The relationship between age and the mandibular cortical bone thickness by using panoramic radiograph (파노라마방사선사진을 이용한 연령별 하악 피질골 두께에 관한 연구)

  • Purpose : This study was to determine the relationship between age and the mandibular cortical bone thickness on panoramic radiograph. Materials and Methods : Panoramic radiographs of 360 patients (180 men and 180 women) over 20 years old, who visited the Chonbuk National University Hospital from January to December in 2007, were assessed. The subjects were divided into 5 age groups. Five indices such as cortical bone thickness at the gonion (GI), antegonion (AI), and below the mental foramen (MI), the panoramic mandibular index (PMI), the mandibular cortical index (MCI) were measured on panoramic radiographs. Results : All five indices including GI, AI, MI, PMI, and MCI showed significant differences between third decade and over 8 decade groups (p<0.05). PMI, MI and GI showed significant differences with gender statistically (p<0.05). Conclusion : The mandibular cortical bone thickness showed negative correlation with age, and the value of the thickness (PMI, MI, and GI) was greater in men than in women.

  • Objectives: There have been few studies to date on the residual effect of bisphosphonate. This study investigated the radiographic changes of mandibular cortical thickness upon bisphosphonate drug holiday. Materials and Methods: This retrospective study includes 36 patients diagnosed with MRONJ (medication-related osteonecrosis of the jaw) at Ajou University Dental Hospital in 2010-2021. All patients stopped taking bisphosphonate under consultation with the prescribing physicians. Panoramic radiographs were taken at the start of discontinuation (T0), 12 months after (T1), and 18 months after (T2) discontinuation of bisphosphonate, respectively. Mental index and panoramic mandibular index were calculated using Ledgerton's method. Paired t-tests were used to analyze differences over time. Results: The difference in indices (mental index and panoramic mandibular index) between T0 and T1 was not statistically significant (paired t-test, P>0.05). However, the difference in these indices between T1 and T2 was statistically significant (paired t-test, P<0.05). Conclusion: The cortical thickness of the mandible decreased in the late stage (after 18 months) as observed by panoramic radiograph.

Comparison of panorama radiomorphometric indices of the mandible in normal and osteoporotic women (정상 및 골다공증 여성의 파노라마 방사선사진 계측치 비교)

  • Purpose : This study was conducted to compare the radiomorphometric indices of the mandible on panoramic radiographs among three groups of normal, osteopenia and osteoporosis and to determine whether panoramic indices have validity in predicting osteoporotic risk. Materials and Methods: Ninty-four postmenupausal women aged 42-72 years (mean 62±7.5) were examined using dual energy X-ray absorptionmetry (DEXA) of the lumbar and classified into three groups by WHO classification. Panoramic mandibular index (PMI), mandibular cortical thickness (Mental index (MI), Antegonial index (AI), Gonial index (G!)) and the mandibular cortical index (MCI) were measured bilaterally on panoramic radiographs and analyzed. Results: There were signigicant differences among normal, osteopenic and osteoporoti groups in PMI (I), PMI (S), MI, AI, GI and MCI. The sensitivities of the MCI (93%) and the combination index of MCI and MI (90%) in diagnosing osteopenic/osteoporotic cases were high. Conclusion: The widths and shapes of the mandibular cortical bone on panoramic radiography may highly reflect the systemic osteopenic/osteoporotic conditions of the patients.

The effect of combined Rehmannia glutinosa Libosch and Eleutherococcus senticosus Max (OPB) extracts on bone mineral density in ovariectomized rats.

  • This study was conducted to investigate the preventing effects of OPB (Rehmannia glutinosa Libosch and Eleutherococcus senticosus Max extracts) and combined OPB/Calcium therapy on bone loss in ovariectomized rats. Sixty Sprague Dawley rats of 12-week-old were divided into eight groups: OVX (ovariectomized), OPBL (OPB 50 mg/kg), OPBM (OPB100 mg/kg), OPBH (OPB 200 mg/kg), OPBL/CAL(OPBL+CAL), OPBM/CAL (OPBM+CAL), OPBH/CAL (OPBH+CAL) and CAL (Calcium citrate 88.33 mg/kg+1$\alpha$, 25-dihydroxy-vitamin $D_3$ 33.33 IU/kg). Bone mineral density (BMD), bone mineral content (BMC), bone strength indices and cortical thickness were analyzed by peripheral quantitative computerized tomography (pQCT). pQCT scanning showed that OVX induced a significant decrease in trabecular bone mineral density and bone mineral content in the proximal tibia $(-36.4\pm2.4%,\;-21.8\pm12.7%)$. These decreases were significantly prevented by the administration of OPBM and OPBM/CAL. Cortical BMD and BMC of tibia were slightly enhanced by OPB and OPB/CAL. However there was no significant difference between OVX and OPB, OPB/CAL treated group. Bone strength indices and cortical thickness were not significantly different. Our results suggest that OPB and combined OPB/Calcium therapy are effective in preventing the development of bone loss induced by ovariectomy in rats.

Effect of Cortical Bone on Acoustic Properties of Trabecular Bone in Bovine Femur In Vitro (생체 외 조건의 소 대퇴골에서 해면질골의 음향특성에 대한 피질골의 효과)

  • The purpose of the present study is to investigate the effect of cortical bone on acoustic properties of trabecular bone, such as speed of sound (SOS) and normalized broadband ultrasound attenuation (nBUA), in bovine femur in vitro. Twelve trabecular bone samples and three cortical bone plates with thicknesses of 1.00, 1.47, and 2.00 mm were extracted from the proximal end of two bovine femurs. The correlations between acoustic properties and trabecular apparent bone density were also examined before and after attaching a cortical bone plate to the trabecular bone samples. SOS increased linearly with increasing thickness of the cortical plate attached to one side of ultrasonic incidence of the trabecular bone samples, whereas nBUA showed a nonlinear dependence on the thickness of the cortical plate. All the SOS (r = 0.95-0.97) and nBUA (r = 0.53-0.73) measurements with and without the cortical bone plate with various thicknesses were found to exhibit high correlations with the trabecular apparent bone density. These results imply that the acoustic properties measured in the femur with lateral cortical layers in vitro can be useful indices for the prediction of trabecular bone mineral density.

Influence of Cortical Endplates on Ultrasonic Properties of Trabecular Bone (피질골판이 해면질골의 초음파 특성에 미치는 영향)

  • The present study investigated the influence of thick cortical endplates on the ultrasonic properties of trabecular bone in a femur with a high fracture risk. Twelve trabecular bone samples were prepared from bovine femurs, and acrylic plates with thicknesses of 1.25, 1.80, and 2.75 mm were manufactured to simulate the cortical endplates using acrylic with a density and a sound speed similar to cortical bone. Although the thickness of the acrylic plates attached to the two sides of the trabecular bone increased, high correlations were observed between the speed of sound and the apparent bone density of the trabecular bone, with Pearson's correlation coefficients of 0.80-0.86. High correlations were also observed between the attenuation coefficient at 0.5 MHz and the apparent bone density of the trabecular bone, with Pearson's correlation coefficients of 0.84-0.91. These results suggest that the speed of sound and attenuation coefficient at a specific frequency measured in a femur with relatively thick cortical endplates compared to the calcaneus could be used as indices for predicting the bone mineral density of the femur.

The combined use of computer-guided, minimally invasive, flapless corticotomy and clear aligners as a novel approach to moderate crowding: A case report

  • The aim of this case report was to describe an innovative orthodontic treatment method that combined surgical and orthodontic techniques. The novel method was used to achieve a positive result in a case of moderate crowding by employing a computer-guided piezocision procedure followed by the use of clear aligners. A 23-year-old woman had a malocclusion with moderate crowding. Her periodontal indices, oral health-related quality of life (OHRQoL), and treatment time were evaluated. The treatment included interproximal corticotomy cuts extending through the entire thickness of the cortical layer, without a full-thickness flap reflection. This was achieved with a three-dimensionally printed surgical guide using computer-aided design and computer-aided manufacturing. Orthodontic force was applied to the teeth immediately after surgery by using clear appliances for better control of tooth movement. The total treatment time was 8 months. The periodontal indices improved after crowding correction, but the oral health impact profile showed a slight deterioration of OHRQoL during the 3 days following surgery. At the 2-year retention follow-up, the stability of treatment was excellent. The reduction in surgical time and patient discomfort, increased periodontal safety and patient acceptability, and accurate control of orthodontic movement without the risk of losing anchorage may encourage the use of this combined technique in appropriate cases.

Diagnostic Coincidence of Radiologic Measurement Methods of the Proximal Femur Anterior-posterior Projection for Assessment of Osteoporosis (근위대퇴부 정면 영상을 이용한 골다공증 평가 시 방사선적 계측의 진단일치도)

  • The purpose of this study was to determine whether there are relationships between radiologic parameters for osteoporosis by the proximal femur radiograhps and bone mineral density, as assessed by Dual Energy X-ray Absorptiometry(DEXA). Proximal femur anterior-posterior projection were taken from 112 individuals who undergoing DEXA. Radiographic parameters including canal-to-calcar ratio(CCR) and cortical thickness indices(CTI) were measured and compared with bone mineral density($g/cm^2$), T-score. The intramedullary femoral canal width(FW) and calcar width(CW) at osteoporosis group was significantly larger than that of control group. The CCR were correlated negatively with bone mineral density($g/cm^2$)(r=-0.340, p<0.01), and the CTI were correlated positively(r=0.624). The diagnostic accuracy of CCR for osteoporosis was 63.4%, and kappa value was 0.271. And the accuracy of CTI was 67.0%, and kappa value was the level of fair agreement(${\kappa}$=0.258). The proximal femur with large CCR and small CTI had lower T-scores. When evaluating proximal femur anterior-posterior radiographs, those patients with the thinnest femur diaphyseal cortices should be referred for further osteoporosis investigation.
