The Influence of Nurses' Organizational Communication and Self-Leadership on Patient Safety Competence in Comprehensive Nursing Service Units: Focusing on Small and Medium-sized Hospitals (간호⋅간병통합서비스병동 간호사의 조직 내 의사소통과 셀프리더십이 환자안전역량에 미치는 영향: 중소병원을 중심으로)

  • Purpose: This study investigated the influence of nurses' organizational communication and self-leadership on patient safety competence in comprehensive nursing care units of small and medium-sized hospitals. Methods: A descriptive design was used and self-reported questionnaires were used to collect data from 165 nurses in Seoul from February to March, 2020. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient, and multiple linear regression with SPSS/WIN 22.0 program. Results: A mean score of organizational communication was 3.20±0.49, self-leadership 3.58±0.50, and patient safety competency 4.01±0.49 out of 5. A significant positive correlation was found between patient safety competence, organizational communication and self-leadership. Self-leadership and combined ward explained 32% of the variance of patient safety competence (Adjusted R2=.32, p<.001). Conclusion: Patient safety competence in comprehensive nursing care units are associated with self-leadership and organizational communication. Systemic education to improve patient safety competence in small and medium sized hospitals should be implemented to promote organizational communication as well as self-leadership.

The factors influencing on Compliance with Job Satisfaction of the Nurses in Comprehensive Nursing Care Service Wards - Focusing on Nursing professionalism and Nursing work environment (간호·간병통합서비스 병동 간호사의 직무만족에 미치는 영향 요인 -간호전문직관과 근무환경을 중심으로-)

  • The purpose of this study was to investigate the factors affecting job satisfaction of nurses in nursing care service ward. The subjects were 127 nurses, nursing job satisfaction, nursing intuition, and nursing work environment using the 5 - point Likert scale. Data were analyzed by t-test, ANOVA, correlation and regression analysis using SPSS WIN 23.0 program. The results showed that the correlation between job satisfaction and independent variables in nursing and nursing care service ward was significant in nursing professionalism (r = .90, p <.001) and nursing work environment (r = .75, p <.001). In addition, factors affecting job satisfaction were in the order of nursing professionalism and nursing work environment (F = 31.516, p <.001), and explanatory power was 84.1%. Therefore, Therefore, the necessity of nursing convergency strategy for improvement of these is suggested.

  • The purpose of this study is to provide basic data for effective human resource management by clarifying the relationship among ward operation support system of ward nurse, nursing performance, job satisfaction, burnout and turnover intention. The subjects of this study were 137 nurses working at comprehensive nursing care service wards of less than 500 beds in B and C city. Data were collected using structured questionnaires, and analysis was conducted using SPSS/WIN 21.0 and AMOS 18.0 programs. As a result of the path analysis, variables directly affecting the turnover intention of nurses working at the comprehensive nursing care service wards are ward operation support system and burnout, and indirect influences are nursing performance and job satisfaction. Burnout has the most direct effect on turnover intention, which is directly influenced by job satisfaction and nursing performance. Ward operation support system was confirmed to have a direct effect on job satisfaction. Therefore, in order to reduce the turnover intention of nurses working at the comprehensive nursing care service wards, strengthening ward operation support system in order to improve job satisfaction and measures to reduce the burnout and burden of nursing performance are needed.

  • Purpose: It is recently reported that the increasing noise in the hospitals has caused psychological and physiological stress problems with patients, and medical staffs. This study intends to investigate and analyze the noise levels in the sub stations in comparison with those in the wards in general. This study tries also to find some alterative solutions to the immediate problems. Methods: Noise measurements are conducted in advance prior to analyzing their results at two general hospitals with more than 900 beds, in the comprehensive nursing service wards and in the main/ sub stations located in the general wards Results: Although the noise level in the comprehensive nursing service wards has been slightly lower than that in the general wards, this result is over the recommended noise levels. Therefore it is recommended that efforts should be made to low down the noise level as an alternative and to replace the aged carts as well. Implications: The comprehensive nursing service wards are required to take measures against the various noise sources.

  • A considerable change of the Korean nursing system has been made during the last decade not only in its philosophy but also in its function and structure to meet political and scientific need of the modern society. The main purpose of this study is to develope the new concept of comprehensive nursing care, both its Philosophy and ethics, as the basic of modern clinical nursing theory and practice. Comprehensive nursing care is the concept of human centered nursing care, and it helps a man to meet the basic physical, psychological, environmental, socioeconomic and teaching needs. It also helps him to help himself to meet these needs. This concept starts from the individualized nursing care and its ultimate goal is to improve a man to have a better position in his own community so that he may able to have a meaningful life. To accomplish this goal, an individualized nursing care plan as a nursing diagnosis and problem solving method should be set up for different patients with similar diagnosis to meet their needs, because each patient has a different social background. from this viewpoint, nursing is a science as well as abstruse humanity. The performance of comprehensive nursing care is a goal and issue of modern clinical nursing care. If nursing is a science and a profession for man, it should have ethics which recognize the dignity of man and offers infinite service voluntarily, and should be able to show leadership in carrying out the nursing responsibility. This leadership finds a person's potential and encourages him to utilize it. Such concepts should develop into a nursing ideology and this ideology should become a priority in comprehensive nursing care. The following statements are the conclusion of this study. 1) Modern nursing has been developed from disease centered nursing care to comprehensive nursing care based on humanity. The primary principle of nursing was to assist in the treatment of disease, but it has been changed to the professional nursing system independently. 2) The concept of nursing is one of continuous or endless scope of dispersion. It proves that nursing is grasping the professional responsibility to be able to coordinate scientific principles Patient health problems are according to scientific principles rather than adhering to nursing technical discipline as a daily work. 3) In chapter I and Ⅱ, the philosophy and ideology of nursing have been discussed and the flow of concept of clinical nursing and the rate of progress which emerges from naturalizing performance of the concept of comprehensive nursing in clinical nursing studied. The discussion developed the theory that a nurse should be to embody nursing ideas and objectives by establishing definite conviction of professions and study. 4) In chapter lil, nursing planning based on nursing diagnosis as a method to attain ideal nursing care for humanity with a definite idea of establishing philosophy of nursing was presented. 5) From the result of survey on patient needs about treatment and nursing, it was observed that all patient had emotional stress from unknown factors. Therefore it was concluded that nurses should not only educate the patient but also give them the opportunity to communicate freely their needs and anxieties. Furthermore complaints and doubts of the patient should be carefully noted and must be considered to meet these needs. 6) Patient teaching is the most important part of comprehensive nursing care. In chapter, Ⅲ, the important of patient teaching was emphasized by demonstrating the effect of patient teaching for diabetic patient. 7) In Chapter Ⅳ, from the result of the study on nurses attitudes to comprehensive nursing care, it was pointed that the evolution of nursing education and the establishment of a complete concept and value of comprehensive nursing was necessary.

  • The purpose of this study was to identify nurses' communication style and the extent of their health literacy and to discern the impact of nurses' communication and health literacy on self-care behaviors, as perceived by elderly diabetic patients in comprehensive nursing-care service wards. This study provides basic data for establishing an efficient nursing-care service-system environment through high-quality self-management. The measurement and improvement of an accurate health literacy level of elderly diabetic patients is an important intervention method to increase the self-care behavior of patients. Therefore, by developing a distinguished nursing intervention plan, early treatment of diabetic patients and quality of care may be possible.

The Relationships among Awareness of Patient Safety Culture, Critical Thinking Disposition and Patient Safety Nursing Activities of Nurses among Comprehensive Nursing Care Service Ward (간호·간병통합서비스병동간호사의 환자안전문화인식, 비판적 사고성향과 환자안전간호활동의 관계)

  • This study was conducted to identify the levels of patient safety culture, critical thinking disposition, and patient safety nursing activities and determine the factors affected by patient safety nursing activities among nurses in comprehensive nursing care service wards. Subjects consisted of 150 nurses from general hospitals in B city, Korea, and data were analyzed using the SPSS Win 21.0 program. The mean scores of awareness of patient safety culture, critical thinking disposition, and patient safety nursing activities were $3.32{\pm}0.32$, $3.50{\pm}0.31$, and $4.28{\pm}0.46$, respectively. The awareness of patient safety culture was significantly different by clinical career in the present unit (F=4.79, p=0.001). The critical thinking disposition was significantly different by age (F=3.89, p=0.010) and position (F=6.40, p=0.002). There were differences in the scores of patient safety nursing activities according to position (F=3.19, p=0.044). Additionally, hospital environment (${\beta}=0.25$, p=0.014), supervisor attitude (${\beta}=0.20$, p=0.046), and position (${\beta}=0.14$, p=0.040) accounted for a 44.4% variance in patient safety nursing activities. To promote patient safety nursing activities in comprehensive nursing care service wards, positive awareness of the hospital's safety environment and support from the manager are required.

Factors Influencing Professional Quality of Life in Intensive Care Unit Nurses of University Hospitals (간호·간병통합서비스병동 간호사의 직무스트레스가 소진에 미치는 영향: 감성지능의 조절 및 매개효과 )

  • Purpose : This study aims to identify job stress, emotional intelligence, and exhaustion of the nurses in comprehensive nursing service units and then verify the moderating and mediating effects of emotional intelligence on the relationship between job stress and exhaustion. Methods : Participants are 118 nurses working in two general hospitals. The collected data are analyzed using the SPSS WIN 27.0 and AMOS 18.0 programs by assessing frequency and percentage, mean and standard deviation, independent t-test and one-way ANOVA, Scheffé test, Pearson correlation coefficient, and hierarchical multiple regression analysis and bootstrapping. Results : Exhaustion has a statistically significant positive correlation with job stress (r=.40, p <.001) and a statistically significant negative correlation with emotional intelligence (r=-.26, p =.004). A partial mediating effect of emotional intelligence is found between job stress and exhaustion, however, there is no moderating effect. Conclusion : It is expected that to reduce the exhaustion of nurses in comprehensive nursing service units, it is necessary to assess job stress and emotional intelligence, and strengthen emotional intelligence along with job stress intervention.

Influences of Positive Psychological Capital, Job Satisfaction, and Social Support on Performance of Nurses in Comprehensive Nursing Care Service Wards (간호·간병통합서비스 병동 간호사의 긍정심리자본, 직무만족, 사회적 지지가 간호업무성과에 미치는 영향)

  • Purpose: This study aimed to prepare basic data for the development of a nursing intervention program to improve nursing performance by identifying the factors affecting nursing performance in an integrated nursing care service ward. Methods: Participants were 166 nurses who had worked for more than six months in the integrated nursing care service wards of three general hospitals located in B city. Data were collected from July 15 to August 30, 2022 using structured questionnaires and analyzed by T-test, ANOVA, Scheffé test, Pearson's correlation coefficients and multiple linear regression using the IBM SPSS/WIN 25.0 statistical program. Results: Factors affecting the nursing performance of participants were age (β=.58, p<.001), positive psychological capital (β=.41, p<.001), and social support (β=.28, p<.001). The total explanatory power of these variables for nursing performance was 55.8%. Conclusion: The results of this study suggest the need for programs that improve psychological capital and social support while considering nurse's age to enhance their performance in nursing integrated service wards.

Optimal Nursing Workforce and Financial Cost to Provide Comprehensive Nursing Service in the National Health Insurance System (국민건강보험 간호·간병통합서비스의 전면 도입을 위한 간호인력 및 재정비용 추계)

  • This study estimated the optimal nursing workforce and financial costs of providing comprehensive nursing services at hospitals under the national health insurance system. Data on registered nurses, nursing aids, medical institutions, and number of patients were obtained from the Health Insurance Review and Assessment Service. The optimal size of the nursing workforce was calculated using the workload model. A bottom-up approach was used to estimate the annual total financial cost of comprehensive nursing services. The number of registered nurses and nursing aids would need to be increased by 81.75% and 83.23%, respectively, in order to fully apply comprehensive nursing care on a national scale. The additional financial costs for comprehensive nursing services at all hospitals was estimated to be as much as 110.39% of the current cost. For the comprehensive nursing service, nurses with a career and newcomers need to be retained at their hospitals, and the validity of the nurse-patient ratio should be continuously checked. The financial shock to the national health insurance system could be minimized by gradually extending the system to all hospitals.
