중환자 가족원의 부담감, 스트레스 및 사회적 지지 (A Study on Burden, Stress and Social Support of Family Caregivers in Intensive Care Unit Patient)

  • This study was to identify the correlation between the degree of burden, stress and social support of family caregivers in ICU patient. The subjects were 146 family care givers observed at a general hospital at D city. The level of stress that family caregivers experience the average points of 2.18, which is considered less than average. The level of burden that average point of 2.51. Points for level of subjective feeling ranged from 1.74 to 3.90. The average point of 3.03 is higher than that for objectively recognized feeling. The points for social support that average point of 3.03 for social support proves that families feel positive about the social support they are receiving. The level of stress which shows that the lower the income, the higher the stress. The effect on stress shows significance in subjective feelings of burden, social support, and employment, indicating that they have correlations with stress. Both the feeling of burden and social support have an impact upon the stress that patient families experience.

치매환자 가족부담감의 한국형 도구개발 (Study on Family Caregiving Burden Scale of Dementia-Korea(FCBSD-K))

  • The purpose of this study was to develop and validate the scale to measure dementia patient's caregiver burden of Korea. In the first phase of the study, 15 caregivers of dementia patients were interviewed to provide narrative data from which items were developed. Initially 65 items were generated from the interview data of 15 caregivers. Content validity was judged by two separate panels of experts with 27 professionals and 30 family caregivers. These items were analyzed through the Index of Content Validity and 33 items were selected which met .80 or more of the CVI. This preliminary FCBSD-K was tested with 207 adult caregivers for reliability and construct validity including item analysis and orthogonal(Varimax) factor analysis. Eight items were deleted because of high or low item-item correlation. The result of the second factor analysis produced six factors that coincided with the conceptual framework posed for the scale developed. The six factors were labeled as 'physio social factor' 'emotional factor' 'family cultural factor' 'role obligation' 'guilt feeling' and 'financial & supportive system factor'. The alpha coefficient relating to internal consistency was .9264 for reliability. In conclusion, cultural factor is related to dementia patient's caregiver burden and FCBSD-K was useful in assessing the dementia patient's caregiver burden in Korea.

주간호제공자 특성에 따른 뇌졸중 환자의 지각된 질병 영향 (Perceived Illness Intrusiveness by the Characteristics of Primary Caregivers in Stroke Patients)

  • Purpose: This study was implemented to investigate perceived illness intrusiveness in stroke patients by the characteristics of primary caregivers. Method: Retrospective cross-sectional survey design was used. The subjects were 225 stroke patients and their primary caregivers. Demographic variables, relationship variables, burden, depression and support were measured as primary caregivers' characteristics. Data were analyzed with SPSS win 14+. T-test, ANOVA and correlation test were used depending on variables. Result: Demographic variables such as age, gender, relation with patients, and living status were not related significantly with illness intrusiveness. But primary caregivers' burden, depression and support were related significantly with perceived illness intrusiveness of stroke patients. That is, burden, depression and quantitative support of primary caregivers were significantly positively correlated with perceived illness intrusiveness of stroke patients. On the other hand, qualitative support was negatively correlated with perceived illness intrusiveness. Conclusion: Primary caregivers' characteristics such as burden, depression and support were found to be correlated with stroke patients perception such as illness intrusiveness. So, it is recommended that nursing intervention targeting burden, depression and support of primary caregivers must be developed to reduce illness intrusiveness of stroke patients.

Developing a Basic Scale for Workers' Psychological Burden from the Perspective of Occupational Safety and Health

  • Background: Organizations are pursing complex and diverse aims to generate higher profits. Many workers experience high work intensity such as workload and work pressure in this organizational environment. Especially, psychological burden is a commonly used term in workplace of Republic of Korea. This study focused on defining the psychological burden from the perspective of occupational safety and health and tried to develop a scale for psychological burden. Methods: The 48 preliminary questionnaire items for psychological burden were prepared by a focus group interview with 16 workers through the Copenhagen Psychosocial Questionnaire II and Mindful Awareness Attention Scale. The preliminary items were surveyed with 572 workers, and exploratory factor analysis, confirmatory factor analysis, and correlation analysis were conducted for a new scale. Results: As a result of the exploratory factor analysis, five factors were extracted: organizational activity, human error, safety and health workload, work attitude, and negative self-management. These factors had significant correlations and reliability, and the stability of the model for validity was confirmed using confirmatory factor analysis. Conclusion: The developed scale for psychological burden can measure workers' psychological burden in relation to safety and health. Despite some limitations, this study has applicability in the workplace, given the relatively small-sized questionnaire.

손자녀를 돌보는 조부모의 부담감과 사회적 지지 (The relationship between the burden and the social support of grandmothers caring their grandchildren)

  • The purposes of this research were to add to the developing knowledge base about the burden and social support of grandmothers involved with caring grandchildren, and to examine whether a significant relationship exists between their burden and social support perceived by the grandmothers. A convenience sample of 102 grandmothers was recruited from five collective apartment areas at a small city in Korea. The criteria of selection of sample were that the grandmothers were raising their grandchildren under the age of 36 months for 3 months or longer at own or gandchildren's home. The instruments used were a 15-item multidemensional burden scale and a 8-item social support scale. Both scales were self report, five point Likert type scales. The higher the score, the higher the degree of burden and social support. Data was collected by two prepared research assistants visiting subjects' home from December 10, 1998 to March 20, 1999. The collected data were analysed using mean, t-test, one-way ANOVA, and Pearson's correlation coefficient computed by SPSS software. The results were as follows. 1. In the age distribution of grandmothers, the over half of subjects(58.8%) were under 60 years old. The majority(69.6%) of subjects were married at time of data collection. 79(77.5%) of the sample reported that their perceived health status was good or over. 2. The burden of the grandmothers was not scored high, and the item means on burden scale were ranged from 2.26 to 4.19 out of 5. 'Short of private time'(4.19) had the highest score, followed by 'fatigue'(3.92), 'short of rest'(3.75), and 'short of contact with friends and neighbors'(3.62). The lowest item was 'family doesn't help me'(2.26), and followed by 'family doesn't understand me'(2.33), 'angry with family' (2.43), and 'angry while caring for grandchild'(2.60). 3. There were significant relationships between the burden and present health status(p<.01), childcaring confidence(p<.01), and motive of caring(p<.01). 4. The score of social support, was ranging from 3.61 to 4.01 out of 5. 5. The relationship between burden and social support was found to be correlated negatively. The relationship was statistically significant(γ= - .2833, p<.001). In conclusion, it was found that the burden was not high and burden of grandmothers caring grandchildren was correlated negatively to social support. Therefore, these results provide a basis for developing a nursing intervention to reduce the burden of grandmothers.

정신과 환자 주 보호자의 삶의 질과 부담 영향 요인 - 정신분열병과 알콜 의존 환자의 보호자 비교 - (Study on Influencing Factors on Subjective Quality of Life and Family Burden of Caretaking Family Members who have a Patient with a Psychiatric Disorder - Comparison between Schizophrenia and Alcohol Dependence -)

  • 연구목적: 만성 정신과 환자 보호자의 부담과 삶의 질을 알아본 후, 상관관계에 대하여 조사하여, 향후 가족 치료를 포함하는 치료적 중재를 위한 기초 자료로 사용하고자 함이다. 방 법: 정신분열병으로 진단을 받은 환자 보호자 78명과 알콜 의존으로 진단을 받은 환자 보호자 54명을 포함하여 총 132명을 대상으로 하여 가족 부담 척도와 스미스클라인 비챰 '삶의 질'척도를 이용하여 조사하였다. 결 과: 가족이 느끼는 부담과 보호자의 성별, 연령, 월 평균 소득과 병전 함께 산기간은 유의한 상관관계를 나타내지 않았다. 삶의 질에서는 남성이 더 삶의 질이 높았으며, 신체적 안녕 요인과 활력 요인에서 연령과 유의한 상관관계를 나타내었다. 스트레스 반응 요인, 부담과 정신과 영역의 장점 요인이 정신과 환자 주 보호자의 삶의 질 전체 변이 중에 약 41%를 설명하였으며, 이 중 긴장 요인이 가장 큰 설명력을 보였다. 결 론: 만성 질환은 간병하는 보호자에게 부담을 주며, 이러한 부담이 보호자의 삶의 질을 악화시키는 요인으로 작용할 수 있다. 특히, 환자의 유병기간이 증가할수록 유능감에 부정적 영향을 줄 수 있다. 그러므로, 보호자가 경험하는 부담과 스트레스를 줄여주어 보호자의 삶의 질을 높일 수 있도록 하는 가족 교육을 포함한 가족 치료적 중재가 필요하리라 사료된다.

  • An stereophonic acoustic echo canceller cannot exactly estimate the echo path in the receiving room, because of the cross-correlation between stereo signals. In this paper, the new preprocessing filter is proposed to reduce the cross-correlation between the signals without influence on stereophonic sound. Two channel signals are linearly decorrelated by using orthogonality principles and the attenuated absolute values of the decorrelated signals are added to each channel input signals. Assuming that the power of each channel signal is larger than that of the cross-correlation, computational burden is reduced.

치매노인을 돌보는 가족의 부담감과 우울반응에 관한 연구 (A Study on the Burdens and Depressive Reactions on Families who Cared for Patients Suffering from Senile Dementia)

  • The purpose of this study was to investigate the relationship between the burden on families who live with an elderly person suffering from senile dementia, and the degree of their depression. There were 400 participants in this study, staying in the Seoul and Kyonggi areas from August 1, 1997 to February 28, 1998. Among the group, 100 participants took care of their patient at home, and another 300 participants left 100 patient at a day-care center, 100 sanatorium for senile dementia(asylum for helpless elderly people), 100 an infirmary for elderly people. Eventually 242 subjects out of the 400 were selected for the data analysis. The Zarit (1980) tool was employed to measure the degree of burden and Zung's(1965) “Self-Rating Depression Scale” was employed for the data analysis. The data was analyzed, and the percentage, t-test, ANOVA and Person's Correlation Coefficient were calculated. The results are as follows : 1. The average degree of burden that care-giving families felt was 49.13, which is somewhat high. 2. The average degree of depression that care -giving families felt was 51.95, which is relatively high. 3. The degree of burden was directly affected by the relation with the patient(F=2.48, P<.05), and the socio-economic status of the family(F=5.17, P<.05). Its also affected by the patient's educational status(F=2.17, P<.05). 4. The degree of depression of the family was significantly dependent on sex(t=-2.05, P<.05), age (F=2.99, P<.05), the relationship with the patient(F=3.65, P<.01), socio-economic status (F=7.74, P<.001), occupation(t=2.82, P<.01), health status(F=4.42, P<.01), and the place of residence(F=4.30, P<.01), The patient characteristics was significantly dependent on his/her educational status(F=3.85, P<.01), the period of suffering from senile dementia(F=2.47, P<.05), and smoking habit(F=6.17, P<.001). 5. The relationship between the degree of burden and that of depression reads r=0.43, which is statistically positive correlation in the high significant level. Upon analyzing the entire summation, most care-givers for elderly patients suffering from senile dementia lack time in caring for themselves. They also experience chronic fatigue and mental discomfort caused by the isolation from society, curtailment of certain activities, a sense of responsibility for their patients, and limits of their endurance in taking care of their patients over time. In conclusion, this study emphasizes the necessity for the following propositions : 1. In order to measure the degree of burden that Korean care-giving families undergo, a new tool must be developed on the basis of Korean culture. 2. An educational program based on the demands that care-giving families undergo must be developed, and its clinical effect also has to be examined.

  • This research was conducted to identify the following: the home care needs of patients with dementia and the burden on the primary family care giver: to provide basic data required to develop nursing intervention for the care giver: and to suggest recommendations for medical institutions and social services that could reduce the burden on the families of people suffering from dementia. subjects of this research were 53 patients of the two Public Health Centers of Cheju Province who are suffering from dementia and their families. The instrument used in the research was Kuen. Jung Don (1994)' s assessment tool of burden in the primary family care giver who has parents with senile dementia and Yoo. Young Mi(1998)'s assessment tool of home care need. modified by the researcher in the questionnaire by a Likert rating scale. The period of data collection was from February 8. 2000 to March 10. 2000. Collected data was analyzed by SPSS, using mean, standard deviation. ANOVA, t-test and Pearson correlation coefficient. The result of this research was that there was not a significant correlation between the burden on the care giver and the level of dementia, its duration, the patient's ability to perform daily tasks, the period of care giving. and the use of social services, although the lower the patient's ability to perform daily tasks. and the worse the care giver's own health situation, the higher the burden on the primary family care giver. The following suggestions are made based on the results of this research. 1. More than half of the subjects don't use social facilities and services. More publicity and referral efforts are needed about medical institutions. nursing institutions and other facilities that specialize in services for dementia sufferers and their families. 2. Nursing services should include intensive education for the primary care giver in the most important aspects of home care. 3. Further research should be done, and should include data from all parts of Cheju Province.

뇌졸중환자 보호자의 삶의 질과 관련 요인에 관한 연구 (A Study of the Quality of Life and Related Factors of Cerebral Infarction Patients' Guardians)

  • This study is about the quality of life of cerebral infarction patients' guardians. The subject of this study was a group of 159 guardians of patients diagnosed of cerebral infarction and being hospitalised in a general hospital located in Seoul. The measurement tool consisted of quality of life of cerebral infarction patients' guardian, self-esteem, burden of responsibility, family unity, health status of the guardian. The collected data have been processed using SPSS/PC 12.0 programme: The results of this study are as follows: 1. The point for quality of life of cerebral infarction patients' guardians was 3.08(five-point scale), and for the related factors, the points for self-esteem was 3.67, burden of responsibility was 3.26, family unity was 4.13, health status was 6.10(ten-point scale). 2. There was a significant difference in the quality of life according to Self-esteem, Burden of responsibility, Family unity and general characteristics. 3. In the quality of life of cerebral infarction patients' guardians, a positive correlation was noted in some factors according to the characteristics of the guardians such as health status, self-esteem, family unity, monthly income, and time spent for caring the patient. Whilst a negative correlation was noted in the others such as burden of responsibility and age of the guardian. By confirming the results above, it is concluded that we must understand the major factors which are related to quality of life, find the causes that deteriorate the quality of life, and apply proper nursing intervention to improve the quality of life of cerebral infarction patients' guardians.
