Being a front-line dentist during the Covid-19 pandemic: a literature review

  • Coronavirus is an enveloped virus with positive-sense single-stranded RNA. Coronavirus infection in humans mainly affects the upper respiratory tract and to a lesser extent the gastrointestinal tract. Clinical symptoms of coronavirus infections can range from relatively mild (similar to the common cold) to severe (bronchitis, pneumonia, and renal involvement). The disease caused by the 2019 novel coronavirus (2019-nCoV) was called Covid-19 by the World Health Organization in February 2020. Face-to-face communication and consistent exposure to body fluids such as blood and saliva predispose dental care workers at serious risk for 2019-nCoV infection. As demonstrated by the recent coronavirus outbreak, information is not enough. During dental practice, blood and saliva can be scattered. Accordingly, dental practice can be a potential risk for dental staff, and there is a high risk of cross-infection. This article addresses all information collected to date on the virus, in accordance with the guidelines of international health care institutions, and provides a comprehensive protocol for managing possible exposure to patients or those suspected of having coronavirus.

Coronavirus Disease 2019 폐렴의 임상적, 영상의학적 소견: 대구의 단일 기관에서 51명의 성인 환자를 대상으로 한 분석 (Clinical and Radiological Findings of Coronavirus Disease 2019 Pneumonia: 51 Adult Patients from a Single Center in Daegu, South Korea)

  • 목적 본 연구는 Coronavirus disease 2019 (이하 COVID-19) 폐렴 환자의 임상 양상과 흉부 전산화단층촬영(이하 CT) 소견을 분석하고자 하였다. 대상과 방법 IRB의 승인을 받은 연구로, 51명의 COVID-19 확진 환자들을 후향적으로 분석하였다. 환자들을 임상 양상에 따라 경증과 중증으로 나누어 두 그룹 간에 임상 양상과 흉부 CT 소견을 비교하였다. 결과 총 51명의 환자(남자 22명, 여자 29명, 평균 56.5 ± 16세, 범위 22~88세) 중 37명(72.5%)은 경증, 14명(27.5%)은 중증이었다. 중증 환자들의 평균 연령(68.7 ± 12.5세)은 경증 환자들(51.8 ± 14.9세)보다 많았다(p < 0.001). 중증 환자가 기저질환을 가지고 있는 경우가 많았으며(71% vs. 41%, p = 0.049), 혈액검사에서 림프구 백분율 감소를 보이는 경우가 많았다(86% vs. 32%, p = 0.001). 흉부 CT 소견은 대부분의 환자들에서 간유리음영과 폐경화가 혼합된 양상이거나(76%) 간유리음영으로(22%) 나타났고, 양측 폐 하부, 후방, 변연부에 나타나는 경우가 많았다. 중증 환자에서 병변이 더 많은 수의 폐엽을 침범했고, CT 위중도 점수도 높았다. 결론 COVID-19 폐렴 확진 환자의 특징적인 흉부 CT 소견을 숙지하는 것이 빠른 진단과 적절한 치료에 도움이 될 것이다.

Single-Cell Genomics for Investigating Pathogenesis of Inflammatory Diseases

  • Recent technical advances have enabled unbiased transcriptomic and epigenetic analysis of each cell, known as "single-cell analysis". Single-cell analysis has a variety of technical approaches to investigate the state of each cell, including mRNA levels (transcriptome), the immune repertoire (immune repertoire analysis), cell surface proteins (surface proteome analysis), chromatin accessibility (epigenome), and accordance with genome variants (eQTLs; expression quantitative trait loci). As an effective tool for investigating robust immune responses in coronavirus disease 2019 (COVID-19), many researchers performed single-cell analysis to capture the diverse, unbiased immune cell activation and differentiation. Despite challenges elucidating the complicated immune microenvironments of chronic inflammatory diseases using existing experimental methods, it is now possible to capture the simultaneous immune features of different cell types across inflamed tissues using various single-cell tools. In this review, we introduce patient-based and experimental mouse model research utilizing single-cell analyses in the field of chronic inflammatory diseases, as well as multi-organ atlas targeting immune cells.

  • 목적: 국내 초중고 학교 등교재개 이후 소아에서의 코로나바이러스감염증-2019 (코로나19) 사례의 감염경로를 파악하고자 하였다. 방법: 2020년 5월 1일부터 7월 12일까지 국가감염병감시체계에 신고된 3-18세 소아 청소년 코로나19 확진자의 사례조사서 및 역학조사서를 분석하였다. 결과: 2020년 5월 국내 초중고 학교 등교 재개 이후 7월 12일까지 총 127명의 소아 청소년 코로나19 확진자가 신고되었다. 그 중 59명(46%)은 가족 및 친지로부터 전파된 사례였으며 18명(14%)은 학원 및 개인교습 중 전파되었다. 8명(6%)은 다중이용 시설에서 전파되었으며 3명(2%)은 학교에서 전파된 사례였다. 결론: 코로나19 감염예방을 위한 관리체계가 사전에 마련되고 준비된 경우 학교 내 코로나19 전파는 드물게 나타났다.

Single Cell Transcriptomic Re-analysis of Immune Cells in Bronchoalveolar Lavage Fluids Reveals the Correlation of B Cell Characteristics and Disease Severity of Patients with SARS-CoV-2 Infection

  • The coronavirus disease 2019 (COVID-19) pandemic (severe acute respiratory syndrome coronavirus 2) is a global infectious disease with rapid spread. Some patients have severe symptoms and clinical signs caused by an excessive inflammatory response, which increases the risk of mortality. In this study, we reanalyzed scRNA-seq data of cells from bronchoalveolar lavage fluids of patients with COVID-19 with mild and severe symptoms, focusing on Ab-producing cells. In patients with severe disease, B cells seemed to be more activated and expressed more immunoglobulin genes compared with cells from patients with mild disease, and macrophages expressed higher levels of the TNF superfamily member B-cell activating factor but not of APRIL (a proliferation-inducing ligand). In addition, macrophages from patients with severe disease had increased pro-inflammatory features and pathways associated with Fc receptor-mediated signaling, compared with patients with mild disease. CCR2-positive plasma cells accumulated in patients with severe disease, probably because of increased CCL2 expression on macrophages from patients with severe disease. Together, these results support the hypothesis that different characteristics of B cells might be associated with the severity of COVID-19 infection.

Structural Factors of the Middle East Respiratory Syndrome Coronavirus Outbreak as a Public Health Crisis in Korea and Future Response Strategies

  • The recent Middle East respiratory syndrome coronavirus (MERS-CoV) outbreak has originated from a failure in the national quarantine system in the Republic of Korea as most basic role of protecting the safety and lives of its citizens. Furthermore, a number of the Korean healthcare system's weaknesses seem to have been completely exposed. The MERS-CoV outbreak can be considered a typical public health crisis in that the public was not only greatly terrorized by the actual fear of the disease, but also experienced a great impact to their daily lives, all in a short period of time. Preparedness for and an appropriate response to a public health crisis require comprehensive systematic public healthcare measures to address risks comprehensively with an all-hazards approach. Consequently, discussion regarding establishment of post-MERS-CoV improvement measures must focus on the total reform of the national quarantine system and strengthening of the public health infrastructure. In addition, the Korea Centers for Disease Control and Prevention must implement specific strategies of action including taking on the role of "control tower" in a public health emergency, training of Field Epidemic Intelligence Service officers, establishment of collaborative governance between central and local governments for infection prevention and control, strengthening the roles and capabilities of community-based public hospitals, and development of nationwide crisis communication methods.

Small Molecule Drug Candidates for Managing the Clinical Symptoms of COVID-19: a Narrative Review

  • Towards the end of 2019, an atypical acute respiratory disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) was identified in Wuhan, China and subsequently named Coronavirus disease 2019 (COVID-19). The rapid dissemination of COVID-19 has provoked a global crisis in public health. COVID-19 has been reported to cause sepsis, severe infections in the respiratory tract, multiple organ failure, and pulmonary fibrosis, all of which might induce mortality. Although several vaccines for COVID-19 are currently being administered worldwide, the COVID-19 pandemic is not yet effectively under control. Therefore, novel therapeutic agents to eradicate the cause of the disease and/or manage the clinical symptoms of COVID-19 should be developed to effectively regulate the current pandemic. In this review, we discuss the possibility of managing the clinical symptoms of COVID-19 using natural products derived from medicinal plants used for controlling pulmonary inflammatory diseases in folk medicine. Diverse natural products have been reported to exert potential antiviral effects in vitro by affecting viral replication, entry into host cells, assembly in host cells, and release. However, the in vivo antiviral effects and clinical antiviral efficacies of these natural products against SARS-CoV-2 have not been successfully proven to date. Thus, these properties need to be elucidated through further investigations, including randomized clinical trials, in order to develop optimal and ideal therapeutic candidates for COVID-19.

  • 우리 나라는 2022년 3월에 5-11세 소아의 코로나19 예방접종을 시작하였다. 5-11세 소아에서 중증 코로나19가 발생할 위험은 높지 않기 때문에, 이 연령에 대한 코로나19 예방접종 정책은 각 국가의 상황에 따라 상이하다. 본 논문에서는 해외 주요 국가의 5-11세 소아 코로나19 예방접종 관련 정책을 살펴보고, 5-11세 코로나19 예방접종을 시작하는 우리 나라에서 고려할 사항에 대하여 언급하였다.

Post-Coronavirus Disease 2019 Pulmonary Fibrosis: Wait or Needs Intervention

  • Coronavirus disease 2019 (COVID-19) has become a major health burden worldwide, with over 450 million confirmed cases and 6 million deaths. Although the acute phase of COVID-19 management has been established, there is still a long way to go to evaluate the long-term clinical course or manage complications due to the relatively short outbreak of the virus. Pulmonary fibrosis is one of the most common respiratory complications associated with COVID-19. Scarring throughout the lungs after viral or bacterial pulmonary infection have been commonly observed, but the prevalence of post-COVID-19 pulmonary fibrosis is rapidly increasing. However, there is limited information available about post-COVID-19 pulmonary fibrosis, and there is also a lack of consensus on what condition should be defined as post-COVID-19 pulmonary fibrosis. During a relatively short follow-up period of approximately 1 year, lesions considered related to pulmonary fibrosis often showed gradual improvement; therefore, it is questionable at what time point fibrosis should be evaluated. In this review, we investigated the epidemiology, risk factors, pathogenesis, and management of post-COVID-19 pulmonary fibrosis.

Herbal Medicine for Coronavirus Disease 2019 (COVID-19) in Children: a Literature Review

  • Objectives Few studies to date reviewed herbal medicines prescribed to pediatric patients with coronavirus disease 2019 (COVID-19). We aimed to summarize and analyze these herbal medicines. Methods Nine databases were searched for studies published up until November 24, 2020. Nine case reports and one case series involving 17 pediatric patients were included in the study. The clinical symptoms, interventions, outcomes, pattern identification, composition, and frequency of herbal medicine administration were analyzed. Results The symptoms of pediatric patients with COVID-19 treated with herbal medicines were mild or moderate. Herbal medicines have been used to alleviate symptoms, such as fever and cough. No adverse events were reported. All of the patients who received herbal medicines were discharged from the hospital after symptomatic relief. Pattern identification revealed that the lungs were the most affected organs. Epidemic toxins, wind, dampness, and heat were the primary etiological factors. More pediatric patients with COVID-19 were prescribed a combination of herbal and Western medicines than herbal medicines alone. The frequently used herbs include Glycyrrhizae Radix et Rhizoma, Lonicerae Flos, Scutellariae Radix, Armeniacae Semen, Citri Reticulatae Pericarpium, and Astragali Radix. Conclusions Our review can serve as a reference for the treatment of COVID-19 in children.
