The thickness of alveolar bone at the maxillary canine and premolar teeth in normal occlusion

  • Purpose: The main purpose of this study was to investigate bone thickness on the buccal and palatal aspects of the maxillary canine and premolars using cone-beam computed tomography (CBCT). The differences between left- and right-side measurements and between males and females were also analyzed. Methods: The sample consisted of 20 subjects (9 males and 11 females; mean age, $21.9{\pm}3.0$) selected from the normal occlusion sample data in the Department of Orthodontics, The Catholic University of Korea. The thickness of the buccal and palatal bone walls, perpendicular to the long axis of the root were evaluated at 3 mm and 5 mm apical to cemento-enamel junction (CEJ) and at root apex. Results: At the canines and first premolars regions, mean buccal bone thickness of at 3 mm and 5 mm apical to CEJ were less than 2 mm. In contrast, at the second premolar region, mean buccal bone thickness at 3 mm and 5 mm apical from CEJ were greater than 2 mm. Frequency of thick bone wall (${\geq}2mm$) increased from the canine to the second premolar. Conclusions: This result should be considered before tooth extraction and planning of rehabilitation in the canine and premolar area of maxilla. Careful preoperative analysis with CBCT may be beneficial to assess local risk factors and to achieve high predictability of success in implant therapy.

치내치를 동반한 감염된 미성숙 영구치의 재생형 근관치료 (Regenerative Endodontic Treatment of Infected Immature Permanent Teeth with Dens Invaginatus : A Report of Two Cases)

  • 치내치를 동반한 미성숙 영구치의 근관치료를 성공적으로 수행하는 것은 치과의사에게 어려운 과제이다. 복잡한 형태의 근관계는 근관 내부를 효과적으로 성형, 멸균, 충전하는 것이 어렵기 때문에 포괄적이고 철저한 치료계획을 수립하는 것이 중요하다. 본 증례는 치내치를 동반한 감염된 미성숙 영구치를 가진 두명의 환자에게 재생형 근관치료를 시행하여 양호한 결과를 얻었기에 이를 보고하고자 한다. 감염된 괴사치수와 각각 Oehlers type I, type III 치내치를 포함하는 두개의 미성숙 상악 측절치에 재생형 근관치료가 시행되었다. 특히 Type III 치내치의 근관치료는 보다 효과적인 감염조절과 근관 충전을 위해 복잡한 근관 내부 구조를 재형성하는 드문 접근법을 이용하였다. 이 때, Cone-beam computed tomography (CBCT)와 미세 수술 현미경의 사용은 근관 내부의 시각화 및 치료에 도움이 되었다. 결과적으로, 복잡한 형태의 치내치를 포함하는 미성숙 영구치의 재생형 근관치료는 임상적, 방사선학적 치유 뿐만 아니라 치근 상아질 벽의 두께 증가를 가능하게하는 효과적인 치료 방법이 될 수 있을 것으로 생각된다.

  • Objective: This study aimed at evaluating the changes in mandibular arch widths and buccolingual inclinations of mandibular posterior teeth after rapid maxillary expansion (RME). Methods: Baseline and post-expansion cone-beam computed tomographic (CBCT) images of patients who initially had bilateral posterior cross-bite and underwent RME with a banded-type expander were assessed in this study. The patients included 9 boys (mean age: $13.97{\pm}1.17$ years) and 11 girls (mean age: $13.53{\pm}2.12$ years). Images obtained 6 months after retention were available for 10 of these patients. Eighteen angular and 43 linear measurements were performed for the maxilla and mandible. The measurements were performed on frontally clipped images at the following time points; before expansion (T1), after expansion (T2), and after retention (T3). Statistical significance was assessed with paired sample $t$-test at $p$ < 0.05. Results: T1-T2 comparisons showed statistically significant post-RME increases for all measurements; similarly, T2-T1 and T3-T1 comparisons showed statistically significant changes. The maxillary linear and angular measurements showed decreases after expansion, and mandibular linear and angular measurements increased after retention. Conclusion: All mandibular arch widths increased and mandibular posterior teeth were uprighted after RME procedure.

Radiographic evaluation of the symphysis menti as a donor site for an autologous bone graft in pre-implant surgery

  • Purpose: This study was performed to obtain a quantitative evaluation of the cortical and cancellous bone graft harvestable from the mental and canine regions, and to evaluate the cortical vestibular thickness. Materials and Methods: This study collected cone-beam computed tomographic (CBCT) images of 100 Italian patients. The limits of the mental region were established: 5 mm in front of the medial margin of each mental foramen, 5 mm under the apex of each tooth present, and above the inferior mandibular cortex. Cortical and cancellous bone volumes were evaluated using SimPlant software (SimPlant 3-D Pro, Materialize, Leuven, Belgium) tools. In addition, the cortical vestibular thickness (minimal and maximal values) was evaluated in 3 cross-sections corresponding to the right canine tooth (3R), the median section (M), and the left canine tooth (3L). Results: The cortical volume was $0.71{\pm}0.23mL$ (0.27-1.96 mL) and the cancellous volume was $2.16{\pm}0.76mL$ (0.86-6.28 mL). The minimal cortical vestibular thickness was $1.54{\pm}0.41mm$ (0.61-3.25 mm), and the maximal cortical vestibular thickness was $3.14{\pm}0.75mm$ (1.01-5.83 mm). Conclusion: The use of the imaging software allowed a patient-specific assessment of mental and canine region bone availability. The proposed evaluation method might help the surgeon in the selection of the donor site by the comparison between bone availability in the donor site and the reconstructive exigency of the recipient site.

Correlation between Radiographic Findings, Clinical Findings and Joint Sounds of Temporomandibular Joint Osteoarthritis Patients

  • Purpose: This study aims to evaluate the correlation between joint sounds and radiographic bone change patterns along with clinical symptoms of temporomandibular joint osteoarthritis (TMJ OA) patients. Methods: The patients for this study were over 19 years of age, diagnosed tentatively with TMJ OA. The patients were examined with temporomandibular disorders analysis test and all three radiographs, including panoramic radiography, transcranial radiography, and cone beam computed tomography (CBCT). Information of the patients' age, pain status, joint sound and mouth opening range were collected. And bone change pattern was examined by reviewing panoramic radiography, transcranial radiography and CBCT images. Results: The patients with crepitus had a higher average active mouth opening (AMO) range than patients without crepitus, and the group with bilateral crepitus had a higher average AMO range than the group with unilateral crepitus (p<0.001). And the patient with pain during mastication was increased in the group with clicking than the group without clicking, and the group with bilateral clicking showed a statistically significant increase in the patient with pain during mastication than the group with unilateral clicking (p<0.05). The analytical results of the relevance of crepitus showed a high correlation with bone change observed from each of the three radiographs. And the agreement in bone change findings from 3 groups of paired radiographs showed high agreement (p<0.001). Meanwhile, 77.2% of CBCT findings showed bone change of condyle without crepitus (p<0.001). Conclusions: This study presented significant results in the evaluation of the correlation with crepitus and bone change of TMJ OA patients from panoramic radiography or transcranial projection. However, the accurate assessment is required through CBCT for the patient with complains of persistent pain, limitation of mouth opening, and occlusal change even if the crepitus does not exist.

A Pilot Study on Hip Bone Mineral Densities Estimation from Forearm CBCT images

  • In this paper, we defined the relative cross-sectional area of forearm cortical bone and investigated its correlation with hip bone mineral density values of total femur, femoral neck, femoral trochanter, femoral inter-trochanter and femoral ward's triangle, respectively. Based on the correlations, we established a linear transformation between the relative cross-sectional area of forearm cortical bone and each hip bone BMD. We obtained forearm images using CBCT and hip bone BMDs using dual-energy X-ray absorptiometry (DXA) for 28 subjects. We also investigated the optimal forearm region to provide the strongest correlation coefficient. We used the optimized forearm region to establish each linear transformation to estimate BMD values for total femur, femoral neck, femoral trochanter, femoral inter-trochanter and femoral ward's triangle from the relative cross-sectional area of forearm cortical bone, respectively. We observed the strong correlations with total femur (r=0.889), femoral neck (r=0.924), femoral trochanter (r=0.821), femoral inter-trochanter (r=0.867) and femoral ward's triangle (r=0.895), respectively. The strongest correlation was observed in the forearm mid-shaft regions. Our results suggest that the hip bone BMD values can be simply estimated from forearm CBCT images in a convenient sitting position without X-ray exposure on a hip including genital organs, and may be useful for screening osteoporosis.

Analysis and evaluation of relative positions of mandibular third molar and mandibular canal impacts

  • Objectives: This study used cone-beam computed tomography (CBCT) images to categorize the relationships between the mandibular canal and the roots and investigated the prevalence of nerve damage. Materials and Methods: Through CBCT images, contact and three-dimensional positional relationships between the roots of the mandibular third molar and the mandibular canal were investigated. With this data, prevalence of nerve damage according to the presence of contact and three-dimensional positional relationships was studied. Other factors that affected the prevalence of nerve damage were also investigated. Results: When the mandibular third molar and the mandibular canal were shown to have direct contact in CBCT images, the prevalence of nerve damage was higher than in other cases. Also, in cases where the mandibular canal was horizontally lingual to the mandibular third molar and the mandibular canal was vertically at the cervical level of the mandibular third molar, the prevalence of nerve damage was higher than in opposite cases. The percentage of mandibular canal contact with the roots of the mandibular third molar was higher when the mandibular canal was horizontally lingual to the mandibular third molar. Finally, the prevalence of nerve damage was higher when the diameter of the mandibular canal lumen suddenly decreased at the contact area between the mandibular canal and the roots, as shown in CBCT images. Conclusion: The three-dimensional relationship of the mandibular third molar and the mandibular canal can help predict nerve damage and can guide patient expectations of the possibility and extent of nerve damage.

  • 이 연구의 목적은 골격성 2급 부정교합 소아·청소년의 상기도 공간을 분석하고, 이에 영향을 미치는 요인을 알아보고자 함이다. 총 67명의 골격성 2급 소아·청소년의 CBCT영상으로 연구를 진행하였다. 상기도 부피와 최소 단면적은 3차원 CBCT 영상을 통해 평가하였으며, 악안면 형태와 골 성숙도는 2차원 두부방사선사진을 통해 평가하였다. 상기도 부피 및 최소 단면적과 다양한 변수들간의 연관성이 분석되었다. 상기도 공간은 최대 성장기 이전의 환자에서 가장 작았으며, 연령과 양의 상관관계를 보였다. 상기도 부피는 전안면 고경 및 연령과, 최소 단면적은 하악 폭경 및 연령과 가장 높은 상관관계를 나타냈다. 골격성 2급 소아·청소년의 상기도 공간은 연령, 골 성숙도, 세 평면에서의 악안면 형태와 유의한 연관성을 가졌다.

Could Crepitus Be an Indication for Early Temporomandibular Joint Osteoarthritis?

  • Purpose: To determine whether crepitus may be a clinical indication for early temporomandibular joint (TMJ) osteoarthritis (OA) and to investigate the correlation between crepitus and the occurrence of TMJ OA with respect to factors, such as patient sex, age, chewing habits, and diagnosis. Methods: This is retrospective analysis of clinical data for 162 TMJs. The criteria for a joint to be included in this study was a minimum of two cone-beam computed tomography (CBCT) scans performed with no OA observed during the initial scan. The Diagnostic Criteria for Temporomandibular Disorders was used for OA diagnosis. Crepitus was recorded when it was objectively palpated during the follow-up period. Correlations between various patient factors and progression to TMJ OA were calculated using the Pearson's chi-square test. A linear-by-linear association was used to analyze trends of OA progression with increasing age. Results: Among the 162 joints, 101 progressed to OA and 61 did not. In the joints where crepitus had been present before OA was confirmed at next or last CBCT, OA progressed at a high rate, and especially higher in female and older patients (p<0.01). Patients in the pain-related disorder group with crepitus were observed to have higher rates of OA progression compared to patients in the intra-articular disorder group (p<0.01). Conclusions: If a patient experiences pain in the TMJs and crepitus, close monitoring through regular CBCT scans is necessary even if there is no evidence of radiologically confirmed OA after the first CBCT.

C-shaped root canals of mandibular second molars in a Korean population: a CBCT analysis

  • Objectives: The purpose of this study was to investigate the C-shaped root canal anatomy of mandibular second molars in a Korean population. Materials and Methods: A total of 542 teeth were evaluated using cone-beam computed tomography (CBCT). The canal shapes were classified according to a modified version of Melton's method at the level where the pulp chamber floor became discernible. Results: Of the 542 mandibular second molars, 215 (39.8%) had C-shaped canals, 330 (53%) had 3 canals, 17 (3.3%) had 2 canals, 12 (2.2%) had 4 canals, and 8 (1.7%) had 1 canal. The prevalence of C-shaped canals was 47.8% in females and 28.4% in males. Seventy-seven percent of the C-shaped canals showed a bilateral appearance. The prevalence of C-shaped canals showed no difference according to age or tooth position. Most teeth with a C-shaped canal system presented Melton's type II (45.6%) and type III (32.1%) configurations. Conclusions: There was a high prevalence of C-shaped canals in the mandibular second molars of the Korean population studied. CBCT is expected to be useful for endodontic diagnosis and treatment planning of mandibular second molars.
