Marginal accuracy and fracture strength of Targis/Vectris Crowns prepared with different preparation designs

  • Statement of problem. Targis/Vectris restorations provide excellent esthetics. Marginal accuracy is significantly influenced by the preparation design. There were no studies to examine the effect of preparation design on the marginal discrepancy and fracture strength of Targis / Vectris crowns. Purpose. This study evaluated the marginal accuracy before and after cementation, and the fracture strength of FRC/Ceromer(Targis / Vectris) crowns according to different preparation design. Material and method. Three metal dies with different convergence angles($6^{\circ},\;10^{\circ},\;15^{\circ}$) were prepared. Total 30 (10 for each angle) Targis/Vectris crowns were made. The restorations were evaluated for adaptation of the margin before and after cementation, then were compressively loaded to failure. Fracture surfaces of the crowns were examined using a SEM. Results. The mean marginal gap was $49{\yen}m\;for6^{\circ},\;55{\S}>for\;10^{\circ}\;and\;70{\S}>for\;15^{\circ}$ and in clinically acceptable level. The mean marginal gap increased significantly after cementation. The increasing amount during cementation was the largest in the $6^{\circ}$ group. The crowns on 60 convergence angle had a significantly higher fracture strength than the crowns on $15^{\circ}$ angle. Mean fracture strength of total crowns regardless of convergence angle was 1390 N, which was higher than all-ceramic crowns. SEM observation showed two-mode fracture pattern. Conclusion. From the results of this study, all of the FRC/Ceromer crowns had clinically acceptable marginal accuracy and could withstand the bite force. Moreover, less convergent angle than all-ceramic crown might be recommended for preparation procedure.

Influence of abutment height and convergence angle on the retrievability of cement-retained implant prostheses with a lingual slot

  • PURPOSE. Cement-retained implant prostheses can lack proper retrievability during repair, and residual cement can cause peri-implantitis. The purpose of this in vitro study was to evaluate the influence of abutment height and convergence angle on the retrievability of cement-retained implant prostheses with lingual slots, known as retrievable cement-type slots (RCS). MATERIALS AND METHODS. We fabricated six types of titanium abutments (10 of each type) with two different heights (4 mm and 6 mm), three different convergence angles ($8^{\circ}$, $10^{\circ}$, and $12^{\circ}$), a sloped shoulder margin (0.6 mm depth), a rectangular shape ($6mm{\times}6.5mm$) with rounded edges, and a rectangular ledge ($2mm{\times}1mm$) for the RCS. One monolithic zirconia crown was fabricated for each abutment using a dental computer-aided design/computer-aided manufacturing system. The abutments and crowns were permanently cemented together with dual-curing resin cement, followed by 24 hours in demineralized water at room temperature. Using a custom-made device with a slot driver and torque gauge, we recorded the torque ($N{\cdot}cm$) required to remove the crowns. Statistical analysis was conducted using multiple regression analysis and Mann-Whitney U tests (${\alpha}=.05$). RESULTS. Removal torques significantly decreased as convergence angles increased. Multiple regression analysis showed no significant interaction between the abutment height and the convergence angle (Durbin-Watson ratio: 2.186). CONCLUSION. Within the limitations of this in vitro study, we suggest that the retrievability of cement-retained implant prostheses with RCS can be maintained by adjusting the abutment height and convergence angle, even when they are permanently cemented together.

디지털 구강스캐너로 모형 없이 제작한 전부지르코니아 수복물의 변연 및 내면 적합도 평가 (Evaluation of marginal and internal gap under model-free monolithic zirconia restoration fabricated by digital intraoral scanner)

  • 목적: 본 연구의 목적은 실제 환자에서 디지털 구강스캐너로 모형 없이 제작한 보철물의 변연 및 내면 적합도를 평가하는 것이다. 대상 및 방법: 전향적 임상시험으로 시행한 본 예비 연구는 총 11개의 수복물을 대상으로 하였다. 구강스캐너(TRIOS, 3shape, Copenhagen, Denmark)로 디지털 구강인상을 채득한 후, 캐드 디자인 및 밀링 가공 과정을 통해 전부지르코니아 수복물을 제작하였다. 완성된 지르코니아관을 환자 구강 내에 시적하였고, 레플리카 술식으로 보철물-지대치 복제물을 얻었다. 이를 근원심, 협설 방향으로 잘라 변연오차, 변연간극과 축벽부, 선각부, 교합면부의 내면간극을 측정하였다. 통계처리는 Kruskal-Wallis 검정과 Mann-Whitney U 검정을 이용하여 통계적 유의성을 분석하였다(${\alpha}=.05$). 결과: 복제물을 통한 적합도 분석 결과, 근원심, 협설 절편 사이에는 통계적으로 유의한 차이가 없었다(P>.05). 변연간극에 불일치가 있었으며, 변연 오차가 변연간극 보다 컸다(P<.01). 결론: 본 연구의 한계 내에서, 구강스캐너로 모형 없이 제작한 전부지르코니아 수복물의 적합도는 임상적으로 허용할만한 결과를 보였다. 그러나 지르코니아관의 변연부위가 과풍융되는 경향이 있었으므로 주의 깊은 임상 적용 및 추적 연구가 요구된다.

전부주조금관 치경부 변연의 형태가 치경부 변연적합에 미치는 영향 (THE EFFECT OF VARIOUS FINISH LINE PREPARATIONS ON THE MARGINAL SEAL OF FULL CROWN PREPARATIONS)

  • 현재 임상에서 사용되고 있는 니켈-크롬합금(Washiloy Soft, Kamemizu Chemical Inc. Ltd., Japan)을 이용하여 Chamfer, shoulder, shoulder with a $45^{\circ}$ bevel의 각 치경부 변연의 형태에 따른 시멘트층 피막후경을 교합면 및 치경부에서 측정, 비교하여 다음과 같은 결론을 얻었다. 금관의 교합면 장착정도는 chamfer가 가장 우수하고, 다음으로는 shoulder, shoulder with a $45^{\circ}$ bevel의 순이며, 이들 간에는 유의성 있는 차이를 나타내었다(p<.05). 금관의 치경부 적합도는 chamfer가 가장 우수하고, 다음으로 shoulder with a $45^{\circ}$ vevel, shoulder의 순이며, 이들 간에는 유의성 있는 차이를 나타내었다(p<.05).

주조에 의한 Core와 CAD-CAM에 의한 Core의 적합도 비교평가 (Comparison of the Marginal and Internal Fit on the Cast and CAD-CAM Cores)

  • 제한된 조건 하에서 수행된 본 연구에서 다음과 같은 결론을 얻었다. 1. 치과용 캐드캠을 이용하여 제작된 지르코니아 코어의 변연 및 내면 적합도보다 전통적인 방식을 이용하여 제작한 금속 코어가 모든 부위에서 우수하였다(p<.001). 2. 두 그룹 모두에서 적합도가 가장 미흡한 부분은 절단부였고, 축벽부가 가장 우수한 것으로 확인되었다. 본 연구 결과를 볼 때 아직까지는 치과용 캐드캠을 이용하여 보철물을 제작하는 것이 기존의 기공 방식을 이용하여 제작된 보철물의 적합도만큼 만족스럽지는 못하였으나 임상적 허용 범위 안에 존재하였다는 점에서 치과용 캐드캠은 임상적으로 사용이 가능한 것으로 판단된다. 앞으로 기계가 개선되고 가공 방식이 지금보다 발전한다면 기존의 기공 방식을 이용하여 제작되는 보철물의 완성도를 능가할 것으로 생각된다.

국내에서 제작된 고정성 보철물의 수명과 실패 요인 및 양상 (LONGEVITY AND FAILURE ANALYSIS OF FIXED RESTORATIONS SERVICED IN KOREA)

  • Statement of problem. Every effort has been continually made to obtain objectivity in measuring the longevity of fixed restorations, such as by establishing unified judgement standard for deciding success and adopting statistical method that analyzes the data of successful and failed cases at the same time. In Korea, however desired level of development has not to be made in this field yet. Purpose. This study, adopting California Dental Association (CDA) quality evaluation system, established objective standard for deciding success, and inferred the longevity of fixed restorations and their failure analysis through adopting Kaplan-Meier survival analysis. Material and method. In order to assess the longevity of flxed restorations serviced in Korea and causes of failure, a total of 1109 individuals (aged 15-74, 716 women and 393 men loaded with 2551 unit fixed restorations, and 1934 abutments) who lived in Kyung-In Province were examined and the findings were as follows : Results. 1. Length of service of fixed restorations serviced in Korea was 6.86$\pm$0.15 yr (mean), 5.5 yr (median), and the rate of success was 65.82% in 5 year survival, and 21.15% in 10 year survival. 2. When there was patient's need for replacing old prosthetics, longevity of fixed restorations was 7.51$\pm$0.27 yr (mean), 7 yr (median), and the rate of success was 61.08% in 5 year survival, and 17.57% in 10 year survival. 3. Longevity of fixed restorations was longest in the over-sixty age group(9.21$\pm$0.66) and that of the teen age group(3.39$\pm$0.28) was shortest (p<0.05). 4. Longevity of fixed restorations of women (7.38$\pm$0.18 years) was longer than that of men (6.00$\pm$0.26) (p<0.05). 5. As for the provider factor (such as unlicensed performers, university hospitals, and private clinic), there was no statistically significant difference in longevity of fixed restorations. 6. Defective margin (34.78%). periodontal disease (12.15%), periapical involvement (11.73%), was the most frequent causes of failure and poor esthetics group showed the longest life above all (p<0.05). Actual frequent causes of failure after removing old prosthetics were defective margin, periapical involvement, periodontal disease and uncemented restoration. In 75.67% of the cases, abutment state after removing old prosthetics was good enough for loading another prosthetics. 7. There was found to have statistically significant influence between longevity of single crown (6.35$\pm$0.20 yr) and that of 3 unit fixed restorations (7.60$\pm$0.30 y) (p<0.05). In each case the most frequent cause of failure was defective margin. 8. The number of cantilever pontic, pontic/abutment ratio, oral hygiene status were found to have no statistically significant influence on longevity of fixed restorations in all groups (p>0.05). 9. Longevity of fixed restorations made of non precious metal was longest (9.60$\pm$0.40 yr) semi precious and precious trailing behind(p<0.05). 10. Group function group (37.04%) and partial group function group (44.62%) were predominant in frequency but showed no correlation between them and among different types of occlusal plane and different types of occlusal surface (p>0.05). 11. Longevity of fixed restorations was longest in the centric interference group(9.35$\pm$0.62) (p<0.05) among different types of occlusal interference. Conclusion. We found that longevity of fixed restorations serviced in Korea is affected by age, gender and type of material, and that most frequent cause of failure is defective margin. In order to assess the accurate longevity of axed restorations, unified research design. overcoming inter-observer difference and establishing the objective research items are needed. Furthermore, it is thought that prospective approach through thorough study and regular follow-ups is needed just from the start of research. Nationwide detailed studies on length of service of fixed restorations manufactured in Korea are hoped to be conducted hereafter.

내측 연결 및 외측 연결 방식으로 설계된 임플란트의 3차원적 유한요소 응력 분석 (Stress distribution of implants with external and internal connection design: a 3-D finite element analysis)

  • 목적: 외측 육각형과 내측 원추형 연결부로 설계된 임플란트 지지 하악 구치 수복물에 교합력을 가할때 발생하는 생역학 현상을 분석하고자 한다. 연구 재료 및 방법: 외측 연결형 임플란트(EXHEX)와 내측 연결형 임플란트(INCON) 그리고 이와 결합할 해당 나사와 지대주 및 크라운을 제작하였고, 하악 무치악 치조골을 설계하였다. 각 부분을 조립하여 2종의 유한요소 모형을 제작하였다. 총 120 N 크기의 수직력(L1)과 45도 측방력(L2)을 가하였고, 유한요소 응력 분석을 시행하였다. 결과: L2 측방력 하중에 의해 발생한 최대 응력은 L1 수직력 하중에 의한 것 보다 6 - 15배 더 컸다. INCON 모델은 EXHEX 모델보다 크라운 교두부에서 2.2배 더 큰 변위량을 보여 주었다. 측방력에 의해 EXHEX 모델은 나사에서, INCON 모델은 임플란트 고정체의 상단 변연부에서 폰미세스 응력의 최대값이 관찰 되었다. INCON 모델에서는 임플란트 내부 계면에서 긴밀한 접촉이 유지 되었다. 결론: 측방력이 큰 변형과 응력을 발생하였으나, 임플란트에서의 최대 응력 발생부위는 INCON과 EXHEX 모델이 서로 상이하였다.

절단피개량과 절단변연형태 및 하중각도가 도재라미네이트 베니어 내의 응력분포에 미치는 영향에 관한 삼차원 유한요소법적 연구 (THREE-DIMENSIONAL FINITE ELEMENT ANALYSIS OF STRESS DISTRIBUTION IN PORCELAIN LAMINATE VENEERS WITH VARIOUS AMOUNTS OF INCISAL COVERAGE AND TYPES OF INCISAL FINISH LINE UNDER TWO LOADING CONDITIONS)

  • The success of porcelain laminate veneer depends on the bond strength between tooth structure and ceramic restoration and the design of tooth preparation. In particular, incisal coverage and incisal finish line are the two most important factors in long-term fracture resistance. Although the majority of clinicians are practicing incisal coverage and there are various opinions on the geo-metrical ratio between the clinical crown length of the remaining tooth structure and the length of incisal extension in porcelain laminate veneer and the optimal incisal finish lines. scientific evidence still loaves much to be desired. The purpose of this study was to determine the effects of the amounts of incisal coverage and the types of incisal finish line on the stress distribution in maxillary anterior porcelain laminate veneers under two different loading conditions. Three-dimensional finite element models of a maxillary anterior porcelain veneer with differ-ent amounts of incisal coverage ; 0, 1, 2, and 3mm and different incisal finish lines feathered edge, incisal bevel, reverse bevel and lingual chamfer with various amounts of lingual extension were developed. 300N force was applied at the point 0.5mm cervical of the linguoincisal edge in two loading conditions ; A) 125 degrees, B) 132 degrees. Tensile and compressive stress in ceramic and shear stress in the resin cement layer were analyzed using three-dimensional finite element method. The results were as follows : 1. The types of incisal finish line had more influence on the stress distribution in porcelain laminate veneer than the amounts of incisal coverage. 2. In case of no incisal coverage, incisal beveled laminate exhibited more evenly distributed tensile stress than feathered edged laminate. And in case of incisal coverage, reverse beveled laminate and lingual chamfered laminate with 1mm lingual extension exhibited more evenly distributed tensile stress than lingual chamfered laminates with 2mm and 3mm lingual extension. 3. As long as the lingual chamfer goes, less tensile stress was found at the incisal edge, while much more tensile stress was found at the lingual margin area in proportion to the length of lingual extension. 4. Under 125 degree load, tensile stress in porcelain laminate veneer had increased compared with that under 132 degree load and the difference exhibited by the change of the amount of tooth support was larger. 5. The types of incisal finish line and the distance from the incisal finish line to the loading point had more influence on the shear stress distribution in the resin cement layer than the amounts of incisal coverage. In contrast loading condition had little influence.
