The Geometric Averaging Technique for Long Bone (긴뼈의 형상 평균화 기법)

  • Many authors issued the feature-preserving averaging technique according to positioning and scaling process using landmarks, which represent the geometric characteristics of three dimensional surface models. Such a technique should be done by manual procedure, choosing and marking the landmarks on each bone surface before averaging process. In this study, we produced another averaging technique without having to use such manual procedure, and made averaging models from three dimensional surface data that were reconstructed from computerized tomography images of Digital Korean Project. The bone models were subjected to orthogonal coordinator system. These models were transformed to coincide mass center and to align principal axis. Then, bone models were scaled according to average length data of sample bone models on all axis(x, y, z). After establishing voxellar hexahedron space which contain all sample bone models, we counted the number of overlapping for each voxel. We generated the three dimensional average surface by displaying the yokels that have more overlapping number than boundary number. The boundary number was decided when the average volume of each bone equal to the volume of bone that would be averaged. Using this technique, we can make a feature-preserving averaging volume of bones.

  • In a 40 year-old man who has complained of epigastric pain and dyspnea for 10 days including melena for 1 month, abdominal ultrasonography and computerized tomography revealed a large, solid mass in the right kidney and thrombus of inferior vena cava (IVC) that extended to the cavoatrial junction. Renal cell carcinoma was performed by percutaneous needle biopsy. IVC thromboembolectomy was performed using centrifugal pump driven venovenous bypass without cardiac arrest and cardiopulmonary bypass (CPB).

  • A geometrical inverse heat conduction problem is solved for the development of Infrared Computerized-Axial-Tomography (IR CAT) Scan by using a boundary element method in conjunction with regularization procedure. In this problem, an overspecified temperature condition by infrared scanning is provided on the surface, and is used together with other conditions to solve the position of an unknown boundary (cavity). An auxiliary problem is introduced in the solution of this problem. By defining a hypothetical inner boundary for the auxiliary problem domain, the cavity is located interior to the domain and its position is determined by solving a potential problem. Boundary element method with regularization procedure is used to solve this problem, and the effects of regularization on the inverse solution method are investigated by means of numerical analysis.

  • Parathyroid cyst are rare lesion of the neck and superior mediastinum. They are classified as either functional or nonfunctional based on the presence or absence of hyperparathyroidism. They typically present as a palpable neck mass, or as an incidental finding during neck surgery. So, it must be included within the differential diagnosis of a neck lump. A 48-year old woman was admitted to our hospital for evaluation of left anterior neck mass. Physical finding showed soft, movable, non-tender mass on the lower left third of the neck. Laboratory findings revealed normal thyroid function and normal serum calcium level. On previous history, patient underwent two times sono-guided fine needle aspiration at local clinic, and about 10cc clear watery aspirate was noted. Computerized tomography findings showed non-enhancing hypodense cystic lesion on left thyroid area with tracheal deviation to right side. Preoperative diagnosis was thyroid cyst, and thyroidectomy was planed. Intra-operative finding showed huge cystic mass occupying the left thyroid area and smooth, shiny, semitransparent thin cyst wall was noted, which was loosely attached to the thyroid. The cyst was easily dissected free from the thyroid and surrounding tissues. After cyst removal, pathological study confirmed as a parathyroid cyst.

Intussusception due to Diverticulum of Ileocecal Area -A case report- (회맹부 게실에 의한 장중첩증 1례 보고)

  • Intussusception is a frequent cause of bowel obstruction in the first five years of life and it is one of the most common surgical emergencies in infancy and early childhood. The age of five months child was administered in Department of Pediatrics of Chunchon Sacred Heart Hospital. His main symptoms were vomiting and high fever for three days. Abdominal sonography, air reduction and abdominal computerized tomography (CT) were performed and the conclusion of these study was intussusception due to cyst mass lesion; duplication cyst, mesenteric cyst or Meckel's diverticulum. He was transferred for operation. We had performed laparotomy for reduction of the intussusception. Operative findings revealed ileocolic type of intussusception due to cystic tumor on ileocecal valve that was invaginated into the cecum, and hyperplasia of the Peyer's patch were seen. But we failed manual reduction because of the tumor in the ileocecal area. So we had performed partial resection of the ileocecum. Diverticulum of the ileum was confirmed by pathologic examination. We experienced unusual cause of the intussusception. So we report this case with a review of the literatures.

Thoracic EndoVascular Stent Graft Repair for Aortic Aneurysm

  • Background: The number of cases employing thoracic endovascular aortic repair (TEVAR) has been increasing due to lower morbidity and mortality compared to open repair technique. The aim of this study is to evaluate the outcome of TEVAR for thoracic aortic diseases. Materials and Methods: Sixteen patients underwent TEVAR from October 2003 to April 2010. Mean age at operation was 59 years (20~78 years), and 11 were male. Indications for TEVAR were large aortic diameter (>5.5 cm) upon presentation in 6 patients, increasing aortic diameter during the follow-up period in 4, traumatic aortic rupture in 3, persistent chest pain in 2, and ruptured aortic aneurysm in one. The mean diameter, length and the number of the stents were 33 mm (26~40 mm), 12 cm (9.5~16.0 cm), and 1.25 (1~2), respectively. Aortography employing Multi-detector computerized tomography (MDCT) technique was performed at one week, and patients were followed up in the out-patient department at one month, 6 months, and one year postoperatively. Results: Primary technical success showing complete exclusion of the aneurysm was achieved in 15 patients. One patient showed a small endo-leak (type 1). Four patients developed perioperative stroke: Three recovered without sequelae, and one showed mild right-side weakness. There was no operative mortality. Diameter of the thoracic aorta covered by stent graft changed within 10% range in 12 patients, decreased by more than 10% in 3, and increased by more than 10% in one during mean follow-up duration of 18 months (1~73 months). There was no recurrence-related death during this period. Conclusion: Intermediate-term outcome after TEVAR was encouraging. Indications for TEVAR could be extended for other thoracic aortic diseases.

A Clinical Analysis of Primary Hyperparathyroidism -A Report of II Cases- (원발성 부갑상선 기능항진증 -11예 보고-)

  • Primary hyperparathyroidism is still uncommomn disease in Korea. However the frequency of this disease has been slowly increased with routine measurement of serum calcium and increasing awareness of hyperparathyroidism in recent years. The diagnosis is established by a persistent elevation of serum calcium and parathyroid hormone and by clinical evaluation. This is a report of eleven patients with primary hyperparathyroidism treated with surgical operation during a period from 1983 to 1997 at Department of Hospital. Authors analyzed the cases to evaluate clinical characteristics and outcome of surgical treatment retrospectively. The result was as follows. 1) In sex distribution, female patients were eight and three were male, the age distribution ranged from 18 to 67 years. 2) The presenting clinical manifestations were renal and urinary stone in eight, bone pain or fracture in six, muscle weakness in four, neurologic symptoms in four, neck mass in three, hypertension in two, and G-I symptoms in one. 3) All patients showed hypercalcemia and elevated serum parathyroid hormone level. 4) Preoperative localization study was performed with computerized tomography, ultrasonography, MRI, arteriography and thyroid scaning. 5) The tumor locations were left lower in eight, left upper in one, right lower in one, and right upper location was one case. 6) Histopathologic findings disclosed adenoma in all cases. 7) All patients were treated by surgical excision and postoperatively transient hypocalcemia occurred in six patients, but no other complication was developed.

  • Maxillary canines have less anatomical diversities than other teeth. They usually have a single root and root canal. This report describes an endodontic treatment of a maxillary canine with two separated root canals which have not been reported through the demonstration of radiography and computerized tomography (CT). Even though appropriated endodontic treatment has been performed, the severe pain could happen due to lack of consideration of anatomical variations of the teeth. Therefore, the clinicians should be well aware of the possibility of anatomical variations in the root canal system during endodontic treatment even if the number of root canals is obvious such as in this case.

Observation of Correlation between Deficiency Syndrome of Kidney and Bone Mineral Density in Osteoporosis Patients (골다공증 환자에서 신허와 골밀도의 상관성에 대한 관찰연구)

  • Objectives : This study was designed to analyze correlation in deficiency syndrome of kidney index and bone mineral density(BMD) in 130 osteoporosis patients. Methods : 130 women who were over 50 years old and visited in Department of Acupuncture & Moxibustion Medicine, Cheonan Oriental Hospital of Daejeon University from January 1, 2012 to December 31, 2013, answered the questionnaire consisting of symptom of deficiency syndrome of kidney, to determine the deficiency syndrome of kidney Index(DSKI). Grade of low back pain were measured in the visual analogue scale(VAS). BMD of the patients were determined by quantitative computerized tomography(QCT). Then, DSKI with BMD, VAS and age were analyzed by correlation analysis, variance analysis and Kruskal-Wallis test. Results : 1. Between DSKI and BMD, patients in total and in-their-50's group did not show a statistically significant result, but in-their-60's group and over-70 group showed a statistically significant result by correlation analysis. 2. Between DSKI and VAS, groups in total, in-the-50's, 60's, and over-70 groups showed a statistically significant result by correlation analysis. Conclusions : DSKI and BMD of the patients group who are over 60, DSKI and VAS of the patiens over 50 years old were associated with each other.

Hypertension, Polydipsia, Polyuria, and Hypokalemia Associated with Renin-Secreting Wilms' Tumor in a Child (소아에서 고혈압, 번갈 다음증, 다뇨증, 및 저칼륨증을 유발한 레닌-분비성 윌름스 종양(Wilms' Tumor))

  • A 3-year-old boy with a Wilms' tumor had unusually severe hypertension, polydipsia, polyuria and hypokalemia. Physical examination on admission was unremarkable except for the presence of a smooth, firm mass in the right abdomen. Computerized tomography showed a tumor occupying the upper two thirds of the right kidney. Plasma renin activity and aldosterone concentration were markedly elevated, 37.7 mg/ml/hour(normal in supine position 0.15-2.33 mg/ml/hour) and 120.1 ng/dL(normal in supine position 1 to 16 ng/dL), respectively. Hypertension varied from 150/90 mmHg to 240/180 mmHg, and was not effectively controlled by antihypertensive drugs. Right nephrectomy was performed on the sixth hospital day. At laparatomy, there was no evidence of mechenical compression of the renal artery by the tumor. The tumor, about 8 cm in diameter, was confined to the renal capsule without involvement of the renal blood vessels at the hilum. Histopathology was Wilms' tumor of favorable histology. On electron microscopy, tumor cells contained intracytoplasmic electron dense secreting graules, suggesting the possibility of renin secreting tumor cells. Shortly after nephrectomy, signs and symptoms were relieved dramatically, and plasma renin activity and aldosterone concentration were also decreased to normal.
