Comparison of bone subtraction CT angiography with standard CT angiography for evaluating circle of Willis in normal dogs

  • Background: Bone subtraction computed tomography angiography (BSCTA) is a useful alternative technique for improving visualization of vessels surrounded by skull bone. However, no studies have compared computed tomography angiography (CTA) and BSCTA for improving the visibility of canine cerebral blood vessels. Objectives: To evaluate the potential benefit of BSCTA for better delineation of brain arteries of the circle of Willis (CoW) in dogs by comparing BSCTA with non-subtraction computed tomography angiography (NSCTA). Methods: Brain CTA was performed for nine healthy beagle dogs using a bolus tracking method with saline flushing. A total dose of 600 mgI/kg of contrast agent with an iodine content of 370 mgI/mL was injected at a rate of 4 ml/s. Bone removal was achieved automatically by subtracting non-enhanced computed tomography (CT) data from contrast CT data. Five main intracranial arteries of the CoW were analyzed and graded on a scale of five for qualitative evaluation. Results: Scores of basilar artery, middle cerebral artery, and rostral cerebral artery in the BSCTA group were significantly higher than those in the NSCTA group (p = 0.001, p = 0.020, and p < 0.0001, respectively). Scores of rostral cerebellar artery (RcA) and caudal cerebral artery (CCA) did not differ significantly between the two groups. However, scores of RcA and CCA in the BSCTA group were higher than those in the NSCTA group. Conclusions: BSCTA improved visualization of intracranial arteries of the CoW with close contact to bone. Thus, it should be recommended as a routine scan method in dogs suspected of having brain vessel disease.

종합검진 수검자의 관상동맥 전산화단층 혈관조영술 검사에서 관상동맥질환의 위험요인과 관련성 분석 (A Study on the Analysis of Risk Factors and Correlations of Coronary Artery Disease of the Examinee taking Coronary Computed Tomography Angiography in a Comprehensive Health Improvement Center)

  • 본 연구는 건강검진 목적으로 시행된 관상동맥 CT에서의 성별, 연령에 따른 관상동맥질환 발견율에 대한 유용성과 글루코스, 총 콜레스테롤, 내장지방, 체질량지수, 중성지방, 고밀도지단백질(HDL), 저밀도지단백질(LDL) 각각의 인자들과 관상동맥질환과의 관련성에 대해 알아보았다. 총 299명의 수검자를 대상으로 후향적 분석을 실시 한 결과 관상동맥질환 발견율에 대해서는 남성, 50세 이상에서의 유병율이 높은 것으로 나타났으며, 통계적으로 유의성을 보였다. 특히 남성의 유병율(37.9%)은 여성의 유병율(17.0%) 보다 약 2배가 높은 것으로 확인되었다. 또한 관상동맥질환과 관계되는 혈액학적 요인으로는 글루코스와 고밀도지단백질(HDL)이 통계적으로 유의성을 보였다. 관상동맥질환의 예방 및 관리를 위해서는 글루코스와 고밀도지단백질(HDL)의 조절이 필요할 것으로 보이며, 다른 혈액학적 인자들과는 통계적으로 유의성은 낮았지만, 관상동맥질환에 대한 관리의 필요성이 확인되었다. 주요 결과로서 선별검사 목적으로 관상동맥 CT의 사용은 방사선 피폭 등의 문제점으로 인해 수검자의 성별, 나이를 고려하여 시행하는 것이 좋을 것으로 생각되며, 관상동맥 CT 시행 이전에 비침습적인 방법들의 선행검사를 통한 관상동맥 질환 검사가 필요함을 제안한다.

Basic Physical Principles and Clinical Applications of Computed Tomography

  • The evolution of X-ray computed tomography (CT) has been based on the discovery of X-rays, the inception of the Radon transform, and the development of X-ray digital data acquisition systems and computer technology. Unlike conventional X-ray imaging (general radiography), CT reconstructs cross-sectional anatomical images of the internal structures according to X-ray attenuation coefficients (approximate tissue density) for almost every region in the body. This article reviews the essential physical principles and technical aspects of the CT scanner, including several notable evolutions in CT technology that resulted in the emergence of helical, multidetector, cone beam, portable, dual-energy, and phase-contrast CT, in integrated imaging modalities, such as positron-emission-tomography-CT and single-photon-emission-computed-tomography-CT, and in clinical applications, including image acquisition parameters, CT angiography, image adjustment, versatile image visualizations, volumetric/surface rendering on a computer workstation, radiation treatment planning, and target localization in radiotherapy. The understanding of CT characteristics will provide more effective and accurate patient care in the fields of diagnostics and radiotherapy, and can lead to the improvement of image quality and the optimization of exposure doses.

Diagnosis of Persistent Primitive Olfactory Artery Using Computed Tomography Angiography

  • Persistent primitive olfactory artery (PPOA) is a rare anomaly of the anterior cerebral artery. We present one case of PPOA incidentally diagnosed with the aid of computed tomography (CT) angiography. A 24-year-old woman was admitted to our hospital after sudden onset of vertigo. Three-dimensional CT angiography revealed an anomalous artery arising from the terminal portion of the right internal carotid artery. The proximal portion of the anomalous artery coursed anteromedially and made a hairpin turn posterosuperior to the midline. PPOA may be overlooked because of its rarity, but CT angiography can be useful in detecting this rare vascular anomaly. Follow-up study is necessary in our case to confirm whether an aneurysm occurs on the PPOA.

Rupturing Anterior Communicating Artery Aneurysm during Computed Tomography Angiography : Three-Dimensional Visualization of Bleeding into the Septum Pellucidum and the Lateral Ventricle

  • Computed tomography angiography (CTA) is commonly used in setting of subarachnoid hemorrhage, but imaging features of aneurysm rupturing taking place at the time of scanning has rarely been described. The author reports a case of actively rebleeding aneurysm of the anterior communicating artery with intraventricular extravasation on the hyperacute CTA imaging. The rebleeding route, not into the third ventricle but into the lateral ventricles, can be visualized by real-time three-dimensional CT pictures. The hemorrhage broke the septum pellucidum and the lamina rostralis rather than the lamina terminalis.

Diagnostic Performance of On-Site Automatic Coronary Computed Tomography Angiography-Derived Fractional Flow Reserve

  • Background and Objectives: Fractional flow reserve (FFR) is an invasive standard method to identify ischemia-causing coronary artery disease (CAD). With the advancement of technology, FFR can be noninvasively computed from coronary computed tomography angiography (CCTA). Recently, a novel simpler method has been developed to calculate onsite CCTA-derived FFR (CT-FFR) with a commercially available workstation. Methods: A total of 319 CAD patients who underwent CCTA, invasive coronary angiography, and FFR measurement were included. The primary outcome was the accuracy of CT-FFR for defining myocardial ischemia evaluated with an invasive FFR as a reference. The presence of ischemia was defined as FFR ≤0.80. Anatomical obstructive stenosis was defined as diameter stenosis on CCTA ≥50%, and the diagnostic performance of CT-FFR and CCTA stenosis for ischemia was compared. Results: Among participants (mean age 64.7±9.4 years, male 77.7%), mean FFR was 0.82±0.10, and 126 (39.5%) patients had an invasive FFR value of ≤0.80. The diagnostic accuracy, sensitivity, specificity, positive predictive value, and negative predictive value of CT-FFR were 80.6% (95% confidence interval [CI], 80.5-80.7%), 88.1% (95% CI, 82.4-93.7%), 75.6% (95% CI, 69.6-81.7%), 70.3% (95% CI, 63.1-77.4%), and 90.7% (95% CI, 86.2-95.2%), respectively. CT-FFR had higher diagnostic accuracy (80.6% vs. 59.1%, p<0.001) and discriminant ability (area under the curve from receiver operating characteristic curve 0.86 vs. 0.64, p<0.001), compared with anatomical obstructive stenosis on CCTA. Conclusions: This novel CT-FFR obtained from an on-site workstation demonstrated clinically acceptable diagnostic performance and provided better diagnostic accuracy and discriminant ability for identifying hemodynamically significant lesions than CCTA alone.

Computed tomographic anatomy of hepatic artery in normal beagle dogs

  • This study was performed to examine the visualization and anatomical variants of the hepatic artery with dual-phase computed tomography (CT) angiography and three-dimensional volume rendering imaging analysis in clinically normal dogs. Seven healthy beagle dogs were enrolled and underwent dual CT angiography. Arterial phase images could be obtained with multi-detector CT angiography using the fixed-scan method in these dogs. Contrast enhancement of the hepatic parenchyma was quite minimal because of the unique blood supply system of the liver. In most dogs, the main hepatic arterial branches were the right lateral branch, left branch, and right medial branch. Although hepatic arterial variation appears to be common in dogs, only one dog in this study had the caudate lobar branch as the first branch of the hepatic artery. Further study on a larger number of dogs with CT images will be needed to identify and classify the pattern of hepatic arterial variations.

Prevalence and Anatomy of Aberrant Right Subclavian Artery Evaluated by Computed Tomographic Angiography at a Single Institution in Korea

  • Objective : Aberrant right subclavian artery (ARSA) is a rare anatomical variant of the origin of the right subclavian artery. ARSA is defined as the right subclavian artery originating as the final branch of the aortic arch. The purpose of this study is to determine the prevalence and the anatomy of ARSA evaluated with computed tomography (CT) angiography. Methods : CT angiography was performed in 3460 patients between March 1, 2014 and November 30, 2015 and the results were analyzed. The origin of the ARSA, course of the vessel, possible inadvertent ARSA puncture site during subclavian vein catheterization, Kommerell diverticula, and associated vascular anomalies were evaluated. We used the literature to review the clinical importance of ARSA. Results : Seventeen in 3460 patients had ARSA. All ARSAs in 17 patients originated from the posterior aspect of the aortic arch and traveled along a retroesophageal course to the right thoracic outlet. All 17 ARSAs were located in the anterior portion from first to fourth thoracic vertebral bodies and were located near the right subclavian vein at the medial third of the clavicle. Only one of 17 patients presented with dysphagia. Conclusion : It is important to be aware ARSA before surgical approaches to upper thoracic vertebrae in order to avoid complications and effect proper treatment. In patients with a known ARSA, a right transradial approach for aortography or cerebral angiography should be changed to a left radial artery or transfemoral approach.

Preoperative Identification of a Perforator Using Computed Tomography Angiography and Metal Clip Marking in Perforator Flap Reconstruction

  • In perforator flap reconstruction, vascular mapping using preoperative computed tomography (CT) angiography is widely used to confirm the existence and location of an appropriate perforator. This study proposes a rapid, accurate, and convenient method for marking the perforator location on the skin surface. For 12 patients who underwent perforator flap reconstruction between November 2011 and November 2013, metal clips were fixed on the skin surface at the anticipated perforator locations, which were decided using a handheld Doppler. CT angiography was used to compare the location between the metal clip and the actual perforator. The metal clip was moved and repositioned, if needed, on the basis of the CT images. The locations of the appropriate perforator and the metal clip, which were observed during the surgery, were then compared. In CT angiography, the mean distance between the metal clip and the perforator was $3{\pm}3.9mm$, and the mean distance that was measured during surgery was $0.8{\pm}0.8mm$. In conclusion, we report a simple, rapid, and precise technique to indicate the accurate location of the appropriate perforator on the skin surface.

  • To determine the utility of spiral computed tomography(CT) in the evaluation of carotid artery stenosis, spiral CT images of 10 patients were compared with both ones of conventional angiography and magnetic resonance(MR) angiography. The severeness of stenosis was determined for each Internal carotid artery on the basis of arterial narrowness : mild stenosis : 40 % or less, moderate stenosis = $40{\sim}70%$, severe stenosis = greater than 70 % of arterial narrowness. The short examination and clear depiction of the arterial lumen In areas of complex or low flow make CT angiography attractive alternative to MR angiography. Spiral CT angiography shows normal and abnormal carotid anatomy well when compared with conventional anglography.
