Three-dimensional soft tissue analysis for the evaluation of facial asymmetry in normal occlusion individuals

  • Objective: To identify the right and left difference of the facial soft tissue landmarks three-dimensionally from the subjects of normal occlusion individuals. Materials and Methods: Cone-beam computed tomography (CT) scans were obtained in 48 normal occlusion adults (24 men, 24 women), and reconstructed into 3-dimensional (3D) models by using a 3D image soft ware. 3D position of 27 soft tissue landmarks, 9 midline and 9 pairs of bilateral landmarks, were identified in 3D coordination system, and their right and left differences were calculated and analyzed. Results: The right and left difference values derived from the study ranged from 0.6 to 4.6 mm indicating a high variability according to the landmarks. In general, the values showed a tendency to increase according to the lower and lateral positioning of the landmarks in the face. Overall differences were determined not only by transverse differences but also by sagittal and vertical differences, indicating that 3D evaluation would be essential in the facial soft tissue analysis. Conclusions: Means and standard deviations of the right and left difference of facial soft tissue landmarks derived from this study can be used as the diagnostic standard values for the evaluation of facial asymmetry.

CT Angiography 영상에서 대동맥 추출을 위한 혈관 분할 알고리즘 성능 평가 (Performance evaluation of vessel extraction algorithm applied to Aortic root segmentation in CT Angiography)

  • 세계보건기구협회에의 통계에 따르면 심장 혈관 질환의 발병률이 가장 높은 것으로 알려져 있다. CTA영상을 사용하여 관상동맥 및 대동맥 질환을 치료 및 검사할 수 있다. 혈관을 3차원으로 복원하는 과정이 의사의 숙련도에 따라 결과가 상이하며 복원 시간이 길다는 단점이 있으며 이를 극복하고자 자동으로 정확한 혈관을 추출하는 연구들이 진행되어 왔다. 본 논문에서는 자동 및 반자동 분할 기법인 Region Competition, Geodesic Active Contour(GAC), Multi-atlas based segmentation, Active Shape Model(ASM) 알고리즘을 CTA영상에 적용하여 대동맥 기부를 추출하였으며 하우스도르프 거리, 볼륨, 영상처리속도, 사용자 관여 여부, 그리고 관상동맥 심문 검출률을 비교 및 분석하였다. 추출된 3차원 대동맥 모델 중 가장 높은 정확도를 나타낸 알고리즘은 GAC인 반면 사용자 관여가 가장 높았기 때문에 실제 시술에 적용하기 위해서는 자동 분할 알고리즘 개선이 필요하다

CT상의 HU 수치에 따른 유한요소모델을 이용한 RME 사용에 따른 응력분포에 대한 연구 (Stress Distribution following Rapid Maxillary Expansion using Different Finite Element Model according to Hounsfield Unit Value in CT Image)

  • With rising prevalency of mouth breathing children caused by developing civilization and increasing pollution, there are many maxillary transverse discrepancy patients with undergrowth of maxilla. For improving this, maxillary mid-palatal suture splitting was often performed. The purpose of this study was to analyse the stress distribution on the craniofacial suture and cranium after rapid maxillary expansion by finite element model. The boy(13Y6M) was chosen for taking computed-tomography for finite element model. Three-dimensional model of maxilla, first premolar, first molar, buccal and lingual part of rapid maxillary expansion were constructed. 1. The alveolar bone adjacent to the first molar and the first premolar that was affected directly by rapid maxillary expansion was displaced laterally approximately 4.04mm at maximum. The force decreased toward anterior region and frontal alveolar bone displaced laterally about 3.18mm. 2. A forward maximum displacement was exhibited at zygomatic process middle region. 3. At maximum, maxillary median part experienced 0.973mm downward repositioning and 0.65mm upward repositioning at lateral alveolar bone. 4. Von mises stress was observed the largest stress distribution around teeth and zygomatic buttress. 5. The largest tensile force was observed around alveolar bone of teeth, while compression force was observed at zygomatic buttress.

이산화탄소 지중저장을 위한 포항분지 시추코어의 공극구조 분석 (An Analysis of Pore Network of Drilling Core from Pohang Basin for Geological Storage of CO2)

  • 이산화탄소 지중저장 시 암석 내부의 공극구조는 지층 내 이산화탄소의 거동에 큰 영향을 미친다. 본 연구에서는 포항분지 영일만 지역에서 채취한 시추코어를 대상으로 X선 단층촬영 기법을 이용해 3차원 영상을 취득하고, 그 내부에 대한 정량적인 분석을 실시하였다. 세 가지 목표지층인 심도 740 m, 780 m, 810 m 부근에서 채취한 사암 시료(T1, T2, T3)의 공극률은 각각 25.22%, 23.97%, 6.28%로 분석되었다. 등가직경, 공극 부피, 공극 표면적, 국부두께 분석을 수행하였으며, 그 결과 T1, T2 시료가 T3 시료에 비해 이산화탄소 지중저장에 더 적합한 것으로 나타났다. 본 연구의 분석 결과는 이산화탄소 주입 조건 결정, 지층 내 이산화탄소 유동 해석 등에 기초자료로 이용될 수 있을 것으로 기대된다.

표적이 규칙적으로 움직일 때 생기는 4DCT 영상의 모션 아티팩트(Motion Artifact) 관련된 원인분석 (Analysis of Acquisition Parameters That Caused Artifacts in Four-dimensional (4D) CT Images of Targets Undergoing Regular Motion)

  • 본 연구는 표적이 규칙적으로 움직일 때 생기는 4DCT 영상의 모션 아티팩트(motion artifact) 관련된 원인들인 partial volume effect (PVE), partial projection effect (PPE), 각각의 프레임의 시작점 사이에 불일치(MMimph)의 원인들은 조사 및 분석을 목적으로 했다. 본 기관에서 제작된 흉부팬텀과 아크릴의 두개의 원기둥팬텀(반지름: 2 cm, 길이: 0.5 cm/ 반지름: 2 cm, 길이: 10 cm)이 규칙적인 움직이는 동안 CT을 이용하여 4D 영상을 획득했다. 촬영은 자동시작과 각각의 프레임의 시작점을 일치시키기 위해 매뉴얼로 두 가지 방법으로 시작하였다. 첫번째 실험에서는 CT 캔트리 rotation time은 5초와 8초가 이용되었다. 각 프레임은 다른 위상으로 시작되었다. 두번째 실험에서는 각 프레임에서 같은 위상으로 시작되게 하기 위해 매뉴얼로 시작했다. 세번째 실험에서는 원기둥팬텀을 2초와 6초를 주기로 움직였다. 각각의 4DCT의 영상에서 표적의 부피를 구했다. 영상으로부터 구한 표적의 부피와 표적의 실제 부피와 비교를 통해 관계를 분석했다. 흉부팬텀 실험에서는 CT의 갠트리 속도가 팬텀의 움직임보다 빠를수록 PVE와 PPE의 영향이 적어짐에 따라 영상에서 얻은 표적부피는 실제에 근접했다. 각각의 프레임의 시작점이 일치할수록 움직임의 속도와 표적의 부피와 상관관계가 높았다. 원기둥팬텀에서는 흉부팬텀의 경우와 같이 갠트리 속도가 팬텀의 움직임보다 빠를수록 영상으로부터 구한 표적부피는 실제 표적부피에 근접했다. 특히 한 slice의 두께가 2.5 mm을 고려 할 때 axial방향의 PVE, PPE, 각각의 프레임의 시작점이 일치가 되는 상태를 시뮬레이션한 길이 10 cm의 원기둥팬텀 실험에서는 영상에서 얻은 표적부피는 표적이 정지되었을 때 영상에서 얻은 부피와 거의 일치했다. 팬텀이 느리게 움직일수록, CT 갠트리의 rotation 시간이 짧을수록 영상에서 얻은 표적부피는 실제 부피에 근접했다. CT 촬영 시 각각의 프레임의 시작점이 같을수록 표적의 속도와 영상에서 의한 표적의 부피는 상관관계가 높게 나타났다. CT 갠트리의 rotation 속도를 팬텀의 움직임보다 빠르게 하고, CT 촬영시 각의 프레임의 시작점을 일치시킬수록 실제의 팬텀부피에 근접하리라고 예상된다.

Nostril Base Augmentation Effect of Alveolar Bone Graft

  • Background The aims of alveolar bone grafting are closure of the fistula, stabilization of the maxillary arch, support for the roots of the teeth adjacent to the cleft on each side. We observed nostril base augmentation in patients with alveolar clefts after alveolar bone grafting. The purpose of this study was to evaluate the nostril base augmentation effect of secondary alveolar bone grafting in patients with unilateral alveolar cleft. Methods Records of 15 children with alveolar clefts who underwent secondary alveolar bone grafting with autogenous iliac cancellous bone between March of 2011 and May of 2012 were reviewed. Preoperative and postoperative worm's-eye view photographs and reconstructed three-dimensional computed tomography (CT) scans were used for photogrammetry. The depression of the nostril base and thickness of the philtrum on the cleft side were measured in comparison to the normal side. The depression of the cleft side pyriform aperture was measured in comparison to the normal side on reconstructed three-dimensional CT. Results Significant changes were seen in the nostril base (P=0.005), the philtrum length (P=0.013), and the angle (P=0.006). The CT measurements showed significant changes in the pyriform aperture (P<0.001) and the angle (P<0.001). Conclusions An alveolar bone graft not only fills the gap in the alveolar process but also augments the nostril base after surgery. In this study, only an alveolar bone graft was performed to prevent bias from other procedures. Nostril base augmentation can be achieved by performing alveolar bone grafts in children, in whom invasive methods are not advised.

Assessment of the Severity of Coronavirus Disease: Quantitative Computed Tomography Parameters versus Semiquantitative Visual Score

  • Objective: To compare the accuracies of quantitative computed tomography (CT) parameters and semiquantitative visual score in evaluating clinical classification of severity of coronavirus disease (COVID-19). Materials and Methods: We retrospectively enrolled 187 patients with COVID-19 treated at Tongji Hospital of Tongji Medical College from February 15, 2020, to February 29, 2020. Demographic data, imaging characteristics, and clinical data were collected, and based on the clinical classification of severity, patients were divided into groups 1 (mild) and 2 (severe/critical). A semiquantitative visual score was used to estimate the lesion extent. A three-dimensional slicer was used to precisely quantify the volume and CT value of the lung and lesions. Correlation coefficients of the quantitative CT parameters, semiquantitative visual score, and clinical classification were calculated using Spearman's correlation. A receiver operating characteristic curve was used to compare the accuracies of quantitative and semi-quantitative methods. Results: There were 59 patients in group 1 and 128 patients in group 2. The mean age and sex distribution of the two groups were not significantly different. The lesions were primarily located in the subpleural area. Compared to group 1, group 2 had larger values for all volume-dependent parameters (p < 0.001). The percentage of lesions had the strongest correlation with disease severity with a correlation coefficient of 0.495. In comparison, the correlation coefficient of semiquantitative score was 0.349. To classify the severity of COVID-19, area under the curve of the percentage of lesions was the highest (0.807; 95% confidence interval, 0.744-0.861: p < 0.001) and that of the quantitative CT parameters was significantly higher than that of the semiquantitative visual score (p = 0.001). Conclusion: The classification accuracy of quantitative CT parameters was significantly superior to that of semiquantitative visual score in terms of evaluating the severity of COVID-19.

유한요소법에 의한 골강도와 micro-CT 지표 및 골밀도간의 상관관계 (Analysis on correlation between bone strength by FEA, micro-CT parameters and bone mineral density)

  • Purpose : Bone mineral density (BMD) and bone microarchitecture are important determinants for bone strength. Recently micro-CT have provided possibilities for measuring a variety of structural indices to characterize bone microarchitecture. The objective of this study was to compare the BMD and micro-CT parameters with Young's modulus calculated by finite element analysis (FEA) for the evaluation of bone strength. Materials and Methods Bone specimens were obtained from the 18 female rabbits aged 16 weeks. Of those, 36 samples (right and left femur) were selected for 3D micro-CT analysis $(ANT^{TM},\;SKYSCAN,\;Belgium)$ and BMD by PIXlmus 2 (GE Lunar Co. USA). Five microstructural parameters of micro-CT, such as trabecular thickness (Tb.Th), bone specific surface (BS/BV), percent bone volume (BV/TV), structure model index (SMI) and degree of anisotropy (DOA) were studied. Young's modulus was obtained by software program (ANSYS 9.0, ANSYS Inc, Canonsburg, PA) based on micro-CT three dimensional images. Results : Young's modulus assessed by FEA correlated significantly with Tb.Th, BV/TV, BS/BV and SMI respectively. Young's modulus showed higher correlation with these rnicrostructural parameters of micro-CT than BMD. Microstructural parameters except DOA showed significant correlations within the examined group. Conclusion The microarchitectural parameters o( micro-CT and BMD represented some informations in the evaluation of bone strength assessed by FEA.

Minimizing the extra-oral time in autogeneous tooth transplantation: use of computer-aided rapid prototyping (CARP) as a duplicate model tooth

  • Objectives: The maintenance of the healthy periodontal ligament cells of the root surface of donor tooth and intimate surface contact between the donor tooth and the recipient bone are the key factors for successful tooth transplantation. In order to achieve these purposes, a duplicated donor tooth model can be utilized to reduce the extra-oral time using the computer-aided rapid prototyping (CARP) technique. Materials and Methods: Briefly, a three-dimensional digital imaging and communication in medicine (DICOM) image with the real dimensions of the donor tooth was obtained from a computed tomography (CT), and a life-sized resin tooth model was fabricated. Dimensional errors between real tooth, 3D CT image model and CARP model were calculated. And extra-oral time was recorded during the autotransplantation of the teeth. Results: The average extra-oral time was 7 min 25 sec with the range of immediate to 25 min in cases which extra-oral root canal treatments were not performed while it was 9 min 15 sec when extra-oral root canal treatments were performed. The average radiographic distance between the root surface and the alveolar bone was 1.17 mm and 1.35 mm at mesial cervix and apex; they were 0.98 mm and 1.26 mm at the distal cervix and apex. When the dimensional errors between real tooth, 3D CT image model and CARP model were measured in cadavers, the average of absolute error was 0.291 mm between real teeth and CARP model. Conclusions: These data indicate that CARP may be of value in minimizing the extra-oral time and the gap between the donor tooth and the recipient alveolar bone in tooth transplantation.

Comparative Analysis of Cervical Lateral Mass Screw Insertion among Three Techniques in the Korean Population by Quantitative Measurements with Reformatted 2D CT Scan Images : Clinical Research

  • Objective : Our purpose of this study is to compare insertion angles and screw lengths from Roy-Camille, Magerl, and our designed method for cervical lateral mass screw fixation in the Korean population by quantitative measurement of reformatted two dimensional (2D) computed tomography (CT) images. Methods : We selected thirty Korean patients who were evaluated with thin section CT scans and reconstruction program to obtain reformatted 2D-CT images of the transversal plane passing the cranio-caudal angle using three different techniques. We measured the minimum angle to avoid vertebral artery (VA) injury, the ideal angle and depth for bicortical screwing of cervical lateral mass. Morphometric measurements of the lateral masses from C3-C7 were also taken. Results : In all three techniques, the mean safety angles from the VA were less than 8 degrees and the necessary depth of the screw was about 14 mm for safety to the VA and for the bicortical purchase. In our designed technique, the mean $\beta$ angles of each level from C3 to C7 were 29.0. 29.8. 29.5. 26.3. and 23.9 degrees, respectively. Conclusion : Results of this study and data from the literature indicate that differences may exist between the Korean and Western people in the length and angle for ideal lateral mass screw fixation. In addition, our technique needs further cadaveric and clinical study for safety and efficacy for being performed as alternative method for cervical lateral mass fixation.
