Cystic Meningiomas

  • Cystic Meningiomas are rare tumor. There is a clear prevalance in infants, according for $10{\sim}19%$ of all intracranial meningiomas in this age group, compared with only $2{\sim}4%$ in adults. But, reports of cystic meningioma have been increased in frequency since the introduction of computed tomography[CT] and magnetic resonance image[MRI]. Authors report two cases of cystic meningioma in adults. Radiological finding showed extraaxial mass with cystic component. Authors performed total surgical resection of the tumor mass and of the entire cystic component with cystic wall in both patients. Postoperative histopathological diagnosis of the tumor mass was a transitional meningioma in both patients. Both patients showed favorable prognosis and no recurrence findings in follow up MRI.

Two Cystic Cavernous Angiomas after Radiotherapy for Atypical Meningioma in Adult Woman : Case Report and Literature Review

  • A correlation between radiation therapy and cavernoma has been suspected since 1994. Since then, only a few cases of radio-induced cavernomas have been reported in the literature (85 patients). Most of them were children, and the most frequent original tumour had been medulloblastoma. The authors report a case of two cystic cavernous angiomas after radiation therapy for atypical meningioma in adult woman. This is the first case of cavernous angioma after radiotherapy for low grade meningioma. A 39-year-old, Latin american woman was operated on for a frontal atypical meningioma with intradiploic component and adjuvant radiotherapy was delivered (6000 cGy local brain irradiation, fractionated over 6 weeks). Follow-up MR imaging showed no recurrences of the tumour and no other lesions. Ten years later, at the age of 49, she consulted for progressive drug-resistant headache. MR imaging revealed two new well defined areas of different signal intensity at the surface of each frontal pole. Both lesions were surgically removed; the histopathological diagnosis was cavernous angioma. This is the first case of cavernous angioma after radiation therapy for atypical meningioma : it confirms the development of these lesions after standard radiation therapy also in patients previously affected by non-malignant tumours.

뇌수막종과 동반된 뇌폐흡충증 - 증례보고 - (A Case of Cerebral Paragonimiasis Combined with a Meningioma - A Case Report -)

  • Cerebral paragonimiasis is an important parasitic infestation found in Eastern Asia, West Africa, and Central/South America. It is frequently associated with abscess, granuloma or cystic lesions in chronic stage. Chronic symptoms include seizure, visual disturbance, movement and perception disorders, usually resulting from the spaceoccupying lesions. The authors report a case of chronic cerebral paragonimiasis combined with meningioma. A 69-year-old man presented with generalized seizure and impaired consciousness. CT scan and MR image of the brain revealed two intracranial lesions ; one was cerebral paragonimiasis in the left temporo-occipital lobe, and the other a huge falx meningioma. Two lesions were successfully removed in staged operations and the generalized seizure was controlled after removal of the cerebral paragonimiasis.

Intraparenchymal Atypical Meningioma in Basal Ganglia Region in a Child : Case Report and Literature Review

  • Intraparenchymal meningiomas without dural attachment are extremely rare, especially when they occur in basal ganglia region in child. An 8-year-old boy was admitted at our hospital, complaining of recurrent headache and vomiting for 3 months. Neurological examination showed impaired vision and mild paresis of the left extremities. Magnetic resonance imaging demonstrated a lesion located in the right basal ganglia region extending to superasellar cistern with solid, multiple cystic and necrotic components. Computed tomography revealed calcification within the mass. Due to the anterior cerebral artery involvement, a subtotal resection was achieved and postoperative radiotherapy was recommended. Histopathological examination indicated that the lesion was an atypical meningioma. The postoperative rehabilitation was uneventful. Mildly impaired vision and motor weakness of left extremities improved significantly and the patient returned to normal life after surgery. To our knowledge, intraparenchymal atypical meningioma in basal ganglia extending to superasellar cistern was never reported. The significance in differential diagnosis of lesions in basal ganglia should be emphasized.

Cystic Hemangiopericytoma in the Third Ventricle

  • Primary intracranial hemangiopericytoma is rare and resemble meningioma on imaging study. It shows meningeal attachment, and is usually isointense with gray matter on T1-weighted MR image with heterogeneous enhancement and prominent vascular flow voids on T2-weighted image. Cystic type of hemangiopericytoma is very rare and only 3 cases have been reported in the literature which arised in the middle fossa, cerebellum, and occipital area. Ventricular hemangiopericytomas were reported in 9 cases, and all of them were solid type. Authors experienced a peculiar case of cystic hemangiopericytoma in the 3rd ventricle and report it with review of the literature.

개에서 척수공동증을 동반한 소낭성 뇌수막종 1례 (Spinal Syringomyelia Secondary to an Intracranial Microcystic Meningioma in a Dog)

  • 체중 2.7 kg의 5세령 암컷 말티즈 견이 양측 슬개골 탈구와 후지 파행으로 지역동물병원에 내원하였다. 도르레절제술과 슬개골의 외측 보강 봉합법을 이용하여 슬개골 탈구 정복술을 실시하였으나, 수술 3주 후 등을 굽힌 자세의 보행과 후지의 운동실조, 흉요추부의 압통, 간헐적인 경련증상으로 재 내원하였다. 자기공명영상 촬영 결과, 우측 전두엽의 종괴와 경수에서부터 3번 요수까지의 공동성 변화가 확인되었다. 병리조직학적 소견으로 뇌종양은 느슨한 망상구조 또는 레이스 모양의 수막세포들로 구성되었으며, 세포 외부에 다양한 크기와 형태의 많은 낭포성 공간이 관찰되었다. 면역조직화학염색 결과 종양세포들은 vimentin 양성, 신경원 특이 에놀라제 및 신경교 섬유질 산성 단백질에는 음성을 나타내었다. 경수와 흉수 부위에는 부정형의 확장된 중심관 유사 동공이 관찰되었다. 이 증례는 개에서 특이적인 두개강 내 소낭성 뇌수막종과 연관되어 2차적으로 발생한 척수공동증의 임상소견과 영상학적 및 병리조직학적 특징을 보여주고 있다.

Diagnostic Value of Susceptibility-Weighted MRI in Differentiating Cerebellopontine Angle Schwannoma from Meningioma

  • Background: Differentiation of cerebellopontine angle (CPA) schwannoma from meningioma is often a difficult process to identify. Purpose: To identify imaging features for distinguishing CPA schwannoma from meningioma and to investigate the usefulness of susceptibility-weighted imaging (SWI) in differentiating them. Materials and Methods: Between March 2010 and January 2015, this study pathologically confirmed 11 meningiomas and 20 schwannomas involving CPA with preoperative SWI were retrospectively reviewed. Generally, the following MRI features were evaluated: 1) maximal diameter on axial image, 2) angle between tumor border and adjacent petrous bone, 3) presence of intratumoral dark signal intensity on SWI, 4) tumor consistency, 5) blood-fluid level, 6) involvement of internal auditory canal (IAC), 7) dural tail, and 8) involvement of adjacent intracranial space. On CT, 1) presence of dilatation of IAC, 2) intratumoral calcification, and 3) adjacent hyperostosis were evaluated. All features were compared using Chi-squared tests and Fisher's exact tests. The univariate and multivariate logistic regression analysis were performed to identify imaging features that differentiate both tumors. Results: The results noted that schwannomas more frequently demonstrated dark spots on SWI (P = 0.025), cystic consistency (P = 0.034), and globular angle (P = 0.008); schwannomas showed more dilatation of internal auditory meatus and lack of calcification (P = 0.008 and P = 0.02, respectively). However, it was shown that dural tail was more common in meningiomas (P < 0.007). In general, dark spots on SWI and dural tail remained significant in multivariate analysis (P = 0.037 and P = 0.012, respectively). In this case, the combination of two features showed a sensitivity and specificity of 80% and 100% respectively, with an area under the receiver operating characteristic curve of 0.9. Conclusion: In conclusion, dark spots on SWI were found to be helpful in differentiating CPA schwannoma from meningioma. It is noted that combining dural tail with dark spots on SWI yielded strong diagnostic value in differentiating both tumors.

3T DWIs with Different b-Values in Brain Tumors

  • Purpose: It is known that diffusion-weighted MR imaging (DWI) is helpful in the evaluation of malignancy grading in brain tumor. This study was to evaluate the DWls with different b-values of various brain tumors in order to determine optimal b-values on 3T MR unit. Method: On a 3T MR unit, DWls with b-values of 1, 000, 3, 000 and 5, 000 s/mm2 were obtained in 20 patients of pathologically-proven brain tumors (7 metastases, 4 high grade gliomas, 2 Iymphomas, 2 low grade gliomas, 2 germinomas, and one each of germinoma, meningioma, hemangioblastoma and central neurocytoma. The overall image quality, contrast between normal brain parenchyma and tumor and signal intensities of solid and cystic components were comparatively evaluated among DWls with different b-values by visual inspection.

두개강내 소아 수막종 16예의 자기공명영상 소견 (MR Imaging of Intracranial Pediatric Meningiomas: Manifestations in 16 Patients)

  • 목적: 이 연구의 목적은 소아 수막종의 임상적, 영상의학적, 그리고 병리학적 특징을 기술함에 있다. 대상 및 방법: 병리학적으로 진단된 수막종을 가진 16예의 소아환자를 대상으로 임상기록과 자기공명영상을 후향적으로 분석하였다. 평균연령은 14세 (3-18세)였다. 자기공명영상은 병변의 크기, 신호강도, 경계부의 특징, 내부 구조, 조영증강, 그리고 경막과 뇌실질의 변화를 검토하였고 다른 영상 방법 및 병리소견을 함께 분석하였다. 결과: 종양의 평균 크기는 5.24 cm (1.3-18.1 cm)였으며 12예는 천막상부에 4예는 천막하부에 위치하였다. T2 강조 영상에서 높은 신호강도를 보이는 종양이 9예 있었고 4예에서 동신호강도를, 3예에서 낮은 신호강도를 보였다. T1 강조 영상에서는 낮은 신호강도 11예, 동신호강도 4예, 그리고 높은 신호강도 1예가 있었다. 모든 종양의 경계가 좋았고 조영증강을 보였다. 5예에서 종양은 균질한 양상을 보였으며 나머지에는 낭종이나 괴사에 의해 비균질한 양상을 보였다. 경막 부착 소견이 11예에서, 뇌실질 부종이 10예에서 동반되었다. 컴퓨터 단층촬영 소견은 6예에서 밝은 음영을 보였으며 5예에서 석회화를 동반하였다. 혈관 조영술 상 3예는 내경동맥에서, 4예는 외경동맥으로부터 혈액 공급이 이루어졌다. 병리검사결과 종양의 아형은 이행성 (4예), 수막세포성 (4예), 척삭성 (2예), 섬유성 (2예), 투명세포성 (1예), 유리질화성 (1예), 횡문근양 유두모양 (1예), 그리고 비정형(1예) 수막종으로 진단되었다. 결론: 소아의 수막종은 주로 10대에 발생하며 다양한 병리학적 아형을 보일뿐만 아니라 비정형적인 영상 소견으로 인해 축내 종양으로 오인될 수 있다.
