 5mm from the outside of the esophageal tube using high dose rate Iridium-192 remotely afterloading brachytherapy machine was given 2 weeks after a total dose of 59-64Gy with external radiotherapy. Induction chemotherapy using cisplatin and 5-FU was performed in 13 patients with median 3 cycles(1-6 cycles), Response rate, local control rate, survival and complications were analysed retrospectively. Results : Two-year overall survival rate and median survival were $15.8{\%}$ and 13.5 months. Response rates were as follows complete remission(CR) 5($25{\%}$): partial remission a(PRa) 7($35{\%}$): partial remission b(PRb) 7($35{\%}$), no response(NR) 1($5{\%}$). Patterns of failure were as follows; local failure 13($65{\%}$), local and distant failure 3($15{\%}$), distant failure 0($0{\%}$). Ultimate local control rate was $20{\%}$. Treatment related complications included esophageal ulcer in two patients and esophageal stricture in one. Conclusion : Though poor local conrol rate, median survival was improved as compared with previous results of radiation therapy alone(8months) and chemoradiation combined treatment(11 months) in Yonsei Cancer Center High-dose-rate intraluminal brachytherapy following external irradiation is an effective treatment modality with acceptable toxicity in esophageal cancer.

  • As a part of studies on potassium(K) behavior in grassland with respect to magnesium(Mg) balance of ruminants, seasonal variation of K and Mg contents of forages including native gasses grown in grazing pasture and meadow were investigated. During an experimental period from April to October of 1984, two times of grazings were carried out in the orchardgrass (Dacfylis glomerata L.) and the tall fescue (Festuca arundinacea Schreb.)dominant grazing pastures, and forage plants (forages and native grasses) were sampled monthly and also K and Mg contents were determined without separating into individual plant species (Experiment 1). All the plant species grown in the two meadows which situated in the grazing pastures were harvested five times during the same period, separated into individual plant species, and botanical composition (SDR, ) and K and Mg contents of the plant species were determined (Experiment 2). The results obtained were as follows: 1. During the experimental period in the orchardgrass grazing pasture K contents of the forage plants were the highest in spring, and the seasonal variation of the contents in the orchardgrass pasture (1.5-5.8 % in a dry matter basis) was more significant than that of forage plants in the tall fescue grazing pasture (3.0- 3.8 %). 2. The Mg contents of forage plants in the orchardgrass grazing pasture ranged under 2.0 mg/g DW from Arpil until July, and the contents in the orchardgrass pasture (1.5-3.1 mg/g DW) was in the lower range than that of forage plants in the tall fescue pasture (2.0-3.8 mg/g DW). (Experiment I). 3. Orchardgrass was the dominant species in the orchardgrass meadow until July, but several species of native grasses were observed from summer (July) and native grasses such as Digitaria adscendens and Echinochlw crus-galli became dominant in autumn (October). 4. Seasonal variation of K contents of orchardgrass was in the range of 3.9-5.9 %, and the contents was higher in spring (May) and in autumn (October). The variation of white clover (Trifolium repens L.) was in the range of 3.6-5.0 %, that of tall fescue 3.8-4.8 %, and that of Italian ryegrass (Lolium multiflorum Lam.) 2.7-3.5 %, respectively. 5 . Seasonal variation of Mg content of white clover was in the range of 2.9-3.7 mg, that of tall fescue 2.0- 3.3 mg, and that of orchardgrass 1.6-2.8 mg/g DW, respectively. The variation of the contents of Italian ryegrass was in the range of 1.3-1.9 mg/g DW. And Mg contents of the forage plants were higher in summer(July) 6. In autumn (October and November ) native grasses such as D. adscendens and E. crus-galli contained lower K contents (1.7-3.9 %), but higher Mg contents (3.2-10.1 mg/g DW) than the forages contained. (Experiment 2) From the results above, it is known that K contents ranged higher in younger forages in viewpoint of growth stage and higher in spring and autumn, and that Mg contents ranged lower in spring. Therefore, the mineral imbalance or hypomagnesaemic (grass) tetany can be triggered in spring or autumn, and more frequently by such plant species as orchardgrass and Italian ryegrass with lower Mg and/or higher K contents than by tall fescue. And it is suggested that the dominant native grasses in autumn such as D. adscendens and E. emsgalli can contribute to the prevention of the tetany with higher Mg and lower K contents.

  • To classify and analyze the forest communities and their structures, the vegetation in Mt. Joghe was investigated from July, 1980 to August, 1989. The results obtained are as follow ; 1. A total of 750 kinds of vascular plant(49 orders, 122 families, 434 genera, 627 species, 1 subspecies, 111 varieties and 11 forma)were observed in Mt. Ioghe. The newly observed plant species were Dioscorea quingueloba, Spiranthes sinensis, Cephalanthera falcata, Angelica gigas, Clematis patents, Paeonia obovata, Hibiscus mulabilis, Ainsliaea acerifolia, Dictamnus dasycarpus, Cynachum ascyrifolia, Vaccinium koreanum, Erythrortium japonicum, Indigofera kirilowii (17species), Broussonetia kazinoki var, humillis, Euonymus, fortunei var. radicans, Juniperus communis var, nippnnica, Callicarpa japonica var. radicans, Joniperus communis var. rzipponica, Callicarpa japonica var. taquetii (4 varieties) and L indera obtusiloba for. billosum (1 forma). 2. The life spectrum of flora in Mt. Joghe was classified into $CH-D_1-R_5-e$ type. Distribution area was identical to Southern type by Nakai, Lee, and Yim. A few subtropical species were also observed. 3. Simpson's species diversity index(Ds) was 0.9 and Shannon-Weiner's diversity index (H') was 1.004. These indice suggest that the vegetation in Mt. Joghe is of complicated forest communities. 4. Pte-Q was 1.81 which was higher than the nationwide mean of 1.68. Urbanization Index (UI) was 28.75 for naturalized plant species, and 17.49 for exotic woody plant species, which were similar to those of Mt. Baekun and Mt. Naejang. 5. The forest vegetation of Mt. Joghe was grouped in 3 vegetation types : 7 natural plant Communities dominated by Quercus serrat, Quercus acutissima, Quercus variabilis, Carpinus laxiflora, Pinus derasiflora and Platycarya strobilacea, 8 substitutional plant communities Styrax japonica, Stewartia koreana, Lindera erytlrrocarpa, Zelkova serrata, Rhtrs chinensis, Controversa, and Frzrxirtus manrlshurica, and 7 plantation Communities composed of Pinus koraiensis, Pinus rigida, Magnolia nbnvata, Chamecyparis obkrsa, Larie ieptolepis, Castanea crenata and Cryptomeria japonica. 6. • The causes of late establishment of barnyardgrass and their density effect on rice yield loss were examined in 1996 and 1977. Herbicide application on 5 to 15DAT(Days after transplanting) increased from 6% to 52.5%, 1988 to 1992, and their ingredient amount for barnyardgrass control decreased to 41.7-87.5% in Korea. Most late establishment of barnyardgrass in machine transplanted rice field were not late germinated but revived ones. The number of late established barnyardgrass were 0.5, 2.0 and 13.3/$m^2$, on direct seeded at May 10, and machine transplanted May 23 and June 9, individually. Relation on rice yield and the number of barnyardgrass at machine transplanted field showed highly significant equation, as y=543.3 4.7x, r=0.9039 in 1996 and y=515.8 10.4x+0.066$x^2$, $R^2$=0.9532 in 1997. Theoritical yield loss by regress equation showed 2% per one plant of barnyardgrass per $m^2$ and 5, 10, 20, 50, and 80 plants of barnyardgrass decreased rice to 10, 19, 35, 69 and 79%, individually.

  • Changes in the fine structure of testicular Leydig cell from hatching to adulthood were studied in Korean native chickens of 1, 2, 4, 6, 8, 10, 12, 14, 16, 18, 21, 24, 28, 32, 44, 52 and 64 weeks (n=13 chickens per group) of age. The objective of this study were to elucidate Leydig cell ultrastructure during testicular development. Testes of chickens were fixed by whole body perfusion using a fixative containing 2.5% glutaraldehyde in cacodylate buffer, processed and embedded in Epon-araldite. Using $1{\mu}m$ sections stained with methylene blue-azure II, qualitative and quantitative(stereological) morphological studies were performed. The ultrastructural changes of the Leydig cell were investigated by ultrathin section with the transmission electron microscope. The stages of the Leydig cell development described focus on mitochondria, endoplasmic reticulum, and lipid droplets which are involved in androgens as fullows. 1) Approaching puberty. The closely packed Leydig cells and sparse intercellular space. The nucleus occupied a large portion of the Leydig cell volume. The population of Leydig cells contained two types of cells that differed in the appearance of their nuclei which were either highly electron-opaque or relatively electron-lucid. The cytoplasm was characterized by large amounts of lipid droplets, relatively few spherical mitochondria, and sparse smooth endoplasmic reticulum. 2) Puberty to adult. The Leydig cells which display features compatible with significant androgen synthesis: large volume of cytoplasm containing extended smooth endoplasmic reticulum, abundant mitochondria, and reduction of lipid droplets.

  • The Korean laparoscopic Gastrointestinal Surgery Study Group made a survey of laparoscopic gastric surgeries which were performed in Korea during 2004. Thirty-eight surgeons from 36 Institutions responded to the questionnaires. One thousand eighty-nine laparoscopic gastric operations were performed during 2004. The cumulative number from 1995 to 2004 was about 2,386. Seven hundred fifty-four operations for a gastric adenocarcinoma were performed during 2004 which is almost two times the number performed during 2003. Laparoscopic radical procedures, such as a laparoscopy-assisted distal gastrectomy or total gastrectomy (LADG and LATG) have increased rapidly since 2001 (55 cases in 2001, 150 cases in 2002, 364 cases in 2003 and 738 cases in 2004). Especially, laparoscopic total gastrectomies were explosively adopted last year (20 cases in 2003 and 112 cases in 2004). However, laparoscopic function-preserving gastrectomies, which included one laparoscopy-assisted pylorus-preserving gastrectomy and laparoscopy-assisted proximal gastrectomy, are rarely performed at this time. One hundred forty-two wedge resections for a gastric submucosal tumor were performed during 2004. Hand-assisted laparoscopic surgery (HALS) was performed in 39 cases in 2001, 55 in 2002, and 49 in 2003; however, only 5 such surgeries were performed during 2004. In 2003, laparoscopic bariatric surgery began, and during 2004, 49 operations were performed. In terms of indications of laparoscopic gastric surgery for adenocarcinoma, 19 surgeons performed a LADG only for a T1 lesion, and 7 surgeons extended their indications to T2N0 lesions. In the near future, laparoscopic procedures for gastric cancer will be widely adopted in Korea if the medical-insurance obstacle is overcome, and the long-term survival results are verified.

  • This study was conducted to investigate the effects of dietary conjugated linoleic acid (CLA) and oil containing unsaturated fatty acid supplementation on egg production and quality in laying hens. Two hundred-eighty eight, 36 week old ISA brown commercial layer, were employed in a 5 week feeding trial. Dietary treatments are 1) BO (basal diet + 1% soybean oil + 1% oat), 2) BS (basal diet + 1% safflower oil + 1% oat), 3) BF (basal diet + 1% free fatty acid + 1% oat), 4) CD (basal diet + 1% CLA containing diglyceride + 1% oat), 5) CT (basal diet + 1% CLA containing triglyceride + 1% oat) and 6) CP (basal diet + 1% CLA by-product + 1% soybean oil). For overall period, hen-day egg production was not significant among treatments (P>0.05). Egg shell breaking strength at 3 weeks in CP treatment was significantly lower than BS, BF, CD and CT treatments (P<0.05) and that at 4 weeks in BO and CP treatments was significantly lower than others (P<0.05). Egg shell thickness at 3 and 4 weeks in CP treatment was significantly lower than BS, BF, CD and CT treatments (P<0.05). Egg weight at 3 and 4 weeks in CP treatment was significantly lower than others (P<0.05). Yolk height at 4 weeks in BF and CT treatments was significantly highest compared to BO, CD and CP treatments (P<0.05) and among BO, CD and CP treatments, that in BO and CD treatments was significantly higher than CP treatment (P<0.05). At 4 weeks, yolk color in CP treatment was significantly higher than BO and BS treatments (P<0.05). Haugh unit at 3 and 4 weeks in BO and CP treatments was significantly lower than others (P<0.05). In conclusion, supplementing CLA and oil containing unsaturated fatty acid for laying hens improved egg shell breaking strength, egg shell thickness, egg weight, yolk height, yolk color and haugh unit.
