Assessment of alveolar bone changes in response to minimally invasive periodontal surgery: A cone-beam computed tomographic evaluation

  • Purpose: The aim of this study was to evaluate 3-dimensional cone-beam computed tomography (CBCT) images of alveolar bone changes in patients who underwent minimally invasive periodontal surgery-namely, the pinhole surgical technique (PST). Materials and Methods: Alveolar bone height was measured and compared on CBCT images of 254 teeth from 23 consecutive patients with Miller class I, II, or III recession who had undergone PST. No patient with active periodontal disease was selected for surgery. Two different methods were used to assess the alveolar bone changes postoperatively. In both methods, the distance between the apex of the tooth and the mid-buccal alveolar crestal bone on pre- and post-surgical CBCT studies was measured. Results: An average alveolar bone gain >0.5 mm following PST was identified using CBCT(P=0.05). None of the demographic variables, including sex, age, and time since surgery, had any significant effect on bone gain during follow-up, which ranged from 8 months to 3 years. Conclusion: PST appears to be a promising treatment modality for recession that results in stable clinical outcomes and may lead to some level of resolution on the bone level. More long-term studies must be done to evaluate the impact of this novel technique on bone remodeling and to assess sustained bone levels within a larger study population.

  • Statement of Problem. As a standard means of diagnostics, an orthopantomogram(OPT) permits to measure the vertical and mesiodistal dimension of available bone at the desired implant site with the help of suitable radioopaque references. Based on the clinical investigation of the dentition and the edentulous sites, information upon the width of the implant site can be obtained and documented in the dental scheme. Both findings permit together systematic primary planning for endosteal implants. Purpose of Study. Contents of the present article are the representation of a semiquantitative classification of available bone with the aim to simplify the primary phase of a systematic implant planning. Results. Thus the ASCIi- system permits a clear protocol of bone findings for the implant case with all information available during the primary appointment for treatment planning as a basis of further diagnostic and therapeutic measures. Conclusion. With the ASCIi system, important parameters such as alveolar height and sub-crestal alveolar width can be documented systematically, easily and time saving in the dental scheme as a basis for exact treatment planning.

RBM 표면 테이퍼형태 임프란트의 단기간 후향적 임상 평가 (Short-Term Retrospective Clinical Study of Resorbable Blasting Media Surface Tapered Implants)

  • Purpose: The aim of the present study was to evaluate the clinical outcome of resorbable blasting media surface tapered implant. Methods: 169 Osstem$^{(R)}$ GS III dental implants in 73 patients who received implant treatments at Seoul National University Bundang Hospital, were included in this study. The incidence of biological and prosthetical complications has been carefully analysed for each implant. Results: The short-term implant survival rate was 97.63%, success rate 94.7%. The prevalence of biological complications was 15.38% and the prevalence of prosthetic complications was 13.04%. The mean value of crestal bone loss was $0.28{\pm}0.57$ mm. The relationship between loading periods and marginal bone loss was small and not statistically significant. In mandible, marginal bone loss was larger than in maxilla, no statistically significant. Also, length and diameter of implant had no relationship with marginal bone loss. Conclusion: We suggest that this implant system could achieve successful and stable results.

S-reamer와 겔 형태의 이식재를 이용한 치조정 접근법을 통한 상악동 거상술 임플란트의 성공률과 생존율: 5년 이상 추적 관찰을 통한 후향적 연구 (Success and survival rate of the implant with crestal sinus lift using S-reamer and gel-type graft material: A retrospective study by more 5-years follow check up)

  • 목적: 이번 후향적 연구의 목적은 수압을 이용하는 방식을 변형한 방식인 S-reamer 와 겔 형태의 이식재를 이용한 상악동 거상술을 시행한 임플란트의 5년 이상 추적 관찰을 함으로써 성공률과 생존률을 조사하여 이 술식에 대한 평가를 하는데 있다. 재료 및 방법: 2008년에서 2014년까지 환자들을 추적 관찰하였다. 관찰된 환자는 59명이었고 식립 임플란트는 117개였다. 남성 34명, 여성 25명이고 연령대는 다양하였다. 잔존 수직 골의 골 량은 1 - 6 mm로 다양하였다. 상악동 거상술은 막의 천공없이 상악동을 천공하는데 S-reamer를 사용하였고 막을 거상하는데 겔 형태의 이식재를 사용하여 상악동 거상술을 시행하였다. 모든 임플란트는 거상과 동시에 식립하였고 5 - 6개월 치유 기간 후 보철물을 장착하였다. 방사선 검사는 추적 검사 할 때와 식립 후 바로 찍은 방사선 검사를 비교하였다 그리고 탐침을 통하여 화농여부, 염증여부, 출혈여부 등등 임상 검사를 통하여 검진 평가 하였다. Buser의 성공 기준을 참고하였고 모든 임플란트를 성공 임플란트, 생존 임플란트, 실패 임플란트로 분류하였다. 결과: 실패하여 제거한 임플란트는 5개였고 골 소실을 동반한 급성 염증 치료를 했거나 염증을 보인 임플란트가 4개였다. 생존율은 95.7%이고 성공률은 92.3%였다. 결론: S-reamer와 겔 형태의 이식재를 이용한 상악동 거상술이 1 - 6 mm 잔존 골이 있는 상황에서 막 천공이 없이 상악동을 거상할 수 있는 성공적인 술식임을 알 수 있었다.

A short-term clinical study of marginal bone level change around microthreaded and platform-switched implants

  • Purpose: The marginal bone levels around implants following restoration are used as a reference for evaluating implant success and survival. Two design concepts that can reduce crestal bone resorption are the microthread and platform-switching concepts. The aims of this study were to analyze the placement of microthreaded and platform-switched implants and their short-term survival rate, as well as the level of bone around the implants. Methods: The subjects of this study were 27 patients (79 implants) undergoing treatment with microthreaded and platform-switched implants between October 2008 and July 2009 in the Dental Hospital of Yonsei University Department of Periodon-tology. The patients received follow-up care more than 6 months after the final setting of the prosthesis, at which time periapical radiographs were taken. The marginal bone level was measured from the reference point to the lowest observed point of contact between the marginal bone and the fixture. Comparisons were made between radiographs taken at the time of fixture installation and those taken at the follow-up visit. Results: During the study period (average of 11.8 months after fixture installation and 7.4 months after the prosthesis delivery), the short-term survival rate of microthreaded and platform-switched implants was 100% and the marginal bone loss around implants was $0.16{\pm}0.08$ mm, the latter of which is lower than the previously reported values. Conclusions: This short-term clinical study has demonstrated the successful survival rates of a microthread and platform-switched implant system, and that this system is associated with reduced marginal bone loss.

임플란트 식립을 위한 상악동 점막 거상술의 결과 (Results of Maxillary Sinus Elevation for Endosseous Implant Placement)

  • Background: Although dental implantation has become widespread and acceptable treatment for dental prosthodontics, maxillary posterior jaw region is often complicated by the pneumatization of the maxillary sinus and physiological resorption of the alveolar bone. When this occurs, the residual bone between the floor of the sinus and the crestal ridge is inadequate for the placement of implants. The sinus elevation procedure provides a way to increase the amount of available bone and to allow the placement of longer implants. Materials & methods: We studied 11 patients requiring the implant placements and the maxillary sinus elevation simultaneously from 1996 to 2003 in our clinic. Nine patients were males and two patients were females, aged from 39 to 72(mean=51.6). Four patients had medical compromised states; angina pectoris, diabetes, hypertension, hepatitis. Patients didn't show any pathologic findings clinically or radiographically. We studied the success and survival rate of implants, factors increasing the osseointegrating capacity of implants. Results: The success rate of osseointegration of implants was 93%. At least 6 months after loading on implants, the survival rate of implants was 78.5%. Autogenous bone graft and adequate residual bone height(>6mm) increased survival rate of implants. Conclusion: Successful implant placement with maxillary sinus elevation mainly depends on sufficient residual bone height, healthy maxillary sinus, autogenous bone graft.

상악동저 거상술 후 상악 구치부에 식립된 임플란트의 생존율에 대한 연구 (Survival analysis of implants placed in the sinus floor elevated maxilla)

  • Objective: The sinus floor elevation procedures have been used to facilitate implant placement in the severely atrophic posterior maxilla. Many variables may have an influence on the outcomes of the sinus floor elevation in combination with implant treatment. The aim of this study was to analyze survival rate of implants placed in the edentulous maxillae of patients in whom sinus floor elevation was undertaken according to variables. Materials and Methods: It consisted of 96 patients(50 male and 46 female), ranging in age from 31 to 70 years(mean 49 years), who underwent sinus floor elevation procedure(94 implants in left side and 106 implants in right side) from 2001 to 2002. A total of 200 implants were placed in the grafted sinus(73 implants in lateral approach and 127 implants in crestal approach). All implants were restored by fixed prosthesis. All patients were healthy. Follow-up periods for implants were between 48 to 60 months. Results: The cumulative survival rate of implants was 91.5%. Gender, age and operation site did not have an influence on the survival rate. There was statistically significant differences for the implants which placed in less than 4 or 5 rom residual bone height, the survival rate was 60%, 81.4% respectively (p<0.05). There was no statistically significant difference of implants survival rate ac- cording to approach technique. The survival rate for 100% autogenous bone grafts was lower with respect to composite grafts containing autogenous bone and 100% substitutes. The survival rate for hydroxyapatite-coated implants was statistically significant lower than other textured group (p<0.05). Conclusion: Residual bone height, surface texture and graft materials have an influence on the survival rate. To use autogenous bone as a part of a composite bone replacement, implant texture which leads to more favorable implant-bone interface were necessary. To determine residual bone height for initial implant stability was important.

  • Implant-supported fixed and removable prostheses provide a proper treatment modality with reliable success. The SS $II^{(R)}$ Implants is a one-stage nonsubmerged threaded titanium implants with Resorbable Blasting Media (RBM) surface developed by Osstem company (Busan, Korea) in October of 2002. This study is to evaluate the survival rate of the SS $II^{(R)}$ Implants for 4 years using radiographic parameters and to review the retrieved implants by the cytotoxicity tests. Since September 2003, 439 SS $II^{(R)}$ implants had been used for 173 patients at Ewha Womans University Medical Center in Korea. Patients consisted of 91 females (52.6 %) and 82 males (47.4 %). The patients' mean age was $42\;{\pm}16$ years, ranging from 21 to 83 years. The follow-up period ranged from 9 to 46 months (mean F/U $24.2\;{\pm}\;10.2$ months). The results are as follows; 1. Of 439 implants, 17 implants were removed and 4-year cumulative survival rate was 96.1%. 2. 82.3% of 17 failed implants were founded during healing phase, and 94.1% of failed fixtures were removed within 5 months after implantation. 3. Crestal bone around the implants was resorbed to 1 mm in 89.0%, to 1 - 2 mm loss of the marginal bone in 8.3%, and the bone loss over 2 mm was occurred in 2.7%. 4. Microscopic examination of the retrieved implants disclosed Grade 0 cytotoxicity in 4 and Grade 1 cytotoxicity in 2 of 6 groups divided according to LOT numbers. Inhibition rate with optical density was acceptable as low as ISO standard.

Finite Element Stress Analysis according to Apical-coronal Implant Position

  • Purpose: The purpose of this study was to evaluate the influence of apical-coronal implant position on the stress distribution after occlusal and oblique loading. Materials and Methods: The cortical and cancellous bone was assumed to be isotropic, homogeneous, and linearly elastic. The implant was apposed to cortical bone in the crestal region and to cancellous bone for the remainder of the implant-bone interface. The cancellous core was surrounded by 2-mm-thick cortical bone. An axial load of 200 N was assumed and a 200-N oblique load was applied at a buccal inclination of 30 degrees to the center of the pontic and buccal cusps. The 3-D geometry modeled in Iron CAD was interfaced with ANSYS. Results: When only the stress in the bone was compared, the minimal principal stress at load Points A and B, with a axial load applied at 90 degrees or an oblique load applied at 30 degrees, for model 5. The von Mises stress in the screw of model 5 was minimal at Points A and B, for 90- and 30-degree loads. When the von Mises stress of the abutment screw was compared at Points A and B, and a 30-degree oblique load, the maximum principal stress was seen with model 2, while the minimum principal stress was with model 5. In the case of implant, the model that received maximum von Mises stress was model 1 with the load Point A and Point B, axial load applied in 90-degree, and oblique load applied in 30-degree. Discussion and Conclusions: These results suggests that implantation should be done at the supracrestal level only when necessary, since it results in higher stress than when implantation is done at or below the alveolar bone level. Within the limited this study, we recommend the use of supracrestal apical-coronal positioning in the case of clinical indications.

나선형 단층방사선사진촬영에서 촬영조건이 악골 단면상의 높이, 폭 및 인지도에 미치는 영향 (The effect of different radiographic parameters on the height, width and visibility of cross-sectional image of mandible in spiral tomography)

  • Purpose : To evaluate the differences in bone height, bone width, and visibility of posterior spiral tomographic images according to various exposure directions, image layer thickness, and inclination of the mandibular inferior border. Materials and Methods: Six partially and completely edentulous dry mandibles were radiographed using Scanora spiral tomography. Spiral tomography was performed at different exposure directions (dentotangential and maxillotangential projection), image layer thicknesses (2 mm, 4 mm and 8 mm), and at various inclinations to the mandibular border (+ 100, 00 and -10°). The bone height and width was measured using selected tomographic images. The visibility of mandibular canal, crestal bone, and buccal and lingual surfaces were graded as 0, 1, or 2. Results : The bone width at the maxillo-tangential projection was wider than at the dento-tangential projection (p < 0.05). The visibility of buccal and lingual surface at the maxillo-tangential projection was higher than at the dento-tangential projection (p<0.05). Thinner image layer thicknesses resulted in greater visibility of buccal and lingual surfaces (p < 0.05). Bone height was greatest in the -10° group, and at the same time the bone width of the same group was the narrowest (p < 0.05). The visibility of alveolar crest and buccal surface of the + 10° group was the highest, while the visibility of the mandibular canal was greatest in the 00 group. Conclusion: When spiral tomography is performed at the mandibular posterior portion for visualization prior to implant surgery, it is important that the inferior border of mandible be positioned as parallel as possible to the floor. A greater improvement of visibility can be achieved by maintaining a thin image layer thickness when performing spiral tomography.
