경두개 직류전류자극과 전산화 인지재활 프로그램이 뇌졸중 환자의 인지기능 및 우울감에 미치는 영향 (The Effect of Transcranial Direct-Current Stimulation on Cognitive Function and Depression in Stroke Patient's through a Computerized Cognitive Rehabilitation Program)

  • Purpose : The purpose of this randomized controlled trial study was to examine the effect of transcranial direct current stimulation (tDCS) on cognitive function and depression in stroke patients. Methods : Thirty stroke patients were randomly divided into an experimental group (n = 15) and a control group (n = 15). The experimental group received tDCS while performing computerized cognitive rehabilitation programs, and the control group was provided with sham tDCS while operating the same programs. The 30-minute intervention was implemented five times per week for six weeks. To assess cognitive function before and after the intervention, the Neurobehavioral Cognitive Status Examination was conducted; the Beck Depression Inventory BDI was employed to assess depression. Results : The experimental group showed statistically significant increases in cognitive function and decreases in depression (p < .05 ). Comparing the amount of variation between the groups after arbitration also showed significant differences in cognitive function and depression between the two groups (p > .05). Conclusion : The application of tDCS and computerized cognitive rehabilitation programs for stroke patients may positively affect their cognitive function and depression. Therefore, tDCS used with computerized cognitive rehabilitation programs is positively applicable to the enhancement of cognitive function in stroke patients and reduction of depression.

  • Purpose : In this study, we applied a computerized cognitive rehabilitation program (CoTras-C) for children with cerebral palsy. Research was conducted to investigate the impact of upper limb function, sensory function, and activities of daily living. Methods : The study period lasted 10 weeks from October 2019 to December 2019. The study subjects were 12 subjects according to the selection criteria, and a computerized cognitive rehabilitation program (CoTras-C) was conducted twice a week for 30 minutes before and after the application of basic occupational therapy. Results : As a result of the computerized cognitive rehabilitation program, scores of upper limb function (QUEST), sensory function (SSP-2), and daily life activity (WeeFIM) were significantly improved (p>.05). From the result of examining the motor area, improvement in fine-motor function and protective extension through touch pad or controller operation was found. It also showed improvement in activities of daily living including motor and activities of daily living including social cognition. In the sensory function evaluation, it was not significant in movement sensitivity. Significant differences were shown in the items excluding olfactory/taste sensitivity. Conclusion : The application of the computerized cognitive rehabilitation program (CoTras-C) showed significant results in upper limb function, sensory function, and daily life activities of children with brain lesions. Based on these results, future studies need to generalize the study by expanding the age or population of children with brain lesions, or by expanding the diversity of diseases and environments.

전산화 인지프로그램 적용이 뇌졸중 환자의 인지기능과 일상생활활동에 미치는 영향 (The Effect of Computerized Cognitive Program on Cognitive Function and Activities of Daily Living of Stroke Patients)

  • Purpose : This study investigates the effects of the application of traditional occupational therapy and the korean computerized cognition training system on the cognitive function and performance of daily activities of stroke patients with cognitive impairment. Methods : From inpatients referred for rehabilitation treatment at L Rehabilitation Hospital located in Busan, 20 patients diagnosed with stroke from April 05. 2021 to May 02. 2021 (study period) were selected, They were divided into two, an experimental group consisting of 10 subjects who underwent a computerized cognitive rehabilitation program and traditional occupational therapy in combination and a control group of 10 subjects who underwent traditional occupational therapy alone. In order to measure the cognitive function of the subjects before the intervention, two assessment tests were conducted: a Neurobehavioral Cognitive Status Examination (NCSE), which evaluates stroke-related cognitive ability, and a Functional Independence Measure (FIM) test, which evaluates life activities. Then, both groups received a total of 20 training sessions at 30 minutes per session, five times a week for four weeks. Results : A statistically significant difference was found in cognitive function between before and after the cognitive training for both the experimental group and the control group. For the FIM scores, statistically significant differences were observed after intervention in the categories of handling personal matters and social cognition, and in the total score. The average scores of the remaining items also improved. Conclusion : The results of this study showed that both the computerized cognition rehabilitation program and the traditional occupational therapy had a positive effect on the improvement of cognitive function in stroke patients.

경두개 직류자극 및 인지훈련 프로그램 적용이 치매환자의 인지기능, 시지각 및 일상생활활동에 미치는 영향 (Does the Application Transcranial Direct Current Stimulation and Visual Perception Program Enhance the Visual Perception Function and ADL Performance of Dementia Patients)

  • Purpose : There is a lack of research on this field in the Republic of Korea, especially those that have seen the effect of interaction between transcranial direct current stimulation and computerized cognitive rehabilitation therapy. This study divided 30 dementia patients into a treatment group, which received transcranial direct current stimulation and computerized cognitive rehabilitation, and a control group, which received pseudo-transcranial direct current stimulation and pseudo-computerized cognitive rehabilitation. This study evaluated the effects of these treatments on the visual perception, cognition functions, and daily activities of dementia patients. Methods : Fifteen subjects were allocated to the treatment group and the other 15 subjects were allocated to the control group. Treatments were given at intervals of five sessions per week (30 minutes per session) for six weeks (30 times in total). This study used the Neurobehavioral Cognitive Status Examination (NCSE) to examine cognitive functions, MVPT to evaluate visual perception, and FIM to test daily living activities before and after applying the treatments. Results : The results of this study showed that cognitive functions, visual perception, and daily living activities significantly (p<.05) improved after the intervention in the treatment group and the control group. The changes in cognitive functions, visual perception, and daily living activities due to the treatments were significantly different between the groups (p<.05). Conclusion : The results indicated that transcranial direct current stimulation and computerized cognitive rehabilitation therapy improved visual perception and daily living activities by increasing cognitive functions. Consequently, it was found that the simultaneous application of transcranial direct current stimulation in conjunction with a computerized cognitive rehabilitation program was an intervention method that could positively affect the visual perception, cognitive function, and daily living activities of dementia patients. Based on the results of this study, the study of arbitration protocols for demential will have to be more active.

전산화 인지재활이 자폐스펙트럼장애 아동의 주의력에 미치는 효과: 단일대상연구 (Effects of Computer-Based Cognitive Rehabilitation on the Attention of Children with Autism Spectrum Disorder: A Single-Subject Study)

  • 목적 : 본 연구는 자폐스펙트럼장애(Autism Spectrum Disorder; ASD) 아동을 대상으로 전산화 인지재활이 주의력에 미치는 효과를 알아보고자 하였다. 연구방법 : 개별대상자 실험연구 방법 중 다중 기초선 설계를 사용하였다. 총 15회기를 주 3회씩 1회기 당 30분 동안 진행하였다. 전산화 인지재활 프로그램의 주의력 영역에서 유지 주의력과 선택적 주의력 검사를 매 회기가 끝난 직후에 평가하였다. 선택적 주의력을 평가하기 위하여 사전과 사후에 별 지우기 검사(star cancellation test)를 실시하였다. 결과 : 전산화 인지재활 중재 후 유지 주의력과 선택적 주의력의 검사에서 정확도는 대상자 모두 향상되었다. 2표준편차 구간을 이용한 분석방법에서 대상자 1, 2가 유의한 증진이 나타났고 대상자 3은 유의하지 않았지만 평균값이 향상되었다. 결론 : 본 연구 결과를 통해 전산화 인지재활 프로그램을 사용한 인지재활은 ASD 아동의 주의력에 긍정적인 영향을 미치는 것을 확인할 수 있었다.

Verification of Effectiveness and Satisfaction Survey for the Korean Computer-based Cognitive Rehabilitation Programs(CoTras)

  • The purpose of this study was to verify the effectiveness of the computerized cognitive rehabilitation program in which areas and to suggest effective ways to utilize the program in the future, being conducted for 20 college students. We lasted this study from May 3 to 23, 2021. As a result of analyzing the groups using the Computer-based Cognitive Rehabilitation Program (CoTras), in terms of the difference in accuracy for the case of visual perception group B was statistically significantly improved than group C(p<0.05). In the case of attention, memory, and orientation, there was no significant difference between groups(p>0.05). In the case of reaction time difference, there was no significant difference between groups in visual perception, concentration, memory, and orientation(p>0.05). And in order to improve attention and visual perception, it is recommended to conduct the program three times with a duration of 20 minutes, and in order to improve orientation and memory, it can be said that it is helpful to conduct one experiment for at least 30 minutes rather than conducting short and frequent experiments. Through this study, we found that it is effective to apply different times according to each area to improve cognitive function. In other words, depending on the purpose of which cognitive function is to be improved, the duration of the program should be applied differently.

뇌질환 환자에서 전산화 인지치료의 임상적 유용성 : 체계적 고찰 및 메타분석 (Clinical Benefit of Computerized Cognitive Therapy in Patients with Brain Disease : A Systematic Review and Meta-Analysis)

  • Purpose : This study aims to confirm the clinical usefulness of computerized cognitive therapy program for patients with brain diseases in Korea and to present basic data that can confirm the effectiveness of computerized cognitive therapy program to experts related to rehabilitation in the future. Methods : A systematic review and meta-analysis research method was used to confirm the clinical effectiveness of computerized cognitive therapy applied to patients with brain diseases in Korea. Based on the national institute of health and medical colleges association (NECA) systematic literature manual and preferred reporting items for systematic reviews and meta-analysis (PRISMA) guidelines for conducting research through systematic review and meta-analysis methods. Systematic review was PICOST-SD was used to research and select papers. For meta-analysis, the data was input and analyzed separately by literature result using the revman manager 5.3 program and the results were presented visually through tables and forest, funnel plot figure. Results : As a result of comprehensive analysis of the contents of the finally selected literature, it as found that studies on stroke and dementia were mainly conducted. In addition, through the evaluation of the quality of the literature, most of the existing computerized cognitive therapy studies were identified as high-quality studies, but the results were insufficient for randomization and blinding. And through meta-analysis, the clinical effectiveness of computerized cognitive therapy on cognitive function, activities of daily living, and visual perception function of patients with brain disease was found. Conclusion : The research literature that applied computerized cognitive therapy to brain disease subjects was comprehensively analyzed and identified. It is thought that this could be used as basic data on the characteristics of cognitive therapy that should be applied to patients with brain diseases in the future and the usefulness of computerized cognitive therapy program in actual clinical practice.

전산화 신경인지기능 프로그램(COMCOG, CNT)을 이용한 뇌졸중 환자의 기억력과 주의력 증진효과 (Effects of Computerized Neurocognitive Function Program Induced Memory and Attention for Patients with Stroke)

  • Purpose: The purpose of this study was to evaluate the effect of computerized neurocognitive function program on cognitive function about memory and attention with stroke. Methods: 24subjects with stroke were recruited. Twelve of subjects received conventional therapy including physical therapy, occupational therapy and language therapy. Another subjects received additional computer assisted cognitive training using Computer-aided Cognitive rehabilitation training system(COMCOG, MaxMedica Inc., 2004). All patients were assessed their cognitive function of memory and attention using Computerized Neurocognitive Function Test(CNT, MaxMedica Inc., 2004) before treatment and 6 weeks after treatment. Results: Before the treatment, two groups showed no difference in cognitive function(p>0.05). After 6 weeks, two groups showed significantly difference in digit span (forward, backward), verbal learning(A5, $A1{\sim}A5$), auditory CPT(n), visual CPT(n)(p<0.05). After treatment, the experimental group showed a significant improvement of digit span(forward, backward), verbal learning(A5, $A1{\sim}A5$), visual span (forward, backward), auditory CPT(n, sec), visual CPT(n, sec), and trail-making (A, B)(p<0.05). Conclusion: Computerized neurocognitive function program would be improved cognitive function of memory and attention in patients with stoke.

전산화 인지재활 프로그램이 뇌졸중 환자의 우울 및 인지기능에 미치는 영향 (The Effects of Computerized Cognitive Rehabilitation Program on Depression and Cognitive Function in Stroke Patients)

  • 본 연구는 부산 M병원에서 전산화 단층촬영이나 자기공명영상촬영에 의해 뇌졸중을 진단받고 입원한 뇌졸중 환자 중 이 연구의 취지를 이해하고 자발적으로 참여하겠다고 서면으로 동의한 환자를 대상으로 전산화 인지재활프로그램을 적용하여 우울 및 인지기능, 일상생활활동에 미치는 영향을 알아보는 연구이다. 뇌졸중환자 44명을 대상으로 2016년 07월 04일부터 2016년 08월 12일까지 6주간 주 5회 1일 30분씩, 총30회기 동안 전산화 인지재활프로그램을 적용한 실험군 22명과 전산화인지재활프로그램을 적용하지 않은 대조군 22명을 6주간 주 5회 30회기를 실시하였다. 중재 전과 후의 우울은 우울척도 검사(BDI)를 통해 알아보았고, 인지기능은 신경학적 인지상태 검사(NSCE), 일상생활활동은 한국판 수정바델지수(k-MBI)를 통해 알아보았다. 연구결과 실험군에서는 우울감의 감소, 인지기능의 증진 및 일상생활활동의 증진을 나타내었다. 그러나, 대조군의 경우 우울감, 인지기능, 일상생활활동이 실험군에 비해 유의하게 증진하지는 않았다는 결과로 나타났다. 따라서 본 연구 결과 전산화 인지재활프로그램을 받은 실험군이 대조군에 비해 우울, 인지기능, 일상생활활동에 긍정적인 영향을 미친다는 것을 알 수 있었으며, 임상에서 뇌졸중 환자의 우울, 인지, 일상생활활동을 위한 치료로서 전산화인지 프로그램을 적극적으로 활용할 수 있을 것이다.

휴대용 전산화 인지훈련 시스템이 경도인지손상이 있는 뇌졸중 환자의 시지각에 미치는 효과 (Effects of a Portable Computerized Cognitive Training System on the Visual-perceptual Function of Stroke Patients with Mild Cognitive Impairment : A Pilot Study)

  • 본 연구는 휴대용 전산화 인지훈련 시스템이 경도 인지손상이 있는 뇌졸중 환자의 시지각에 미치는 효과를 알고자 하였다. 재활병원에 입원 중인 경도인지손상이 있는 12명의 뇌졸중 환자가 본 연구에 참여하였다. 모든 대상자들은 실험군과 대조군으로 6명씩 할당되었다. 모든 대상자들은 실험군 또는 대조군의 훈련을 하루 30분, 주 5회, 4주 동안 수행하였다. 실험군은 휴대용 전산화 인지훈련 시스템을 실시하였으며, 대조군은 보편적인 인지훈련을 수행하였다. 시지각을 측정하기 위하여, MVPT-3(Motor-Free Visual Perception-3)을 측정하였다. 연구 결과, 두 군은 중재 전과 후로 MVPT-3 total score에서 유의한 향상을 보였다(p<.05). 두 군 간 변화량 비교에서 실험군은 대조군보다 MVPT-3 total score와 visual short-term memory of sub-item of MVPT-3에서 유의하게 더 큰 향상을 보였다(p<.05). 이러한 결과는 휴대용 전산화 인지훈련 시스템이 경도인지손상이 있는 뇌졸중 환자의 시지각 능력 향상에 긍정적인 효과를 이끌어 낼 수 있는 중재방법이 될 수 있음을 시사한다.
