Endoscopic Management of Gastric Subepithelial Tumor (위상피하종양의 내시경적 진단 및 치료)

  • Diagnosis of gastric subepithelial tumors (SETs) is sometimes difficult with conventional endoscopy or tissue sampling with standard biopsy, so non-invasive imaging modalities such as endoscopic ultrasound (EUS) and computed tomography are used to evaluate the characteristics of SETs features (size, location, originating layer, echogenicity, shape). However imaging modalities alone is not able to distinguish among all types of SETs, so histology is the gold standard for obtaining the final diagnosis. For tissue sampling, mucosal cutting biopsy and mucosal incision-assisted biopsy and EUS-guided fine-needle aspiration or biopsy (EUS-FNA or EUS-FNB) is commonly recommended. Endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD) are used for resection of SETs involving the mucosal and superficial submucosal layers, could not treat adequately and safely the SETs involving the deep mucosa and muscularis propria. Submucosal tunneling endoscopic resection (STER) and endoscopic full-thickness resection (EFTR) is used as a therapeutic option for the treatment of SETs with the development of reliable endoscopic closure techniques and tools.

  • Accurately identifying brain tumors is crucial for medical imaging's precise diagnosis and treatment planning. This study presents a novel approach that uses cutting-edge image processing techniques to automatically segment brain tumors. with the use of the Pyramid Network algorithm. This technique accurately and robustly delineates tumor borders in MRI images. Our strategy incorporates special algorithms that efficiently address problems such as tumor heterogeneity and size and shape fluctuations. An assessment using the RESECT Dataset confirms the validity and reliability of the method and yields promising results in terms of accuracy and computing efficiency. This method has a great deal of promise to help physicians accurately identify tumors and assess the efficacy of treatments, which could lead to higher standards of care in the field of neuro-oncology.

Development of a Solar Concentrator having the Reflecting Surface with the 2m Class Diameter (직경 2m급 반사면을 가지는 태양열 집광 장치 개발)

  • Purpose: To develop a solar concentrator having the reflecting surface with the 2m class diameter. Methods: In order to make the reflecting surface for the solar concentrator, the shape of the reflecting surface sector is required. So, first, we induced the formula that can produce this shape. After that, using Delphi 6.0 language, we developed a program which uses this formula and produces the shape and the numerical data of the reflecting surface sector with the input variables such as the external diameter of the reflecting mirror, the reflecting mirror's radius of curvature at the paraxial range, the number of reflecting mirror sector, the size of the center hole of the reflecting mirror, and the interval of the output data. Results: This program, which was developed to produce the shape and the numerical data of the reflecting surface sector, enables us to see the shape of sector on the monitor and to save the numerical data files for the shape of sector. As a result, the user of this program can easily access the numerical data of the reflecting surface sector. Conclusions: Developing the program which produce the reflecting surface sector used to make the reflecting surface of the solar concentrator, we could succeed in making the prototype products by applying it to the development of the real solar concentrator with the diameter of the 2m class.

Morphological and Biomechanical Study of the Pulley System of the Thumb

  • The purpose of this study was to define more precisely the anatomy of the thumb flexor pulley system and to determine the relative contribution of each of the pulleys to the biomechanics of thumb motion at the metacarpophalangeal (MP) and interphalangeal (IP) joints. For this, 22 hands from 11 cadavers were used and randomly assigned to two groups. In the first group, the first annular (A1) pulley was cut first followed by the variable annular (Av) pulley and then the oblique pulley. In the second group, the oblique pulley was cut first followed by the, pulley and then the Av pulley. In 7 of 22 hands, it was a transverse structure parallel to the, pulley with a gap between the A1 and Av pulleys, referred to here as type I. In 9 hands, the A1 and Av pulleys were connected without any gap (type II). In 6 hands, the space between the A1 and Av pulleys were triangular in shape with fibers of the Av pulley converging toward the radial side (type III). In biomechanical study of both first and second experiments, there was no significant difference in MCP joint flexion between the all intact, A1 section, A1/Av section, A2 intact (A1/Av/oblique section), and no pulley configuration (p>.05). In occurring displacements less than 10 mm, there was no significant difference in IP joint flexion (p>.05). However, there was a significant decrease in IP joint flexion occurred in both 15 mm and 20 mm excursion (p<.05), when the oblique pulley was resected additionally after cutting the A1 and Av pulleys in first experiment, and when the A1 pulley was resected additionally after cutting the oblique pulley. According to the results, the injury of only the oblique pulley does not decrease thumb motion significantly. The oblique pulley injury with both the A1 and Av pulleys laceration decreased thumb motion significantly. The additional laceration of the A2 pulley does not decrease thumb motion.

A Study on Products Analysis of the Domestic and Overseas Cycling Wears (시판 국내외 사이클 웨어의 제품 분석)

  • This study provided basic data to develop functional and fitness cycling wear for cyclists. We analyzed the domestic and overseas cycling wear brand patterns for tops and tights as well as compared pads attached to tights. The results were as follows. The tops of the cycling wear were designed with cutting lines in the lateral or yoke in the back to reflect cycling posture. It is necessary to study if there is a difference in the air resistance or fitness of the athlete. The sleeve hem band affected by arm movement during cycling, proper sewing and band pressure are also important. The tights have a brand that does not insert a band in front of the waist in consideration of the cyclist's breathing. It should take into account methods of constructing a proper garment pressure that does not interfere with cycling. Analyzing the cycling pad indicated that the three-dimensional shape and size of the pad show many differences for each brand, indicating a difference in cycling movement suitability. In addition, many of the cutting lines should account for the productive aspects because of the high consumption of the fabric in order to design a pattern considering the cycling posture.

A study on the havesting process and operating behaviour of working ships for farming laver (김 양식장 채취선의 운항거동과 수확조업에 관한 연구)

  • We analyzed the cutting mechanism of laver harvesting machine in the sea area near Gooam Port in Goheung, Jeollanam-do, and investigated the change and efficiency of laver collecting operation in the working ship. The laver working ship slides uniformly from the bow to the upper part of the laver collecting machine on the deck and cuts the wet laver attached to the bottom of the net at the blade of the havesting machine. The laver farming net, which was loaded with laver turrets on the deck by gravity and collected primitives, consisted of a ship structure that led to the stern side and into the sea. The working ship operation is in harvesting process while driving in a S-shape that is separated by one space to efficiently collect the laver net. During laver working ship operation, the speed was 0.51 m/s in the access stage, 0.56 m/s in the havesting stage, and 0.52 m/s in the exit stage. Considering the cutting edge life and production efficiency of the laver harvesting machine, it is appropriate to harvest 1.15 to 1.26 kg/rpm by operating at a rotational speed of about 700 to 800 rpm rather than forcibly harvesting the product at high speed. On the deck of the working ship, 959.7 kg of starboard and 1048.7 kg of center were 964.7 kg of port side. Based on the starboard, 9.3% of the central part and 0.5% of the port side appeared. The reason for this was due to the difference in harvest time according to the turning direction of the working ship.

  • This study was carried out to investigate the native distribution of wax myrtle (Myrica rubra) in Cheju Province and its propagation method. Wax myrtle is heavily distributed at 100 to 400m above sea level of Donhong-chun and Hyodon-chun which is bordered by Youngchon-dong of Seogwipo City and Haryeri of Namcheju county. BA, IBA, Kinetin, IAA, and GA3 were applied to promote rooting of cutting but were ineffective in promoting rooting. Treating cuttings with both AgNO,1 which removes rooting inhibitor, tannin and growth regulators such as BA, IBA and Kinetin were also ineffective in promoting rooting. Incubating seeds at 4$0^{\circ}C$ for 30 days resulted in 39% germination. There was distinct difference in leaf shape between seedlings and mature trees.

  • Interest in the environment is rising in all the sectors of industry and culture. Even in the fashion industry, fashion designers have recently begun delivering messages on the environment. In particular, $Issey$ $Miyake$ has been constantly interested in environmental issues, and has been deemed as a designer who values practicality and universality in clothing. This study seeks to examine Issey Miyake's fashion from the perspective of an eco-friendly design. This research analyzed collections of $Issey$ $Miyake$ from 1980 to 2010 through photo works and www.firstview.com. The research method was to study 8 people who have professionalism in fashion design and have analyzed 201 works of $Issey$ $Miyake$ from 1980s to 2000s that have the characteristics of eco-friendly design. The results showed the following characteristics of eco-friendly design in Issey Miyake's fashion : naturality, simplicity, sustainability and transformability. First, $Issey$ $Miyake$ preferred natural materials and used Japan traditional dyeing that integrated modern techniques for naturality. Second, he expressed simplicity in his fashion by applying the least cutting and sewing in geometric panels. Third, he pursued sustainability with comfortable clothing that anyone could wear regardless of trend, age and body shape. Finally, he developed the A-POC system that eliminated the usual needs for cutting and sewing and tried transformability on clothing using buttons, strings, belts, zippers, and layered styling.

The Utility of a Three-Dimensional Approach with T-Shaped Osteotomy in Osseous Genioplasty

  • Background Facial beauty depends on the form, proportion, and position of various units of the face. In terms of the frontal view and facial profile, the chin is the most prominent aesthetic element of the lower third of the face. Many methods have been implemented to obtain good proportions of the lower face. In this study, we applied the T-shaped genioplasty method to correcting chin deformities. Methods All of the procedures in 9 cases were performed under general anesthesia. For genioplasty, a horizontal cutting line and 1 or 2 vertical cutting lines were drawn 5 mm below the mental foramen. Osteotomed bone segments of the chin were used for horizontal widening using bone grafts or for horizontal shortening. Likewise, they were used as bone grafts for vertical lengthening or vertical shortening. The bone segments were approximated in the midline and held in place using miniplates. Results The postoperative appearance of the 9 cases showed that the lower third of the face had been naturally changed. At the same time, vertical lengthening or shortening, and horizontal widening or shortening could be implemented during the operation. Satisfactory results were obtained based on reviews of the patients' preoperative and postoperative photographs. The patients were also satisfied with the outcomes. Conclusions Using T-shaped genioplasty, we efficiently adjusted the shape and position of the chin to obtain good proportions of the lower face and change its contour to obtain an aesthetically appealing oval face in accordance with East Asians' aesthetic preferences.

Feasibility Study of a Custom-made Film for End-to-End Quality Assurance Test of Robotic Intensity Modulated Radiation Therapy System

  • This paper aims to verify the clinical feasibility of a custom-made film created by a laser cutting tool for End-to-End (E2E) quality assurance in robotic intensity modulated radiation therapy system. The custom-made film was fabricated from the Gafchromic EBT3 film with the size of $8^{{\prime}{\prime}}{\times}10^{{\prime}{\prime}}$ using a drawing that is identical to the shape and scale of the original E2E film. The drawing was created by using a computer aided design program with the image file, which is obtained by scanning original E2E film. Beam delivery and evaluations were respectively performed with the original film and the custom-made film using fixed-cone collimator on three tracking modes: 6D skull (6DS), Xsight spine (XS), and Xsight lung (XL). The differences between total targeting errors of the original and custom-made films were recorded as 0.17 mm, 0.3 mm, and 0.17 mm at 6DS, XS, and XL tracking modes, respectively. This indicates that the custom-made film could yield nearly equivalent results to those of the original E2E film, given the uncertainties caused by distortions during film scanning and vibrations associated with film cutting. By confirming the clinical feasibility of a custom-made film for E2E testing, it can be expected that economic efficiency of the testing will increase accordingly.
