Associated Factors and Prevention of Upper Pole Rippling in Prepectoral Direct-to-Implant Breast Reconstruction

  • Background Despite its many advantages, prepectoral breast reconstruction also carries the risk of implant rippling. The recent introduction of partial superior implant coverage using a pectoralis muscle slip in prepectoral direct-to-implant (DTI) breast reconstruction has shown the potential to minimize upper pole rippling. The purpose of this study was to identify factors associated with rippling and the effectiveness of our surgical technique. Methods In total, 156 patients (186 breasts) who underwent prepectoral DTI breast reconstruction between August 2019 and March 2021 were identified retrospectively. Patient data were analyzed from medical records. Univariable and multivariable logistic analyses were performed to contextualize the risks associated with rippling deformity relative to demographic characteristics and other clinical factors. Retrospective propensity-matched analysis was performed to identify the relationship between rippling deformity and the reconstruction method. Results Patients with body mass index (BMI; odds ratio [OR], 0.736; p < 0.001), those with a postoperative chemotherapy history (OR, 0.324; p = 0.027) and those who received breast reconstruction via the superior coverage technique (OR, 0.2; p = 0.004), were less likely to develop rippling deformity. The median follow-up period was 64.9 weeks, and there were no significant differences between patients in types of mastectomy, implant, or acellular dermal matrix. Patients who underwent superior coverage technique-based reconstruction showed significantly reduced rippling (OR, 0.083; p = 0.017) Conclusion Patients with higher BMI and prior postoperative chemotherapy were less likely to develop rippling deformity. The superior coverage technique can be effective in minimizing upper pole rippling.

Toothbrushing Behavior and Oral Health State of Immigrant Workers (외국인 이주노동자의 칫솔질행태와 구강건강상태)

  • This study analyzed the tooth brushing behavior and oral health status of foreigner immigrant workers and obtained the following results. Statistical significance based on different general characteristics. Gender, educational background, period of stay, health insurance, medical expense coverage, and whether or not one has friends or regular gatherings were significant traits for frequency, while age, marital status, income, and health insurance were traits for part (p<0.05). Statistical significance (p<0.05) was found in marital status and health insurance for time used to brush teeth, period of stay and whether or not one attends a regular gathering for tooth brushing training, and gender, educational background, period of stay, roommate status, medical expense coverage, and whether or not one attends a regular gathering for supplemental oral product usage. The averages of decayed, missing, and filled teeth (DMFT) index and community periodontal index of treatment needs (CPITN) for general characteristics were 6.98 and 3.03 respectively, and gender, roommate status, income, health insurance for DMFT index, and gender, age, marital status, educational background, roommate status, income, health insurance for CPITN were statistically significant (p<0.05). Tooth brushing frequency and part showed statistical significance (p<0.05) for DMFT index based on tooth brushing behavior, while tooth brushing part and time used to brush teeth were significant for CPITN. DMFT index was high at 8.34 for '3 times or more' in tooth brushing frequency, while DMFT index was high at 7.91 and CPITN was low at 2.94 for tooth brushing part. '3 minutes or longer' in time used to brush teeth was the lowest in CPITN at 2.88. As a result of analyzing the correlation between tooth brushing behavior and DMFT index, CPITN, tooth brushing frequency and part in DMFT index, and tooth brushing part and time used to brush teeth in CPITN showed significance (p<0.05). Based on the above results, we must promote the significance of brushing teeth and implement continuous training for correct tooth brushing management to enhance the oral health of foreign immigrant workers.

  • This study investigated the environmental indices in environmental administration, and to presents how to develop a sustainable environmental evaluation index. The index should be used to evaluate composite plans in the environmental aspects as well as means for setting goals of basic plans for the environment and its management. Specifically, the goals of composite plans are to check environments and land use frame, and evaluate individual measures (public projects). Environment policies are required to be 'a policy creating a clean environment' from 'a policy of environment regulation'. For the organization of a basic framework of environment policies, five topics are needed to be considered, such as the clarification of the idea of environment, establishment of environment regulations, environment management plan, plan to support civilian's action, and local government's efforts for the global matters of environment. For the amenity environments, people should expand their environmental behavior based on their voluntary intentions in transformation of personal life styles, projects with less load on the environment, development of environmentally-friendly projects with consideration for their impact on the earth. This study suggests that they should put more efforts into the development and implementation of useful environmental indices which effectively analyse the regional character to protect the natural ecology and create an sustainable environment with wide coverage.

  • Optimum field size for the whole body stereotactic radiosurgery was studied. Dose distributions from the various sizes of targets (diameter 1cm to 7cm, icm interval) were used for this study. Planing scores, expressed as the Target Coverage Index (TCI), were calculated for various target Margin ranged 0cm to 0.5cm. Highest scores were obtained for no Margin to the target size. The target Margin -0.5cm to 0cm to the target showed best TCI the cases of the target size larger than 6cm diameter. No Margin or 0.5cm Margin generated best TCI for less than 2cm cases. Prescription to 80~90% gives best results.

Analysis of Business Performance of Local SMEs Based on Various Alternative Information and Corporate SCORE Index

  • Purpose: The purpose of this study is to compare and analyze the enterprise's score index calculated from atypical data and corrected data. Research design, data, and methodology: In this study, news articles which are non-financial information but qualitative data were collected from 2,432 SMEs that has been extracted "square proportional stratification" out of 18,910 enterprises with fixed data and compared/analyzed each enterprise's score index through text mining analysis methodology. Result: The analysis showed that qualitative data can be quantitatively evaluated by region, industry and period by collecting news from SMEs, and that there are concerns that it could be an element of alternative credit evaluation. Conclusion: News data cannot be collected even if one of the small businesses is self-employed or small businesses has little or no news coverage. Data normalization or standardization should be considered to overcome the difference in scores due to the amount of reference. Furthermore, since keyword sentiment analysis may have different results depending on the researcher's point of view, it is also necessary to consider deep learning sentiment analysis, which is conducted by sentence.

  • As the technologies advance, mobile peer-to-peer (MP2P) networks or systems become one of the major ways to share resources and information. On such a system, the information retrieval (IR), including the development of scalable infrastructures for indexing, becomes more complicated due to a huge increase on the amount of information and rapid information change. To keep the systems on MP2P networks more reliable and consistent, the index structures need to be updated frequently. For a semantic IR system, the index structure is even more complicated than a classic IR system and generally has higher update cost. The most well-known indexing technique used in semantic IR systems is Latent Semantic Indexing (LSI), of which the index structure is generated by singular value decomposition (SVD). Although LSI performs well, updating the index structure is not easy and time consuming. In an MP2P environment, which is fully distributed and dynamic, the update becomes more challenging. In this work, we consider how to update the sematic index generated by LSI and keep the index consistent in the whole MP2P network. The proposed Concept Space Update (CSU) protocol, based on distributed 2-Phase locking strategy, can effectively achieve the objectives in terms of two measurements: coverage speed and update cost. Using the proposed effective synchronization mechanism with the efficient updates on the SVD, re-computing the whole index on the P2P overlay can be avoided and the consistency can be achieved. Simulated experiments are also performed to validate our analysis on the proposed CSU protocol. The experimental results indicate that CSU is effective on updating the concept space with LSI/SVD index structure in MP2P semantic IR systems.

Analysis of effects of burning in grasslands with quantifying succession stages by life-history traits in Kirigamine, central Japan

  • To quantitatively analyze the effects of burning, we conducted a vegetation survey in the grasslands in Kirigamine, central Japan. We classified each species into stages of succession based on the life-history traits of the species and defined the score of the species in each stand based on the classification. We weighted the scores with a v-value, the product of coverage and height in the quadrat, and summed them to calculate the index of dynamic status. With these indices, we were able to quantitatively compare the stands in the study area and discern minute differences between the stands with different lengths of restoration periods since the disturbance of burning. These indices correlated with the v-value of trees, suggesting that the disturbance of burning seemed to affect the trees in the stand. We then calculated the growth of the tree species Pinus densiflora to evaluate its contribution to the index of dynamic status.

Characteristics of 10-day composite NDVI and LAI in Korea Peninsula Using NOAA AVHRR Data (NOAA AVHRR데이터를 이용한 한반도의 순별 NDVI와 LAI 특성)

  • This study proposes a particular approach to assess information about NDVI(Normalized Difference Vegetation Index) and LAI(Leaf Area Index) from the spectroradiometer and NOAA/AVHRR satellite data. AVHRR data were collected in twelves months over a one year period in 2004. We calculated 10-day composite NDVI using daily composite AVHRR surface reflectance products(1km spatial resolution). The 10-day composite NDVI have a great effect on the plant growth conditions. Considerably, NDVI was increased by developing muscle fiber tissue from April to May. Then the NDVI increased until the August and then decreased until February. The highest month was at August and the lower month was at December. The difference NDVI analysis using December and another months data was conducted, the results were provided information on the variation of vegetation coverage. The result suggest that a relationship established between the LAI and NDVI in 2004.

Influence of different boost techniques on radiation dose to the left anterior descending coronary artery

  • Purpose: The purpose of this study is to compare the dosimetry of electron beam (EB) plans and three-dimensional helical tomotherapy (3DHT) plans for the patients with left-sided breast cancer, who underwent breast conserving surgery. Materials and Methods: We selected total of 15 patients based on the location of tumor, as following subsite: subareolar, upper outer, upper inner, lower lateral, and lower medial quadrants. The clinical target volume (CTV) was defined as the area of architectural distortion surrounded by surgical clip plus 1 cm margin. The conformity index (CI), homogeneity index (HI), quality of coverage (QC) and dose-volume parameters for the CTV, and organ at risk (OAR) were calculated. The following treatment techniques were assessed: single conformal EB plans; 3DHT plans with directional block of left anterior descending artery (LAD); and 3DHT plans with complete block of LAD. Results: 3DHT plans, regardless of type of LAD block, showed significantly better CI, HI, and QC for the CTVs, compared with the EB plans. However, 3DHT plans showed increase in the $V_{1Gy}$ at skin, left lung, and left breast. In terms of LAD, 3DHT plans with complete block of LAD showed extremely low dose, while dose increase in other OARs were observed, when compared with other plans. EB plans showed the worst conformity at upper outer quadrants of tumor bed site. Conclusion: 3DHT plans offer more favorable dose distributions to LAD, as well as improved target coverage in comparison with EB plans.

Variation of optimization techniques for high dose rate brachytherapy in cervical cancer treatment

  • High dose rate (HDR) brachytherapy treatment planning usually involves optimization methods to deliver uniform dose to the target volume and minimize dose to the healthy tissues. Four optimizations were used to evaluate the high-risk clinical target volume (HRCTV) coverage and organ at risk (OAR). Dose-volume histogram (DVH) and dosimetric parameters were analyzed and evaluated. Better coverage was achieved with PGO (mean CI = 0.95), but there were no significant mean CI differences than GrO (p = 0.03322). Mean EQD2 doses to HRCTV (D90) were also superior for PGO with no significant mean EQD2 doses than GrO (p = 0.9410). The mean EQD2 doses to bladder, rectum, and sigmoid were significantly higher for NO plan than PO, GrO, and PGO. PO significantly reduced the mean EQD2 doses to bladder, rectum, and sigmoid but compromising the conformity index to HRCTV. PGO was superior in conformity index (CI) and mean EQD2 doses to HRCTV compared with the GrO plan but not statistically significant. The mean EQD2 doses to the rectum by PGO plan slightly exceeded the limit from ABS recommendation (mean EQD2 dose = 78.08 Gy EQD2). However, PGO can shorten the treatment planning process without compromising the CI and keeping the OARs dose below the tolerance limit.
