A case report of a Phantom Limb Pain (PLP) patient treated with integrative Korean and Western medicine

  • Objectives : We report a case of Phantom Limb Pain (PLP) and propose the possibility of integrative Korean and Western medicine in PLP treatment. Methods : For 42 days, a patient who complained of pain and insomnia with PLP (December 20, 2016 to January 20, 2017) received Korean and Western medical treatment. The patient was treated with analgesic injection, a neuropathic pain inhibitor, the combined herbal formula powders Bojungikgitang, and acupuncture. Symptom improvement was assessed using the numerical rating scale (NRS) and continuous sleep time. Results : The patient's NRS scores decreased, and continuous sleep time increased after treatment. The NRS scores decreased from 10 to 0, and the continuous sleep time increased from 2 hours to 7 to 8 hours. Conclusion : According to the results, this report suggests that integrative Korean and Western medicine could be effective in the treatment of PLP patients.

연속 뜸의 열 자극에 대한 수치해석 (A Numerical Study on the Thermal Stimulation of Continuous Moxibustion)

  • 본 연구에서는 연속 뜸의 열자극 특성을 분석하기 위하여 복합대류 열전달 간이 모형을 만들어 ANSYS-FLUENT 를 이용하여 수치해석을 수행하였다. 연속 뜸의 주기가 열자극 효과에 미치는 영향을 파악하기 위해서 여러 장의 뜸을 다양한 주기로 적용하였다. 수치해석을 통하여 뜸의 장수가 증가할수록 유효자극영역의 깊이가 증가하기 위해서는 교체온도가 체온과 같거나 체온보다 커야 함을 확인하였고, 뜸의 적용 수가 증가함에 따른 유효자극 영역 깊이의 증가율은 감소함을 보였다. 또한, 유효자극의 지속시간이 최대가 되는 연속 뜸의 최적의 교체 주기가 존재함을 확인하였다.

족삼리(足三里) 침자(鍼刺)가 동통억제(疼痛抑制)에 미치는 영향(影響) (Effects of Acupuncture at Chok-samni(ST36) on Pain in Rats)

  • The purpose of this study was finding the pain inhibitory effect of acupuncture at ST-36. The pain at dentes incisor was evoked by noxious electric stimulation and EMG changes based on time interval were measured. To do this, the opioid antagonist was administered intraperitoneally. Followings are some of the results. 1. The pain inhibitory effect of acupuncture at ST-36 was 93.82%, 87.00%, 75.30%, 69.76% at 10 20, 30 and 40 minutes respectively. All the data were calculated based on 100% at 0 minute. The effect was very significant at 40 minutes. 2. The continuous effect of acupuncture at ST-36 after taking out the needle was 77.46%, 79.66%, 87.60%, 91.50%, 95.14%, 99.48% at 50, 60, 70, 80, 90 and 100 minutes respectively. The overall continuous effect was shown its maximum 20 minutes after taking out the needle and reached to the initial value at 60 minutes. 3. The pain inhibitory effect following the naloxone administration and acupuncture application ST-36 was 93.44%, 94.58%, 90.80%, 88.04% at 10, 20, 30 and 40 minutes respectively. All values showed similar tendency to the initial data. 4. The continuous effect of acupuncture at ST-36 after taking out the needle which was preceded by the naloxone administration was 91.26%, 91.90%, 92.06%, 93.66%, 94.12%, 93.50% at 50, 60, 70, 80, 90 and 100 minutes respectively. This trend was not a significant fluctuation. It Was concluded that effect of acupuncture at ST-36 was shown in conjunction with secretion of analgesic substance. Therefore it is implied that the acupuncture application will play a major role in treating many diseases with more revelation of scientific acupuncture mechanism. Further studies of acupuncture manipulation are needed in the future based on our study.

매선요법을 이용한 부분비만치료 9례 보고 (The Effect of Thread-embedding therapy on 9 Patients with Partial Obesity)

  • Objectives : The purpose of this study was to examine the effect of thread-embedding therapy against partial obesity. Methods : 9 women from 20 to 30 with partial obesity were treated with thread-embedding therapy and then surveyed satisfaction, reaction after treatment and whether they were re-treated or not. The results of before and after treatment were compared with physical measurement, body composition tests and fat thickness measured by ultrasound. Results : The satisfaction of patients was high and side effects were pain, bruises and unnaturalness of action, but 8 of 9 patients wanted to re-treatment. The effect of thread-embedding therapy against partial obesity was continuous and was most pronounced during the first week. Conclusions : Thread-embedding therapy may be useful to reduce partial obesity without different treatments of obesity, diet, exercise etc.

급성기 뇌경색 환자에서 한방치료와 지속적 유로키나제 정주요법 병용시 안전성에 대한 임상적 고찰 (Study for Safety of Oriental Medical Therapy and Continuous Intravenous Urokinase combined Therapy in Acute Cerebral Infarction.)

  • Objectives : This Study was prepared for investigating the safety of oriental medicine and continuous intravenous urokinase combined therapy in acute cerebral infarction. Methods : We prospectively estimate safety of hemorrhagic transformation occurred in oriental medical therapy and continuous intravenous urokinase combined therapy. We estimate National Institute of Health Stroke Scale Score and Modified Barthel Index. Results : Hemorrhagic transformation was not noted. and Others are not fatal complication. Conclusions : oriental medical therapy and continuous intravenous urokinase combined therapy are safety method in treatment of acute cerebral infarction. We think this can be a good model of Oriental and western cooperative therapy.

A Case of Gouty Arthritis Patient Treated with Collaborative Oriental and Western Medicine with Acute Inflammation and Liver Injury

  • Objectives : To examine the effects of the collaborative Oriental and Western medicine, we treated a gouty arthritis patient with acute inflammation and liver injury with a combination of Oriental and Western treatments. Methods : Acupuncture, Bangphungtongsung-San(Fangfengtongsheng-san)', 'Kangwhaljetong-Um(Qianghuochutong-yin)', and 'Sosiho-Tang(Xiaochaihu-tang) were offered to an acute gouty arthritis patient with NSAIDs, Corticosteroids and allopurinol. Laboratory data were observed for the duration of hospital days. Results : In spite of Oriental treatments, NSAIDs administration caused liver injury, but continuous Oriental treatments with small amount of Corticosteroids and allopurinol brought recovery of liver function and gouty arthritis. Confusion : Collaborative treatments of Oriental and Western medicine are better than independent Western treatment for gouty arthritis with acute inflammation and liver injury. Further studies will be required to ascertain the collaborative treatment with Oriental and Western medicine for gouty arthritis and other diseases.

합곡(合谷) 침자극(鍼刺戟)이 동통억제(疼痛抑制)에 미치는 영향(影響) (Effects of Acupuncture at HapGuk$(LI_4)$ on Pain in Rats)

  • The purpose of this study was finding the pain inhibitory effect of acupuncture at $LI_4$. The pain at dentes incisivi was evoked by noxious electric stimulation and EMG changes based on the interval were measured. To do this, the opioid antagonist was administered intraperitoneally. Following are some of results. 1. The pain inhibitory effect of acupuncture at $LI_4$ was 94.35%, 84.56%, 57.62%, 54.40%, at 10, 20, 30 and 40 minutes respectively. All the data were calculated based on 100% at 0 minute. The effect was very significant at 40 minutes. 2. The continuous effect of acupuncture at $LI_4$ after taking out the needle was 58.58%, 57.62%, 66.22%, 73.18%, 83.70%, 92.68%, at 50, 50, 70, 80, 90 and 100 minutes respectively. The overall continuous effect was shown its maximum 20 minutes after taking out the needle and reached to the initial value at 60 minutes. 3. The pain inhibitory effect following the naloxone administration and acupuncture application at $LI_4$ was 95.96%, 96.04%, 94.86%, 94.92% at 10, 20, 30 and 40 minutes respectively. All values showed similar tendency to the initial data. 4. The continuous effect of acupuncture at $LI_4$ after taking out the needle which was preceded by the naloxone administration was 94.48%, 96.02%, 96.02%, 98.00%, 98.46%, 97.18% at 50, 60, 70, 80, 90 and 100 minutes respectively. This trend was not a significant fluctuation. It was concluded that effect of acupuncture at $LI_4$ was shown in conjunction with secretion of analgesic substance. Therefore it is implied that the acupuncture application will play a major role in treating many diseases with more revelation of scientific acupuncture mechanism. Further studies of acupuncture manipulation are needed in the future based on our study.

  • By process of treatment for a case which diagnosed as glossopharyngeal neuralgia and admitted from the 4th, August, 2000 to the 1st, September, 2000, the results are as follows. Method & Resuits : This patient was diagnosed as Flaring up of Heart-fire(心火上炎), medicated Gamichengsimyeonja-eum(JiaWeiQingXinLianZi-Yin) and acupunture therapy was taken on proximal acupoints of right throat and parotid and distal acupoints inciuding Yuji(L10, 魚際), Taixi(K6,太谿), Zhaohai(K3, 照海), Hegu(LI4, 合谷) and Taichong(Liv3, 太衝). And negative therapy was taken on some pressure points of Jianzhongshu(SI15, 肩中兪) Jianjing(G21, 肩井) and Futu(LI18, 扶突). As a result, the patient's paroxysmal megalgia was improved. Conclusion : I consider that idiopathic pain diseases including glossopharyngeal neuralgia can improved by Oriental-Western collaborative medical treatment and continuous research must be accumulated subsequently.

경추증성 척수증 환자 1례에 대한 임상적 고찰 (Clinical Study on a Case of Cervical Myelopathy)

  • Objective : Cervical myelopathy is one of the most severe disease, as well as a common diesaes. However, its clinical study and report about efficiency of oriental treatment are absolutely insufficient. Therefore, we report the treatment about 1 case of Cervical myelopathy like followings. Methods : From November 27th, 2002 to January 11th, 2003, we observed patient hospitalized because of cervical myelopathy, and for its valuation, JOA score(Japanese Orthopedic Association score), Nutric grading, VAS(visual analog scale), and D.I.T.I(Digital Infrared Thermal Imaging)were used. Results : After being hospotalized for 50 days, the patients' JOA score, Nuric grading system, and VAS were improved each from 7 to 13, from grade IV to grade II, and from 8 to 2. Conclusions : We considered that oriental treatment has an useful effect on cervical myelopathy patients' treatment and recovery. But continuous studies, such as comparative studies accompanied by more treatment case.

  • By process of trearment for case diagnosed as Oral Dyskinesia and admitted from the 3lth, July, 2001 to the 1st, September, 2001, the results are as follows. Method and Results : This patient was diagnosed as Oral Dyskinesia of Shaoyang disease with interior syndrome that was based by disorder of hepatic qi, medicated shihogayonggolmoreutang(shihujiayonggumolie-tang), gamisoyo-san(jiaweishaoyao-san) and acupun - cturetherapy was taken on proximal and disital acupoints including Yingu(B10, 陰谷), Zulingyi(G41, 足臨泣), Jianshi(P5, 間使), Shenmen(H6, 神門), Sanyinjiao(Sp6, 三陰交), Yi - ntang(Extra point, 印堂), Lianquan(CV23, 康泉), Qihai(CV6, 氣海) and Danhange(Damhangyuk, 膽寒格), Ganjeongge(ganjeonggyuk, 肝正格), Pizhengge(Bijeonggyuk, 脾正格) of Shayen - zhenfa(Shayamchimbeop, 舍岩鍼法). Conclusion : I consider that psychosomatic tremer disease including Oral Dyskinesia can improved by Oriental medical treatment and continuous research must be accumulated subsequently.
