Cost Structure of Medical Services in Korean National Health Insurance (건강보험 의료행위의 비용구조)

  • Health insurance fees are set by relative value scales and conversion factors. Since 2008 the conversion factor has been classified into 7 according to the provider type, and a separate contract has been made respectively. As such classification of the conversion factor reflects only the different characteristics of providers, however, further classification to reflect the different cost structures of providers is proposed. Cost varies according to the type of not only providers but also services each provider supply. In fact different cost structures of providers are the result of their different services. This study analyzed the cost structure of medical services to propose a new approach to the classification of the conversion factor. This study analyzed the cost structure of medical services using cost data constructed in the revision study of relative value scales. The cost data consist of doctor's fee, support staff's fee, cost of medical equipments, cost of medical supplies and indirect cost. The proportion of each cost component to the total cost was analyzed in terms of service department and service type. 72 service groups are defined in terms of the combination of service department and service type. Through cluster analysis, 72 service groups were reduced into 7 clusters each of which has a similar cost structure. Conversion factor is contracted annually to reflect the change in the cost of providing medical services. So the classification of conversion factor has to be based on the cost structures of medical services, not the characteristics of providers. Service clusters derived in this study can be used as a new classification for health insurance fee contract.

  • Color conversion technology using unique color conversion film for OLED back light was developed to achieve renovative performance for high- end display. It can reduce the production cost more than 20% due to cheaper cost for blue OLED and conversion film and also has a free chromaticity control capability for 10% raising color gamut with respect to LCD color filter. The OLED BLU by color conversion technology also shows excellent performances such as chromaticity stability. White efficiency using PLF + Blue OLED is 17cd/A@4.4V, CIExy(0.29, 0.31).

Life Cycle Cost Comparative Analysis for Main Conversion System in 8200 Series Electric Locomotive (8200호대 전기기관차 주변환장치 LCC 비교 분석)

  • Parts and units of 8200 series electric locomotive are aging, but and source technology is not secured. As a results, maintenance costs are increasing steadily due to using expensive substitute parts. Therefore, various studies have been conducted to reduce maintenance costs. In this paper, the life cycle cost(LCC) of the developed and conventional products were compared and analyzed about main conversion system in 8200 Series electric locomotive. As a result of analysis, the material cost was the highest in the conventional product among the various item costs. On the other hand, it is confirmed that preventive cost was the highest among the costs about the developed product.

  • PURPOSES : This paper presents a description of the current issues facing road managers regarding the surface-type conversion of low-volume roads for cost savings. METHODS: The paper reviews previous works conducted toward this end, acknowledges gaps in the current research, and lays out what information is needed for further studies. RESULTS : If the cost to maintain an unsurfaced road is less than the cost of maintaining a surfaced road, then there is potential for cost savings for the management agency. However, the problem is bigger than simply maintaining the roads that already exist. If unsurfaced roads prove to be more economical than surfaced roads, then the cost to convert from a surfaced to an unsurfaced roadway, and vice versa, when necessary, must also be examined. CONCLUSIONS : No other studies have addressed the un-surfacing of a road for cost savings, and it is therefore unknown whether substantial savings can be realistically obtained by converting from a surfaced to an unsurfaced road. To determine whether a conversion policy would be a viable option, additional data and research are needed.

  • 8200 Series Electric Locomotives are mostly imported from overseas due to aging and uncertainty of source technology, and it's the maintenance cost is increasing. We are analyzed life cycle costs based on international standards such as IEC 60300-3-3 and IEC62278. The main conversion system of the 8200 series electric locomotive is closely related to vehicle operation and is one of the subsystems requiring frequent maintenance. In this paper, the life cycle cost of the main conversion unit in 8200 series electric locomotive is analyzed based on the maintenance manual. As a result of analysis, maintenance cost of GTO module and control device is relatively high, and it is confirmed that the cost increases according to the useful life.

  • This study aims to derive the conversion factor of oriental medicine based on the publicly available information to overcome the problem of the small sample size and reliability of cost analysis which spend much data gathering expense in spite of small sample. This study confirms the fairness and validity of results of analysis by using the publicly available information composed of large sample. Because of the large variation of the conversion factor depending on the types of data and methodologies, it is required to use the analysis using publicly available information in company with the cost analysis. build the health data system for cost analysis. For more reasonable conversion factor, it is urgent to construct the healthcare data system including the individual oriental medicine office.

  • The online advertising industry's business model undertakes the change from CPM (cost-per-mille)-based to CPC (cost-per-click)-based. However, due to the problem of 'Click Fraud', CPA (cost-per-action) has been regarded as a new step. For CPA, publishers need to get information after a user clicks an advertisement. Therefore, in CPA, the key is to get Conversion Action Data (CAD). This paper introduces two existing mechanisms for getting CAD, compare their characteristics, and analyze their limitations. Then the two new mechanisms are introduced and their requirements and feasibility are analyzed. Lastly, we compare the existing two and the new two mechanisms, and point out each mechanism's business possibility, value and Application Area. This paper will help publishers choose the most appropriate mechanism on the basis of their situation.

  • The rapidly growing demand for electric power systems in electric vehicles (EVs) and hybrid electric vehicles (HEVs) require simpler, cost-effective, and higher performance components. In this paper, a novel power conversion system for hybrid electric vehicles is proposed for these needs. The proposed power conversion system reduces the conversion system cost while preserving same functionality. The proposed power conversion system can boost multi-sources to drive a traction motor and to store energy at the same time reducing number of switching components. In this paper, all operational modes of the proposed converter are explained in detail and verified by a computer simulation first. Then, the topology and operational modes are experimentally verified. Based on the results, the feasibility of the proposed multi-mode single leg power conversion system for EV and HEV applications is discussed.

Methods and Applications to estimate the Conversion Factor of Resource-based Relative Value Scale for Nurse-Midwife's Delivery Service in the National Health Insurance (조산원(助産院)의 분만간호서비스에 대한 건강보험수가 산출방법과 적용방안)

  • Purpose: This paper analyzed alternative methods of calculating the conversion factor for nurse-midwife's delivery services in the national health insurance and estimated the optimal reimbursement level for the services. Methods: A cost accounting model and Sustainable Growth Rate (SGR) model were developed to estimate the conversion factor of Resource-Based Relative Value Scale (RBRVS) for nurse-midwife's services, depending on the scope of revenue considered in financial analysis. The data and sources from the government and the financial statements from nurse-midwife clinics were used in analysis. Results: The cost accounting model and SGR model showed a 17.6-37.9% increase and 19.0-23.6% increase, respectively, in nurse-midwife fee for delivery services in the national health insurance. The SGR model measured an overall trend of medical expenditures rather than an individual financial status of nurse-midwife clinics, and the cost analysis properly estimated the level of reimbursement for nurse-midwife's services. Conclusion: Normal vaginal delivery in nurse-midwife clinics is considered cost-effective in terms of insurance financing. Upon a declining share of health expenditures on midwife clinics, designing a reimbursement strategy for midwife's services could be an opportunity as well as a challenge when it comes to efficient resource allocation.
