The Effect of Structured Information on the Sleep Amount of Patients Undergoing Open Heart Surgery (계획된 간호 정보가 수면량에 미치는 영향에 관한 연구 -개심술 환자를 중심으로-)

  • The main purpose of this study was to test the effect of the structured information on the sleep amount of the patients undergoing open heart surgery. This study has specifically addressed to the Following two basic research questions: (1) Would the structed in formation influence in the reduction of sleep disturbance related to anxiety and Physical stress before and after the operation? and (2) that would be the effects of the structured information on the level of preoperative state anxiety, the hormonal change, and the degree of behavioral change in the patients undergoing an open heart surgery? A Quasi-experimental research was designed to answer these questions with one experimental group and one control group. Subjects in both groups were matched as closely as possible to avoid the effect of the differences inherent to the group characteristics, Baseline data were also. collected on both groups for 7 days prior to the experiment and found that subjects in both groups had comparable sleep patterns, trait anxiety, hormonal levels and behavioral level. A structured information as an experimental input was given to the subjects in the experimental group only. Data were collected and compared between the experimental group and the control group on the sleep amount of the consecutive pre and post operative days, on preoperative state anxiety level, and on hormonal and behavioral changes. To test the effectiveness of the structured information, two main hypotheses and three sub-hypotheses were formulated as follows; Main hypothesis 1: Experimental group which received structured information will have more sleep amount than control group without structured information in the night before the open heart surgery. Main hypothesis 2: Experimental group with structured information will have more sleep, amount than control group without structured information during the week following the open heart surgery Sub-hypothesis 1: Experimental group with structured information will be lower in the level of State anxiety than control group without structured information in the night before the open heart surgery. Sub-hypothesis 2 : Experimental group with structured information will have lower hormonal level than control group without stuctured information on the 5th day after the open heart surgery Sub-hypothesis 3: Experimental group with structured information will be lower in the behavioral change level than control group without structured information during the week after the open heart surgery. The research was conducted in a national university hospital in Seoul, Korea. The 53 Subjects who participated in the study were systematically divided into experimental group and control group which was decided by random sampling method. Among 53 subjects, 26 were placed in the experimental group and 27 in the control group. Instruments; (1) Structed information: Structured information as an independent variable was constructed by the researcher on the basis of Roy's adaptation model consisting of physiologic needs, self-concept, role function and interdependence needs as related to the sleep and of operational procedures. (2) Sleep amount measure: Sleep amount as main dependent variable was measured by trained nurses through observation on the basis of the established criteria, such as closed or open eyes, regular or irregular respiration, body movement, posture, responses to the light and question, facial expressions and self report after sleep. (3) State anxiety measure: State Anxiety as a sub-dependent variable was measured by Spi-elberger's STAI Anxiety scale, (4) Hormornal change measure: Hormone as a sub-dependent variable was measured by the cortisol level in plasma. (5) Behavior change measure: Behavior as a sub-dependent variable was measured by the Behavior and Mood Rating Scale by Wyatt. The data were collected over a period of four months, from June to October 1981, after the pretest period of two months. For the analysis of the data and test for the hypotheses, the t-test with mean differences and analysis of covariance was used. The result of the test for instruments show as follows: (1) STAI measurement for trait and state anxiety as analyzed by Cronbachs alpha coefficient analysis for item analysis and reliability showed the reliability level at r= .90 r= .91 respectively. (2) Behavior and Mood Rating Scale measurement was analyzed by means of Principal Component Analysis technique. Seven factors retained were anger, anxiety, hyperactivity, depression, bizarre behavior, suspicious behavior and emotional withdrawal. Cumulative percentage of each factor was 71.3%. The result of the test for hypotheses show as follows; (1) Main hypothesis, was not supported. The experimental group has 282 minutes of sleep as compared to the 255 minutes of sleep by the control group. Thus the sleep amount was higher in experimental group than in control group, however, the difference was not statistically significant at .05 level. (2) Main hypothesis 2 was not supported. The mean sleep amount of the experimental group and control group were 297 minutes and 278 minutes respectively Therefore, the experimental group had more sleep amount as compared to the control group, however, the difference was not statistically significant at .05 level. Thus, the main hypothesis 2 was not supported. (3) Sub-hypothesis 1 was not supported. The mean state anxiety of the experimental group and control group were 42.3, 43.9 in scores. Thus, the experimental group had slightly lower state anxiety level than control group, howe-ver, the difference was not statistically significant at .05 level. (4) Sub-hypothesis 2 was not supported. . The mean hormonal level of the experimental group and control group were 338 ㎍ and 440 ㎍ respectively. Thus, the experimental group showed decreased hormonal level than the control group, however, the difference was not statistically significant at .05 level. (5) Sub-hypothesis 3 was supported. The mean behavioral level of the experimental group and control group were 29.60 and 32.00 respectively in score. Thus, the experimental group showed lower behavioral change level than the control group. The difference was statistically significant at .05 level. In summary, the structured information did not influence the sleep amount, state anxiety or hormonal level of the subjects undergoing an open heart surgery at a statistically significant level, however, it showed a definite trends in their relationships, not least to mention its significant effect shown on behavioral change level. It can further be speculated that a great degree of individual differences in the variables such as sleep amount, state anxiety and fluctuation in hormonal level may partly be responsible for the statistical insensitivity to the experimentation.

  • In this paper, we address two position control scheme; the lead-lag control and the sliding mode control for a stage system, which is levitated and driven by electric magnetic actuators. This consists of a levitating object (called platen) with 4 permanent magnetic linear synchronous motors in parallel. Each motor generates vertical force for suspension against gravity and propulsion force horizontally as well. This stage can generate six degrees of freedom motion by the vertical and horizontal forces. Dynamic equations of the stage system are derived simply. The sliding mode control algorithm is more effective than the lead-lag control algorithm to reduce effects from movements and disturbances of other axis.

  • The purpose of this study was to investigate the inhibitory effect of the aroma therapy of three kinds of aroma oil mixtures on asthma. 1. The percentage of granulocytes/lymphocytes population in mouse OVA-induced asthma lung cells was decreased significantly compared with those of control group. 2. The number of CCR3+ cells, CD4+ cells, CD8+ cells, CD23 and CD3e+/CD69+ in lungs of the mice group treated with M1 were decreased significantly compared with those of control group. 3. The number of IgE+/B220+ cells in the lungs of the mice group treated with M1 decreased significantly compared with those of control group. But the number of B220+ cells in the lunes of the mice group treated with M1 didn't show significant difference compared with those of control group. 4. The number of Gr-1+/CD11b+ cells in lungs of the mice group treated with M1 didn't show significant difference compared with those of control group. But the number of CD11b+ cells in lungs of the mice group treated with M1 decreased significantly compared with those of control group.

The Study on the Effect of Stage Based Exercise Motivational Intervention Program for the Elderly (운동행위변화단계에 근거한 노인 운동 동기화 중재프로그램의 효과에 관한 연구)

  • This study aims at confirming exercise effects on obesity, mobility, self-efficacy, process of change, and decisional component by stage based exercise motivational intervention program for the elderly. The stage based exercise intervention program was constructed based on Transtheoretical Model. Methods: The design of this study is nonequivalent control group with repeated measuring by quasi-experimental study. The subjects of this study, composing of experimental group of 32 and control group of 28 were selected at one institution for the aged in Seoul. Results: 1) The body fat (weight, BMI and circumference of waist), of the intervention group was significantly decreased than the control group. 2) The mobility of the intervention group was not significantly increased than control group. 3) The self-efficacy, Pros, Process of Change for exercise of the intervention group was not significantly increased than the control group. 4) The Cons for exercise of intervention group was not significantly decreased than the control group. Conclusion: The above result have informed us that a stage-based exercise motivational intervention program for the elderly has the effect of decreasing old persons' body fat and has value as an effective means of nursing for the elderly.

Correlation Analysis between MBI and MMSE after Exercise Program for Dementia Elderly (치매노인을 위한 운동프로그램 적용후 MBI와 MMSE 사이의 상관관계분석)

  • 29 dementia patients over 60 years of no in the nursing home were assigned to execute rehabilitation program a month for 3 months. 1 evaluated MBI scores and MMSE scores and analyzed correlation between both scores. Analyses of results were as follows : 1. In the experimental group with rehabilitation program. ADL and cognitive function were improved significantly(p<0.05). 2. In the control group with no rehabilitation program. ADL was decreased significantly(p<0.05), cognition was not changed (pgt;0.05). 3. There were significant rises in MBI scores and MMSE scores in the experimental group(p<0.05). 4. There was a significant fall in MBI scores in the control group(p<0.05). a no change in MMSE scores in the control group(p>0.05). 5. There was strong correlation between ADL and cognition in the experimental group and the control group. 6. Inverse correlation revealed between the experimental group and the control group in ADL. 7. Inverse correlation revealed between the experimental group and the control group in cognition.

Effects of Dan Jeon Breathing Exercise on Physical Health Promotion in Middle Aged Women (중년여성의 신체적 건강증진에 미치는 단전호흡의 효과)

  • Purpose: This study was to examine the effects of Dan Jeon Breathing on physical health promotion in middle aged women. Method: This study was quasi-experimental with a nonequivalent control group pre-post test design. The subjects were classified into an experimental group and a control group. The experimental group had 21 middle-aged women who did Dan Jeon Breathing, the control group consisted of 22 middle aged women. Dan Jeon Breathing was carried out for 40 minutes a day, 3 times a week for 6 weeks. The difference between the experimental group and control group was verified with an $x^2$-test, t-test and paired and unpaired t-test. Results: The score of physical symptoms for the experimental group was lower than that of the control group. The score of physical strength for the experimental group was higher than that of the control group. Conclusion: Dan Jeon Breathing can be a physical health promotion for middle aged women. In terms of nursing practice, Dan Jeon Breathing can be utilized as a nursing intervention for physical well-being of middle aged women.

  • Objectives : In order to study effects of Dansamyeum on Hyperlipidemia which causes Hypertension, Arteriosclerosis, Cerebral vascular disease and Ischemic heart disease Methods : The changes of serum total lipid, triglyceride, total cholesterol, HDL-cholesterol, LDL-cholesterol, body weight and organ weight were observed after the liquid extracts of Single-dosage Dansamyeum and Double-dosage Dansamyeum were administered p.o to the Hyperlipidemic rats induced by 1% cholesterol diet for 10, 20 and 30 days. Results : The body weight was significantly decreased in Double-dosage Dansamyeum group for 20 and 30 days compared with control group. The contents of total lipid were significantly decreased in Single-dosage Dansamyeum group for 20 days, but Double-dosage Dansamyeum group did not show any significant change compared with control group. The contents of total cholesterol were significantly decreased in Single-dosage Dansamyeum group for 20 days, and Double-dosage Dansamyeum group showed significant changes for 10 and 20 days compared with control group. The contents of triglyceride were significantly decreased in Single-dosage Dansamyeum group for 30 days, and Double-dosage Dansamyeum group showed significant changes for 10 days compared with control group. The contents of HDL-cholesterol were significantly increased in Single-dosage Dansamyeum group and Double-dosage Dansamyeum group for 10 days compared with control group. The contents of LDL-cholesterol were significantly decreased in Single-dosage Dansamyeum group for 10, 20 and 30 days, and Double-dosage Dansamyeum group showed significant changes for 10, 20 and 30 days compared with control group. The liver weight was significantly decreased in Single-dosage Dansamyeum group and Double-dosage Dansamyeum group compared with control group. The kidney and testis weight was significantly decreased in Single-dosage Dansamyeum group compared with control group. The spleen weight was significantly decreased in Double-dosage Dansamyeum group compared with control group. Conclusions : It was thought that Single-dosage Dansamyeum and Double-dosage Dansamyeum could be applied effectively to the Hyperlipidemia.

Effects of Yijin-tang (Erchen-tang) and GamiYijin-tang (Jiaweierchen-tang) on the Gastrointestinal Functions of Rats (이진탕 및 가미이진탕이 흰쥐의 위장관기능에 미치는 영향에 대한 연구)

  • Objectives: This study was undertaken to investigate the effects of Yijin-tang and GamiYijin-tang on the gastrointestinal functions of rats Methods: Sprague-Dawley rats were used as experimental animals, and were administered Yijin-tang (Sample I group, 47.5 mg/ml) and GamiYijin-tang(Sample II group, 38.37 mg/ml, Sample ill group, 85.3 mg/ml) water extract once a day. Changes of gastric juice volume and intestinal mobility index were measured. The effects on colitis induced by dextran sulfate sodium in the rats were also observed. Results: 1. Gastric juice volume was decreased significantly in the sample I group (P<0.05) compared to the control group; there was not significant effect in the sample II and sample III groups. 2. The moving distance of carbon bolus was increased significantly in the sample n (p<0.05) and sample II (p<0.05) groups compared to the control group; there was not significant effect in the sample I group. 3. The intestinal mobility index was increased significantly only in the sample II group (P<0.05) compared to the control group. 4. The feces consistency was increased significantly on the 3rd and 5th day of the sample I group (P<0.05), on 3rd, 4th, and 5th day of the sample II (p<0.05) and the sample III (p<0.05) groups compared to the control group. 5. The feces property index was increased significantly only on the 5th day of the sample III group (p<0.05) compared to the control group. 6. The number of WBC and RBC, levels of hemoglobin and hematocrit were not changed in all sample groups compared to the control group. 7. The number of the type B Goblet cells were increased significantly in the sample II (p<0.05) and the sample III (P<0.05) groups, but the number of the type C Goblet cells were decreased significantly only in the sample ill group (P<0.05) compared to the control group. Conclusions: According to the above results, GamiYijin-tang compared to the Yijin-tang were decreased hight significantly in gastrointestinal mucose and histological antidiarrheal function with protection of the goblet cell more excellently were observed.

Coffee intake can promote activity of antioxidant enzymes with increasing MDA level and decreasing HDL-cholesterol in physically trained rats

  • This study investigated the effect of coffee intake and exercise on the antioxidative activity and plasma cholesterol profile of physically trained rats while they were exercising. Forty eight rats were under either the control diet with water (C) or control diet with coffee (CF) and at the same time they were given physical training for 4 weeks. In terms of physical training, the rats were exercised on a treadmill for 30 minutes everyday. At the end of 4 weeks, animals in each dietary group were subdivided into 3 groups: before-exercise (BE); during-exercise (DE); after-exercise (AE). Animals in the DE group were exercised on a treadmill for one hour, immediately before being sacrificed. Animals in the AE group were allowed to take a rest for one hour after exercise. TG levels were significantly high in coffee intake group than in control group. Also TG level of AE group was significantly higher than that of BE group. Exercise and coffee-exercise interaction effects were significant in total cholesterol (P = 0.0004, 0.0170). The AE of coffee intake group showed highest total cholesterol levels. HDL-cholesterol was significantly lower in coffee intake group than in control group. Coffee, exercise, and coffee-exercise interaction effects were significant in SOD (P = 0.0001, 0.0001, and 0.0001). The AE and BE of coffee intake group showed higher SOD levels than the other four groups. Catalase activities were significantly higher in coffee intake group than control group. No significant main effect was found in GSH/GSSG. Coffee, exercise, and coffee-exercise interaction effects were significant in MDA levels (P = 0.0464, 0.0016, and 0.0353). The DE and AE of coffee intake group and the DE of control group showed higher MDA levels than the BE of control group. Therefore, coffee intake can promote activities of antioxidant enzyme but it also increases MDA and decreases HDL-cholesterol in physically trained rats.

Clinical Study of Emotional State and Personality Characteristics in Patients with Alopecia Aerata (원형탈모증 환자의 정서상태와 성격특성에 관한 임상적 연구)

  • Objectives: This study was aimed to investigate the emotional state and personality characteristics of alopecia aerata patient and to determine whether these characteristics are similar with those of neurotic patient. Methods : 20 alopecia aerata patients, 20 neurotic patients, and 20 normal healthy controls were studied. Beck Depression Inventory(BDI), State and Trait Anxiety Inventory(STAI) and Korean standardized edition of Catell's 16 Personality factors Questionnaire(16PF) were used for assessment. Results: 1) Total score of BDI was higher in neurosis group than alopecia aerata group and normal control group(p<0.05). Inspire of statistical non-significance, total score of in alopecia aerata group was higher than normal control group. 2) Total scores of state anxiety and trait anxiety in alopecia aerata and neurosis group were significantly higher than normal control group(p<0.001). 3) In first-stratum source traits of 16PF, normal control group was significantly higher than alopecia aerata group and neurosis group in B-factor(intelligence) and C-factor(ego-strength)(p<0.05). Alopecia aerata group and neurosis group were significantly higher than normal control group in O-factor(guilt-proneness) and $Q_4-factor(tension,\;anxiety)$(p<0.001). 4) In second-stratum source traits of 16PF, alopecia aerata group and neurosis group were significantly higher than normal control group in ANX-factor(anxiety)(p<0.05). Conclusions : These results suggest that emotional state and personality characterstics of patients with alopecia aerata are a distinguishable different to compare with normal control, and are very similar with those of neurotic patients. The authors propose that psychiatric management in patient with alopecia aerata will be more benificial.
