Considering the well known environmental issues of cement manufacturing (direct and indirect levels of $CO_2$ emissions), clinker replacement by supplementary cementing materials (SCM) can be a very promising first step in reducing considerably the associated emissions. However, such a reduction is possible up to a particular level of SCM utilization, influenced by the rate of its pozzolanic reaction. In this study a (4-step) structured methodology is proposed in order to be able to further adjust the concrete mix design of a particular SCM, in achieving additional reduction of the associated levels of $CO_2$ emissions and being at the same time accepted from a derived concrete strength and service life point of view. On this note, the aim of this study is twofold. To evaluate the environmental contribution of each concrete component and to provide the best possible mix design configuration, balanced between the principles of sustainability (low environmental cost) and durability (accepted concrete strength and service life ). It is shown that such a balance can be achieved, by utilising SCM by-products in the concrete mix, reducing in this way the fixed environmental emissions without compromising the long-term safety and durability of the structure.
In recent years, contamination of drinking water sources has emerged as a serious social problem, such as a large number of impurities in tap water or groundwater or the supply of suitable water due to rust of pipes. Although the government and public institutions are implementing various measures to protect water sources, they cannot improve water quality in a short period of time because of the enormous cost involved. Therefore, in recent years, preference has been given to a device that converts tap water, which is hard water, into soft water by installing a separate water softener at the faucet from which tap water is discharged. However, the existing filtration device has a problem that filtration performance is gradually lowered when impurities accumulate in the filter, requiring continuous filter replacement. In this study, the optimal design of the filter housing was performed to develop a water softener that can be washed when impurities accumulate on the filter inside the water softener connected to the faucet. For optimal design of the filter housing, fluid and fluid-structural interaction analysis were performed on the design pressure to determine the shape and thickness of the housing, and design review was performed through prototype.
Reverse total shoulder arthroplasty (RTSA) is widely popular among shoulder surgeons and patients, and its prevalence has increased dramatically in recent years. With this increased use, the indicated pathologies associated with RTSA are more likely to be encountered, and challenging patient presentations are more likely to be seen. One prominent challenging presentation is RTSA patients with severe glenoid bone loss. Several techniques with varying degrees of invasiveness, including excessive reaming, alternate centerline, bone grafting, and patient-specific implants (PSIs), have been developed to treat patients with this presentation. PSI treatment uses a three-dimensional reconstruction of a computed tomography scan to design a prosthetic implant or component customized to the patient's glenoid morphology, allowing compensation for any significant bone loss. The novelty of this technology implies a paucity of available literature, and although many studies show that PSIs have good potential for solving challenging shoulder problems, some studies have reported questionable and equivocal outcomes. Additional research is needed to explore the indications, outcomes, techniques, and cost-efficiency of this technology to help establish its role in current treatment guidelines and strategies.

Background Simulation training is becoming an increasingly important component of skills acquisition in surgical specialties, including Plastic Surgery. Non-living simulation models have an established place in Plastic Surgical microsurgery training, and support the principles of replacement, reduction and refinement of animal use. A more sophisticated version of the basic chicken thigh microsurgery model has been developed to include dissection of a type 1-muscle flap and is described and validated here. Methods A step-by-step dissection guide on how to perform the chicken thigh adductor profundus free muscle flap is demonstrated. Forty trainees performed the novel simulation muscle flap on the last day of a 5-day microsurgery course. Pre- and post-course microvascular anastomosis assessment, along with micro dissection and end product (anastomosis lapse index) assessment, demonstrated skills acquisition. Results The average time to dissect the flap by novice trainees was $82{\pm}24$ minutes, by core trainees $90{\pm}24$ minutes, and by higher trainees $64{\pm}21$ minutes (P=0.013). There was a statistically significant difference in the time to complete the anastomosis between the three levels of training (P=0.001) and there was a significant decrease in the time taken to perform the anastomosis following course completion (P<0.001). Anastomosis lapse index scores improved for all cohorts with post-test average anastomosis lapse index score of $3{\pm}1.4$ (P<0.001). Conclusions The novel chicken thigh adductor profundus free muscle flap model demonstrates face and construct validity for the introduction of the principles of free tissue transfer. The low cost, constant, and reproducible anatomy makes this simulation model a recommended addition to any microsurgical training curriculum.
