Successful Treatment of Post-operative Keloid with Combined Cryotherapy and Ablative Fractional CO2 Laser

  • Keloids are pathologic fibroproliferative conditions characterized by excessive collagen deposition during wound healing. The pathogenesis of keloids is not fully understood, and current treatment options show variable results. In this case report, the patient developed a keloid after bilateral total thyroidectomy, and was treated with a combination approach using fractional ablative laser systems along with cryotherapy and triamcinolone injection. After seven monthly sessions of combination treatment, the patient presented marked improvement of the scar texture and symptoms. Furthermore, there was no recurrence for up to 2 years. We propose this combination as a safe and effective treatment option for keloid patients.

Eliminating Potato Virus Y (PVY) and Potato Leaf Roll Virus (PLRV) Using Cryotherapy of in vitro-grown Potato Shoot Tips

  • Potato virus Y (PVY) and potato leafroll virus (PLRV) are among the most damaging potato viruses and prevalent in most potato growing areas. In this study, cryopreservation was used to eradicate PVY and PLRV using two cryogenic methods. Potato shoot tips proliferated in vitro were cryopreserved through droplet-vitrification and encapsulation-vitrification using plant vitrification solution 2 (PVS2; 30% glycerol + 15% dimethyl sulfoxide + 15.0% ethylene glycol + 13.7% sucrose) and modified PVS2. Both cryogenic procedures produced similar rates of survival and regrowth, which were lower than those from shoot tip culture alone. The health status of plantlets regenerated from shoot tip culture alone and cryopreservation was checked by reverse transcription-polymerase chain reaction. The frequency of virus-free plants regenerated directly from highly proliferating shoot tips reached 42.3% and 48.6% for PVY and PLRV, respectively. In comparison, the frequency of PVY and PLRV eradication after cryopreservation was 91.3~99.7% following shoot-tip culture. The highest cryopreserved shoot tip regeneration rate was observed when shoot tips were 1.0~1.5 mm in length, but virus eradication rates were very similar (96.4~99.7%), regardless of shoot tip size. This efficient cryotherapy protocol developed to eliminate viruses can also be used to prepare potato material for safe long-term preservation and the production of virus-free plants.

A Study on the Effect of Oral Cryotherapy on Nausea Vomiting and Oral Intake by Anti Cancer Chemotherapy in Pediatric Cancer Patient (구강냉요법이 암환아의 오심구토와 구강섭취량에 미치는 효과)

  • This research objected to the diagnosed patients as acute lymphoblastic leukemia, acute myelogenous leukemia, neuroblastoma, non-Hodgkins lymphoma, Hodgkin's disease, kidney tumor, myelodysplastic syndrom and juvenile chronic leukemia after admission in the 'P' hospital in Pusan from Aug. 1. 1999 to Jan. 31. 2000. The results of this study are summarized as follows. 1. On the specific character between the experimental(exp.) group and the control (con.) group : there were 7 of 4-7 years old patients(the most) in the experimental group(53.8%), 5 of 12 years old or older patients in the control group (38.5%). Patients who experienced operation were 7 in the exp. group(53.8%) and 6 in con. group(46.2%). The largest number of the patients' diagnosis was acute lymphoblastic leukemia by 5 in the exp. group(38.5%) and 4 in the con. group (30.8%). The hardest nausea came on the second day by 5 in the exp. group(38.5%), 9 in the con. group(69.2%). 2. P-score of the nausea vomiting on the number of daily anticancer drug administration : first day, the exp. group got 9.6 and the con. group 17.6(P = 0.03). 2nd day, 10.9 and 19.4(P = 0.00), 3rd day, 10.6 and 18.3(P = 0.00), 4th day 10.0 and 18.0, 5th day 10.9 and 16.8(P = 0.05). The score showed statistically significant difference(P < .05). 3. Oral intake didn't show statistically significant difference between two groups. However the average of Oral intake of the exp. group was continually higher than the con. group except to the first day after administration. In conclusion, nursing intervention and nutrition care are much more needed on the 2-3th day after administration to reduce nausea vomiting, and for remission of nausea and enlarging oral intake it is utilizable to apply the easy, economic Oral Cryotherapy to the young patients who undergo chemotherapy.

Local Cervical Immunity in Women with Low-grade Squamous Intraepithelial Lesions and Immune Responses After Abrasion

  • Minor trauma to the uterine cervix is supposed to induce local immunity to prevent cervical lesions caused by human papillomavirus (HPV) infection. This study aimed to investigate the local cervical immunity in women with low grade squamous intraepithelial lesion (LSIL) and effects of abrasion after cryosurgery or Pap smear. One hundred women with LSIL and known results of HPV detection were recruited. HPV positive women were randomly divided according to abrasion into cryotherapy and Pap smear observation groups. Cervical tissues and cervico-vaginal lavage (CVL) were collected at 6 and 12 months after allocation. The levels of cytokines at first recruitment were compared with cytokine levels at 6 months after abrasions. The mRNA of IFN-${\gamma}$, TNF-${\alpha}$ and IL-10 in cervical tissues and these cytokines secreted in CVL were determined using real time PCR and ELISA, respectively. Anti-HPV16 IgG and IgA antibodies in CVL were assessed by western blotting. At first recruitment of women with LSIL (100 cases), IL-10 mRNA and cytokine in HPV positive group (60 cases) was significantly higher than negative group (40 cases). IFN-${\gamma}$ and TNF-${\alpha}$ mRNA level in both groups were comparable but their secretions in CVL were significantly increased in HPV negative group. After abrasion for 6 months in HPV-positive women, all mRNA and secreted cytokines were changed, but no significant difference was observed between cryotherapy and observation groups. When individuals were compared between first recruitment and after abrasion for 6 months, IFN-${\gamma}$ mRNA and anti-HPV16 L1 IgA antibodies were significantly increased in the cryotherapy group. The results suggest that modulation of local cervical immunities by abrasion might promote different effects in clearance of HPV-related cytological abnormalities.

  • Bronchial carcinoid tumors are relatively uncommon neoplasms that are considered to be malignant tumors of low to intermediate grade. They are classified by pathologic features as typical or atypical carcinoids and have distinctly different prognoses and therapeutic options. Surgery is the treatment of choice in typical and atypical carcinoid tumors but the approach has been changing. Recently, several studies have described experiences using other technologies as adjuncts to bronchoscopic resection, technologies such as laser and cryotherapy with curative intent in endoluminal typical carcinoids. Here we present a case of atypical bronchial carcinoid that was treated with bronchoscopic cryotherapy.

Two Cases of Diagnosis and Removal of Endobronchial Hamartoma by Cryotherapy via Flexible Bronchoscopy

  • Although endobronchial hamartoma is a rare benign tumor, most patients with endobronchial hamartoma have respiratory symptoms such as obstructive pneumonia, hemoptysis, cough, or dyspnea due to bronchial obstruction. It can cause irreversible post-obstructive pulmonary destruction, thus early diagnosis and treatment is very important. Recently, there have been cases of neodymium-doped yttrium aluminum garnet (Nd:YAG) laser and electrocautery procedures for bronchoscopic treatment of malignant or benign central airway obstruction with comparable therapeutic efficacy and few complications. Bronchoscopic cryotherapy is a newly developed technique for management of central airway obstruction. Moreover, it provides diagnostic methods with improving diagnostic yield and safety. We report two cases of endobronchial hamartoma, each diagnosed and definitively treated with bronchoscopic techniques. Endobronchial biopsy and removal was successfully performed by cryotherapy via flexible bronchoscopy without notable complications. Follow-up bronchoscopic examinations excluded residual or recurrent disease.

The Efficacy of Korean Medical Treatment for Recalcitrant Hand and Foot Warts with an Experience of Cryotherapy (냉동치료 경험이 있는 손과 발의 난치성 사마귀에 대한 한방치료의 효과)

  • Objective : The purpose of this study is to know the efficacy of Korean medical treatment for recalcitrant hand and foot warts with an experience of cryotherapy. Methods : Twenty-two patients(male 12, female 10) with recalcitrant warts visited Amar Korean Medicine Clinic and were treated with herbal medicine, herbal acupuncture, acupuncture, moxibustion and bloodletting for at least 3 months. After treatment, a clearance rate was divided into 4 scales such as; complete clearance / partial clearance-good / partial clearance-poor / no change. Results : Mean age of patients was 23.4 years and mean period of disease was 43.6 months. Mean duration of treatment was 3.0 months and mean dosage(貼) of herbal medicine was 89.1. Nineteen patients(86.4%) showed complete clearance, one(4.5%) partial clearance-good, and two(9.1%) partial clearance-poor. Conclusion : This data demonstrates that Korean medical treatment could be an effective treatment for recalcitrant warts.

  • The purpose of this study was to determine the effects of cold application for reducing spasticity in patients with traumatic brain min injury cerebrovascular accident who did not take neurosurgical treatment and medication except patients with spinal cord injury. The participants consisted of 24 men and 21 female were ramdomly assigned to three groups with each 15. The result were as followings after making an observation about the change of skin temperature and spasticity throughout cryotherapy which was performed with ice stick in Group I (1 min), Group II (5 min) and Group III(10 min). 1. It was found that the Group III, Croup II, Group I, in order named, had the statistically significant reduction of skin temperature, the reduction of spasticity showed statistically significance in Group III, Group II, in order named, but did not show it even though there was a little increase in Group I. 2. The reduction of skin temperature and spasticity did not continue over 24 hours at the same time every day for 5 days. 3. The longer cryotherapy was applied the mere skin temperature and spasticity reduced, the more skin temperature was reduced the more spasticity reduced.

A Combination of Herbal Medicine and Moxibustion Treatment is Effective for a Recalcitrant Cutaneous Warts in a Pediatric Patient: A Case Report

  • Although warts are a common skin condition/disease that recurs in childhood, treatment completion is difficult owing to the pain associated with conventional therapies. In this case, Korean herbal medicine, Guijakjihwang-Tang, and indirect moxibustion was used to treat a recalcitrant cutaneous wart without pain and recurrence. A 7-year-old boy presented with recurrent cutaneous warts on his right hand and received cryotherapy 3 times in the previous year. The wart recurred during the course of cryotherapy treatment, and the patient did not want to suffer any more pain from the previous treatment. Therefore, parents sought an efficient and painless therapy. The patient was treated for 2 months using Guijakjihwang-Tang combined with indirect moxibustion. The prominent lesion separated in the 2nd month of treatment and completely recovered without adverse events in the 3rd month. There was no recurrence over 12 months of follow-up. This report provides meaningful insights into the use of Korean medicine as a safe, painless and efficient treatment for warts in pediatric patients.

Effect of Oral Care Protocol on Oral Cavity Status and Self-care Performance in Solid Cancer Patients Receiving Chemotherapy (구강관리 프로토콜 적용이 항암화학요법을 받는 고형암 환자의 구강상태 및 자가간호수행에 미치는 효과)

  • The purpose of this study was to identify the effectiveness of oral care protocol including individual oral care education, cryotherapy and benzydamine gargling on oral cavity status and self-care performance in solid cancer patients receiving chemotherapy. Thirty-one patients were enrolled by convenience sampling in this study and allocated to control (n=18) and experimental group (n=13). In the intervention group, individual oral care education, oral cryotherapy and benzydamine gargling were applied, while the control group received pre-existed oral care. Oral cavity status and self-oral care behavior were measured in five periods. The data were analyzed using the Mann-Whitney test, Friedman test, and Chi-square test. There was no significant difference in oral cavity status between the groups. But the two groups showed the poorest oral cavity status on $14^{th}$ day. Self-care behavior was significantly higher in the intervention group than the control group. This effect had maintained for 4months after completion of study. The results showed that individual oral care education by oncology nurse is effective to encourage patients to do self-care behavior and a further study is needed to explore the effect of cryotherapy and benzydamine gargling on oral cavity status of patient with solid tumor.
