The Study on the Comprehensiveness of Medical Appraisal (의료감정(鑑定)에 있어 포괄성에 대한 고찰)

  • The conventional medical appraisal which was done in the process of medical lawsuit was requested from the court to the designated hospital and was delivered as a pattern of one question and one answer in each. However, the comprehensiveness of medical appraisal which was pursued, for example, in Korea Medical Dispute Mediation and Arbitration Agency, could be guaranteed in terms of in-depth medical analysis as well as the broader capacity of the causality estimation besides. The comprehensiveness of appraisal would also include how well organized hospital system of medical care is and how well correlated job system among medical staffs, when medical dispute was happened at the hospital. This comprehensiveness will exert a big contribution on making a demonstrative medical care to prevent from the medical dispute and it could achieve the national plan of building the patient safety net which is effective in restoring the worsened quality of contemporary medical service. Therefore, the comprehensiveness of medical appraisal has to be designed to go forward interdisciplinary fused speciality rather than one division of medicine, which is also aiming at the reliable and consistent appraisal with the supreme dignity from one window. In addition to that, the objective and concrete frame of comprehensive appraisal under the computed connection has to be deliberated to make itself possible in collaboration with positive participation of medical community. The comprehensiveness of medical appraisal would serve to expand not only the capacity of speciality but also the ability of influence on a restorative justice, so that it give effect to an increased number of mediation and arbitration rather than medical lawsuit as well as a decreased number of the social cost and social conflict.

Subjective Satisfaction with Medical Care among Older People: Comprehensiveness, General Satisfaction and Accessibility (노인 의료이용의 주관적 만족도: 포괄성, 전반적인 만족도, 접근성을 중심으로)

  • Objectives : The changing population age structure and rapidly increasing medical costs make providing high-quality, effective medical care for the elderly a challenge. This study assessed the satisfaction with medical care in terms of comprehensiveness, general satisfaction, and accessibility among community-dwelling Korean elders. Methods : Data were obtained from a nationwide representative sample of the older adults(aged 65 years old or older) living in the community, who participated in a 2006 telephone survey conducted using random digit dialing(n=881). General satisfaction, comprehensiveness and accessibility were measured using a 10-item satisfaction survey questionnaire. Descriptive analysis was used to assess the distribution of each of three components of subjective satisfaction. Analysis of covariance(ANCOVA) was used to examine the association of each of the three components with socioeconomic variables. Results : Comprehensiveness and general satisfaction were low among older people with a high socioeconomic status. Accessibility was evaluated as low among older people of low socioeconomic status, those living in rural areas and those who were medical aid beneficiaries. Conclusions : Urgent interventions should be considered in order to improve accessibility to medical care for elders of low socioeconomic status and those living in rural communities. Given the rapid aging of the population, we need to develop a monitoring system to improve the quality of geriatric care.

Effects of Analyzability, Comprehensiveness, and Flexibility on Wage (분석성, 포괄성, 유연성이 임금에 미치는 영향)

  • We measure a worker's analyzability by how correctly he grasps its causes when a problem occurs, comprehensiveness by how much he understand the overall process of his firm, and flexibility by how frequently unusual situations occur at his job. After controling firm-specific fixed of fandom effects, his age, tenure, position, and rank, the characteristics increase a worker's wage. A worker who grasps correctly the causes of problem is paid more by 5.2% than a worker who scarcely does. A worker who understands the overall process of his firm is paid more by 2.52% than a worker who understands roughly the process of his team or department. A worker whose job is mew one by one is paid more by 2.98% than a worker whose job is routine and repetitive.

An Algorithm for Predicting the Relationship between Lemmas and Corpus Size

  • Much research on natural language processing (NLP), computational linguistics and lexicography has relied and depended on linguistic corpora. In recent years, many organizations around the world have been constructing their own large corporal to achieve corpus representativeness and/or linguistic comprehensiveness. However, there is no reliable guideline as to how large machine readable corpus resources should be compiled to develop practical NLP software and/or complete dictionaries for humans and computational use. In order to shed some new light on this issue, we shall reveal the flaws of several previous researches aiming to predict corpus size, especially those using pure regression or curve-fitting methods. To overcome these flaws, we shall contrive a new mathematical tool: a piecewise curve-fitting algorithm, and next, suggest how to determine the tolerance error of the algorithm for good prediction, using a specific corpus. Finally, we shall illustrate experimentally that the algorithm presented is valid, accurate and very reliable. We are confident that this study can contribute to solving some inherent problems of corpus linguistics, such as corpus predictability, compiling methodology, corpus representativeness and linguistic comprehensiveness.

Patient Assessment of Primary Care for Health Cooperative Korean Medicine Clinics in South Korea (의료생활협동조합 한의원의 일차의료서비스 수준 평가)

  • Objectives : Health cooperative movement is of increasing concern among medical consumers and professionals in Korea. Most health cooperative clinics provide Western Medicine and Korean Medicine(KM) to patients. This study aimed to evaluate the primary care level of health cooperative KM clinics and compare it with local KM clinics in Korea. Methods : Face to face survey was performed at the 3 health cooperative KM clinics and 5 local KM clinics with the Korean Primary Care Assessment Tool (KPCAT). The KPCAT consists of 5 domains (21 items): first contact (5), coordination function (3), comprehensiveness (4), family/community orientation (4), and personalized care (5). Subjects were patients or guardians who had visited KM clinics five times or more during the last 3 months. We compared primary care scores of each domain between health cooperative KM clinics and local KM clinics. Results : Data were collected from 200 respondents (100 patients from health cooperative KM clinics and 100 local KM clinics). Total average scores of the KPCAT for health cooperative clinics and local KM clinics were $81.1{\pm}12.0$ and $75.4{\pm}9.5$, respectively. Among primary care domains, personalized care was the highest ($89.2{\pm}12.0$, $89.6{\pm}8.4$, respectively), and comprehensiveness function was the lowest ($68.5{\pm}22.5$, $54.5{\pm}22.0$, respectively). Significant differences between two groups were noted in comprehensiveness function (68.5 vs. 54.5, P=0.000), family-community orientation (79.5 vs. 73.0, P=0.004), first contact(89.2 vs 84.0, p=0.001) and coordination function(74.0 vs 68.7, p=0.025). Conclusions : Based on the patients assessment, health cooperative KM clinics provide more primary care-oriented services than local KM clinics. This means that health cooperative clinic can be one of alternatives to strengthen the primary health care in Korea. Future researches are recommended to measure patients satisfaction and treatment effectiveness in the health cooperative clinics.

The Policy Proposal for Selecting Facility-stems of Rural Living Environment Renewal Program (농촌생활환경정비 대상 시설항목의 선정: 정책적 제안)

  • The problem of outdated and surplus facility-items in existing Rural Living Environment Renewal Program(RLERP) makes this study focus on restricting the range of facility sorts for the purpose of coㅁins with the changing rural living environment. In the four aspects of plan establishment, project implementation, comprehensiveness and linkage between programs, this study made an analysis of the existing RLERP that has 4-stage hierarchy, namely, Gun(county), Myun(primary administrative district), village and project site level Firstly, Myun and upper level policy programs fulfill the comprehensiveness and planning aspects, but have defects in the implementation and linkage aspects. Secondly, the village level programs satisfies more or less the four condition, but give rise to the problems of equity between the project target village and the non-target, and of generalization of the planning method applying to a specific village. At last, project level programs, controlled by several ministries, have defects in comprehensiveness and linkage aspects, exposing the sectoralism of central government. Above all, the existing RLERP has so many facility items, which have led to inefficient investment till now. Therefore, it's necessary that the environmental facilities should be selected(simultaneously reduced ), referring to following 5 criteria. (1) Exception of facilities relating to agricultural production. (2) Confined to public facilities to be constructed by government support. (3) Common-use facilities located in small town and below level area. (4) Consideration of not only the present need but also the future need of rural residents. (5) Reflection of a regional difference of mountain, plane and suburban area. Just 13 kinds of living environmental facilities are finally selected as the result of the selecting work, which can be an new alternative for RLERP. Concludingly, the selected facility items should be integrated into a designated project site by the prepared plan in order to enhance the efficiency of RLERP, investment and also to satisfy the four analytic aspects mentioned above.

Analysis of Online Reviews on Hotel Booking Intention: An Empirical Study in Indonesia

  • This study aims to determine the direct effect of positive online reviews, negative online reviews, the usefulness of online reviews, reviewers' expertise, timeliness of online reviews, the volume of online reviews, and comprehensiveness of online reviews on accommodation booking intentions and also the indirect effect of positive online reviews on the intention of booking accommodations through trust as mediation. Research respondents are users of the accommodation booking application in Yogyakarta. Hypothesis testing was carried out using SEM (Partial Least Square). Data was collected by distributing questionnaires to 135 respondents. The results of this study indicate that the Usefulness of Online Reviews, Volume of Online Reviews, and Comprehensiveness of Online Reviews have a direct positive and significant influence on the accommodation booking Intention of booking application users in Yogyakarta. The variables of Negative Online Reviews and Timeliness of Online Reviews have negative and significant influences on the accommodation booking Intention of booking application users in Yogyakarta. Positive Online Reviews and Reviewer Expertise variables are not significant in this study. At the same time, the Trust variable has a full mediation relationship in an indirect relationship between the Positive Online Reviews variables and the accommodation booking Intention of booking application users in Yogyakarta.

Development of a Consecutive Clinical Nursing Practicum Module using Simulation (시뮬레이션을 이용한 연속적 간호실습교육 모듈개발)

  • Purpose: This is a project report of the development of consecutive clinical nursing practicum modules using simulation encompassing the essentials of nursing over 4 years of university level education. Methods: The project was conducted from May to December in 2011 in a college of nursing in Seoul, Korea. Six nursing faculty members from different major subjects at the university participated in the project. The theoretical framework was sought in the first phase, resulting in utilizing Neuman's Systems Model. The principles of developing the contents of and the links between the modules were set up in the second phase, presented as complexity, diversity, and comprehensiveness. The details in the individual module were fleshed out in the third phase. Results: Eight clinical nursing modules using simulation were developed and presented in a table in detail. The complexity, diversity, and comprehensiveness of each module increased in depth and breadth in a consecutive order. Conclusion: It is hoped that this module is a decent curricular exemplar demonstrating comprehensive nursing practice education using a simulation technique.

National Health Expenditure Account of Korea: Sources and Estimation Methods (국민의료비 계정에 관한 연구 - 자료원 및 추계방법을 중심으로-)

  • National health expenditure account describes expenditure flows both public and private within the health sector. It describes the sources and uses and channels for all funds utilized in the health sector and is a basic requirement for optimal management of the allocation of health sector resources. Constructing a national health expenditure account should begin with sound estimates. This paper thoroughly examines the sources and discusses the estimation methods, and provides the national health expenditure account of Korea by function and source of funding category The national health expenditure account produced in this parer has, however, some drawbacks and followings are proposed fur enhancing the comprehensiveness and consistency of the account. First, comparable data un health related expenditures of local government and private sector should be produced because data sets on the sectors are very limited. Second, we need further study un overall scope and boundaries of health expenditure estimates in order to improve compatibility of other main aggregates.

  • Objectives : Classical texts such as 『Donguibogam』 and 『Yixuerumen』 have previously been used as primers to students of Korean Medicine. However, their massiveness in volume and comprehensiveness in contents make it unfit for students whose school curriculum lacked classical chinese. This paper suggests another introductory text that would be more practical in the current situation. Methods :Based on the translation of the main text and annotations, the clinical meanings of the contents were studied. Afterwards its practical application as a primer was considered. Results : The text focuses on the medically important issues in simple and accessible form, making it an important text for beginners to establish the foundation in medicine. Conclusions : Beginners will be able to establish a standard for basic medical knowledge through this text and also apply its contents to diseases that are relatively easy to treat.
