한국인 7-17세 아동의 두개저, 상악, 하악의 성장에 관한 준종단적 연구 (A Semilongitudinal Study on Cranial Base, Maxillary and Mandibular Growth of Korean Children Aging 7 to 17 Years Old)

  • 한국인 7세-17세 아동의 두개저, 상악, 하악의 사춘기 성장 양상을 파악해 보고자 남자 251개, 여자 286개의 측모두부계측방사선 사진을 이용, 10개의 계측점과 16개의 계측항목을 설정하고 계측항목에 대한 계측치와 각 연령별 년간 누년차를 산출한 다음 이들간의 통계적 유의성을 검증하였으며 각 계측항목을 두개저, 상악, 하악의 세군으로 분류하고 또 남녀별로 분류하여 비교함으로써 다음과 같은 결론을 얻었다. 1. 한국인 성장기 아동에 있어서의 성장은 남자 아동에 비해 여자 아동이 빠른 시기에 일어났다. 2. 두개저, 상악골, 하악골 모두 사춘기 성장을 나타내었으나 두개저의 성장은 안면골 성장에 비해 상대적으로 작았다. 3. 두개저 성장에 있어서 전두개저 길이의 증가에 비해 중두개저 및 후두개저의 길이 증가가 현저했으며 사춘기 성장증가(circumpuberal growth spurt) 양상도 보다 명확했다.4. 상악골의 전하방 이동은 상악골 자체의 길이 성장과 상악골 주위 봉합부의 성장에 따른 변위가 종합된 결과로 두개저에 대한 상악골의 상대적 위치를 나타내는 Ar-ANS와 Ar-Pr이 상악골 자체의 길이 성장을 나타내는 ANS-PNS보다 많은 성장량을 보였다. 5. 하악골은 수평 성장량에 비해 수직 성장이 약간 크게 나타났으나 유의할 수준은 아니다. 6. 상악 치조골과 하악 치조골은 유치 탈락후 영구치 맹출시기에 최대 성장률을 보인후 감소하는 경향을 보이지만 하악 치조골의 수직성장에서는 사춘기 성장증가가 관찰되었다.

Anterior Cranial Base Reconstruction in Complex Craniomaxillofacial Trauma: An Algorithmic Approach and Single-Surgeon's Experience

  • Management of traumatic skull base fractures and associated complications pose a unique reconstructive challenge. The goals of skull base reconstruction include structural support for the brain and orbit, separation of the central nervous system from the aerodigestive tract, volume to decrease dead space, and restoration of the three-dimensional appearance of the face and cranium with bone and soft tissues. An open bicoronal approach is the most commonly used technique for craniofacial disassembly of the bifrontal region, with evacuation of intracranial hemorrhage and dural repair performed prior to reconstruction. Depending on the defect size and underlying patient and operative factors, reconstruction may involve bony reconstruction using autografts, allografts, or prosthetics in addition to soft tissue reconstruction using vascularized local or distant tissues. The vast majority of traumatic anterior cranial fossa (ACF) injuries resulting in smaller defects of the cranial base itself can be successfully reconstructed using local pedicled pericranial or galeal flaps. Compared with historical nonvascularized ACF reconstructive options, vascularized reconstruction using pericranial and/or galeal flaps has decreased the rate of cerebrospinal fluid (CSF) leak from 25 to 6.5%. We review the existing literature on this uncommon entity and present our case series of n = 6 patients undergoing traumatic reconstruction of the ACF at an urban Level 1 trauma center from 2016 to 2018. There were no postoperative CSF leaks, mucoceles, episodes of meningitis, or deaths during the study follow-up period. In conclusion, use of pericranial, galeal, and free flaps, as indicated, can provide reliable and durable reconstruction of a wide variety of injuries.

치조열에서 블록 골이식을 이용한 임플란트 동시 매식법 (Simultaneous Implant Installation Using the Block Bone Graft)

  • Simultaneous implant installation with bone graft was performed in 15 cases. Four cases were cleft alveolus patients. 56 implants were placed immediately with block bone grafts. 2 cases were cranial bone grafts and the others were iliac bone grafts. Three of 56 implants were lost(94.6% Survival rate). One of three was cleft alveolus case. The cleft alveolus patients with simultaneous implants installation showed functional and esthetic results without infraocclusion and positional changes. Bergland index was considered to be type I after 12 months later. Immediate implant installation with bone graft is one of choice of treatment in closing cleft alveolus hoping simultaneous implant installation could be related with function which might result in less resorption of graft. Functional and esthetic results are satisfaction ; there was no infraocclusion and positional changes.

Comparison of the bone healing capacity of autogenous bone, demineralized freeze dried bone allograft, and collagen sponge in repairing rabbit cranial defects

  • Objectives: This study sought to evaluate the efficacy of collagen graft materials, as compared to other graft materials, for use in healing calvarial defects in rabbits. Materials and Methods: Ten mm diameter calvarial defects were made in ten rabbits. The rabbits were then divided into 4 groups: control, autogenous bone graft, SureOss graft, and Teruplug graft. Bone regeneration was evaluated using histological and radiographic methods. Results: Based on visual examination, no distinct healing profile was observed. At 4 weeks after treatment, histological analysis showed there was no bone regeneration in the control group; however, at 8 weeks after treatment, new bone formation was observed around the margin of the defective sites. In the autogenous bone graft group, new bone formation was observed at 4 weeks after treatment and mature bone was detected around the grafted bone after 8 weeks. In the SureOss graft group, at 4 weeks after treatment, acute inflammatory and multinuclear cells were noted around the grafted materials; at 8 weeks after treatment, a decrease in graft materials coupled with new bone formation were observed at the defective sites. In the Teruplug graft group, new bone formation was detected surrounding the bone margin and without signs of inflammation. There were statistically significant differences observed between the graft and control group in terms of bone density as evidenced by radiographic analysis using computed tomography (P<0.05), particularly for the autogenous bone graft group (P<0.001). Conclusion: These results suggested that autogenous bone, SureOss and Teruplug have the ability to induce bone regeneration as compared to an untreated control group. The osteogenic potential of Teruplug was observed to be lower than that of autogenous bone, but similar to that of SureOss.

Huge Size Intracranial Plasmacytoma Treated with Surgery and Fractionated Stereotactic Radiotherapy

  • Surgery and radiotherapy are mainly used for plasma cell neoplasm which constitutes about $1{\sim}2%$ of human malignancy. The authors carried out Fractionated Stereotactic Radiotherapy[FSRT] on the residual tumor after the subtotal removal of Intracranial plasmacytoma. A huge mass lesion was observed on MRI [magnetic resonance image] in the left anterior and middle cranial fossa of a 63-year-old man with left exophthalmus which lasted for a month, and was suspected as a meningioma with strong contrast enhancement. Extramedullary plasmacytoma was diagnosed on histopathological examination. After the surgery, FSRT was also carried out on the residual tumor which invaded the skull base. One-year follow up after FSRT showed contrast enhancement only in the left sphenoid bone on MRI, which indicated significant decrease in the size of the tumor without any abnormal neurologic deficits. We treated intracranial plasmacytoma which invaded left anterior and middle cranial fossa and surrounded cavernous sinus without cranial nerve deficit through subtotal tumor removal and FSRT.

가토 두개골 결손부에 이식된 ${\beta}$-TCP의 골치유과정에서 맥동전자기장의 영향에 관한 연구 (THE EFFECT OF PULSED ELECTROMAGNETIC FIELDS ON ${\beta}$-TCP GRAFT IN RABBIT CRANIAL BONE DEFECT)

  • The purpose of this research was to investigate whether pulsed electromagnetic field (PEMF) stimulation applied to the rabbit cranial defects grafted with ${\beta}$-tricalcium phosphate (${\beta}$-TCP) could affect the new bone formation. With 16 New Zealand white rabbits under the same condition, bilateral calvarial bone defects were formed around the sagittal suture line. The defect on the left side was grafted with ${\beta}$-TCP, while on the right side was grafted by harvested autogenous bone. PEMF was applied to 8 rabbits for 8 hours per day. The bony specimen were divided into 3 groups, the group 1 was autogenous bone grafted specimen, the group 2 was ${\beta}$-TCP grafted with PEMF, and the group 3 was ${\beta}$-TCP grafted without PEMF. We investigated the bone regeneration & growth factor expression at 2, 4, 6, and 8 weeks. As a result, BMP 2 was expressed in the group 1 from 2 weeks, the group 2 from 4 weeks, and the group 3 from 6 weeks. BMP 4 was expressed in the group 1 from 2 weeks, in the group 2 and the group 3 from 4 weeks. 4. There was no significant difference in expression pattern of BMP 7, PDGF, VEGF, and TGF-${\beta}$1 during grafted bone regeneration in group 1, 2, and 3. According to our results, PEMF stimulation could be effective on the new bome formation in animal study, and have a feasibility of clinical use.

광범위하게 진행된 다발성 섬유성이형성증 (A Case of Extensive Polyostotic Fibrous Dysplasia)

  • Fibrous dysplasia is a benign disorder of bone consisting of intramedullary proliferation of fibrous tissue and irregularly distributed, poorly developed bone. The disease manifests itself in the monostotic form in which only one bone is involved and the polyostotic form in which multiple bones at different sites are affected. We reported a extensive case of polyostotic fibrous dysplasia with involvement of craniofacial bones, mandible, ribs and extremities. A 18-year-old man showed remarkable right facial swelling who had been treated on right femur 3 years ago with a bone graft for pathologic fracture and he recognized facial swelling 5 years ago. Extraoral radiograms and computed tomogram showed diffuse sclerosis with a ground glass appearance of the most cranial bones, facial bones. The right mandibular lesion showed very expansile lesion with mottled appearance. Bone scans showed mutifocal increased uptakes in craniofacial bones, right mandible, bilaterally in ribs, humerus, femur, tibia and characteristic varus deformity of right femur (shepherd's crook defomity). This case showed exceptionally bilateral, extensive nature of bone lesion and didn't show any features of skin pigmentation and endocrine disturbances.

Posttraumatic Giant Extradural Intradiploic Epidermoid Cysts of Posterior Cranial Fossa: Case Report and Review of the Literature

  • We reported a unique case of posttraumatic giant infratentorial extradural intradiploic epidermoid cyst. A 54-year-old male, with a previous history of an open scalp injury and underlying linear skull fracture in the left occipital region in childhood, presented with a painful subcutaneous swelling, which had been developed gradually in the same region and moderate headache, nausea, vomiting and cerebellar ataxia. The duration of symptoms on admission was 3 months. Imaging studies revealed occipital bone destruction and giant extradural intradiploic lesion. The preoperative diagnosis was giant infratentorial extradural intradiploic epidermoid cyst. Surgery achieved total removal of the lesion, which was histologically confirmed and the postoperative course was uneventful. To our knowledge, this is the first case of giant infratentorial extradural intradiploic epidermoid cyst with a traumatic etiology described in the literature.

비화농성 뇌염을 동반한 개 수두증 1례 (A Case of Canine Hydrocephalus with Nonsuppurative Encephalitis)

  • A Shih Tzu Puppy had clinical onset of anotexia crying and progression of neurological sings when enlargement of the cranial vault at 1 month old and died after showing clinical signs during 2 months period. Radiological and pathological examinations were performed. Radiological findings were homogeneous appearance of the calvaria with cortical thinning, loss of the normal convolutional skull markings and persistent fontanelles. Grossly enlargement of the cranial vault thinning of the bone and defective closure of the fontanelles were also observed. The entire subcortical area of the cerebral hemispheres with severe, dilatation of ventricles and cerebrospinal fluid(CSF) wits absent. There was parenchyma atrophy affecting chiefly in the white mater and the cerebral cortices, axon degeneration and necrosis and gitter cell infiltration in the whiter matter and the subependymal area. Mononuclear perivacular cuffing in the cerebrum and the pons was shown. Based on the radiological, gross and histopathological findings, this case was believed to have congenital hydrocephalus with nonsuppurative encephahitis. Possible etiology on the case is also discussed.

코에 국한된 희귀 안면 갈림 (Rare Cleft of the Nose)

  • Congenital facial cleft is a rare entity and appears along by the line of different processes of the facial development. An isolated cleft of the nose has been reported not often in the literature. We treated a patient with an isolated nasal cleft associated with undefined cranial anomaly. On 3D CT scan was seen a bony cleft traversing the pyriform aperture lateral to the anterior nasal spine. The nasal septum and frontal process of the maxilla were intact. There also was found bilateral bony defects in the frontal bone and bilateral frontal boss. The nasal cleft and frontal defect and boss were corrected by two stages: anterior two-third of the cranial vault with bilateral frontal boss was remodeled at the age of two years and the nasal cleft was repaired with a local rotation flap at age 3.
