INTRAVENOUS SEDATION OF CEREBRAL PALSY PATIENT FOR DENTAL IMPLANT CT TAKING -A CASE REPORT (정신지체가 동반된 뇌성마비 환자의 임플란트 치료를 위한 CT 촬영 시 진정법 시행 -증례보고-)

  • A 33-years-old female pateint with cerebral palsy showing spastic quardriplegia and severe mental retardation was scheduled for dental implant restorations. Before implant surgery we had to take implant CT. But, because of her involuntary motion and communication difficulty, sedation was needed in order to take CT. After 8 hour NPO, propofol infusion sedation with TCI (target controlled infusion) system was administered. The propofol blood concentration of the patient was maintained 2-3 ${\mu}$/ml to keep deep sedation to prevent uncontolled movement. During sedation, we monitored ECG, pulse oximetry, blood pressure, capnometry for patient safety. Oxygen was administered via nasal prong for preventing hypoxemia and to keep airway during sedation some bands were applied to lift mandible. Total duration was 20 minutes for taking CT, and she was discharged from hospital after 30 minute rest without complication.

Antihypertensive effect of Meridian Tai Ji exercise treatment in hypertension patients (고혈압 환자에서 경락도인태극권의 혈압강하 효과)

  • Objectives : The purpose of the study was to determine the effects of a 4 weeks Meridian Tai Ji exercise treatment on blood pressure reducing in hypertensive patients. Methods : We measured the blood pressure of 3 patients who were visited in the Gwacheon Health center from 21th November 2007 to 17th December 2007. Eligible participants had systolic blood pressure ${\geq}$ 120 mmHg or diastolic blood pressure ${\geq}$ 80 mmHg with antihypertensive drug. Blood pressure measurements were after the patient had been in rest for at least 10 minute. Twelve sessions of Meridian Tai Ji exercise treatment over 4 weeks were performed in the patients. Blood pressure were measured twice before and after each session. Results : After 4 weeks, blood pressure reduction was observed in the treatment patients, with an average decline of systolic blood pressure up to 5.67 mmHg and diastolic blood pressure up to 0.17 mmHg. But, it was not statistically significant. Blood pressure increased between measurements taken before and after an Meridian Tai Ji exercise treatment session, although individual responses were quite variable. No significant difference was found in the immediate effect. Conclusion : Twelve sessions of Meridian Tai Ji exercise treatment do not seem to control hypertension. Further research is required to determine whether Meridian Tai Ji exercise treatment can enhance clinical management of hypertension if it is used in combination with more enrolled participants, over longer periods, or randomized controlled trial.

Nutrient Intakes and Dietary Habits of Single Living Korean Adults by Age Group

  • In order to investigate the differences in nutrient intakes and eating habits between people living alone and people living together with family or others by age group, dietary survey data of the subjects aged 20 years or older from 2001 Korean National Health and Nutrition Survey were analyzed. Living status of the subjects was defined as 'single' when the subjects' household member was one person. Age, gender, income, education were adjusted during the comparative analysis. The subjects living alone had diets with lower score of nutritional adequacy ratio and lower quality, and drank more alcoholic beverages when compared with the subjects living together. Females were more greatly affected in dietary intakes by living alone situation than males. Of the four age groups, a group with ages from 30 to 39 years showed less nutrient intake patterns in persons living alone than in persons living together, but the rest three groups with ages from 20 to 29, from 50 to 64, and 65 or older did not show any significant differences. Eating habits of the subjects living alone, such as skipping meals, kinds of snacks, dining-out, were worse as a whole than the other. In conclusion, single living particularly of females or of 30 to 39 years of age group had negative influences on dietary intakes and behavior. There may be statistical errors if socioeconomic and demographic factors such as age, gender, income, and education are not controlled in the population study investigating the effect of living alone on dietary intakes. Further studies will be needed to know the age-specific reasons for the worse nutrient intakes of single living persons.

A Study on the Wastewater and Air Pollution, Noise and Vibration Management and Discharge Control at the Industries (환경오염의 방지시설의 운영에 관한 실태조사)

  • 510 random samples were studied during the months of may through November 1985 at the various industries and conclustions were made as follows; 1. $43.94\%$ of the plants studied operated their plants with semiautomatic control system, and better efficiency were observed at the plants where automatic control systems emplorid and also large industries showed more tendency adopting the automatic plant control system. 2. Overall efficiency of the treatment plants were seen much higher at the first and secand discharge class categories then the lower discharge classes, $80.79\%$ of the plants were see their daily plant operation being controlled by the operator himself. 3. The main causes of the plant stopage and in efficient discharge control were found to be malfunctioning of the plants machineries and equipment or inadequate decision made by the management to save chemicals or electricity. 4. The study showed $60\%$ of the industry treated their wastwater wholly and the rest discharged only with dilution without receiving any further treatment, and this tendency pronounced at the 4th and 5th class discharge category industries. 5. $66.17\%$ of the industry had their storage capacity to accommodate the waste discharge during plants outage while $92.67\%$ of the air pollution discharge industries had no means for the plant outage. 6. $56.77\%$ of the studied industry maintained 24 hour operation of their discharge control systems whill $18.67\%$ of air pollution discharge industries and $10.53\%$ of the waste water discharge industries showed no control effort during the night.

Acupuncture for Subacute and Chronic Post-thoracotomy Pain in Patients with Traumatic Multiple Rib Fractures: A Study Protocol for a Randomised-controlled, Two-arm, Parallel Design, Pilot Trial

  • Background: The aim of this study is to assess the feasibility of acupuncture treatment for the management of subacute and chronic post-thoracotomy pain in patients with traumatic multiple rib fractures. Methods: A total of 30 participants who have undergone thoracotomy after traumatic multiple rib fractures will be recruited. Participants will be invited and equally randomised into acupuncture plus usual care and usual care alone groups. A computer-generated random number sequence will be used and concealed using opaque, sealed, sequentially numbered envelopes. Twelve sessions of manual and electrical acupuncture performed by Korean medicine doctors will be provided over a span of 3 months to participants allocated to the acupuncture group. Participants in the usual care group will continue pain medication, exercise and physical therapy as required. Study feasibility will be measured based on the proportion of patients who complete the measurement of pain at 12 or 24 weeks after baseline. The clinical outcomes will include; the average pain intensity over the recent week at rest, movement and cough, quality of life, patient's global assessment of recovery, respiratory function measured by the pulmonary function test and use of pain medication at 4, 8, 12 and 24 weeks after enrolment. Adverse events will be recorded for all participants. Written informed consent will be obtained from all participants. The local ethics committee has approved the study. This pilot trial will inform further studies investigating the potential role of acupuncture for subacute and chronic post-thoracotomy pain in patients with traumatic multiple rib fractures.

Educational Usage of a Teaching Assistant Robot (교사 보조 로봇의 교육적 활용)

  • Robots evolve from tools to information media since they generates information by interacting with human. As studies on robot-aided education are still in a starting phase, attempts need to be made to use robots for educational purposes and to investigate the effects of the use. It was showed that robot-aided learning was friendlier than other media assisted learning, and especially effective for motivating children. We developed the prototype robot Jenny that can help teachers as a educational media in class(i.e. as a T.A. robot, it can present robot contents on its chest to screen and explain about it when teacher asks). is a schoolmate for 5th or 6th grade children or an elder schoolmate for the rest. We performed the field trial at an elementary school. We carried out 9 classes for three subjects(english, korean, music) with -students in $4th{\sim}5th$ grade. They thought Jenny who was 13 years old as an elder schoolmate in 6th grade. Also, a significant difference was found in the interest and concentration of experimental groups from controlled groups.

Systemic Review of Hyaluronate for the Treatment of Osteoarthritis (골관절염 치료제 히알우론산 임상연구결과의 Systemic Review)

  • Background: The multitude of the therapeutic usefulness of intra-articular injection of hyaluronate on oateoarthritis of the knee is still in question. The objective of this systemic review was to elucidate both the therapeutic efficacy and the safety of intra-articular administration of hyaluronic acid for degenerative osteoarthritis of the knee joints. Methods: I searched MEDLINE and Korea Medical Database (KMbase) from January 1990 to April 2007 using a combination search terms for knee osteoarthritis and hyaluronic acid and a filter for randomized controlled trials. I extracted data on pain at rest, and during or just after movement, on joint function, and on adverse events. Results: Ten trials that reported usable quantitative information on any of the predefined end points were identified and included in the systemic review. Intra-articular injection of hyaluronic acid can decrease symptoms of osteoarthritis of the knee. The study revealed significant improvements in pain and functional outcomes with few adverse effects. However, there was significant between-study heterogeneity in the estimates of the efficacy of hyaluronic acid. Sub-group analysis showed that lower methodological quality such as a single-blind or single-center design resulted in higher estimates hyaluronic acid efficacy, and that patients older than sixty years of age and those with the most advanced radiographic stage of osteoarthritis were less likely to benefit from intra-articular injection of hyaluronic acid. Conclusion: According to the currently available evidence, intra-articular hyaluronic acid has been proven clinically effective for the patients bearing the knee osteoarthritis with NSAID-induced GI troubles or inapplicable to any surgery, and may be associated with lower risk of adverse events.

Exposure assessment of musculoskeletal disorder risk factors in non routinized work: An application of PATH-KOSHA observational tool to hospital workers (비정형작업 근골격계질환 위험요인의 노출평가: 일부 병원근로자에 대한 PATH-KOSHA 관찰도구 적용사례)

  • This study was conducted to assess exposure to musculoskeletal disorder(MSD) risk factors in hospital personnel who performed non-routinized work tasks. A tool ("PATH-KOSHA" version) was newly revised from PATH(Posture, Activity, Tools and Handling) method and uploaded into a personal digital assistant(PDA). The version was used, on a basis of direct-observation, to collect PATH data at the 2 hospital settings in different regions. Job analysis was performed to get various information (e.g., work and rest time, task type) as well. The data collected were visually checked for data cleaning and stored for future data analysis. A total of 1,992 PATH observations were made for 37 hospital workers. Exposure levels varied across 18 items of the MSD risk factors. The highest percent time spent on non-neutral postures was 53% for wrist deviation, followed by 47%(pinch grip), 35%(trunk posture), 23%(neck posture), and 20%(shoulder/arm posture). The highest percent time spent among hand activity level(HAL) variables was 55% for HAL-cat2 (HAL: 3.3 - <6.7). The percent time of items with respect to both loads with more than 5kg and contact stress was less than 4%. Vibration was not exposed in the study workers. Different aspects were discussed for findings. The study results showed that wrist deviation was highest in percent time spent on awkward posture while HAL-cat2 was highest in hand repetition. The study suggests that distal upper extremity posture and HAL should be primarily addressed and controlled in non-routinized work including the hospital settings.

The Effect of Massage to the Different Regions of the Body on the Relaxation in Healthy Women (부위별 마사지가 건강한 성인여성의 신체이완에 미치는 영향)

  • This study was to determine whether the massage would enhance the physical relaxation in healthy women, and whether the effects of massage on the relaxation would be different by the applied regions of the body. Five healthy young women participated in the two test sessions of back and lower extremities massage in a chamber controlled at an air temperature of $28^{\circ}C$ and a relative humidity of $50{\pm}5%$. The slow stroke massage with lavender aroma oil for 30 minutes was applied to the subjects with a 10-min. and a 20-min rest time before and after the application, respectively. Rectal temperature decreased and the chest and thigh skin temperatures increased with the massage application, and the changes of the temperatures were greater with lower extremities massage than with back massage. Heart rate also decreased during massage period and tended to be lower with lower extremities massage. Systolic and diastolic blood pressure decreased with lower extremities massage. These results show that the slow stroke massage for 30 minutes can induce physiological responses related with the enhanced relaxation in healthy women, and lower extremities massage is more effective on the physical relaxation rather than back massage.

The depressive effect of S'NC nerve control treatment in hypertension patients (고혈압 환자에서 뇌신경조절의학 S'NC의 혈압강하 효과)

Methods: We measured the blood pressure of 5 patients who were admitted in the Oriental Medical Clinic of S'NC Medicine Research Institute from 19th April 2007 to 29th June 2007. Eligible participants had systolic blood pressure ${\ge}$140mmHg or diastolic blood pressure ${\ge}$90mmHg. Blood pressure measurements were after the patient had been in bed rest for at least 10 min. Ten sessions of S'NC nerve control treatment over 2 weeks were performed in the patients. Blood pressure were measured twice before and after each treatment. Results : After 2 weeks, blood pressure reduction was observed in the treatment patients, with an average decline of systolic blood pressure up to 21mmHg and diastolic blood pressure up to 20mmHg. But, there were not statiscally significant The effects of S'NC nerve control treatment by measuring time on blood pressure were as follows: In a systolic blood pressure(p=0.087) and diastolic blood pressure(p=0.609) was gradually deceased not significantly from 1st to 10th. Conclusion: These results suggest that S'NC nerve control treatment may be efficacious in decreasing arterial blood pressure in hypertensive patients. Controlled trials investigating the efficacy of S'NC nerve control treatment for lowering blood pressure are warranted.
