Supernumerary teeth that are present in the molar region may be evident based on crowding and impaction, but most cases are asymptomatic and discovered as incidental findings during routine radiological examinations. This article reports the case of a 29-year-old woman who presented with a severe feeling of pressure in the region of the maxillary third molars that had been increasing in intensity for weeks. A clinical examination revealed crowding of the maxillary anterior teeth despite the completion of orthodontic treatment and an erupted third molar with localized gingivitis in the second quadrant. A radiographic examination revealed bilateral supernumerary maxillary fourth and fifth molars, so cone-beam computed tomography was performed to locate the supernumerary teeth precisely for a preoperative diagnosis and comprehensive treatment planning. This report presents the radiological and surgical case management of a rare case of bilateral supernumerary molars and reviews the literature regarding epidemiology and treatment options.

Sialolithiasis is a condition in which the salivary gland excretory duct is obstructed due to the formation of calcareous deposits and is uncommon in children compared to adults. The treatment modalities range from a conservative approach involving hydration to a surgical approach. Though several studies have analyzed the sialolith micromorphology structures, studies on pediatric sialoliths remain scarce. This brief communication aims to describe the sialolith micromorphology to understand the mechanism of mineralization and growth of pediatric sialoliths. A 6-year-old Korean female presented with swelling under her tongue. The intraoral examination revealed a painless yellowish hard mass beneath the tongue near the Wharton's duct which was suspected as a sialolith. After receiving the informed consent, the sialolithotomy was performed under local anesthesia. The obtained stone was analyzed through histopathology and transmission electron microscope examinations to understand the mechanism of mineralization and growth of pediatric sialolith. The micromorphology and growth processes of pediatric sialolith remain undescribed. More comprehensive microscopic studies are needed regarding their distinctive characteristics. By expanding knowledge about sialoliths micromorphology, development of new preventive, diagnostic and patient-tailored treatment methods of pediatric sialolithiasis will be enhanced.
Objective: The aim of this systematic review was to assess the occlusal outcome and duration of fixed orthodontic therapy from clinical trials in humans with the Objective Grading System (OGS) proposed by the American Board of Orthodontics. Methods: Nine databases were searched up to October 2016 for prospective/retrospective clinical trials assessing the outcomes of orthodontic therapy with fixed appliances. After duplicate study selection, data extraction, and risk of bias assessment according to the Cochrane guidelines, random-effects meta-analyses of the mean OGS score and treatment duration were performed and 95% confidence intervals (CIs) were calculated. Results: A total of 34 relevant clinical trials including 6,207 patients (40% male, 60% female; average age, 18.4 years) were identified. The average OGS score after treatment was 27.9 points (95% CI, 25.3-30.6 points), while the average treatment duration was 24.9 months (95% CI, 24.6-25.1 months). There was no significant association between occlusal outcome and treatment duration, while considerable heterogeneity was identified. In addition, orthodontic treatment involving extraction of four premolars appeared to have an important effect on both outcomes and duration of treatment. Finally, only 10 (39%) of the identified studies matched compared groups by initial malocclusion severity, although meta-epidemiological evidence suggested that matching may have significantly influenced their results. Conclusions: The findings from this systematic review suggest that the occlusal outcomes of fixed appliance treatment vary considerably, with no significant association between treatment outcomes and duration. Prospective matched clinical studies that use the OGS tool are needed to compare the effectiveness of orthodontic appliances.

Background: Dental sedation reduces fear and phobia during dental treatment and helps patients get quality treatment by inducing adequate consciousness control. Propofol has recently grabbed the spotlight, but no meta-analysis for efficacy and safety of propofol in dentistry has yet been performed. Thus, the purpose of this study was to perform meta-analysis to verify the efficacy and safety of propofol for use in dental sedation. Methods: Articles published between 1980 and 2010 were searched in the web sites, journals and medical database including The Cochrane Library, MEDLINE and EMBASE. And a total of 22 studies were selected among the randomized controlled trials (RCTs) that compared the use of propofol with other sedatives (control group). The data was collected from these studies and meta-analysis for efficacy and safety was performed using Comprehensive Meta-Analysis 5.0 (CMA 5.0). Results: The patient recovered significantly faster and discharged significantly earlier in the propofol group (SMD = -1.442, P < 0.001). The satisfaction of patient and that of operator was higher in the propofol group (P < 0.05). The incidence of arrhythmia and apnea/ hypoventilation was significantly lower in the propofol group (OR = 0.071, P < 0.05), and there was no significant difference in the other side effects. On the level of sedation, although the sedation score was significantly lower in the propofol group (SMD = -0.430, P < 0.05). Conclusions: The present analysis showed that the use of propofol resulted in high satisfaction levels on the part of the patients and operators, a shorter recovery time, and faster hospital discharge. The incidence of complications, however, was lower in the propofol groups or not much different between the propofol and control groups. Thus, the adequate use of propofol in dentistry is believed to be helpful for the effective and safe sedation of the patients.

Soft tissue calcification is a pathological condition in which calcium and phosphate salts are deposited in the soft tissue organic matrix. This study presents an unusual calcification noted in the cartilaginous portion of the Eustachian tube. A 67-year-old woman presented for dental treatment, specifically for implant placement, and cone-beam computed tomography (CBCT) was performed. The CBCT scan was reviewed by a board-certified oral and maxillofacial radiologist and revealed incidental findings of 2 distinct calcifications in the cartilaginous portion of the Eustachian tube. To the authors' knowledge, no previous study has reported the diagnosis of Eustachian tube calcification using CBCT. This report describes an uncommon variant of Eustachian tube calcification, which has a significant didactic value because such cases are seldom illustrated either in textbooks or in the literature. This case once again underscores the importance of having CBCT scans evaluated by a board-certified oral and maxillofacial radiologist.
The success in periodontal-prosthetic therapy lies in patient satisfaction with subjective and customized treatment regimen that has been refined for individual needs. To fulfill these requirements, multiple predictors may have to be taken into consideration in the comprehensive treatment planning. Incorporation of complex factors that guarantee the evidence-based therapy would include, but not limited to, osseous morphology, biotype of gingival around abutment, bone morphology of interdental bone and those around furcation area, abutment or bridge mobility. The periodontal-restorative interface should be of mutually protective. Also strategic and esthetic value of abutment teeth to be restored should also be taken into consideration. Taken together, all these should work in concert to enhance the predictability and longevity of abutment teeth in periodontal-prosthetic therapy.
Genioplasty may considerably change a person's face which needs surgical correction of deformed chin. Comprehensive treatment planning is therefore decisive for the treatment to be successful. A three-dimensional analysis of chin relative to cranial base, upper and lower jaws using the frontal and lateral facial photographs and cephalographs permits classification of the defective chin position and provides a basis for operation planning and deciding upon the operation method. A chin miniplate system has been developed for intraoperative registration securing the sagittal, vertical and horizontal position of the osteotomized chin and ould reduce the relapse rate.
This study was carried out to examine comparison of relating influencing factors on medical service satisfaction between general dental patients and orthodontic patients and to improve quality of the medical services by the treatment type of the visit. The data were collected from 421 patients who visited dental clinics and hospitals in Daegu and Gyeongbuk region, using self-administered questionnaire for the period from Sep 1 to Sep 30, 2015. The multiple regression showed that the medical staff and medical fee were common statistically significant independent variables to influence service satisfaction in both groups, however, fear in the treatment was added to statistically significant variables in only the orthodontic patients. Therefore, based on the provision of high - quality medical services and adequate medical care for patients' satisfaction, comprehensive plans including tailor-made counseling management and reducing the fear of dental treatment for general dental patients should be needed.
Purpose : The purpose of this study was to compare the plaque removal effects of vacuum oral cleaner developed for the patients with limited mobility with those of manual toothbrushes and high pressure injection oral cleaner (dental water jet). Meterials and methods : Thirty human subjects were measured with Patient Hygiene Performance index (PHP index) and O'Leary index before and after the use of toothbrush, high pressure injection oral cleaner and vacuum oral cleaner. These three different oral hygiene methods were conducted with seven-day intermittence. Then the statistical analysis was carried out to define plaque removal rate of three different oral hygiene methods (${\alpha}=.05$). Results : According to the efficacy analysis of plaque removal before and after the oral cleaning using each of three methods, significant reduction in plaque after the treatment compared to the previous state when using toothbrush, high pressure injection oral cleaner, and vacuum oral cleaner was observed (P < 0.001). PHP index of tooth brushing was higher than that of the high pressure injection oral cleaner, while PHP index of vacuum oral cleaner did not show significant difference from either of the other two methods. There was no significance difference in O'Leary index among the three methods. Conclusion : Effect of plaque removal using the vacuum oral cleaner is comparable to that of manual tooth brush or high pressure injection oral cleaner, so it will be helpful for self-oral hygiene care of the patients with limited mobility.
In our daily practice, we think about the diagnosis of our patient and get into a situation wherein we have to make a clinical decision. Diagnosis and treatment come from the knowledge and experiences that each dentist should have, but sometimes, we can have doubts on our decisions. "On what evidence did I make such decision? Was that really right?" Drawing our attention these days as a possible answer to this question, evidence-based dentistry seeks to apply the best available evidence gained from the scientific method to medical decision making. To make a good decision, the strength of evidence is assessed. Specifically, randomized controlled trial, systematic review, and meta-analysis are considered the highest level of evidence; cohort study, case control study, case series, animal study, bench test, and biological plausibility follow. With the approach of evidence-based dentistry, we can make objective, scientifically sound clinical decisions. It is also patient-oriented, incorporating clinical experiences and stressing good judgments; thorough and comprehensive, it uses transparent methodology. That is the reason evidence-based dentistry can be better than other assessment methods when we make a clinical decision in modern dentistry.
