한국 성인의 두경부자세와 두개안면형태에 관한 두부 X-선 계측학적 연구 (STUDY OF CRANIOCERVICAL POSTURE AND CRANIOFACIAL MORPHOLOGY IN KOREAN YOUNG ADULTS)

  • 본 연구는 한국성인의 두경부자세와 두개안면형태를 두개내 및 두개외 기준선에 대하여 알아보고 그 상관성을 평가하여 향후 진단시 보조적 자료로 사용하고자하여 시행되었다. 양호한 안모와 1급 구치관계를 지닌 성인 남, 녀 각 25명씩 총 50명에 대하여 natural head position 측모 두부 X-선 계측사진을 촬영하여 투사도를 그린후 posterior nasal spine을 지나는 true vertical line과 sella를 지나는 true horizontal line을 기준으로하여 자세변수 및 형태변수를 계측, 계산하였다. SPSS통계처리 프로그램을 이용하여 남녀평균, 표준편차를 구하고 student t-test로 유의차를 검정한후 자세변수와 형태변수간의 상관관계를 보았으며, 또한 자세변수를 이용하여 두경부자세의 재현성을 평가하여 다음과같은 결과를 얻었다. 1. 한국성인에서의 두경부자세변수와 두개안면형태변수의 평균과 표준편차를 얻었다. 2. 한국성인은 OPT/CVT간 각에 있어서 $3.55{\pm}2.58^{\circ}$의 경추 전방만곡을 보였다. 3. 자세변수와 형태변수의 상관관계에서는 두경부자세와 안면의 수직적인 비율, 안면돌출도, 하악골의 회전이 높은 상관성을 보였다. 4. 두경부자세와 상하악간 관계, 전후방적 악골비율의 상관관계는 미약하였다. 5. Natural head position에서의 head positioning error는 $1.65^{\circ}$로 SN line의 개인간 변이도 $3.31^{\circ}$ 보다 작았다. 다른 자세변수도 head positioning error가 각 변수들의 개인간 변이도보다 작았다.

Clinical Analysis of Lobular Keloid after Ear Piercing

  • Background: Lobular keloid appears to be a consequence of hypertrophic inflammation secondary to ear piercings performed under unsterile conditions. We wish to understand the pathogenesis of lobular keloids and report operative outcomes with a literature review. Methods: A retrospective review identified 40 cases of lobular keloids between January, 2005 and December, 2010. Patient records were reviewed for preclinical factors such as presence of inflammation after ear piercing prior to keloid development, surgical management, and histopathologic correlation to recurrence. Results: The operation had been performed by surgical core extirpation or simple excision, postoperative lobular compression, and scar ointments. Perivascular infiltration was noted in intra- and extra-keloid tissue in 70% of patients. The postoperative recurrence rate was 10%, and most of the patients satisfied with treatment outcomes. Conclusion: Histological perivascular inflammation is a prominent feature of lobular keloids. Proper surgical treatment, adjuvant treatments, and persistent follow-up observation were sufficient in maintaining a relatively low rates of recurrence.

The influence of age on lip-line cant in adults: a cross-sectional study

  • Objective: The aims of this study were to assess the direction and degree of lip-line cant in Korean adult orthodontic patients and to identify the effects of sex and age on changes in the cant severity. Methods: In this cross-sectional retrospective study, lip-line cant was measured in the frontal photographs of 585 Korean patients (92 men and 493 women) aged 18-48 years. The outcome variables (direction and degree of lip-line cant) were assessed in terms of predictor variables (sex, age, sagittal skeletal relationship, and menton deviation angle). Results: The direction of lip-line cant did not differ according to sex, age, or skeletal classification. Patients had $1.6^{\circ}$ of lip-line cant on average before orthodontic treatment. Middle-aged adults displayed a significant trend toward a lower degree of lip-line cant compared to younger adults (p < 0.01). Multiple linear regression analysis showed that the degree of lip-line cant was weakly negatively correlated with age (p < 0.001). Conclusions: While the direction of lip-line cant did not differ according to the parameters explored here, the degree of cant was correlated with age in adults, independent of menton deviation. Specifically, middle-aged adults tended to display significantly lower degrees of lip-line cant than did younger adults.

  • For the purpose of interpretation of positional changes of craniofacial structures in Class III malocclusion between mixed and permanent dentition, 73 normal samples and 103 Class III samples of mixed dentition and 125 normal samples and 168 Class III samples of permanent dentition were selected. Comparative cephalometric analysis was undertaken between them respectively by mesh diagram method to evaluate the positional changes of maxilla and mandible in anteroposterior direction and vertical direction and also the inclination changes of maxillary and mandibular incisors in labio-lingual direction. The following results were obtained : 1. The antero-posterior positional changes of the maxilla and mandible were posterior direction of maxilla and anterior direction of mandible. 2. The vertical positional changes of the maxilla and mandible were superior direction of both maxilla and mandible. 3. The labio-lingual inclination changes of the maxillary and mandibular incisors were lingual direction of both maxillary and mandibular incisors.

Delayed formation of sterile abscess after zygomaticomaxillary complex fracture treatment with bioabsorbable plates

  • We present a patient who showed a sterile abscess after facial bone fixation with bioabsorbable plates and screws. He had zygomaticomaxillary complex and periorbital fracture due to falling down. The displaced bones were treated by open reduction and internal fixation successfully using bioabsorbable plate system. However, at postoperative 11 months, abrupt painless swelling was noted on the previous operation sites, left lateral eyebrow and lower eyelid. By surgical exploration, pus-like discharge and degraded materials were observed and debrided. The pathologic analysis revealed foreign body reaction with sterile abscess. This complication followed by bioabsorbable device implantation on maxillofacial bone surgery has been rarely reported in which we call attention to the maxillofacial plastic surgeons.

Staged excision of basal cell carcinoma arising from a tattooed eyebrow

  • Side effects of tattoos can occur due to unexpected body responses. Concerns about the side effects of tattoos are growing concomitantly with the increasing number of tattooed individuals. We report a rare case of basal cell carcinoma (BCC) on a tattooed eyebrow. A 48-year-old woman with no family history or occupational risk of skin cancer had her eyebrows tattooed 6 years prior, and she noticed a black mass on her right eyebrow 1 year before presentation. Staged excision was planned due to the patient's reluctance regarding the possibility of a wide scar. After the first surgical procedure, a histopathological examination confirmed BCC. Wide excision and primary closure were performed for remnant BCC, and no recurrence was observed. The mechanism of skin neoplasms involving tattoos has not been clearly identified. Unapproved tattoo inks contain carcinogenic substances, and previous case reports indicate that skin neoplasms may differ depending on the tattoo color. Therefore, the carcinogenic effects of these unknown components of tattoo ink might cause skin neoplasms. Accurate component analysis and systematic management of tattoo ink is necessary, and medical practitioners must also pay attention to this possibility because it is easy to overlook tattoos as a causal factor contributing to cancer.

Analysis of Facial Asymmetry

  • Facial symmetry is an important component of attractiveness. However, functional symmetry is favorable to aesthetic symmetry. In addition, fluctuating asymmetry is more natural and common, even if patients find such asymmetry to be noticeable. However, fluctuating asymmetry remains difficult to define. Several studies have shown that a certain level of asymmetry could generate an unfavorable image. A natural profile is favorable to perfect mirror-image profile, and images with canting and differences less than $3^{\circ}-4^{\circ}$ and 3-4 mm, respectively, are generally not recognized as asymmetry. In this study, a questionnaire survey among 434 medical students was used to evaluate photos of Asian women. The students preferred original images over mirror images. Facial asymmetry was noticed when the canting and difference were more than $3^{\circ}$ and 3 mm, respectively. When a certain level of asymmetry is recognizable, correcting it can help to improve social life and human relationships. Prior to any operation, the anatomical component for noticeable asymmetry should be understood, which can be divided into hard tissues and soft tissue. For diagnosis, two-and three-dimensional (3D) photogrammetry and radiometry are used, including photography, laser scanner, cephalometry, and 3D computed tomography.

Simple Aesthetic Correction for Patients with Acute Auriculocephalic Angle

  • Background: Acute auriculocephalic angle refers to an ear with helix that is spaced closely to the cranium. An increasing number of patients with acute auriculocephalic angle wish to undergo corrective operation for aesthetic purposes. However, there is a paucity of data regarding acute auriculocephalic angle. This paper proposes a treatment protocol for patients with acute auriculocephalic angle. Methods: We performed a retrospective analysis of patients undergoing acute auriculocephalic angle (4 patients, 6 ears). Patient records were reviewed for demographic data as well as auricular measurements at preoperative, immediate postoperative and final follow-up evaluations. Results: All of the patients were men with a mean age of 36.5 years (range, 23-52 years). The mean follow-up period was 47.5 months (range, 28-60 months). Postoperative auriculocephalic angle was close to the normal auriculocephalic angle ($25^{\circ}-30^{\circ}$) without notable scars. Moreover, the patients had minimal contractions of the skin flaps without any hematoma or relapse. Conclusion: We propose the following three treatment protocols for patients with acute auriculocephalic angle: the posterior auricular muscle should be sufficiently released, the mastoid area should be augmented using implants, the skin should be repositioned with a superior auricular flap.

Evaluation and management of facial granuloma caused by various injection materials

  • Background: Various substances are currently being used for cosmetic or postsurgical reconstruction reasons. Injecting of various materials into the face may provoke inflammatory or granulomatous reactions, infection and tissue necrosis. Among these reactions are foreign body granulomas. This study aims to describe the clinical characteristics of and therapeutic approaches used in patients with facial foreign body granulomas caused by the injection of various substances. Methods: From 2007 to 2020, a total of 64 patients visited our hospital due to inflammatory signs, palpable masses or surface irregularities. We reviewed patient characteristics, type of injected material, medical history and treatment outcomes. The treatment results were scored with patient satisfaction and statistical analysis of the treatment period was performed. Results: Most patients underwent conservative treatment followed by surgical treatment because of persistent symptoms; one patient did not require surgery. All patients reported good treatment results and satisfaction. The earlier the surgical treatment was performed, the shorter the treatment period and the higher patient satisfaction scoring. Conclusion: Granulomatous changes to the face are an emerging concern in various cosmetic procedures and surgeries, including fillers, silicone implants and autologous fat injections. Our findings strongly suggest that patient symptoms require accurate diagnosis and surgical treatment.

Outcome analysis of biplanar mandibular distraction in adults

  • Background: Mandibular deficiency leading to facial asymmetry causes cosmetic deformity as well as psychological stigma for the patient. Correction of these mandibular asymmetries is a major challenge. The study investigates the efficacy of bidirectional mandible distraction for the treatment of mandibular deficiency. Methods: This prospective study included six individuals aged between 17 and 24.4 years. Five patients had hemifacial microsomia and one had unilateral temporomandibular joint ankyloses. All patients underwent mandibular distraction osteogenesis. Postoperative skeletal changes in affected mandible, and changes in occlusal plane and oral commissure cant were evaluated using three-dimensional reconstruction. Patient satisfaction and understanding of the procedure were assessed through three questionnaires administered during pre-distraction, distraction and post-distraction phases. Results: In pre-distraction phase, aesthetic appearance seemed to be the primary indication for surgery. In distraction phase, pain while chewing was the primary handicap. In post-distraction phase all patients were satisfied with the aesthetic outcome. The facial deformity was improved through mandibular distraction osteogenesis. On the affected side in all the patients, height and length of the mandible increased. Canting of the occlusal plane and oral commissure was corrected. Conclusion: Bidirectional mandible distraction is an effective treatment for correction of mandible deformities in adult patients.
