CHANGES IN ROOT CANAL CONFIGURATION USING DIFFERENT FILE TYPES AND TECHNIQUES (근관형성 기구 및 방법에 따른 근관 형태의 변화)

  • The purposes of this study were to evaluate the changes in root canal configuration with canal instrumentation using different file types and techniques and to investigate most appropriate instrumentation technique in maintaining the original canal configuration with different file types. Fifty curved mesiobuccal or distobuccal canals of extracted human maxillary molar teeth were instrumented using a step-back technique with stainless steel K-files or nickel-titanium K-files, a crown-down pressureless technique with stainless steel K-files or nickel-titanium K-files and nickel-titanium engine-driven files. Radiographs were taken before and after instrumentation using a specially designed device that allowed for the pre-and postinstrumentation canals to be taken with the same X-ray angulation. Magnified X-ray images on a magnifier screen were traced and post instrumentation canal images were compared with the preinstrumentation ones. Changes in canal curvature and the incidence of procedural accidents were analyzed. The results were as follows : Crown-down pressureless technique with nickel-titanium K-files and nickel-titanium engine-driven filing produced no significant changes in canal curvature (p>0.05), while the step-back technique with stainless steel K-files or nickel-titanium K-files (p<0.01) and the crown-down pressureless technique with stainless steel K-files (p<0.05) produced significant changes. With nickel-titanium K-file, crown-down pressureless technique produced significantly less changes in canal curvature than step-back technique (p<0.05), while there was no significant difference between techniques with stainless steel K-files (p>0.05). File types exerted no significant influences in the changes of canal curvature both in the step-back technique and crown-down pressureless technique (0>0.05). Regardless of the file types used, step-back technique produced more procedural accidents such as ledge or elbow formation, apical zipping and apical transportation than the crowndown pressureless technique and nickel-titanium engine-driven filing. Both with stainless steel K-files and with nickel-titanium K-files, the incidence of apical extrusion of canal debris was higher in step-back technique than in crown-down pressureless technique.

Comparison of shaping ability between single length technique and crown-down technique using Mtwo rotary file (Mtwo 전동 파일을 사용한 single length technique과 crown-down technique의 근관성형 효율 비교)

  • The aims of this study were to compare the shaping effect and safety between single length technique recommended by manufacturer and crown-down technique using Mtwo rotary file and to present a modified method in use of Mtwo file. Sixty simulated root canal resin blocks were used. The canals were divided into three groups according to instrument and the manner of using methods. Each group had 20 specimens. Group MT was instrumented with single length technique of Mtwo, group MC was instrumented with crown-down technique of Mtwo and group PT was instrumented with crown-down technique of ProTaper. All of the rotary files used in this study were operated by an electric motor. The scanned canal images of before and after preparation were superimposed. These superimposed images were evaluated at apical 1 to 8 mm levels Angle changes were calculated. The preparation time, weight loss, instrument failure and binding, canal aberrations, and centering ratio were measured. Statistical analysis of the three experimental groups was performed with ANOVA and Duncan's multiple range tests for post-hoc comparison and Fisher's exact test was done for the frequency comparison. In total preparation time, group MT and group MC were less than group PT. In the aberrations, group MT had more elbows than those of group MC and group PT. The binding of group MC was least and group MT was less than group PT (P < 0.05). Under the condition of this study, crown-down technique using Mtwo rotary file is better and safer method than single length technique recommended by the manufacturer.

EFFECT OF CANAL PREPARATION METHODS ON THE APICAL EXTRUSION OF DEBRIS (근관형성법이 근관잔Δ사의 치근단 정출에 미치는 영향)

  • Apical extrusion of canal debris is occurred inadvertently during root canal preparation and this could produce interappointment discomfort or postinstrumentation pain. The purpose of this study was to investigate the influence of canal preparation methods on the apical extrusion of canal debris by means of comparing the amounts of apically extruded debris with several kinds of instrumentation methods. In the first experiment, 40 incisors were divided into four groups of 10 each. They were instrumented using one of the four techniques: Step-back, crown-down pressureless technique with stainless steel K-files, engine-driven instrumentation with Quantec series 2000, and Profile .04 taper series 29. Root canal irrigation was done with 2.52% sodium hypochlorite solution. In the second experiment, 80 incisors were divided into five groups of 16 each and instrumented using step-back, crown-down pressureless technique with stainless steel K-files, engine-driven instrumentation such as Quantec SC, Quantec LX, and Profile .04 taper series 29 No irrigation procedure was performed in this second experiment. Extruded debris from each tooth was collected in a container and weighed by the use of an electronic balance after desiccation. With or without canal irrigation, step-back technique produced significantly more amount of apical debris than the other groups (p<0.05). However, there was no significant difference among crown-down pressureless technique, engine-driven instrumentation with Quantec LX, Quantec SC, or Profile. Therefore, either by hand or engine-driven instrumentation, it is concluded that to minimize apical debris, techniques using reaming motion of files should be applied rather than filing motion.

A Study on The Shape of a Canal Prepared With 'TWO-FILE' Technique (Two-File' 방식에 의한 근관 형성시 근관의 형태에 관한 연구)

  • 이 연구의 목적은 'Two File' 방식에 의한 근관형성시 근관의 형태변화 및 전이정도를 분석하고 이를 기존의 근관형성방법과 비교하고자 하는 것이다. 만곡된 근관을 가진 투명한 레진블럭 상에서 step back방식, crown down방식 및 'Two File' 방식으로 근관형성을 시행하고 이를 똑같은 위치가 재현가능한 고정틀에 위치시킨 후 사진의 이중노출 기법을 이용하여 근관형성 전후의 근관의 형태변화 및 전이정도를 분석 한 결과 다음과 같은 결과를 얻었다. 1. step back방식의 경우 형성된 근관이 taper하지 못하고 불규칙한 형태를 보였으며 근관의 전이정도도 다른 두 가지 엔진구동 방식에 의한 경우에 비해 유의하게 컸다(p<0.05) 2. crown down방식의 경우 형성된 근관이 taper한 형태를 보였고, 근관의 전이 정도도 step back방식에 의한 경우보다 유의하게 작았다(p<0.05). 3. 'Two File' 방식의 경우 형성된 근관이 taper한 형태를 보였으며 근관의 전이정도도 step back방식에 의한 경우보다 유의하게 작았으나 crown down방식에 의한 경우와 유의할 만한 차이는 없었다(p<0.05).

EFFECTS OF DIFFERENT PREPARATION METHODS ON THE MORPHOLOGIC CHANGE OF SIMULATED ROOT CANALS IN RESIN BLOCK (수종 근관형성방법에 따른 레진모형상의 근관형태의 변화)

  • In this study, 24 curved resin blocks were prepared by one of the following four methods: 1) Conventional technique using K-flexo files 2) Step-back technique using K-flexo files 3) Crown-down technique using K-flexo files 4) Canal Master instrumentation using Canal Master Resin blocks were sectioned, photographed, and evaluated the mean centering ratio and the mean area of dentin removed before and after the instrumentation. The results were as follows : I. the mean centering ratio 1. In the level 1 and level 3, there was no significant difference in the mean centering ratio. 2. In the level 2, Step-back technique showed the worst mean centering ratio among the tested groups(p<0.001) and there was no significant difference between the other three groups. 3. In the level 4, Canal Master instrumentation and Step-back technique showed better mean centering ratio than the other two techniques(p<0.001) and there was no significant difference between the two techniques. II. the mean area of dentin removed 1. In the level l and level 3, there was no significant difference in the mean area of dentin removed. 2. In the level 2, Canal Master instrumentation removed less dentin than the other three techniques(P<0.01). 3. In the level 4, Crown-down technique removed less dentin than the other three techniques(P<0.05).

Profile로 crown-down방식에 의해 형성한 경우 taper한 근관형태를 보였고, 근관의 전이 정도도 스테인레스강 화일의 경우보다 작았다(p<0.01). 3. Profile로 'Three-File'방식에 의해 형성한 경우 taper한 근관형태를 보였고 근관의 전이정도도 스테인레스강 화일의 경우보다 유의하게 작았으나(p<0.01), crown-down방식에 의해 형성한 경우와 유의할 만한 차이는 없었다(p>0.01).

A STUDY ON THE ACCURACY OF THE ROOT-ZX ACCORDING TO THE VARIOUS CONDITIONS OF ROOT CANALS (근관내 여러 가지 조건에 따른 Root-ZX의 정확도에 관한 연구)

  • Currently frequency-dependent type electronic apex locators have been widely used to determine the working length in endodontic treatment. But, accuracy of electronic apex locators is controversial. The purpose of this study was to evaluate the accuracy of Root-ZX(Morita Co., Japan) at different kinds of conditions of root canals compared with the radiographic working length. The 40 extracted human anterior teeth with fully formed apices and without any caries were used. The radiographs were taken for working length with the 0.5mm short of #15 K-file tip just visible at the foramen under the surgical microscope(Carl Zeiss Co. Germany) at 25X. Then the electronic working lengths were determined with Root-ZX at the different kinds of conditions of root canals according to the presence of electrolyte and Crown-down pressureless technique. The results were as follows ; 1. There was no significant statistical difference in working length between radiograph and Root-ZX. 2. There was no significant statistical difference in electronic working length between the canal with electrolyte and without electrolyte. 3. There was no significant statistical difference in electronic working length between the canal without any instrumentation and after Crown-dow pressureless technique. 4. Of the total 40 root canals, 85% in Group I, 92.5% in Group II, 95% in Group III and 95% in Group IV using Root-ZX showed coincidence within 0.5mm accuracy compaing with the radiographic working length. The results showed that the Root-ZX can be use effectively for measuring the working length of root canal after instrumentation with Crown-down pressureless technique regardless of the presence of electrolyte in root canal.

A survey of experience-based preference of Nickel-Titanium rotary files and incidence of fracture among general dentists

  • Objectives: The purpose was to investigate the preference and usage technique of NiTi rotary instruments and to retrieve data on the frequency of re-use and the estimated incidence of file separation in the clinical practice among general dentists. Materials and Methods: A survey was disseminated via e-mail and on-site to 673 general dentists. The correlation between the operator's experience or preferred technique and frequency of re-use or incidence of file fracture was assessed. Results: A total of 348 dentists (51.7%) responded. The most frequently used NiTi instruments was ProFile (39.8%) followed by ProTaper. The most preferred preparation technique was crown-down (44.6%). 54.3% of the respondents re-used NiTi files more than 10 times. There was a significant correlation between experience with NiTi files and the number of re-uses (p = 0.0025). 54.6% of the respondents estimated experiencing file separation less than 5 times per year. The frequency of separation was significantly correlated with the instrumentation technique (p = 0.0003). Conclusions: A large number of general dentists in Korea prefer to re-use NiTi rotary files. As their experience with NiTi files increased, the number of re-uses increased, while the frequency of breakage decreased. Operators who adopt the hybrid technique showed less tendency of separation even with the increased number of re-use.

ROOT CANAL TREATMENT ON PRIMARY TEETH USING NICKEL-TITANIUM NOTARY FILES (회전식 nickel-titanium file을 이용한 유치의 근관치료)

  • The pulpectomy or root canal treatment on primary teeth is to be done when there is evidence of chronic in flammation or necrosis in the radicular pulp. Due to the tortuous and ribbon-shaped anatomy of the primary teeth, the instrumentation of endodontic hand files and barbed broaches is not an easy procedure. Recently, many automatic Ni-Ti rotary instruments have been developed and has made endodontic treatment easier and faster. This report describes two cases of root canal treatment on primary molars using Ni-Ti rotary files. The cervical constricture was eliminated by the crown-down method, as smaller file proceeded unhindered into the apical third of the canal. In addition, the crown-down technique enhanced the efficacy of the endodontic irrigant. The use of rotary instrumentation for primary teeth seemed to be a more effective way to debride the uneven walls of primary teeth.
